Introduction: Fluoxetine raises the levels of BDNF (brain-derived neurotrophic factor). BDNF is known to improve neurogenesis and plasticity, so it seems to improve Anterior Ischemic Optic Neuropathy (AION). The goal of this study was to find out how Fluoxetine affects the clinical outlook of people with AION. Methods: In this double-blind, placebo-controlled, randomized clinical trial, patients with AION split into two groups: the fluoxetine group (n=50), which took 20 mg of Fluoxetine every day, and the control group (n=50), which took a placebo pill instead. Both groups were followed for six months. Before and after the trial, patients were given clinical and non-clinical evaluations. Results: 100 people took part in this study and were evaluated. Subjects in the Fluoxetine group had better visual acuity than those in the placebo group. They had lower scores on the LogMAR scale (P: 0.008 and 0.002), better MD parameters of perimetry (P: 0.003 and 0.002), and shorter VEP latencies (P (in 1st minute): 0.001 and 0.001, P (in 15th minute): 0.038 and 0.011. After the trial of Fluoxetine therapy, there were no changes in color vision, Rnfl in all dimensions, PSD parameter of perimetry, or VEP amplitudes (Ps> 0.05. Conclusion: Fluoxetine showed promise as a therapy for people with AION, and it was safe to use as a treatment option in addition to corticosteroids

Purpose: To report a rare case of primary pneumosinus dilatans (PSD) and to specify the cardinal imaging findings associated with this condition. Methods: A 20-year-old patient presented with bilateral profound visual loss as a result of primary PSD. A detailed review of clinical findings and presumed pathophysiological basis of vision loss was performed. Results: Other than undiagnosed primary hypothyroidism, no other abnormalities were found. With the diagnosis of PSD, the patient underwent optic nerve decompression through transnasal sphenoidotomy. However, after nine months of follow-up, no improvement in the patient's vision was attained. Conclusion: Unlike previous reports of favorable visual results after sphenoidotomy and bilateral decompression of the optic nerves, vision recovery was not achieved in this case.

Introduction: To compare the optic nerve head (ONH) structure and microvasculature in patients with optic atrophy due to non-arteritic anterior ischemic optic neuropathy (NAION), compressive optic neuropathy (CON), methanol-induced optic neuropathy (MION), and traumatic optic neuropathy (TON) using optical coherence tomography angiography.Methods: In this comparative, cross-sectional study, 32 eyes with NAION, 18 eyes with CON, 32 eyes with MION, 23 eyes with TON, and 55 normal eyes were enrolled. Radial peripapillary capillary (RPC) vessel density, peripapillary retinal nerve fiber layer (RNFL) thickness, disc area, cup volume, and cup/disc area ratio were obtained using the RTVue XR Avanti system (Optovue Inc., Fremont, CA, USA).Results: RPC vessel density and peripapillary RNFL thickness in all patients were significantly lower than normal subjects. A positive correlation was found between the RPC vessel density and peripapillary RNFL thickness in normal subjects and all study groups. The positive correlation between the inside and outside disc RPC vessel density was only found in the NAION (r = 0.36, P = .042) and MION (r = 0.42, P = .018) groups. No significant difference was found among the groups in terms of peripapillary and inside disc vascular densities (all P > .05). Disc area and cup volume in patients with MION was larger than the values in patients with NAION (P = .018) and TON (P = .044) and normal subjects (P = .015). The discriminating features among the study groups were the larger cup volume and cup/disc area ratio in patients with MION, and lower RNFL thickness in patients with TON.Conclusions: There was a positive correlation between the RNFL thickness and peripapillary RPC vessel density regardless of the cause of optic disc pallor. Structural evaluation of the ONH seems to be a better way to differentiate the cause of optic nerve head atrophy than the microangiographic changes.

Purpose: To evaluate the results of overminus lens therapy in the management of children with intermittent exotropia or X(T). Methods: In this retrospective study, 163 consecutive patients with X(T) who were treated with overminus spectacles with at least 12 months of follow-up were included in the study. The outcome measures were the level of X(T) control evaluated using the Jampolsky's qualitative assessment method and refractive error changes under overminus lens treatment. Results: The mean angle of deviation at the initial visit was 24.7 ± 15.1 prism diopters (PD) that improved to 10.6 ± 4.2 PD with overminus glasses with a median follow-up of 38 months (P = 0.02). One hundred and nine patients (66.8%) achieved good controlled X(T) or orthotropia by overminus lens therapy after 1 year. Three patients progressed to esotropia, which disappeared after discontinuing overminus lens therapy. Overminus lens therapy did not have a statistically significant effect on the mean spherical equivalent of cycloplegic refraction in each eye (right eye: P = 0.13; left eye: P = 0.15). Conclusions: Overminus lens therapy can be effective for improving the control of X(T) in young children. It can defer the requirement for surgery or decrease the rate of surgical intervention.

Ischaemic optic neuropathy (ION) following non-ocular surgeries is a rare but devastating complication. Spine and cardiovascular surgical procedures are well known to be accompanied by this complication. ION after liposuction surgery is gaining attention during the past decades due to the escalating demand for this procedure. We report a case of unilateral anterior ION (AION) after abdominoplasty and liposuction in an eye with optic disc drusen (ODD), which has not been reported previously. The presence of ODD is not only an independent risk factor for AION in young patients, but could also be the underlying cause of the ischaemic insult to the optic nerve head in haemodynamically unstable patients.

Vascular insufficiency of the optic nerve head is the cause of non-arteritic ischemic optic neuropathy (NAION). Carotid artery abnormalities are now considered as a predisposing factor for developing NAION. In this report, we present a case of NAION associated with ipsilateral internal carotid artery hypoplasia.A 69-year-old female was referred for evaluating the cause of vision loss in her left eye. The patient was diagnosed with left NAION, however, the magnetic resonance imaging (MRI) showed a considerably smaller left internal carotid artery (ICA) than the right side. The magnetic resonance angiography (MRA) confirmed the disparity as well as perfusion abnormalities of intracranial carotid arteries. The diagnosis of left ICA hypoplasia was made and verified by depicting a left side small carotid canal on the computed tomography (CT) scan.This report suggests that ICA hypoplasia could be associated with NAION.

Purpose This report describes the clinical course of the traumatic chiasmal syndrome and treatment of diplopia due to the hemifield-slide phenomenon. Case report A 20-year-old male with decreased vision in both eyes following blunt head trauma was referred to the neuro-ophthalmology clinic. Neuroimaging revealed disruption of the optic chiasm and skull base fracture. After partial vision recovery, the patient noticed complete bitemporal hemianopia. Three months later, he was exotropic and complained of diplopia and difficulty in near visual tasks. His symptoms were successfully controlled by injection of botulinum toxin A in both lateral rectus muscles. Conclusions There is some potential for improvement in vision in patients with traumatic chiasmal syndrome. Botulinum toxin could be used in patients with the hemifield-slide phenomenon to predict the sensory outcome of surgery and relieve the bothersome symptoms.

To evaluate the optic disc microvasculature in optic nerve head drusen (ONHD) vasculature in comparison to acute nonarteritic anterior ischemic optic neuropathy (NAION) and normal eyes using optical coherence tomography angiography (OCT-A).Ten eyes with ONHD, 10 eyes with acute NAION, and 10 healthy eyes were included in this prospective, comparative, observational case series. OCT-A imaging was performed on the optic discs. Qualitative grading was performed for dilation and tortuosity of the peripapillary vessels by 3 graders. Quantitative comparison was performed for peripapillary and inside disc vessel densities in nerve head (NH) and retinal peripapillary capillary (RPC) slabs.The intergrader agreement for dilation and tortuosity of the peripapillary vessels was poor (0.313 and 0.182 for vascular dilation in nerve head and radial peripapillary capillary enface images, respectively, and 0.478 and 0.490 for vascular tortuosity in nerve head and radial peripapillary capillary enface images, respectively). In NH en face images, the vessel density measurements were statistically significantly different between the 3 groups (all p0.05). In RPC en face images, the vessel density measurements were statistically significantly different between the 3 groups (all p0.05) except for nasal peripapillary sector (0.08).Despite poor intergrader agreement in qualitative analysis, quantitative OCT-A evaluation may differentiate optic disc edema due to NAION from pseudodisc edema due to ONHD.

Clinical relevance The existence of a correlation between the results of paraclinical retinal imaging and clinical tests such as contrast sensitivity can reduce time and cost in diagnosing optic neuritis (ON). Background To demonstrate whether changes of peripapillary retinal nerve fibre layer (RNFL) thickness are correlated with the results of visual acuity, contrast sensitivity and colour vision in eyes with acute ON. Methods Thirty patients with acute ON, 23 females and seven males, who had no previous history of ON in neither eye, were examined. Inclusion criteria were: subjects had spherical refraction of less than ±5D, no prior ON or optic disc swelling, no history of amblyopia or colour blindness, and no history or ophthalmoscopic evidence of glaucoma, diabetic retinopathy, or maculopathies. Visual acuity, contrast sensitivity and colour vision were tested and optical coherence tomography was performed for all patients after complete ophthalmologic examinations including refraction, biomicroscopy, and funduscopy with a 90D lens by a neuro-ophthalmologist. Results Thirty patients who were 18-45 years old entered the study. There was an inverse correlation between contrast sensitivity and nasal (r = -0.430, p = 0.018), inferior (r = -0.503, p = 0.005) and mean (r = -0.510, p = 0.004) RNFL thickness. The multiple linear regression model, after adjustment for age and sex, showed a significant association between visual acuity and nasal RNFL thickness (coefficient = 0.025, p = 0.032). There was also a significant inverse correlation between contrast sensitivity and nasal (coefficient = -0.003, p = 0.036), inferior (coefficient = -0.006, p = 0.010) and mean (coefficient = -0.007, p = 0.012) RNFL thickness. No significant correlation was found between colour vision and RNFL thickness. Conclusion Contrast sensitivity is more correlated with changes in RNFL thickness in comparison with visual acuity and colour vision. Contrast sensitivity measurement therefore can be used as one of the first assessments in patients with ON.

Introduction: Radiation-induced optic neuropathy (RION) is still a devastating complication of brain and skull base radiation that has no effective treatment up until today, thus uttermost caution must be taken in treating patients that brain radiotherapy is needed. We present two cases of RION that happened in seemingly safe radiation doses. Case description: A 48-year-old female with a history of pleomorphic pituitary adenoma developed bilateral and painless loss of vision 10 months after radiation to the brain; the total radiation dose was 45 Gy in 25 fractions and no other risk factors of RION were found. Magnetic resonance imaging of the brain depicted bilateral prechiasmatic optic nerve enhancement with involvement of the optic chiasm. Treatment with high doses of corticosteroids was unsuccessful. A 62-year-old female with a history of lung adenocarcinoma and brain metastases presented with a 1-month history of decreased vision in both eyes. He had undergone whole-brain radiotherapy with a total dose of 30 Gy over 10 fractions and concurrent chemotherapy with cisplatin and pemetrexed. Brain magnetic resonance imaging (MRI) with contrast showed bilateral intracranial optic nerve enhancement. Conclusions: This is the second case report of RION in a patient with a history of brain radiotherapy and concurrent chemotherapy with pemetrexed. History of chiasmal compression, concurrent use of chemotherapeutic agents, and high fraction size (despite the safety of total radiation dose) were possible contributing risk factors to develop RION in our cases. Hence, adjusting the radiation dose according to the presence of these risk factors is recommended.

Background: assessing the risk of conversion to multiple sclerosis (MS) in patients with optic neuritis (ON) has been the topic of numerous studies. However, since the risk factors differ from population to population, the extension of conclusions is a matter of debate. This study focused on the Iranian patients with optic neuritis and assessed the probability of conversion to multiple sclerosis by using a machine-based learning decision tree. Methods: in this retrospective, observational study the medical records of patients with optic neuritis from 2008 to 2018 were reviewed. Baseline vision, the treatment modality, magnetic resonance imaging (MRI) findings, and patients’ demographics were gathered to evaluate the odds of each factor for conversion to MS. The decision tree was then obtained from these data based on their specificity and sensitivity to predict the probability of conversion to MS. Results: the overall conversion rate to MS was 42.2% (117/277). 63.1 percent of patients had abnormal MRIs at baseline. The presence of white matter plaque had the highest odds for the conversion followed by the positive history of optic neuritis attack and gender. The regression tree showed that the presence of plaque was the most important predicting factor that increased the probability of conversion from 16 to 51 percent. Conclusion: the decision tree could predict the probability of conversion to MS by considering multiple risk factors with acceptable precision.

A previously healthy 28-year-old female developed bilateral painless vision loss, more prominent in the right eye than in the left, following abdominoplasty and liposuction surgery. Laboratory studies showed severe peri- and post-operative anemia. Over a 5-month follow-up, visual function remained decreased but stable in the right eye and improved in the left eye. This is the second reported case of anterior ischemic optic neuropathy in one eye and posterior ischemic optic neuropathy in the other eye after liposuction. Objective: To describe an isolated unilateral sixth nerve palsy, as a rare neuro-ophthalmic presentation of nasopharyngeal carcinoma. Methods: We report a 54-year-old female, known case of nasopharyngeal carcinoma who was treated with chemo-radiotherapy, and presented with isolated right sixth nerve palsy. Magnetic resonance imaging (MRI) indicated extension of tumor to intracranial fossa with clival involvement. Results: The patient was referred to an otolaryngologist for further evaluation and necessary intervention due to invasion of the cancer to intracranial fossa with involvement of right abducens nerve. Conclusions: Although, isolated sixth nerve palsies in adults over the age of 50 are usually ischemic; but in enduring cases, neoplastic processes should be considered.

Purpose To measure the foveal avascular zone (FAZ) areas and vessel densities of patients with diabetic retinopathy and to study their relationship with diabetic cystoid changes and retinal thickness. Methods Prospective case series of 51 eyes of 31 patients with diabetic retinopathy. The eyes were grouped based on the presence or absence of cystoid edema and evaluated using optical coherence tomography angiography. The FAZ areas and vessel density were compared. Results The FAZ area at the superficial capillary plexus level was equal between the eyes with and without cystoid edema. Vessel density did not differ as well. There was no correlation with retinal thickness. In eyes with cystoid changes, FAZ area changes at the deep capillary plexus level were difficult to interpret. Conclusion The FAZ area and vessel density at the superficial capillary plexus level are reproducible and independent of the presence of cystoid edema.

BACKGROUND Methanol poisoning can cause an optic neuropathy that is usually severe and irreversible and often occurs after ingestion of illicit or homemade alcoholic beverages. In this study, we evaluated the potential neuroprotective effect of erythropoietin (EPO) on visual acuity (VA) in patients with methanol optic neuropathy. METHODS In a prospective, noncomparative interventional case series, consecutive patients with methanol optic neuropathy after alcoholic beverage ingestion were included. All patients initially received systemic therapy including metabolic stabilization and detoxification. Treatment with intravenous recombinant human EPO consisted of 20,000 units/day for 3 successive days. Depending on clinical response, some patients received a second course of EPO. VA, funduscopy, and spectral domain optical coherence tomography were assessed during the study. Main outcome measure was VA. RESULTS Thirty-two eyes of 16 patients with methanol optic neuropathy were included. Mean age was 34.2 years (±13.3 years). The mean time interval between methanol ingestion and treatment with intravenous EPO was 9.1 days (±5.56 days). Mean follow-up after treatment was 7.5 months (±5.88 months). Median VA in the better eye of each patient before treatment was light perception (range: 3.90-0.60 logMAR). Median last acuity after treatment in the best eye was 1.00 logMAR (range: 3.90-0.00 logMAR). VA significantly increased in the last follow-up examination (P < 0.0001). Age and time to EPO treatment after methanol ingestion were not significantly related to final VA. No ocular or systemic complications occurred in our patient cohort. CONCLUSIONS Intravenous EPO appears to improve VA in patients with methanol optic neuropathy and may represent a promising treatment for this disorder.

Intravenously administered erythropoietin (EPO) was firstly commenced (phase 1) in patients with indirect traumatic optic neuropathy (TON) by this group in 2011. It was re-tested by another group (phase 2) in 2014. This multicenter clinical trial was designed to compare its effect with intravenous steroid and observation. Included were TON patients ≥5 years of age and with trauma-treatment interval of ≤3 weeks. Follow-up visits were set at 1, 2, 3, 7, 14, 30, and at least 90 days after treatment. EPO and methylprednisolone were infused intravenously every day for three consecutive days. Primary outcome measure was change in the best corrected visual acuity (BCVA). Secondary outcomes included change in color vision and relative afferent pupillary defect (RAPD), side effects, and factors affecting the final visual improvement. Out of 120 patients, 100 (EPO: 69, steroid: 15, observation: 16) were finally included. All three groups showed a significant improvement of BCVA which was not significantly different between the groups (adjusted for pretreatment BCVA). Color vision was significantly improved in the EPO group. Late treatment (>3 days) (odds ratio = 2.53) and initial BCVA of NLP (odds ratio = 5.74) significantly worsened visual recovery. No side effect was observed in any group. EPO, steroid, and observation showed a significant improvement of BCVA in patients with TON. Initial BCVA of NLP and late treatment (>3 days) were significant risk factors for visual improvement.

Purpose: To compare two methods for treating inferior oblique overaction (IOOA): disinsertion versus myectomy of the muscle. Methods: In this prospective interventional case series, patients were randomly assigned to undergo either IO myectomy or disinsertion. The changes in vertical and horizontal deviations following these two surgical procedures were evaluated. The postoperative IO function of grade 0 or +1 and the fundus extorsion of grade 0 or +1 was considered as the successful outcome. Results: Thirty-six patients (50 eyes) with a mean age of 12.67 ± 4.05 years were included. In the myectomy group, the mean preoperative hyperdeviation in adduction was 29.5 ± 9.32 prism diopter (PD), which decreased to 9.15 ± 7.86 PD after surgery (P = 0.001). In the disinsertion group, these measurements were 32.73 ± 12.42 and 12.65 ± 9.34 PD before and after the surgery, respectively (P = 0.001). The success rate of surgery based on the IOOA grading was 87.4% and 92.3% in the myectomy and disinsertion groups, respectively (P = 0.780). The successful correction rate of abnormal fundus torsion was 91.6% in the myectomy and 88.4% in the disinsertion group (P = 0.821). In comparison, 48% of the cases in the myectomy group and 50% in the disinsertion group were within the normal range of torsional position postoperatively (P = 0.786). There was no statistically significant difference in terms of changes in the horizontal or vertical deviations, V-pattern, and dissociated vertical deviation between the two groups. Conclusion: Both surgical techniques seem to be effective for treatment of inferior oblique muscle overaction.

Purpose: To report a case of inadvertent inferior oblique extirpation during orbital decompression, its management, and postoperative result. Methods: A 38-year-old female with thyroid eye disease underwent cosmetic right orbital decompression during whichinferior oblique extirpation was noticed. Result: The muscle was repaired on the same session (illustrated in the article) with no postoperative diplopia at 3-month follow-up. Conclusion: Inferior oblique injury should be considered among the uncommon yet important complications of orbital decompression. It can be easily found and repaired in the same session as demonstrated in this case report.

Purpose: To report the frequency of lateral wound dehiscence (LWD) after upper blepharoplasty (UB), a technique and its outcome to prevent LWD. A 44-year-old man suffered sequential episodes of anterior ischaemic optic neuropathy in first the left and then right eye. He had suffered a previous episode of anterior ischaemic optic neuropathy in the right eye. Recent studies have shown neuroprotective properties of erythropoietin. Based on our previous studies on erythropoietin in anterior ischaemic optic neuropathy, we used intravenous erythropoietin in this patient. The patient received intravenous human recombinant erythropoietin. Visual functions significantly improved following treatment. Intravenous erythropoietin, as a neuroprotective agent, may herald a new modality of treatment in anterior ischaemic optic neuropathy.

To compare the intraocular pressure (IOP)-lowering effect of 0.005% latanoprost used once every other day versus once daily in patients with primary open-angle glaucoma (POAG) or ocular hypertension (OHT).Forty eyes of 20 patients with bilateral OHT or early POAG were enrolled in this prospective, randomized, crossover study. After a washout period of 1-4 weeks, each patient started latanoprost once daily in 1 randomly assigned eye and once every other day in the other eye. After 1 month, eyes were crossed over to the opposite dosing regimen for another month. IOP was measured at 9 am, 4 pm, and 9 pm, both at baseline and on 2 consecutive days, at the end of the first and second months. The main outcome measure was IOP reduction on the second day at the end of the first and second months.Mean±standard deviation (SD) baseline IOPs (mmHg) in the right and left eye were 25.6±2.6 and 25.5±2.7 (P=0.8), respectively. Mean±SD diurnal IOPs decreased (mmHg) by 8.6±2.5 in the once-daily group versus 7.3±2.3 in the every-other-day group (P0.001) at the end of the first month and 8.4±2.5 versus 7.7±2.5 at the end of the second month (both on day 2) (P=0.01).Once every-other-day administration of latanoprost significantly reduced the IOP although the IOP reduction was less pronounced 36-48 h after drug application.

Purpose: To compare treatments with wavefront-optimized and topography-guided ablations. Methods: This prospective, randomized, contralateral study comprised 40 eyes (20 patients) with low to moderate myopia with or without astigmatism that underwent topography-guided photorefractive keratectomy (PRK) (ALLEGRO Topolyzer, Alcon Laboratories Inc) in one eye and wavefront-optimized PRK (ALLEGRETTO WAVE software version 2.020 default treatment, Alcon Laboratories Inc) in the fellow eye. Visual acuity, refractive error, contrast sensitivity, and Orbscan (Bausch & Lomb) 3- and 5-mm corneal irregularities were measured preoperatively and 3 and 6 months postoperatively. The results were compared between the two eyes. Results: In both groups, preoperative corrected distance visual acuity (CDVA) (0.03±0.09 logMAR for topography-guided and 0.01±0.06 logMAR for wavefront-optimized [ P =.1]), 3-month postoperative uncorrected distance visual acuity (UDVA) (−0.01±0.03 logMAR for topography-guided and −0.01±0.03 logMAR for wavefront-optimized [ P =.4]), and 6-month postoperative UDVA (0.01±0.03 logMAR for topography-guided and 0.0±0.01 logMAR for wavefront-optimized [ P =.3]) were the same. All wavefront-optimized and 18 (90%) topography-guided eyes had UDVA of 20/20 or better. No significant differences were noted between groups in pre- and postoperative spherical and cylindrical refractive errors or corneal irregularity and contrast sensitivity measurements. Six months postoperatively, contrast sensitivity values at 3, 6, 12, and 18 cycles/degree were 5.7±0.7, 6.0±1.5, 6.1±1.3, and 5.5±1.5, respectively, in the topography-guided group, and 6.2±0.6, 6.4±1.2, 6.4±1.1, and 5.8±1.3, respectively, in the wavefront-optimized group ( P =.3, P =.5, P =.4, and P =.6, respectively). Conclusions: Using the WaveLight excimer laser platform for PRK, CDVA and contrast sensitivity outcomes were statistically similar between the wavefront-optimized and topography-guided ablations in eyes with low to moderate myopia with and without astigmatism.

To determine the visual outcome and safety of intravenous erythropoietin in patients with indirect traumatic optic neuropathy (TON).In a university-based hospital, from December 2008 to March 2010, seven cases with indirect TON (EPO group) received three intravenous (IV) injections of 10,000 IU of recombinant human erythropoietin in three successive days. They were compared to eight patients with indirect TON who had received no specific treatment from December 2006 to November 2008 (observation group). The primary end point was best-corrected visual acuity (BCVA) in both groups at last follow-up and secondary measure was systemic adverse effects of medication in the EPO group.Mean ages of patients in the observation and EPO groups were 23.3 (SD = 10.8) and 19.7 (SD = 12.1) years, respectively. Mean follow-up in the observation and EPO groups were 5.8 (SD = 3.8) and 7.0 (SD = 5.8) months, respectively. Mean best-corrected visual acuity (BCVA) significantly (p = 0.028) improved from 1.82 logMAR (SD = 1.27) at initial visit to 0.94 logMAR (SD = 0.82) at last follow-up in the EPO group, whereas the observation group showed an insignificant (p = 0.285) improvement from 2.55 log MAR (SD = 0.62) to 2.32 logMAR (SD = 0.83). Mean final BCVA was significantly higher in the EPO group than in the observation group (p = 0.012). Two cases developed mild transient systemic hypotension during EPO injection.Intravenous erythropoietin may be a new effective and safe treatment for patients with indirect TON.

Purpose To compare the effects and complications of botulinum toxin injection into the medial rectus muscle with and without electromyographic (EMG) assistance for treatment of abducens nerve palsy. Methods In a prospective, comparative, interventional case series, botulinum toxin was injected into the medial rectus muscle of 23 patients with esotropia secondary to abducens nerve palsy within 3 months of onset. Patients were randomly assigned to two groups. The injection was given with EMG assistance in the first (EMG group) and without EMG in the second group (no-EMG group). Success was defined as a distance esotropia of no more than 10 Δ in the primary position. Angle of deviation was recorded preoperatively and at 1 week, 3 months, and 6 months; any postoperative complications and side effects were noted. Results There were 11 patients in the EMG and 12 patients in the no-EMG group. The ocular deviations were significantly improved in both groups with an overall 6 month success rate of 52.2% ( p = 0.008 in the EMG group, p = 0.006 in the no-EMG group). Success rates and mean changes of the angle of deviation were not statistically different between the two groups. Postinjection blepharoptosis was found to be significantly more in the EMG group ( p = 0.009). Conclusions The effectiveness of botulinum toxin injection is the same with or without EMG assistance into the medial rectus muscle for treatment of abducens nerve palsy. EMG assistance is associated with more blepharoptosis.
