BackgroundUp to now several studies estimate the prevalence of HBV, HCV, and TB among people living with HIV (PLWH) in Iran; however, their results are inconsistent. This study aimed to estimate the overall prevalence of HBV, HVC, and TB among Iranian PLWH.MethodsIn this systematic review and meta-analysis six databases including Medline, Web of Science, Scopus, MagIran, Scientific Information Database (SID), and Barakat Knowledge network system were searched up to October 2023 with no language restriction. All studies estimated the prevalence of HBV, HCV, and TB among PLWH in Iran were included. The random-effects model was used to report the study estimates. Results were reported at a 95% confidence interval (CI).ResultsOut of 1050 retrieved references, 58 articles met the eligibility criteria. Overall among PLWH, HBV prevalence was 13.0% (95% CI: 11.0, 15.0), HCV prevalence was 54% (95% CI: 45.0, 64.0), and TB prevalence was 19% (95% CI: 13.0, 24.0). The results from multivariate meta-regression analysis showed no statistically significant association between HBV and TB prevalence with the year of study, quality of studies, age, gender, and persons who inject drugs (PWID). HCV prevalence was significantly associated with PWID.ConclusionWe found HBV, HCV, and TB infections are common among PLWH in Iran and required to be screened and treated with effective and timely services.

BackgroundIran is one of the first few countries that was hit hard with the coronavirus disease 2019 (COVID-19) pandemic. We aimed to estimate the total number of COVID-19 related infections, deaths, and hospitalizations in Iran under different physical distancing and isolation scenarios.MethodsWe developed a susceptible-exposed-infected/infectious-recovered/removed (SEIR) model, parameterized to the COVID-19 pandemic in Iran. We used the model to quantify the magnitude of the outbreak in Iran and assess the effectiveness of isolation and physical distancing under five different scenarios (A: 0% isolation, through E: 40% isolation of all infected cases). We used Monte-Carlo simulation to calculate the 95% uncertainty intervals (UIs).ResultsUnder scenario A, we estimated 5 196 000 (UI 1 753 000-10 220 000) infections to happen till mid-June with 966 000 (UI 467 800-1 702 000) hospitalizations and 111 000 (UI 53 400-200 000) deaths. Successful implantation of scenario E would reduce the number of infections by 90% (ie, 550 000) and change the epidemic peak from 66 000 on June 9, to 9400 on March 1, 2020. Scenario E also reduces the hospitalizations by 92% (ie, 74 500), and deaths by 93% (ie, 7800).ConclusionWith no approved vaccination or therapy available, we found physical distancing and isolation that include public awareness and case-finding and isolation of 40% of infected people could reduce the burden of COVID-19 in Iran by 90% by mid-June.

Plague, a lethal zoonotic disease, primarily circulates within rodent populations and their fleas. In Iran, the widely distributed jird, Meriones persicus, serves as the principal reservoir for plague, with a belief in the existence of five out of its six recognized subspecies within the country. However, these subspecies are classified into four mitochondrial cytochrome b sub‐lineages (IA, IB, IIA, IIB). This discrepancy, combined with the presence of an unnamed sub‐lineage in central Iran awaiting taxonomic clarification, has left intraspecific taxonomy unsettled and obscured the true alignment between mtDNA sub‐lineages and nominal subspecies. In this study, we investigated the intraspecific variation in the cytb gene across populations sampled throughout Iran, focusing on underexplored regions between the Zagros and Alborz Mountains and central Iran. While our genetic data generally support reported subspecies validity in Iran, we raise questions about M. p. baptistae, emphasizing the need for further data from its type territory in Pakistan. Two main lineages of M. persicus (I and II) exhibit geographical isolation, with limited overlap in the central Zagros Mts., where three subspecies (M. p. ambrosius, M. p. rossicus, and M. p. persicus) coexist. Superimposing infected rodents' geographic coordinates onto updated sub‐lineages' distribution revealed a potential association between sub‐lineage IA (M. p. rossicus) and all enzootic plague cases from 1946 to 2023. M. persicus rossicus extends into the Caucasus (where plague infections are common), Eastern Turkey, and Iraq. Backgroundworldwide, hepatitis C and B virus infections (HCV and HCV), are the two most common coinfections with human immunodeficiency virus (HIV) and has become a major threat to the survival of HIV-infected persons. The review aimed to estimate the prevalence of HIV, HBV, HCV, HIV/HCV and HIV/HBV and triple coinfections in different subpopulations in Iran.MethodFollowing PRISMA guidelines, we conducted a systematic review and meta-analysis of reports on prevalence of HIV, HBV, HCV and HIV coinfections in different subpopulations in Iran. We systematically reviewed the literature to identify eligible studies from January 1996 to March 2012 in English or Persian/Farsi databases. We extracted the prevalence of HIV antibodies (diagnosed by Elisa confirmed with Western Blot test), HCV antibodies and HBsAg (with confirmatory laboratory test) as the main primary outcome. We reported the prevalence of the three infections and coinfections as point and 95% confidence intervals.FindingsHIV prevalence varied from %0.00 (95% CI: 0.00-0.003) in the general population to %17.25 (95% CI: 2.94-31.57) in people who inject drugs (PWID). HBV prevalence ranged from % 0.00 (95% CI: 0.00-7.87) in health care workers to % 30.9 (95% CI: 27.88-33.92) in PWID. HCV prevalence ranged from %0.19 (95% CI: 0.00-0.66) in health care workers to %51.46 (95% CI: 34.30-68.62) in PWID. The coinfection of HIV/HBV and also HIV/HCV in the general population and in health care workers was zero, while the most common coinfections were HIV/HCV (10.95%), HIV/HBV (1.88%) and triple infections (1.25%) in PWID.ConclusionsWe found that PWID are severely and disproportionately affected by HIV and the other two infections, HCV and HBV. Screenings of such coinfections need to be reinforced to prevent new infections and also reduce further transmission in their community and to others.

Due to its faster evolution rate compared to nuclear genes, haploid mitochondrial DNA (mtDNA) is a promising species identification tool. This has led to its significant use in taxonomic and phylogenetic studies. While mtDNA is subject to selective constraints that prevent the accumulation of deleterious mutations, the prevalence of nuclear-mitochondrial fragments (known as NUMTs or pseudogenes) in mammals has complicated the use of mtDNA for taxonomy. In the present study, a pseudogene of the mitochondrial cytochrome b (Cytb) was detected in Meriones persicus. This pseudogene differed from its mitochondrial counterpart at 235 out of 1140 sites and is characterized by frame-shift mutations, indels, and accumulation of non-synonymous substitutions. It is the first report of the Cytb pseudogene in a jird, highlighting the risk of misidentifying NUMTs as authentic mtDNA and the importance of addressing this potential pitfall in taxonomic studies.
