The Antarctic Circumpolar Current (ACC) is the world’s strongest zonal current system that connects all three major ocean basins of the global ocean and therefore integrates and responds to global climate variability. Its flow is largely driven by strong westerly winds and constricted to its narrowest extent in the Drake Passage. Transport of fresh and cold surface and intermediate water masses through the Drake Passage (cold-water route) strongly affects the Atlantic Meridional Overturning Circulation together with the inflow of Indian Ocean water masses (warm-water route). Both oceanographic corridors are critical for the South Atlantic contribution to Meridional Overturning Circulation changes. In contrast to the Atlantic and Indian sectors of the ACC, and with the exception of drill cores from the Antarctic continental margin and off New Zealand, the Pacific sector of the ACC lacks information on its Cenozoic paleoceanography from deep-sea drilling records. To advance our knowledge and understanding of Miocene to Holocene atmosphere-ocean-cryosphere dynamics in the Pacific and their implications for regional and global climate and atmospheric CO2, International Ocean Discovery Program (IODP) Expedition 383 recovered sedimentary sequences at (1) three sites located in the central South Pacific (U1539, U1540, and U1541), (2) two sites at the Chile margin (U1542 and U1544), and (3) one site from the pelagic eastern South Pacific (U1543) close to the entrance to the Drake Passage. Because of persistently stormy conditions and the resulting bad weather avoidance, we were not successful in recovering the originally planned Proposed Site CSP-3A in the central South Pacific in the Polar Frontal Zone. The drilled sediments at Sites U1541 and U1543 reach back to the late Miocene, and those at Site U1540 reach back to the early Pliocene. High sedimentary rate Pleistocene sedimentary sequences were drilled both in the central South Pacific (Site U1539) and along the Chile margin. Taken together, the sites represent a depth transect from ~1100 m at the Chile margin site (U1542) to ~4070 m in the central South Pacific (Site U1539) and allow investigation of changes in the vertical structure of the ACC, a key issue for understanding the role of the Southern Ocean in the global carbon cycle. The sites are located at latitudes and water depths where sediments will allow the application of a wide range of siliciclastic-, carbonate-, and opal-based proxies to address our objectives of reconstructing with unprecedented stratigraphic detail surface to deep-ocean variations and their relation to atmosphere and cryosphere changes through stadial to interstadial, glacial to interglacial, and warmer than present time intervals.

Background: The National Institute of Neurological Disorders and Stroke (NINDS) Common Data Elements (CDEs) have been generated to standardize and define terms used by the scientific community. The widespread use of these CDEs promotes harmonized data collection in clinical research. The aim of the NINDS Unruptured Intracranial Aneurysms (UIA) and Subarachnoid Hemorrhage (SAH), and Subject Characteristics working group (WG) was to identify, define, and classify CDEs describing the characteristics of patients diagnosed with an UIA and SAH. Thus, “Participant/Subject characteristics” is a set of factors defining a population of selected individuals and allowing comparisons with a reference population and overtime. Methods: Based on standard terms defined by the United States’ Census Bureau, CDEs previously defined by several (Stroke, Epilepsy and Traumatic Brain Injury) NINDS CDE working groups literature and expert opinion of the WG, the “Participant/Subject characteristics” domain has been defined. Results: A set of 192 CDEs divided in 7 subsections: demographics (8 CDEs), social status (8 CDEs), behavioral status (22 CDEs), family and medical history (144 CDEs), pregnancy and perinatal history (8 CDEs), history data source reliability (3 CDEs), and prior functional status (3 CDEs) was defined. SAH is characterized by 6 core elements, all classified in the “Participant/Subject characteristics” domain. Four exploratory elements out of the 39 for SAH overall are in the “Participant/Subject characteristics” domain, and all remaining 182 CDEs in the “Participant/Subject characteristics” domain are classified as Supplemental-Highly Recommended elements. Conclusions: These CDEs would allow the development of best practice guidelines to standardize the assessment and reporting of observations concerning UIA and SAH.

We have performed incubation experiments in order to examine the behaviour of soil-derived branched glycerol dialkyl glycerol tetraether (brGDGT) membrane lipids upon entering an aquatic environment and to evaluate the processes that potentially take place during their fluvial transport from land to sea. We incubated a soil from the Rakaia River catchment on the South Island of New Zealand using Rakaia River water and ocean water collected near the river mouth as inocula for a period of up to 152 days. The concentrations, as well as the relative distribution of brGDGTs derived from intact polar ("living"; IPL) lipids and core ("fossil"; CL) lipids remained unaltered over the course of the experiment. Although the stability of the brGDGTs may be a consequence of the higher than natural soil : water ratio used in the laboratory experiment, the substantial increase (27–72%) in the total pool of isoprenoid GDGTs (isoGDGTs) in all incubation setups, including the control using distilled water, indicates that entering an aquatic environment does influence the behaviour of soil-derived GDGTs. However, the availability of water appears to be more important than its properties. As a consequence of increasing isoGDGT concentrations, a decrease in Branched and Isoprenoid Tetraether (BIT) index values – a proxy for the relative input of fluvially discharged soil material into a marine system – became evident after an incubation period of 30 days, with a maximum final decrease of 0.88 to 0.74 in the experiment with river water. The relative distribution within the isoGDGT pool shows changes with time, suggesting that isoGDGT producers may either have different rates of membrane adaptation or production/degradation, or that preferential release from the soil matrix or a shift in source organism(s) may take place. While the apparent stability of soil brGDGTs during this incubation experiment reinforces their potential as tracers for land–sea transport of soil organic carbon and their use in paleoclimate reconstructions, the distributional differences between GDGTs in river water and nearby soil, as well as in river and ocean water, indicate that further research is needed to pinpoint the sources of GDGTs that are ultimately discharged to the oceans and are subsequently archived in continental margin sediments., Biogeosciences, 12 (4), ISSN:1726-4170

Objective: We sought to determine the relationship of greater adherence to Mediterranean diet (MeD) and likelihood of incident cognitive impairment (ICI) and evaluate the interaction of race and vascular risk factors. Methods: A prospective, population-based, cohort of individuals enrolled in the Reasons for Geographic and Racial Differences in Stroke (REGARDS) Study 2003-2007, excluding participants with history of stroke, impaired cognitive status at baseline, and missing data on Food Frequency Questionnaires (FFQ), was evaluated. Adherence to a MeD (scored as 0-9) was computed from FFQ. Cognitive status was evaluated at baseline and annually during a mean follow-up period of 4.0 ± 1.5 years using Six-item-Screener. Results: ICI was identified in 1,248 (7%) out of 17,478 individuals fulfilling the inclusion criteria. Higher adherence to MeD was associated with lower likelihood of ICI before (odds ratio [lsqb]OR [rsqb] 0.89; 95%confidence interval [lsqb]CI[rsqb] 0.79-1.00) and after adjustment for potential confounders (OR 0.87; 95% CI 0.76-1.00) including demographic characteristics, environmental factors, vascular risk factors, depressive symptoms, and self-reported health status. There was no interaction between race (p = 0.2928) and association of adherence to MeD with cognitive status. However, we identified a strong interaction of diabetes mellitus (p = 0.0134) on the relationship of adherence to MeD with ICI; high adherence to MeD was associated with a lower likelihood of ICI in nondiabetic participants (OR 0.81; 95%CI 0.70-0.94; p = 0.0066) but not in diabetic individuals (OR 1.27; 95% CI 0.95-1.71; p = 0.1063). Conclusions: Higher adherence to MeD was associated with a lower likelihood of ICI independent of potential confounders. This association was moderated by presence of diabetes mellitus. © 2013 American Academy of Neurology.

Background Laboratory studies suggest that nitrous oxide augments brain injury after ischemia or hypoxia. The authors examined the relation between nitrous oxide use and outcomes using data from the Intraoperative Hypothermia for Aneurysm Surgery Trial. Methods The Intraoperative Hypothermia for Aneurysm Surgery Trial was a prospective randomized study of the impact of intraoperative hypothermia (temperature = 33 degrees C) versus normothermia (temperature = 36.5 degrees C) in patients with aneurysmal subarachnoid hemorrhage undergoing surgical clipping. Anesthesia was dictated by a limited-options protocol with the use of nitrous oxide determined by individual anesthesiologists. All patients were assessed daily for 14 days after surgery or until hospital discharge. Neurologic and neuropsychological testing were conducted at 3 months after surgery. Outcome data were analyzed via both univariate tests and multivariate logistic regression analysis correcting for factors thought to influence outcome. An odds ratio (OR) greater than 1.0 denotes a worse outcome in patients receiving nitrous oxide. Results Outcome data were available for 1,000 patients, of which 373 received nitrous oxide. There was no difference between groups in the development of delayed ischemic neurologic deficit. At 3 months after surgery, there were no significant differences between groups in any outcome variable: Glasgow Outcome Score (OR, 0.84; 95% confidence interval [CI], 0.63-1.14; P = 0.268), National Institutes of Health Stroke Scale (OR, 1.29; 95% CI, 0.96-1.73; P = 0.087), Rankin Disability Score (OR, 0.84; 95% CI, 0.61-1.15; P = 0.284), Barthel Activities of Daily Living Index (OR, 1.01; 95% CI, 0.68-1.51; P = 0.961), or neuropsychological testing (OR, 1.26; 95% CI, 0.85-1.87; P = 0.252). Conclusions In a population of patients at risk for ischemic brain injury, nitrous oxide use had no overall beneficial or detrimental impact on neurologic or neuropsychological outcomes.

Located at 54ºS in the heart of the Island of Tierra del Fuego, Lago Fagnano occupies the deepest of a chain of en-echelon tectonic depressions along the Magallanes-Fagnano Transform system (MFT). A recent geophysical campaign combining 3.5 kHz (pinger) single-channel with 1 in3 airgun multi-channel systems surveyed more than 100 m of glacio-lacustrine sediments filling two main sub-basins. These data provide a unique opportunity to visualize the most recent lacustrine sequence with high-resolution while simultaneously imaging the oldest infill. A preliminary seismic stratigraphic analysis of the high-resolution 3.5 kHz pinger data allowed the identification of three major seismostratigraphic units (A, the oldest and C, the youngest). While unit A is interpreted as glacially derived sediments, the overlying unit B is interpreted as fining upward sequences of proglacial turbidites reflecting sediment pulses released by the retreating Fagnano glacier during the last deglaciation. A major environmental change occurred during deposition of unit C when pelagic style of sedimentation is intercalated by sequences of downslope mass flow events probably triggered by relatively strong tectonic pulses along the MFT system. Gravity cores show a regular alternation of light and dark laminae occasionally interrupted by homogenous sedimentary units interpreted as turbidites. Ultra-high resolution X-ray fluorescence micro-profiles show fluctuations in major trace elements at mm scale that may indicate seasonal variations in the sedimentary influx. These core data provide a unique record of decadal changes in regional climate that can be compared with other marine and continental archives to improve our understanding of the forcing mechanisms behind climate change.

BACKGROUND: Surgery for intracranial aneurysm often results in postoperative neurologic deficits. We conducted a randomized trial at 30 centers to determine whether intraoperative cooling during open craniotomy would improve the outcome among patients with acute aneurysmal subarachnoid hemorrhage.METHODS: A total of 1001 patients with a preoperative World Federation of Neurological Surgeons score of I, II, or III ("good-grade patients"), who had had a subarachnoid hemorrhage no more than 14 days before planned surgical aneurysm clipping, were randomly assigned to intraoperative hypothermia (target temperature, 33 degrees C, with the use of surface cooling techniques) or normothermia (target temperature, 36.5 degrees C). Patients were followed closely postoperatively and examined approximately 90 days after surgery, at which time a Glasgow Outcome Score was assigned.RESULTS: There were no significant differences between the group assigned to intraoperative hypothermia and the group assigned to normothermia in the duration of stay in the intensive care unit, the total length of hospitalization, the rates of death at follow-up (6 percent in both groups), or the destination at discharge (home or another hospital, among surviving patients). At the final follow-up, 329 of 499 patients in the hypothermia group had a Glasgow Outcome Score of 1 (good outcome), as compared with 314 of 501 patients in the normothermia group (66 percent vs. 63 percent; odds ratio, 1.14; 95 percent confidence interval, 0.88 to 1.48; P=0.32). Postoperative bacteremia was more common in the hypothermia group than in the normothermia group (5 percent vs. 3 percent, P=0.05).CONCLUSIONS: Intraoperative hypothermia did not improve the neurologic outcome after craniotomy among good-grade patients with aneurysmal subarachnoid hemorrhage.
