On September 5, 2022, the Luding M6.8 earthquake occurred in the Moxi-Shimian segment of the Xianshuihe fault, coinciding with the historical ruptured zone of the 1786 Moxi earthquake. Its seismogenic environment provides a foundation for comprehending the mechanism of the earthquake and its future hazard. In the Moxi-Shimian segment, we establish a series of near-field Global Navigation Satellite Systems (GNSS) stations to enhance the spatial resolution of observational data for the inversion of the interseismic kinematic parameters. In this study, with an elastic screw dislocation model constrained by GNSS observations, the slip rate of the Moxi-Shimian segment is estimated to be 10.9 ± 1.0 mm/yr, while the locking depth is 15.7 ± 6.2 km. Additionally, we utilize a block-dislocation model to invert the interseismic fault coupling along the Kangding-Moxi-Shimian segment. The result indicates a gradual deepening of the locking depth along the section from Kangding to Shimian. The coseismic rupture of the 2022 event occurred within the high coupling regions in the Kangding-Moxi-Shimian segment, which indicates that the rupture kinematics in this event might be controlled by the interseismic deformation. The seismic moment accumulated within the ruptured zone of the Luding earthquake since 1786 ranges in [1.42–3.40] × 1019 N·m, which is significantly greater than the seismic moment released during the 2022 event. As a result, we infer that the Luding earthquake released only a portion of the accumulated energy within the original rupture zone since 1786, indicating that the 2022 event has not caused a complete rupture in the Moxi-Shimian segment. Consequently, there remains a substantial seismic hazard in this area.

BackgroundBronchopulmonary Dysplasia (BPD) is a chronic lung disease affecting preterm infants, with limited prevention and treatment options. Inhaled Nitric Oxide (iNO) is sometimes used to treat Persistent Pulmonary Hypertension of the Newborn (PPHN) and Hypoxemic Respiratory Failure (HRF), and its impact on BPD development remains debated.ObjectiveTo assess whether iNO-related factors are potential contributors to the development of BPD Grade Ⅱ-Ⅲ in very premature infants (VPI) diagnosed with PPHN or HRF at birth using Propensity Score Matching (PSM).MethodsWe conducted a retrospective cohort study of infants born at 22–32 weeks gestation with PPHN or HRF, treated with iNO for over 3 h. PSM matched groups by gestational age, birth weight, and gender, etc. Multivariate logistic regression evaluated the association between iNO treatment and BPD outcomes to identify influencing factors, while Restricted Cubic Spline (RCS) and mediation analysis examined iNO dose effects and potential mediators like mechanical ventilation time and oxygenation index (OI).ResultsA higher initial iNO dose was significantly associated with a reduced risk of BPD Grade Ⅱ-Ⅲ (adjusted OR = 0.68, 95% CI: 0.52–0.89, p < 0.01). Additionally, administration of iNO within the first 7 days of life was identified as an important influencing factor No significant mediation effects were observed for factors such as mechanical ventilation time and OI.ConclusionA higher initial iNO dose within the first 7 days was associated with a reduced risk of BPD Grade Ⅱ-Ⅲ in VPI with PPHN or HRF.

Abstract Background Benzodiazepines are a class of medications that are being frequently prescribed in Canada but carry significant risk of harm. There has been increasing clinical interest on the potential “sparing effects” of medical cannabis as one strategy to reduce benzodiazepine use. The objective of this study as to examine the association of medical cannabis authorization with benzodiazepine usage between 2013 and 2021 in Alberta, Canada. Methods A propensity score matched cohort study with patients on regular benzodiazepine treatment authorized to use medical cannabis compared to controls who do not have authorization for medical cannabis. A total of 9690 medically authorized cannabis patients were matched to controls. To assess the effect of medical cannabis use on daily average diazepam equivalence (DDE), interrupted time series (ITS) analysis was used to assess the change in the trend of DDE in the 12 months before and 12 months after the authorization of medical cannabis. Results Over the follow-up period after medical cannabis authorization, there was no overall change in the DDE use in authorized medical cannabis patients compared to matched controls (− 0.08 DDE, 95% CI: − 0.41 to 0.24). Likewise, the sensitivity analysis showed that, among patients consuming ≤5 mg baseline DDE, there was no change immediately after medical cannabis authorization compared to controls (level change, − 0.04 DDE, 95% CI: − 0.12 to 0.03) per patient as well as in the month-to-month trend change (0.002 DDE, 95% CI: − 0.009 to 0.12) per patient was noted. Conclusions This short-term study found that medical cannabis authorization had minimal effects on benzodiazepine use. Our findings may contribute ongoing evidence for clinicians regarding the potential impact of medical cannabis to reduce benzodiazepine use. Highlights • Medical cannabis authorization had little to no effect on benzodiazepine usage among patients prescribed regular benzodiazepine treatment in Alberta, Canada. • Further clinical research is needed to investigate the potential impact of medical cannabis as an alternative to benzodiazepine medication.

Abstract Small-field-of-view reconstruction CT images (sFOV-CT) increase the pixel density across airway structures and reduce partial volume effects. Multi-instance learning (MIL) is proposed as a weakly supervised machine learning method, which can automatically assess the image quality. The aim of this study was to evaluate the disparities between conventional CT (c-CT) and sFOV-CT images using a lung nodule system based on MIL and assessments from radiologists. 112 patients who underwent chest CT were retrospectively enrolled in this study between July 2021 to March 2022. After undergoing c-CT examinations, sFOV-CT images with small-field-of-view were reconstructed. Two radiologists analyzed all c-CT and sFOV-CT images, including features such as location, nodule type, size, CT values, and shape signs. Then, an MIL-based lung nodule system objectively analyzed the c-CT (c-MIL) and sFOV-CT (sFOV-MIL) to explore their differences. The signal-to-noise ratio of lungs (SNR-lung) and contrast-to-noise ratio of nodules (CNR-nodule) were calculated to evaluate the quality of CT images from another perspective. The etiology of hemorrhagic fever with renal syndrome (HFRS) is significantly impacted by a variety of immune cells. Nevertheless, the existing techniques for sequencing peripheral blood T cell receptor (TCR) or B cell receptor (BCR) libraries in HFRS are constrained by both limitations and high costs. In this investigation, we utilized the computational tool TRUST4 to generate TCR and BCR libraries utilizing comprehensive RNA-seq data from peripheral blood specimens of HFRS patients. This facilitated the examination of clonality and diversity within immune libraries linked to the condition. Despite previous research on immune cell function, the underlying mechanisms remain intricate, and differential gene expression across immune cell types and cell-to-cell interactions within immune cell clusters have not been thoroughly explored. To address this gap, we performed clustering analysis on 11 cell subsets derived from raw single-cell RNA-seq data, elucidating characteristic changes in cell subset proportions under disease conditions. Additionally, we utilized CellChat, a tool for cell–cell communication analysis, to investigate the impact of MIF family, CD70 family, and GALECTIN family cytokines—known to be involved in cell communication—on immune cell subsets. Furthermore, hdWGCNA analysis identified core genes implicated in HFRS pathogenesis within T cells and B cells. Trajectory analysis revealed that most cell subsets were in a developmental stage, with high expression of transcription factors such as NFKB and JUN in Effector CD8+ T cells, as well as in Naive CD4+ T cells and Naive B cells. Our findings provide a comprehensive understanding of the dynamic changes in immune cells during HFRS pathogenesis, identifying specific V genes and J genes in TCR and BCR that contribute to advancing our knowledge of HFRS. These insights offer potential implications for the diagnosis and treatment of this autoimmune disease.

Background:The pathogenesis of juvenile idiopathic arthritis (JIA) is strongly influenced by an impaired immune system. However, the molecular mechanisms underlying its development and progression have not been elucidated. In this study, the computational methods TRUST4 were used to construct a T-cell receptor (TCR) and B-cell receptor (BCR) repertoire from the peripheral blood of JIA patients via bulk RNA-seq data, after which the clonality and diversity of the immune repertoire were analyzed.Results:Our findings revealed significant differences in the frequency of clonotypes between the JIA and healthy control groups in terms of the TCR and BCR repertoires. This work identified specific V genes and J genes in TCRs and BCRs that could be used to expand our understanding of JIA. After single-cell RNA analysis, the relative percentages of CD14 monocytes were significantly greater in the JIA group. Cell-cell communication analysis revealed the significant role of the MIF signaling pathway in JIA.Conclusion:In conclusion, this work describes the immune features of both the TCR and BCR repertoires under JIA conditions and provides novel insight into immunotherapy for JIA.

Background: For patients with atrial fibrillation who have an ischemic stroke or transient ischemic attack (TIA) despite taking direct oral anticoagulants (DOACs), the optimal strategy for ongoing anticoagulation is unknown. Methods: Using provincial administrative databases in Alberta, Canada, we compared anticoagulant use before/after the breakthrough stroke/TIA and assessed recurrence of stroke/TIA or bleeding, with consideration of medication adherence. Adherence was defined as the proportion of days covered (PDC) being ≥ 80%. Results: Among 985 patients, the median age was 80 years (interquartile range 13), with a mean CHADS2 score of 1.7± 1 prior to the index event. Patients were followed for a median of 643 days (interquartile range 836). Following the index stroke/TIA event, 623 patients (63%) filled a prescription for the same DOAC regimen, 83 (8%) filled a prescription for a different dose, 155 (16%) switched DOAC agents, 51 (5%) switched to warfarin, and 73 (7%) filled no oral anticoagulant prescription. Patients who kept the same regimen more commonly had TIA index events (59%); patients who changed dose or drug more often had stroke index events (55%-78%). During follow-up, 135 (14%) had stroke/TIA recurrence, and 46 (5%) had bleeding; rates of each did not differ between prescribing patterns. Post-index event, the proportion of patients with a proportion of days covered ≥ 80% improved from 55% to 80%. Conclusions: Although most maintained the same DOAC regimen after stroke/TIA, rates of recurrent stroke/TIA and bleeding were similar across prescribing patterns. Stroke/TIA severity may have influenced prescribing practices. DOAC prescription adherence improved poststroke/TIA and signals an opportunity for optimization in patients with atrial fibrillation. Résumé: Contexte: Chez les patients atteints de fibrillation auriculaire qui subissent un accident vasculaire cérébral (AVC) ischémique ou un accident ischémique transitoire (AIT) malgré la prise d’anticoagulants oraux directe (AOD), la stratégie optimale pour la poursuite de l’anticoagulation est inconnue. Méthodologie: À partir des bases de données administratives provinciales en Alberta, au Canada, nous avons comparé l’utilisation d’anticoagulants avant/après l’AVC/AIT survenu pendant l’anticoagulothérapie et avons évalué la récurrence d’un AVC/AIT ou d’un saignement, en tenant compte de l’adhésion au traitement médicamenteux. L’adhésion a été définie comme une proportion de jours couverts (PJC) de 80 % ou plus. Résultats: Chez 985 patients, l’âge médian était de 80 ans (écart interquartile de 13) et le score CHADS2 moyen, de 1,7 ± 1 avant l’événement de référence. Les patients ont été suivis pendant une médiane de 643 jours (écart interquartile de 836). Après l’AVC/AIT de référence, 623 patients (63 %) ont fait exécuter une ordonnance du même schéma d’AOD, 83 (8 %) ont fait exécuter une ordonnance d’une dose différente, 155 (16 %) sont passés à d’autres AOD, 51 (5 %) sont passés à la warfarine et 73 (7 %) n’ont fait exécuter aucune ordonnance d’anticoagulant oral. Chez les patients qui ont continué à recevoir le même schéma, la plupart (59 %) avaient eu un AIT comme événement de référence; chez les patients qui ont changé de dose ou de médicament, la plupart (55 à 78 %) avaient eu un AVC comme événement de référence. Durant le suivi, 135 (14 %) ont connu un AVC/AIT récurrent et 46 (5 %) ont présenté un saignement; les taux de chaque manifestation ont été similaires pour les différents schémas de prescription. Après l’événement de référence, le pourcentage de patients ayant une PJC ≥ 80 % a augmenté, passant de 55 à 80 %. Conclusions: Malgré le maintien du même schéma d’AOD chez la plupart des patients après l’AVC/AIT, les taux d’AVC/AIT récurrent et de saignement ont été similaires avec tous les schémas de prescription. La gravité d’un AVC/AIT pourrait avoir influencé les pratiques de prescription. L’adhésion aux AOD prescrits s’est améliorée après un AVC/AIT et témoigne d’une possibilité d’optimisation chez les patients atteints de fibrillation auriculaire.
