Abstract Background Over the past fifty years, the incidence of atrial fibrillation (AF) has tripled. Traditionally, the main treatment for this condition has been pulmonary vein isolation (PVI) performed using radiofrequency catheter ablation (RFCA). However, another technique known as cryoballoon ablation (CBA) has been developed as another option for managing this heart rhythm disorder. This study evaluated the efficacy and safety of CBA and RFCA for the treatment of AF. Methods This study compared the safety and effectiveness of CBA and RFCA for the treatment of AF using a thorough review of randomized controlled trials up until June 1, 2023. Results The results revealed that CBA and RFCA had similar effectiveness and safety profiles in achieving freedom from AF (RR: 1.00; 95% CI: 0.93 to 1.07, p = 0.99) and paroxysmal atrial fibrillation (PAF) (RR: 0.99; 95% CI: 0.89 to 1.10, p = 0.79). Developments in low-cost techniques for growing high-quality nickel oxide thin films (NiOTFs) are critical enablers for the fabrication of NiO-based devices, particularly solar cells, light-emitting diodes, lasers, and many other applications. In this study, it has been demonstrated that the optical, electrical, and structural characteristics of NiOTFs coated on glass substrate employing a low-cost solution method are strongly influenced by post-thermal annealing treatments in ambient air. A comprehensive study was carried out on the characteristics of the deposited NiOTFs. X-ray diffraction (XRD) measurements indicate the enhancement of the crystal quality of films after annealing. There is a noticeable change and improvement in coated NiOTFs with heat treatment at 300 and 600 ℃ due to clear shifts in density. During the heating temperature increment, the energy of the band gap was shifted from 3.63 to 3.44 eV, and the electrical resistance of the NiOTFs varied from 1020 to 700 Ω-cm. Furthermore, the results indicate that the low-cost sol-gel (SG) deposition technique has a greater potential for producing high-quality p-type NiOTFs with minimal defects for electronic device applications.

Poliovirus is a deadly disease that needs to be eradicated in the world. Recently, issues with circulating vaccine-derived polioviruses (cVDPVs) in the Democratic Republic of the Congo (DRC) have been raised in the country. This article aims to determine the increase in Type 1 cVDPV1 and Type 2 cVDPV2 in the DRC. Relevant articles on PubMed, Google Scholar, ResearchGate, and Web of Science were searched from 2010 to 2023. Our findings indicate that the Democratic Republic of Congo has struggled with polio outbreaks, with the virus primarily linked to cVDPVs produced from vaccines rather than wild poliovirus. These cVDPVs have the potential to revert to their paralyzing capabilities by evolving from the weakened virus seen in the oral polio vaccination (OPV). Several regions in DRC have reported cVDPV outbreaks of cVDPVs. Numerous cVDPV2 outbreaks were documented in various provinces during the 2017–2018 period. Addressing the cVDPV outbreak in the DRC requires a concerted global effort, involving collaboration among governments, international health organizations, and donor agencies. There should be global support and collaboration among governments, international health organizations, and donor agencies to address the cVDPV outbreak in the DRC.

Malaria is a significant public health challenge in the Democratic Republic of the Congo (DR Congo), with a high prevalence and mortality rate, particularly among children under five years old. This study examines the impact of malaria in eastern DR Congo, where armed conflict and humanitarian crises have resulted in overcrowded refugee and internally displaced persons (IDP) camps, exacerbating malaria transmission. Malnutrition, limited access to healthcare, and poor living conditions makes children under the age of five particularly vulnerable. Despite attempts by organizations such as the World Health Organization and various non-governmental organizations to supply insecticide-treated bed nets and antimalarial drugs, implementation in refugee camps is impeded by war, resource constraints, and insufficient healthcare facilities. A focused elimination approach that includes integrated vector control, enhanced diagnostic access, healthcare professional training, and community engagement is critical. Surveillance and research are critical for determining malaria prevalence and resistance patterns. Effective malaria treatment in refugee camps necessitates broad strategies to protect vulnerable children and lower disease burdens.

This study presents designing and evaluating a reliable wireless power transfer (WPT) mechanism to charge electric vehicle (EV) batteries using resonant coupling. The EV wireless system was created applying the principles of mutual inductance whereby the receiving and transmitting coils were interlinked and connected to their circuits. Significant mathematical work was also done to obtain the equations for the impedance, the power input, and output power. Proteus software was employed to draw a Printed Circuit Board (PCB) for the reception of the receiver and the transmitter circuits. The simulations were conducted using ANSYS Maxwell and MATLAB softwares, utilizing the key parameters of the transmitter and the receiver coils. The experimental setup included an EV installed with transmitter and receiver circuits, copper coils, a compensation network of capacitors, and a 12 V battery voltage monitor. The study showed that the efficiency of wireless power transfer was 78% with the enhanced power density for the gap ranging between 6 and 20 cm. The innovations presented in this paper are a new lower operating frequency of 70 kHz, coil optimization, and a dynamic study on coil misalignment all of which enhance the efficiency and reduce cost of the proposed system.

Abstract Objective To determine the relationship of Neutrophil Lymphocyte Ratio (NLR), Monocyte Lymphocyte Ratio (MLR), and Neutrophil Monocyte Ratio (NMR) with treatment response in Pulmonary Tuberculosis (PTB) patients during intensive phase treatment (IPT). Methods This analytical cross-sectional study was conducted at Ojha Institute of Chest Diseases (OICD), Dow University of Health Sciences, from February to December 2021. 100 patients were enrolled using purposive sampling technique. Both male and female of age 18 and above, rifampicin sensitive newly diagnosed cases of PTB by Acid Fast Bacilli (AFB) microscopy and Gene Xpert MTB/RIF were included. SPSS version 26 was used to analyze data. Numerical data was expressed in median and interquartile range and categorical data was expressed in frequencies and percentages. Results Out of total 100 patients, 81% (n = 81) showed treatment response with negative AFB Sputum Smear Microscopy (SSM) after 2nd month. Out of 81% (n = 81) of the patients who achieved treatment response, 83.9% (n = 68) also had decreased NLR, 85.2% (n = 69) had decreased MLR and 83.9% (n = 68) had decreased NMR from baseline. However 19% (n = 19) did not achieved treatment response with positive AFB SSM after 2nd month of ATT (Anti tuberculosis treatment), among them 10.52% (n = 2) were INH resistant with no decrease in all the ratios after 2nd month. Conclusion Leukocyte ratios decreased significantly from baseline as PTB was treated in patients who achieved treatment response with negative AFB SSM after two months of ATT and hence these ratios could be used as markers to monitor the treatment response.

Objective: We aimed to conduct an audit to investigate whether the steps of phlebotomy performed by nurses in our emergency department were in line with the WHO guidelines on drawing blood. Based on our investigation, we could then make interventions to ensure that the guidelines were followed diligently. This would be followed by a repeat audit to determine if there was an improvement in adherence to the guidelines. Methods: This clinical audit was conducted in the Emergency Department of the Holy Family Hospital, Rawalpindi. Nurses working in the department were included in the audit. Medical and nursing students, along with doctors were excluded. The initial audit was conducted from 1st to 7th July 2021. A total of 54 instances of phlebotomy were observed and analyzed. The results of this analysis were used to devise interventions for improvement. Interventions included an interactive training session for the nursing staff, installation of visual cues in the department, ensuring better availability of equipment and formation of a team to ensure that the guidelines are being followed. A repeat audit was performed from 23rd to 30th August 2021 in which a total of 58 instances of phlebotomy were covertly observed. The purpose of this research is to identify and provide understanding and invitation to everyone to ground Islamic communication in social life so that communication patterns between people are well maintained in accordance with the principles of Islamic communication. The research method used is a type of library research. his research is carried out by reading, examining, and analyzing various existing sources, in the form of the Qur'an, Hadith, journals and books, as well as other research results related to communication in social life. The results showed that Islamic communication in social life has a very important role in human life, this can be seen from social meaning, social coverage and social support to being able to limit negative words against someone. Islamic communication is a means of conveying or exchanging information using the principles and methods of communication according to the Qur'an as one of the motivations for Muslims to ground Islamic communication in social life directly and through social media. Meanwhile, in the context of coffee shops as a means of social communication in the city of Banda Aceh, coffee shops are not only a place to drink coffee or rest but also a source of the latest information and also a gathering place for various communities, ranging from ordinary people, community leaders, government employees, and so on. The conclusion is that communication is one of the important reasons in society, of course, in the rules of Islam, besides that Islamic communication and social life play a very large role in the context of coffee shops in the city of Banda Aceh.

Inflammation and oxidative stress (OS) are crucial processes essential in various physiological and pathological states. The imbalance in reactive oxygen species (ROS) interrupts the body’s natural defense system, making fatal disorders more likely to develop. Walnut, also known as Juglans Regia L. is a valuable nut that can significantly reduce OS and inflammation. Walnut contains a considerable amount of polyunsaturated fatty acids (PUFA), flavonoids and phenolic compounds, specifically ellagitannins, that have emerged as major anti-oxidative and anti-inflammatory contributors by activating the Nrf2 signaling pathway. Moreover, quercetin, gallic acid, epigallocatechin-3-gallate and chlorogenic acid scavenge free radicals, modulate pro-inflammatory pathways and suppress the production of inflammatory cytokines. Currently, there is a limited compilation of previous research studies on walnut’s anti-oxidative and anti-inflammatory potential, along with their curative insights against other physiological conditions. This review presents an overview of the latest research studies of walnuts in different forms, as a whole, its extract, oil, protein hydrolysate, peptides, etc., supporting its antioxidant and anti-inflammatory potential in tabular form, along with the figures to demonstrate the underlying mechanisms of its active components through animal and clinical trials. The regulatory effects of the bioactive substances against hyperlipidemia, diabetes, arthritis, Alzheimer’s, seizures, hypertension, ischemic stroke, and cancer have also been elaborated. The findings of the present compilation of research studies suggest that walnuts have a remarkable potential to alleviate oxidative stress and inflammation due to its high concentration of bioactive compounds, by activating the Nrf2 signaling pathway, scavenging free radicals, and modify pro-inflammatory pathways. The incorporation of walnuts in daily diet is an effective therapeutic approach to support overall health and wellness.

The purpose of this work was to confirm the traditional usage of the food plant Coronopus didymus (CD) against ovalbumin-induced asthmatic symptoms, as well as chemical characterization of the extract by Gas Chromatography-Mass Spectrometry. Aceh Province, which is a Special Autonomous Region based on Islamic Sharia, based on 2021 data shows that the implementation of the Teaching Factory has not yet occurred completely in all vocational schools in Aceh. This is a strong indication that the main factor has not yet seen the outcomes expected by the community. Seeing the gap in the quality of Teaching Factory implementation among vocational schools in Aceh, it is necessary to conduct a study regarding the integration of Islamic education through the Teaching Factory model as an effort to develop learning attitude and work ethic of students in vocational schools in Aceh. The research design used is a sequential explanatory design or combination model research. The total sample for quantitative data was 228 respondents using the Structural equation Modeling analysis method. Meanwhile, for qualitative data, the sample size was 509 respondents. Quantitative data analysis uses the Structural equation Modeling analysis method, while qualitative data describes the results descriptively by linking them to existing facts and supporting theories from the results of data integration from the implementation of the Teaching Factory and Islamic Education. The research results show that teaching and learning activities in vocational schools are not just coincidental or incidental, but are part of a planned effort to integrate Islamic education into the learning system and daily life through Teaching Factory learning. This integration has a positive and holistic impact on students' personalities, skills, attitude, and values, with a focus on success in the world of business run according to Islamic principles.

BackgroundPatients with small cell lung cancer (SCLC) often respond to first-line chemoimmunotherapy. However, relapse is inevitable and is associated with a poor prognosis. Treatments for relapsed SCLC, such as lurbinectedin and topotecan, are limited by modest efficacy and significant hematologic adverse events, leaving a need for newer therapeutic agents or regimens. The combination of gemcitabine and nab-paclitaxel is active and safe in other types of malignancies, such as pancreatic cancer.Patients and methodsWe conducted a phase II trial evaluating the efficacy and safety of gemcitabine and nab-paclitaxel in patients with relapsed/refractory SCLC. The primary endpoint was objective response rate (ORR), defined as the proportion of patients with confirmed complete or partial response. Secondary endpoints included time to progression (TTP), progression-free survival (PFS), overall survival (OS), and safety.ResultsBetween October 2016 and May 2021, 32 patients were enrolled. Patients were followed for a median of 9.3 months (range 1.8–65.2). Median age was 65 years (range 48–81). Fifty percent of patients were female. Fifty-three percent of patients had platinum-resistant/refractory relapsed SCLC. The ORR was 28.1% (95% confidence interval [CI] 15.5–100%). Median PFS was 2.9 months (95% CI 2.4–3.6), and median OS was 9.3 months (95% CI 5.2–12.4). Seven patients (21.9%) developed grade 3 or 4 neutropenia.ConclusionOur study showed that the combination of gemcitabine and nab-paclitaxel led to encouraging outcomes in relapsed/refractory SCLC. OBJECTIVE: To determine the Frequency of Germinal Center B (GCB) type Diffuse Large B Cell Lymphoma (DLBCL) in Karachi, Pakistan, and to screen the DLBCL patients for EZH2 mutation. METHODOLOGY: A cross-sectional study was conducted on DLBCL patients, and IHC analysis, PCR, and sequencing of the EZH2 gene were used. This study was conducted from August 2016 to July 2020. Surgical specimens received in the histopathology section of DDRRL centrally and from different collection units throughout Sindh having been diagnosed with DLBCL were collected. Three markers (CD10, BCL6, and MUM1) were used for subtyping. DNA was manually extracted from the FFPE Blocks of DLBCL cases using phenol: chloroform: isopropanol alcohol. Polymerase chain reaction (PCR) was performed, and samples were sent from Dow University to Korea (Macrogen) for sequencing. The amplified product was 189bp. Primers were used for EZH2 to flank the region containing the tyrosine domain on exon 16 and exon 18. RESULTS: Out of 116 cases, 60.3% and 39.7% were male and female, respectively. The mean age was 49.73±16.38 years. The mean survival duration of cases diagnosed with GCB was 8.21±7.39 months. Polymorphism of EZH2 mutation (Tyr 701 Asn) was found to be positive in GCB-DLBCL for three samples (16.6%). CONCLUSION: In the current study, Sanger sequencing was used to detect any mutation in the coding region of EZH2, and a unique mutation was reported (Tyr701Asn).

Mineral oil is the most commonly used insulating oil in power transformers due to its low cost, universal availability and high dielectric strength. The potential of vegetable oils to serve as an alternative to the conventional mineral oil for power transformers is being considered due to its better dielectric strength and high biodegradability. Furthermore, the addition of small quantity of nanotubes to vegetable oils has the potential to considerably enhance their dielectric strength. In this paper, a detailed experimental investigation on the dielectric characteristics of Mustard oil infused with Titanium dioxide (TiO2) nanotubes has been carried out. The investigation covers the analysis of physical properties including moisture content, specific gravity, pour-point, flash point, viscosity, and acidity. Additionally, the breakdown strength and partial discharges of Mustard oil and nanofluid were also examined. All the tests were performed according to ISO and IEC standards. The detailed characterization of nanotubes was investigated with the help of X-ray diffraction and Scanning Electron Microscopy. The TiO2 nanotubes were mixed with pure Mustard oil at the concentration levels of 0.02g/L, 0.05g/L, and 0.1g/L. The experimental results indicate that the nanofluid with a concentration of 0.05g/L exhibits better properties compared to the other samples in its potential as a replacement of the traditional mineral oil. The investigation also revealed that the AC breakdown voltage increased by approximately 40%, whereas a three-fold rise in the partial discharge extinction and inception voltages was observed when 0.05g/L TiO2 was added into the Mustard oil compared to the pure mineral oil. A comprehensive evaluation of the dielectric properties of this mixture, in comparison to the mineral oil and other vegetable oils, underscores its cost-effectiveness as a viable alternative for power transformers. Hence, the addition of 0.05g/L TiO2 nanofluids into Mustard oil can be considered as a practical and economically feasible substitute to both mineral and vegetable oils.

Myelofibrosis (MF) is a rare BCR-ABL negative myeloproliferative neoplasm characterized by clonal proliferation of stem cells, with mutations in JAK2, CALR, or MPL genes. MF presents in primary and secondary forms, with common symptoms including splenomegaly, anemia, and thrombocytopenia. Diagnostic criteria involve bone marrow examination and mutation studies. Current treatments are limited, with allogeneic stem cell transplant as the only curative option. Recent FDA approval of Momelotinib (MMB) offers new promise for MF patients with anemia. MMB, a JAK1/2 and ACVR1 inhibitor, effectively reduces spleen size, improves hemoglobin levels, and decreases transfusion dependency. The MOMENTUM trial compared MMB to danazol in JAK inhibitor-treated MF patients with anemia, showing MMB’s superior symptom relief and transfusion independence rates. Additionally, the SIMPLIFY-1 and SIMPLIFY-2 trials evaluated MMB in JAK inhibitor-naïve and experienced patients, respectively, confirming MMB’s non-inferiority to ruxolitinib in spleen volume reduction and highlighting its benefits in transfusion requirements. MMB’s unique dual inhibition mechanism addresses anemia by suppressing hepcidin production, thus enhancing erythropoiesis. These trials collectively suggest MMB as an effective treatment for MF, improving quality of life and offering a survival advantage for patients with anemia. Despite challenges, such as trial design limitations and adverse events, MMB represents a significant advancement in MF management, providing a new therapeutic option for a previously underserved patient population.

Background: Non-small cell lung cancer is classified into different subtypes. It is now mandatory for the histopathologists to classify the NSCLC into its exact classification because of the increase in promising personalized therapy against each of its subtypes. In order to better classify non-small cell lung cancers, the current study evaluates the diagnostic value of p40 and Napsin-A along with the comparison with the conventional markers TTF-1 and P63. Methods: 147 blinded diagnosis of NSCLC were included and classified based on histological findings. After histological review, all the specimens were stained with the conventional markers and new markers i.e., p40, p63 for squamous-cell-carcinoma (SQC) and TTF-1, Napsin-A for adenocarcinoma (ADC). Results: Regarding baseline data, out of 147 NSCLC samples, 106 (72.10%) were males and 41 (27.90%) were females with the mean age of the patient was 57.6 years. The new markers combination reclassified tumors as 60 (40.8%) as ADC, 80 (54.4%) as SQC and 7.0 (4.8%) as Transdifferentiated (TD). The sensitivity and specificity of newly proposed markers, that is, Napsin-A and p40 were better than the conventional markers TTF-1 and p63. Conclusion: Napsin-A and p40 are better markers than presently used markers (TTF-1and p63). These better markers are suggested to be added or the existing markers (TTF-1 and p63) should be preferred by Napsin-A and p40 in the histopathological diagnostic workup of NSCLC.

BackgroundThis study aims to evaluate real-world (rw) outcomes of immunotherapy (IO) for advanced stage NSCLC at King Hussein Cancer Center (KHCC) in Jordan.MethodsAdvanced stage NSCLC patients who received IO at KHCC between 2017 and 2022 were included. The data were retrospectively collected. PFS and OS were estimated for patients with ECOG performance status (ECOG PS) 0-1. Cox regression analyzed predictors of OS in first-line (1L) IO, regardless of performance status.ResultsThe total number of patients included was 244. Out of those, 160 (65%), 67 (28%), and 17 (7%) patients received IO as 1L, second-line (2L), or third-line or beyond (3L or beyond), respectively. The median age for all patients was 59 years. Male were 88%, and 77% were smokers. The median follow-up time was 12.5 months. The median PFS and OS for 1L IO were 7 [95% CI 5.8 – 10.3] and 11.8 [95% CI 8.8 – 14.4], months, respectively. In the first 3 months after starting 1L IO, 34/160 (21%) patients had died. For those who survived beyond 3 months after starting 1L IO, the median PFS and OS were 11.3 [95% CI 8.3 – 16.5] and 15.4 [95% CI 13.2 – 21] months, respectively. In the Cox regression model of 1L IO patients with any performance status, ECOG PS 2 was predictive of worse OS compared to ECOG PS 0-1 (p= 0.005).ConclusionThis real-world study of advanced-stage NSCLC patients treated with immunotherapy at KHCC reveals outcomes that fall short of those anticipated from clinical trials. The inclusion of Middle Eastern patients in lung cancer trials is essential to ensure adequate representation of various ethnicities in clinical research.

Precoding is a technique employed to enhance transmission rates in various communication systems, including massive multiple-input multiple-output (MIMO) and optical wireless communication (OWC). In this study, we focus on network massive MIMO OWC (NM-MIMO-OWC) systems and investigate the precoder design to enhance the sum rate and improve the system performance. We present the network’s massive MIMO OWC framework. By utilizing this framework, we are able to calculate the achievable sum rate. Subsequently, we consider the precoding design for maximizing the sum rate while adhering to the total power constraint. To solve this optimization problem, we provide a necessary condition of the optimal solution based on the Karush–Kuhn–Tucker (KKT) conditions, and propose a low-complexity algorithm to further enhance the efficiency of the proposed precoding technique. The numerical results demonstrate that the proposed precoder design significantly improves the transmission rate and effectively maximizes the sum rate.

Poly-herbal therapies for chronic diseases like diabetes mellitus (DM) have been practiced in south Asia for centuries. One of such therapies comprises of Hordeum vulgare, Elettaria cardamomum and Cicer arietinum that have shown encouraging therapeutic potential in the treatment of diabetes and obesity. Therefore, poly-herbal granules (PHGs) of this formula were developed and investigated for their anti-diabetic and anti-obesity potential in obese-diabetic rats. The developed PHGs were chemical characterized and the virtual molecular docking was performed by Discovery studio visualizer (DSV) software. For in-vivo experiment, obesity in rats was induced with high-fat high-sugar diet. After that, diabetes was induced by alloxan monohydrate 150 mg/kg i.p. injection. The diseased rats were treated with PHGs at 250, 500 and 750 mg/kg/day for four weeks. GC-MS analysis of PHGs demonstrated the presence of 1,3-Benzenedicarboxylic acid bis(2-ethylhexyl) ester and 1,2-Benzenedicarboxylic acid di-isooctyl ester and phenol, 2,4-bis(1,1-dimethylethyl). Molecular docking of these compounds demonstrated higher binding energies with receptor than metformin against α-amylase and α-glucosidase. PHGs exhibited a decline in body weight, HbA1c, hyperlipidemia, hyperglycemia, and insulin resistance in diseased rats. The histopathological examination revealed that PHGs improved the alloxan-induced damage to the pancreas. Furthermore, PHGs increased the SOD, CAT and GSH while and the decreased the level of MDA in the liver, kidney and pancreas of diseased rats. Additionally, the PHGs had significantly downregulated the TNF-α and NF-κB while upregulated the expression of NrF-2. The current study demonstrated that the PHGs exhibited anti-diabetic and anti-obesity potential through amelioration of oxidative stress, NF-κB, TNF-α, and NrF-2 due to the presence of different phytochemicals.

Smog is a form of extreme air pollution which comprises of gases such as ozone, sulfur dioxide, nitrogen and carbon oxides, and solid particles including particulate matter (PM2.5 and PM10). Different types of smog include acidic, photochemical, and Polish. Smog and its constituents are hazardaous to human, animals, and plants. Smog leads to plethora of morbidities such as cancer, endocrine disruption, and respiratory and cardiovascular disorders. Smog components alter the activity of various hormones including thyroid, pituitary, gonads and adrenal hormones by altering regulatory genes, oxidation status and the hypothalamus-pituitary axis. Furthermore, these toxicants are responsible for the development of metabolic disorders, teratogenicity, insulin resistance, infertility, and carcinogenicity of endocrine glands. Avoiding fossil fuel, using renewable sources of energy, and limiting gaseous discharge from industries can be helpful to avoid endocrine disruption and other toxicities of smog. This review focuses on the toxic implications of smog and its constituents on endocrine system, their toxicodynamics and preventive measures to avoid hazardous health effects.

The most common cause of infertility and metabolic problems among women of reproductive age is polycystic ovary syndrome (PCOS), a multifaceted disorder. It is an endocrine disorder that occurs in approximately one in seven women. Among these PCOS patients, two thirds will not ovulate on a regular basis and seek treatment for ovulation induction. The symptoms vary in their severity, namely ovulation disorders, excessive androgen levels, or polycystic ovarian morphology. All these symptoms require a therapeutic approach. Many drugs are used to eradicate PCOS symptoms, like metformin, clomiphene citrate, spironolactone, and pioglitazone. Long-term treatment is required to achieve the desired outcome, which is often accompanied by significant adverse reactions. Some herbs and phytochemicals are equally effective for treating PCOS and produce minimal side effects. Recently, herbal products are gaining popularity due to their wide biological activities, safety, availability, and efficacy. The present review covers aetiology, current treatment, pathophysiology, and detailed pre-clinical and clinical studies on plants and phytochemicals that are proven to be useful for the treatment of symptoms associated with PCOS.

Diabetes mellitus is a metabolic disorder. Synthetic antidiabetics are the commonly used treatment options associated with complications. The objective of this study was to explore the antioxidative and antidiabetic potential of Euphorbia helioscopia whole plant ethanolic extract using in vitro and in vivo models. For that purpose, the antioxidative potential was explored by using 2,2-diphenyl-1-picrylhydrazyl analysis. In vitro antidiabetic potential of the extract was evaluated using amylase inhibitory analysis. In vivo antidiabetic activity of the extract was assessed in diabetic rats using streptozotocin/nicotinamide (60 mg/kg/120 mg/kg) as an inducing agent. Metformin was used as standard. The results indicated the presence of significant quantities of phenolic 82.18±1.28 mgg−1 gallic acid equivalent (GAE) and flavonoid 66.55±1.22 mgg−1 quercetin equivalent (QE) contents in the extract. Quantitation of phytoconstituents exhibited the presence of sinapic acid, myricetin, and quercetin using HPLC analysis. The extract inhibited α-amylase by 84.71%, and an antiglycemic potential of 50.34% was assessed in the OGTT assay. IoT underlies the existence of smart home technology. One of the advantages of smart home technology is that homeowners can control conditions and monitor home security using only one device. Unfortunately, at SMKs Darul Ulum Layoa, Bantaeng, Sulawesi Selatan (South Sulawesi), learning about IoT still needs to be improved. It becomes the basis for why we hold a community service activity. This activity provided understanding and training on the Internet of Things, which 40 teachers and students attended. Pretest and posttest methods are used to determine the activity's success level. Both results were processed using the SPSS (Statistical Package for the Social Sciences) method. SPSS is a statistical software used to perform data analysis, including descriptive and inferential analysis. By utilizing the Wilcoxon Signed Rank Test, an analysis of the data revealed a Z value of -5.026 and a p-value of 0.000. These findings indicate a significant difference between the pretest and posttest groups or a significant increase in scores between the pretest and posttest. Therefore, the intervention had a positive effect on the outcome being measured.

To know the results of the face mask detection system, one must be near a computer. This problem makes it difficult to reprimand and provide face masks to violators. One of the ways to prevent the spread of the virus is to wear a mask. This study focuses on making a face mask detection system connected to a cellular device. This study aims to make obtaining information more effortless, and monitoring officers can find out from a smartphone. As a medium of communication, we use the telegram application. Smartphone users widely use this application compared to existing messaging media applications. This study uses the YoloV4 algorithm to detect face mask and JSON to send information to the telegram application. The test consists of two stages, the first stage is to determine the accuracy of the face mask detection system and the second stage is to determine the average time required until the information is sent. The two tests performed obtained 97.57% and 0.255 seconds, respectively. The test results show that the system created can solve the existing problems. The researcher can do further research by increasing the number of datasets to increase the accuracy of face mask detection. There is a critical need to use the abundantly available solar energy worldwide due to the global energy crisis. The goal of this study is to demonstrate the residential use of a stirling engine powered by solar energy in Karachi, Pakistan. The design was carried out to produce a power output of 5 kW in order to address the issues with household and rural area power generating. The design was simulated using MATLAB. The use of solar energy as the engine's heat input was one of the design's key components. This allowed the engine to be built for locations with a lot of solar radiation. In regions with abundant solar energy, solar cookers, sun air warmers, and other appliances may be used. For power generation, however, the Stirling engine was chosen. Calculations of the sun intensity showed that 5 kW of Stirling engine output could be generated from 12 kW of solar electricity. The concentrator for concentrating and reflecting the incoming radiations to the Stirling engine was selected as a solar parabolic dish. This process heated the engine's working fluid, which then expanded and contracted due to thermodynamic forces to produce the engine's power strokes.

Nanobiotechnology has gained attention in the field of medicine and can provide solutions to many questions of medicine including antibiotic resistance. This study was performed to assess the antibacterial activity of iron nanoparticles synthesized by green synthesis, using Syzgium aromaticum (clove) as a reducing and stabilizing agent for an anticipatory use to address antibiotic resistance. Antibacterial activities of iron nanoparticles (FeNPs) analyzed against Gram-positive (Bacillus subtilis, Enterococcus faecalis) and Gram-negative (Escherichia coli, Klebsiella pneumonia, and Pseudomonas aeruginosa) bacterial species anticipated the use of earlier to serve the purpose. FeNPs exhibited the highest antibacterial potential against Enterococcus faecalis. Further studies should be performed to know the exact potential of FeNPs against these bacterial strains.

BackgroundNumerous reviews of the epidemiology and risk factors for breast cancer have been published previously which heighted different directions of breast cancer.AimThe present review examined the likelihood that incidence, prevalence, and particular risk factors might vary by geographic region and possibly by food and cultural practices as well.MethodsA systematic review (2017-2022) was conducted following Preferred Reporting Items for Systematic Reviews and Meta-analyses (PRISMA) guidelines, reporting on epidemiological and risk factor reports from different world regions. Medical Subject Heading (MeSH) terms: “Breast neoplasm” “AND” country terms such as “Pakistan/epidemiology”, “India/epidemiology”, “North America/epidemiology”, “South Africa/epidemiology” were used to retrieve 2068 articles from PubMed. After applying inclusion and exclusion terms, 49 papers were selected for systematic review.ResultsResults of selected articles were summarized based on risk factors, world regions and study type. Risk factors were classified into five categories: demographic, genetic and lifestyle risk factors varied among countries. This review article covers a variety of topics, including regions, main findings, and associated risk factors such as genetic factors, and lifestyle. Several studies revealed that lifestyle choices including diet and exercise could affect a person’s chance of developing breast cancer. Breast cancer risk has also been linked to genetic variables, including DNA repair gene polymorphisms and mutations in the breast cancer gene (BRCA). It has been found that most of the genetic variability links to the population of Asia while the cause of breast cancer due to lifestyle modifications has been found in American and British people, indicating that demographic, genetic, and, lifestyle risk factors varied among countries.ConclusionThere are many risk factors for breast cancer, which vary in their importance depending on the world region. However, further investigation is required to better comprehend the particular causes of breast cancer in these areas as well as to create efficient prevention and treatment plans that cater to the local population.

Soft tissue sarcomas (STSs) are mesenchymal malignant lesions that develop in soft tissues. Despite current treatments, including radiation therapy (RT) and surgery, STSs can be associated with poor patient outcomes and metastatic recurrences. Neoadjuvant radiation therapy (nRT), while effective, is often accompanied by severe postoperative wound healing complications due to damage to the surrounding normal tissues. Thus, there is a need to develop therapeutic approaches to reduce nRT toxicities. Avasopasem manganese (AVA) is a selective superoxide dismutase mimetic that protects against IR-induced oral mucositis and lung fibrosis. We tested the efficacy of AVA in enhancing RT in STSs and in promoting wound healing. Using colony formation assays and alkaline comet assays, we report that AVA selectively enhanced the STS (liposarcoma, fibrosarcoma, leiomyosarcoma, and MPNST) cellular response to radiation compared to normal dermal fibroblasts (NDFs). AVA is believed to selectively enhance radiation therapy by targeting differential hydrogen peroxide clearance in tumor cells compared to non-malignant cells. STS cells demonstrated increased catalase protein levels and activity compared to normal fibroblasts. Additionally, NDFs showed significantly higher levels of GPx1 activity compared to STSs. The depletion of glutathione using buthionine sulfoximine (BSO) sensitized the NDF cells to AVA, suggesting that GPx1 may, in part, facilitate the selective toxicity of AVA. Finally, AVA significantly accelerated wound closure in a murine model of wound healing post RT. Our data suggest that AVA may be a promising combination strategy for nRT therapy in STSs.

Diabetes mellitus (DM) is the most prevalent endocrine disorder. Numerous individual herbs possess anti-diabetic activity. The seeds of Hordeum vulgare, Elettaria cardamomum and Cicer arietinum are traditionally used to manage DM. The ambition of this work was to formulate the poly-herbal granules (PHGs) comprising of these three functional foods and evaluate their in-vitro antioxidant and antidiabetic potential. The dried seed extracts of Hordeum vulgare, Elettaria cardamomum and Cicer arietinum were used in a ratio of 2.5:1:1 to formulate PHGs by wet granulation method. The ratio of extracts was selected on the basis of traditional phytotherapies popularly used by local Hakeems of Pakistan to achieve glycemic control in diabetic patients resistant to traditional allopathic regime of medicine. The flow properties of developed PHGs were evaluated. The UV–Visible spectroscopic, Fourier Transform Infrared (FTIR) and HPLC-DAD of all seed extracts and PHGs were performed. The in-vitro antioxidant DPPH, FRAP, total antioxidant capacity (TAC) and Nitric Oxide (NO) scavenging assays were carried out on PHGs. The in-vitro antidiabetic activity of PHGs was investigated by alpha-amylase and alpha-glucosidase enzyme inhibition activity. The developed PHGs exhibited excellent flow properties. The UV–Vis spectra of all seed extracts and PHGs demonstrated peak at 278 nm showing the presence of flavonoids and phenols. The FTIR spectra confirmed the existence of flavonoids, and phenols along with amines in seed extracts as well as PHGs. The HPLC-DAD test revealed the existence of gallic acid, ascorbic acid, Quercetin-3-(caffeoyldiglucoside)-7-glucoside, Rosmarinic acid, delphinidin-3,5-diglucosides, Kaempferol-3-feruloylsophoroside-7-glucoside and Phloroglucinol in PHGs. The PHGs exhibited IC50 of 51.23, 58.57, 55.41 and 53.13 μg/mL in DPPH assay, FRAP assay, TAC, Nitric oxide scavenging assays respectively. The PHGs also demonstrated IC50 of 49.97 and 36.16 μg/mL in alpha-amylase and in alpha-glucosidase inhibition assays respectively in dose dependent manner. The developed PHGs exhibited an excellent flow property. These exhibit significant in-vitro antioxidant and antidiabetic profile by virtue of flavonoid and phenolic acid derivatives.

DPD also holds potential for treating chronic kidney disease anemia in non-dialysis patients and may have benefits for patients with coexisting renal anemia and heart failure, pending further research and trials.

Technology can serve as a valuable option for learning English vocabulary, as it provides various resources such as language learning apps that can enhance vocabulary and language skills. Quizlet is one such application that assists students in learning vocabulary concepts interactively and enjoyably. Student perceptions in learning activities are important things to know. This study aims to find out how students perceive the use of the Quizlet application in learning vocabulary whether they have negative or positive perceptions. This study uses qualitative methods in data collection. Ten students from grade seven became participants in this study. Researchers used two instruments, namely questionnaires and interviews. After distributing the questionnaires to students, the researcher also conducted interviews with five students to obtain more detailed information. The results showed that students had a positive perception of using Quizlet.

Hundreds of 90-s iEEG records are typically captured from each NeuroPace RNS System patient between clinic visits. While these records provide invaluable information about the patient’s electrographic seizure and interictal activity patterns, manually classifying them into electrographic seizure/non-seizure activity, and manually identifying the seizure onset channels and times is an extremely time-consuming process. A convolutional neural network based Electrographic Seizure Classifier (ESC) model was developed in an earlier study. In this study, the classification model is tested against iEEG annotations provided by three expert reviewers board certified in epilepsy. The three experts individually annotated 3,874 iEEG channels from 36, 29, and 35 patients with leads in the mesiotemporal (MTL), neocortical (NEO), and MTL + NEO regions, respectively. The ESC model’s seizure/non-seizure classification scores agreed with the three reviewers at 88.7%, 89.6%, and 84.3% which was similar to how reviewers agreed with each other (92.9%–86.4%). On iEEG channels with all 3 experts in agreement (83.2%), the ESC model had an agreement score of 93.2%. Additionally, the ESC model’s certainty scores reflected combined reviewer certainty scores. When 0, 1, 2 and 3 (out of 3) reviewers annotated iEEG channels as electrographic seizures, the ESC model’s seizure certainty scores were in the range: [0.12–0.19], [0.32–0.42], [0.61–0.70], and [0.92–0.95] respectively. The ESC model was used as a starting-point model for training a second Seizure Onset Detection (SOD) model. For this task, seizure onset times were manually annotated on a relatively small number of iEEG channels (4,859 from 50 patients). Experiments showed that fine-tuning the ESC models with augmented data (30,768 iEEG channels) resulted in a better validation performance (on 20% of the manually annotated data) compared to training with only the original data (3.1s vs 4.4s median absolute error). Similarly, using the ESC model weights as the starting point for fine-tuning instead of other model weight initialization methods provided significant advantage in SOD model validation performance (3.1s vs 4.7s and 3.5s median absolute error). Finally, on iEEG channels where three expert annotations of seizure onset times were within 1.5 s, the SOD model’s seizure onset time prediction was within 1.7 s of expert annotation.

Abstract In this study, we have investigated the association between the baseline gene expression profile in periapical granuloma and periapical wound healing after surgical endodontic treatment. Twenty-seven patients aged between 15 and 57 years underwent periapical surgery. The retrieved periapical tissue sample was used for mRNA expression analysis of COL1A1, VTN, ITGA5, IL-4, TNF, ANGPT, VEGFA, and CTGF. All patients were recalled after 6 and 12 months for periapical healing evaluation. Healing was then correlated with baseline gene expression. Healing was observed in 15 patients at the end of 6 months, which increased to 21 patients after 12 months. Six patients showed no healing even after 12 months. Background: Radiation therapy (RT) is an integral and commonly used therapeutic modality for primary lung cancer. However, radiation-induced lung injury (RILI) limits the irradiation dose used in the lung and is a significant source of morbidity. Disruptions in iron metabolism have been linked to radiation injury, but the underlying mechanisms remain unclear. Purpose: To utilize a targeted radiation delivery approach to induce RILI for the development of a model system to study the role of radiation-induced iron accumulation in RILI. Methods: This study utilizes a Small Animal Radiation Research Platform (SARRP) to target the right lung with a 20 Gy dose while minimizing the dose delivered to the left lung and adjacent heart. Long-term pulmonary function was performed using RespiRate-x64image analysis. Normal-appearing lung volumes were calculated using a cone beam CT (CBCT) image thresholding approach in 3D Slicer software. Quantification of iron accumulation was performed spectrophotometrically using a ferrozine-based assay as well as histologically using Prussian blue and via Western blotting for ferritin heavy chain expression. Results: Mild fibrosis was seen histologically in the irradiated lung using hematoxylin and eosin-stained fixed tissue at 9 months, as well as using a scoring system from CBCT images, the Szapiel scoring system, and the highest fibrotic area metric. In contrast, no changes in breathing rate were observed, and median survival was not achieved up to 36 weeks following irradiation, consistent with mild lung fibrosis when only one lung was targeted. Our study provided preliminary evidence on increased iron content and ferritin heavy chain expression in the irradiated lung, thus warranting further investigation. Conclusions: A targeted lung irradiation model may be a useful approach for studying the long-term pathological effects associated with iron accumulation and RILI following ionizing radiation.

Introduction: This research was conducted to validate the folkloric use of Quercus leucotrichophora (QL) leaf extracts against inflammation and arthritis and to determine the chemical composition using HPLC.Method: The aqueous and methanolic extracts of QL were evaluated by in vitro anti-oxidant, anti-inflammatory (inhibition of protein denaturation and membrane stabilization) assays, and in vivo anti-inflammatory (carrageenan and xylene-induced edema) and anti-arthritic models. For anti-arthritic potential, 0.1 mL Complete Freund’s Adjuvant (CFA) was inoculated into the left hind paw of a Wistar rat on day 1, and oral dosing with QL methanolic extract (QLME) at 150, 300, and 600 mg/kg was begun at day 8 till the 28th day in all groups, except disease control that was given distilled water, while methotrexate was given as standard treatment.Results and discussion: There was a noteworthy (p < 0.05–0.0001) restoration in body weight, paw edema, arthritic index, altered blood parameters, and oxidative stress biomarkers in treated rats as compared to the diseased group. Moreover, QLME treatment significantly (p < 0.0001) downregulated TNF-α, IL-6, IL-1β, COX-2, and NF-κB, while significantly (p < 0.0001) upregulating IL-10, I-κB, and IL-4 in contrast to the diseased group. The QLME exhibited no mortality in the acute toxicity study. It was concluded that QLME possessed substantial anti-oxidant, anti-inflammatory, and anti-arthritic potential at all dosage levels prominently at 600 mg/kg might be due to the presence of quercetin, gallic, sinapic, and ferulic acids.

Introduction: Vidutolimod, a CpG-A TLR9 agonist, was investigated in a phase 1b study (CMP-001-003; ClinicalTrials.gov, NCT03438318) in combination with atezolizumab with and without radiation therapy (RT) in patients with advanced NSCLC. Methods: Patients with progressive disease after anti–programmed cell death protein 1 or programmed death-ligand 1 therapy received either vidutolimod and atezolizumab (part A) or vidutolimod, atezolizumab, and RT (part B). The primary objective was to evaluate the safety of vidutolimod and atezolizumab with and without RT. Key secondary end point was best objective response rate per Response Evaluation Criteria in Solid Tumors, version 1.1. Results: Between March 28, 2018, and July 25, 2019, a total of 29 patients were enrolled and received at least one dose of vidutolimod (part A, n = 13; part B, n = 16). Intratumoral injections of vidutolimod were administered successfully, including injection of visceral lesions. The most common treatment-related adverse events (≥30%) were flu-like symptoms and hypotension. No objective responses were observed; 23.1% and 50.0% of the patients in parts A and B, respectively, had stable disease as best response. Itraconazole (ICZ) is a broad spectrum antifungal drug, but used as second or third line therapy due to its low and erratic oral bioavailability. This work was carried out to prepare and characterize matrix type lipid-polymer hybrid nanoparticles (LPHNPs) for dissolution enhancement of ICZ. LPHNPs were prepared using solvent diffusion/emulsification technique. Matrix LPHNPs were composed of chitosan (polymer), glyceryl monostearate (lipid) and poloxamer 188 (stabilizer). LPHNPs loaded with ICZ (LPHNPs-1, LPHNPs-2, LPHNPs-3 and LPHNPs-4) were developed using varying concentration of chitosan whereas LPHNPs (LPHNPs-5, LPHNPs-6, LPHNPs-7 and LPHNPs-8) were prepared using varying concentrations of poloxamer 188. LPHNPs loaded with ICZ were further evaluated for entrapment efficiency, particle size, polydispersity index (PDI), zeta potential and dissolution profiles at biorelevant pH conditions. The particle size (LPHNPs-1 to LPHNPs-4) was found to be in range of 421–588 nm with PDI values 0.34–0.41. The particles size of LPHNPs-5 to LPHNPs-8 was found to be in range of 489–725 nm with PDI 0.34–0.74. The entrapment efficiency of LPHNPs-1 to LPHNPs-4 was found to be in range of 85.21%–91.34%. The entrapment efficiency of LPHNPs-5 to LPHNPs-8 was found to be in range 78.32%–90.44%. . The scanning electron microscopy of optimized formulations LPHNPs-1 and LPHNPs-5 indicated formation of oval shaped nanoparticles. DSC thermogram of ICZ loaded LPHNPs also depicted the conversion of crystalline form of ICZ into amorphous form demonstrating the internalization and dissolution enhancement of drug in the hybrid matrix. The cumulative drug dissolved at acidic pH 1.2 was found to be 23.3% and 19.8% for LPHNPs-1 and LPHNPs-5 respectively. Similarly at basic pH values 7.4, cumulative amount of drug dissolved was 90.2% and 83.4% for LPHNPs-1 and LPHNPs-5 respectively. Drug dissolution kinetics exhibited fickian diffusion best described by Korse-meyer Peppas model. The results suggested that chitosan and glyceryl monostearate based matrix LPHNPs could be used as promising approach for dissolution enhancement of ICZ which could further increase its bioavailability.

Polycystic ovary syndrome (PCOS) in females is an endocrine pathological condition of reproductive age which is usually caused by insulin resistance, hyperlipidemia, and oxidative stress. This research was aimed at evaluating the therapeutic effect of the Centratherum anthelminticum seed extract (CA) against PCOS in rodents as it is traditionally used to treat diabetes, inflammation, and gynecological problems. The CA was chemically characterized by high-performance liquid chromatography-diode array detection (HPLC-DAD). For the induction of PCOS, a high-fat diet (HFD) was given to all female Wistar rats for nine weeks except the normal control group, which was given a normal chow diet. Estradiol valerate was given to all rats except normal control. After the induction of PCOS, oral metformin (300 mg/kg) was given to the standard group, while CA was orally administered to diseased rats at 250, 500, and 750 mg/kg/day for 28 days. HPLC-DAD analysis revealed that kaempferol-3-pcoumaroylglucoside was present in the highest amount (146.8 ± 1.8 mg/g) of the extract followed by ferulic acid and malvidin-3-(6-caffeoyl)-glucoside. The in vivo results revealed a marked reduction in cholesterol and triglyceride levels in CA treatment groups. A significant rise was observed in progesterone and follicle stimulating hormone with a decrease in luteinizing hormone in the treatment groups as compared to disease control, which indicated normalization of the estrus cycle. The decrease in insulin resistance was characterized by low serum insulin levels in treatment groups. Treatment with CA also reduced inflammatory markers, such as IL-6 and NF-κB in PCOS rats. NrF2 and oxidative stress markers such as catalase, superoxide dismutase, malondialdehyde, and reduced glutathione were also improved by CA in the ovary of diseased rats. Histopathological examination showed the different developmental stages of normal follicles in CA-treated diseased rats which were indicative of a normal fertile estrous cycle. Overall, the results confirmed the efficacy of CA against PCOS in treating estradiol-HFD-induced PCOS due to its antidiabetic, anti-inflammatory, antihyperlipidemic, and antioxidant properties.

Abstract Background Matrix metalloproteinases (MMPs) catalyzes the degradation of the extracellular matrix components and have a major role in many physiological processes including wound healing. In the current study, we examined the correlation of baseline MMPs 1, 2, 7, and 9 expressions with periapical wound healing after surgical endodontic treatment. Methods 27 patients aged between 15 and 57 years presenting with chronic apical periodontitis or chronic apical abscess of an anterior tooth with previously attempted or failed root canal treatment were included in this study. During surgical endodontic treatment, tissue from the periapical lesion sample was collected and used for gross histopathological analysis as well as mRNA expression analysis of MMPs 1, 2, 7, and 9. Patients were recalled for follow-up after 6 months to evaluate the healing status both clinically and radiographically and healing was correlated with baseline MMP expression. Results Out of 27 patients, healing was observed in 15 patients at the end of 6 months, and in 21 patients after 12 months.. Six patients showed no healing even after 12 months. Analysis of baseline MMP 1, 2, 7, and 9 expression levels with healing status showed the mean relative expression of MMP2 and MMP9 to be considerably increased in the non-healing group as compared to the healing group. Conclusion Overexpression of MMP2 and MMP9 may be considered as a potential prognostic biomarker for periapical wound healing after surgical endodontic treatment. However, further studies are desirable to establish its precise relationship with periapical wound healing.

Background: Parkinson’s disease (PD) is associated with progressive neuronal damage and dysfunction. Oxidative stress helps to regulate neurodegenerative and neuronal dysfunction. Natural compounds could attenuate oxidative stress in a variety of neurological disorders. B. juncea is a rich source of antioxidants. The present study aimed to evaluate the therapeutic potential of B. juncea leaves for the treatment of PD by applying behavioral, in vivo and in silico studies. For in vivo studies rats were divided into six groups (n = 6). Group-I served as normal control (vehicle control). Group-II was disease control (haloperidol 1 mg/kg). Group-III was kept as a standard group (L-Dopa 100 mg/kg + carbidopa 25 mg/kg). Groups (IV–VI) were the treatment groups, receiving extract at 200-, 400- and 600 mg/kg doses respectively, for 21 days orally. Results: In vivo study results showed that the extract was found to improve muscles strength, motor coordination, and balance in PD. These behavioral outcomes were consistent with the recovery of endogenous antioxidant defence in biochemical analysis which was further corroborated with histopathological ameliorations. Dopamine levels increased and monoamine oxidase B (MAO-B) levels decreased dose-dependently in the brain during the study. Herein, we performed molecular docking analysis of the proposed extracted phytochemicals has explained that four putative phytochemicals (sinapic acid, rutin, ferulic acid, and caffeic acid) have presented very good results in terms of protein-ligand binding interactions as well as absorption, distribution, metabolism, excretion & toxicity (ADMET) profile estimations. Conclusion: The undertaken study concluded the anti-Parkinson activity of B. juncea and further suggests developments on its isolated compounds in PD therapeutics.

Abstract Background Though the gut microbiome has been associated with efficacy of immunotherapy (ICI) in certain cancers, similar findings have not been identified for microbiomes from other body sites and their correlation to treatment response and immune related adverse events (irAEs) in lung cancer (LC) patients receiving ICIs. Methods We designed a prospective cohort study conducted from 2018 to 2020 at a single-center academic institution to assess for correlations between the microbiome in various body sites with treatment response and development of irAEs in LC patients treated with ICIs. Patients must have had measurable disease, ECOG 0–2, and good organ function to be included. Data was collected for analysis from January 2019 to October 2020. Patients with histopathologically confirmed, advanced/metastatic LC planned to undergo immunotherapy-based treatment were enrolled between September 2018 and June 2019. Nasal, buccal and gut microbiome samples were obtained prior to initiation of immunotherapy +/− chemotherapy, at development of adverse events (irAEs), and at improvement of irAEs to grade 1 or less. Results Thirty-seven patients were enrolled, and 34 patients were evaluable for this report. 32 healthy controls (HC) from the same geographic region were included to compare baseline gut microbiota. Compared to HC, LC gut microbiota exhibited significantly lower α-diversity. The gut microbiome of patients who did not suffer irAEs were found to have relative enrichment of Bifidobacterium (p = 0.001) and Desulfovibrio (p = 0.0002). Responders to combined chemoimmunotherapy exhibited increased Clostridiales (p = 0.018) but reduced Rikenellaceae (p = 0.016). In responders to chemoimmunotherapy we also observed enrichment of Finegoldia in nasal microbiome, and increased Megasphaera but reduced Actinobacillus in buccal samples. Longitudinal samples exhibited a trend of α-diversity and certain microbial changes during the development and resolution of irAEs. Conclusions This pilot study identifies significant differences in the gut microbiome between HC and LC patients, and their correlation to treatment response and irAEs in LC. In addition, it suggests potential predictive utility in nasal and buccal microbiomes, warranting further validation with a larger cohort and mechanistic dissection using preclinical models. Cisplatin, a potent chemotherapeutic agent, is marred by severe nephrotoxicity that is governed by mechanisms involving oxidative stress, inflammation, and apoptosis pathways. The transcription factor Nrf2, pivotal in cellular defense against oxidative stress and inflammation, is the master regulator of the antioxidant response, upregulating antioxidants and cytoprotective genes under oxidative stress. This review discusses the mechanisms underlying chemotherapy-induced kidney injury, focusing on the role of Nrf2 in cancer therapy and its redox regulation in cisplatin-induced kidney injury. We also explore Nrf2's signaling pathways, post-translational modifications, and its involvement in autophagy, as well as examine redox-based strategies for modulating Nrf2 in cisplatin-induced kidney injury while considering the limitations and potential off-target effects of Nrf2 modulation. Understanding the redox regulation of Nrf2 in cisplatin-induced kidney injury holds significant promise for developing novel therapeutic interventions. This knowledge could provide valuable insights into potential strategies for mitigating the nephrotoxicity associated with cisplatin, ultimately enhancing the safety and efficacy of cancer treatment.

Wettability is a key parameter for optimizing the residual oil recovery from geological rock formations and it provides a path for improved oil recovery and geo-storage of energy. Thus, the key motive behind wettability alteration from hydrophobic to hydrophilic is to enhance the oil productivity. Thus, this work concentrates on Sui main limestone reservoir core samples’ wettability alteration (altering their surface wetting behavior from an oil-wet to water-wet state) for enhanced oil recovery. Hence, we examine the effectiveness of alumina nanofluid as well as a new chemical methyl blue to alter the wettability. Methyl blue is released on a large scale from various industries, i.e., pharma, textile, and food industries, which is a key environmental concern; subsequently, it contaminates the water table. Hence, the study explores the effects of MB and alumina nanofluid on wettability. The effect of nanofluids formulated via dispersing the alumina nanoparticles in aqueous solutions at various concentrations (0. 0.05, 0.3, 0.50, 0.75, and 1.0 wt. %) were tested for wettability modifications under different physio-thermal conditions. Subsequently, the wettability change was examined for these samples treated with different concentrations of MB (10, 15, 30, 50, and 100 mg/L) for 7 days at two different temperatures (25 and 50 °C). The results show that the hydrophobicity of the SML carbonate rock significantly reverses while treating with alumina nanofluids and MB. Thus, the wettability modification/reversal via the treatment of MB and alumina nanofluids can be an effective mechanism for hydrogen injections and EOR processes.

Background: There are concerns with inappropriate prescribing of antibiotics in hospitals especially broad spectrum in Pakistan and the subsequent impact on antimicrobial resistance rates. One recognized way to reduce inappropriate prescribing is for empiric therapy to be adjusted according to the result of culture sensitivity reports. Objective: Using culture sensitivity reports to optimize antibiotic prescribing in a teaching hospital in Pakistan. Methods: A retrospective observational study was undertaken in Ghurki Trust Teaching Hospital. A total of 465 positive cultures were taken from patients during the study period (May 2018 and December 2018). The results of pathogen identification and susceptibility testing from patient-infected sites were assessed. Additional data was collected from the patient’s medical file. This included demographic data, sample type, causative microbe, antimicrobial treatment, and whether empiric or definitive treatment as well as medicine costs. Antimicrobial data was assessed using World Health Organization’s Defined Daily Dose methodology. Results: A total of 497 isolates were detected from the 465 patient samples as 32 patients had polymicrobes, which included 309 g-negative rods and 188 g-positive cocci. Out of 497 isolates, the most common Gram-positive pathogen isolated was Staphylococcus aureus (Methicillin-sensitive Staphylococcus aureus) (125) (25.1%) and the most common Gram-negative pathogen was Escherichia coli (140) (28.1%). Most of the gram-negative isolates were found to be resistant to ampicillin and co-amoxiclav. Most of the Acinetobacter baumannii isolates were resistant to carbapenems. Gram-positive bacteria showed the maximum sensitivity to linezolid and vancomycin. The most widely used antibiotics for empiric therapy were cefoperazone plus sulbactam, ceftriaxone, amikacin, vancomycin, and metronidazole whereas high use of linezolid, clindamycin, meropenem, and piperacillin + tazobactam was seen in definitive treatment. Empiric therapy was adjusted in 220 (71.1%) cases of Gram-negative infections and 134 (71.2%) cases of Gram-positive infections. Compared with empiric therapy, there was a 13.8% reduction in the number of antibiotics in definitive treatment. The average cost of antibiotics in definitive treatment was less than seen with empiric treatment (8.2%) and the length of hospitalization also decreased. Conclusions: Culture sensitivity reports helped reduced antibiotic utilization and costs as well as helped select the most appropriate treatment. We also found an urgent need for implementing antimicrobial stewardship programs in hospitals and the development of hospital antibiotic guidelines to reduce unnecessary prescribing of broad-spectrum antibiotics.
