Hippuric acid is a biotransformation product in humans which is excreted through urine. Dietary intake of phenolic compounds increases its concentration in urine. Hippuric acid is experimentally proved to be a regulator of calcium oxalate super saturation in human urine and has a solvent effect on oxalate salts. Non-steroidal anti-inflammatory drugs (NSAIDs) are widely used analgesics employed in associated renal colic of urolithiasis. The objective of the present study was to chemically link hippuric acid through anhydride linkage to various NSAIDs to synthesize potential mutual prodrugs for urolithiasis and associated renal colic. Hippuric acid was linked to eleven NSAIDs through anhydride linkage by first synthesizing hippuryl chloride using oxalyl chloride as chlorinating agent, followed by its reaction with sodium salts of NSAIDs. Structures of obtained compounds were elucidated using UV spectrophotometry, FTIR spectrometry, 1H NMR and C-13 NMR spectrometry. Synthesized compounds were evaluated for in vitro antioxidant, in vivo acute toxicity, in vivo antiurolithic, in vivo analgesic and in vitro hydrolysis studies. Molecular docking analysis on TNF-α and COX-2 was carried out to determine target protein binding. Synthesized compounds showed stability at acidic pH which indicate potential gastro protective effect of synthesized conjugates. Compounds P9 an P6 showed maximum in vivo antiurolithic activity while P9 and P8 showed maximum in vitro antioxidant activity. All the conjugates except P2 showed significant analgesic activity which show that conjugation of hippuric acid to NSAIDs did not result in loss of analgesic potential. Docking studies showed better affinity with TNF-α as compared to COX-2.

This research addresses the issue of an individual's ability to drive and especially of those individuals that are questionably demented (dementia is suspected) or are in a state of very mild dementia and are therefore the most difficult to identify. A methodology has been developed for categorization of drivers by considering three driving performance indices/parameters simultaneously. This novel approach precluded the previous technique whereby only a single driving performance index (an omnibus approach without the ability to discriminate between normal driving behavior and risky driving habits primarily due to cognitive decline) is considered. Driving performance of 28 young and 28 old drivers was gauged by 24 driving performance parameters through two designed drives on driving simulator. Normal Mixture Model Cluster Analysis was used in the performance-based categorization of drivers. It was found that out of a total of 56 drivers, 8 turned out to be "poor drivers". Results from neuropsychological/cognitive tests showed on average lower cognitive performance for the "poor drivers" group. This methodology will preclude the need for measurement of driving skills through driving instructors.

The aim of this research is to explore the factors influencing housing prices in Pakistan. The author used monthly time series data for the period from 2011 to 2020, which were obtained from different sources: housing prices data from zameen.com, Karachi interbank offered rate (KIBOR) as a proxy for monetary policy, consumer price index as a proxy for inflation, and exchange rate data from the State Bank of Pakistan. Various methods, such as autoregressive distributed lag (ARDL), comparative analysis and deductive analysis were employed. Before using the ARDL technique, a proper lag length was selected, which turned out to be 11 months. Various diagnostic tests indicated model stability with no autocorrelation or structural breaks. The author concluded that the KIBOR rate negatively affected housing prices, while inflation and exchange rates affected house prices positively.

Marble dust waste powder generated by the marble cutting industry has a high alkalinity. In this research, the use of marble dust (MD) as a mineral filler substitute in hot mixed asphalt (HMA) was evaluated. The Marshall mix design was used to determine the optimum bitumen content (OBC) for all of the mixtures. For each of the four MD contents, i.e., 0, 2%, 4%, and 6% by weight of the total aggregates, four different bitumen percentages were used. The results of the Marshall stability test showed that the optimum filler content was 4% MD. Samples were prepared with 0 MD in the control mix and varying percentages of MD as an alternate filler. In addition, MD aided in increasing the Marshall stability, rutting resistance, and permanent deformation and reduced the fatigue life of asphalt mixtures. As the percentage of MD increases, the rutting resistance and stiffness at high temperatures both increase. As the percentage of MD increases, the fatigue life reduces. Rut resistance in high-temperature conditions can be improved by using MD in HMA as a partial substitute for stone dust (SD). In areas where extensive MD waste is present, MD can be incorporated into HMA mixtures instead of conventional fillers.

Background: Human health is seriously threatened by particulate matter (PM) pollution, which is a major environmental problem. A better indicator of biological responses to PM exposure than its mass alone is the PM ''oxidative potential (OP),'' or ability to oxidize target molecules. When reactive oxygen species (ROS) are generated in the OP in excess of the antioxidant capacity of body due to PM components such metals and organic species, it causes inflammation, deoxyribonucleic acid (DNA), proteins, and lipids damage. Method: The samples of fine particulate matter (PM2.5) are collected from the brick kiln site and the roadside in Lahore, Pakistan. The organic carbon (OC) and elemental carbon (EC) were estimated by carbon analyzer (DRI 2001A) using the thermal/optical transmittance (TOT) protocol. The water-soluble organic carbon (WSOC) concentration was determined using a total organic carbon analyzer (Shimadzu TOC-L CPN). Ion chromatography (Dionex ICS–900) with a conductivity detector was used to analyze the water-soluble anions (Cl−, NO3−, and SO42−) and cations (NH4+, Na+, K+, Mg2+, and Ca2+). Inductively coupled plasma-mass spectrometry (iCAP TQ ICP-MS, Thermo Scientific) was used to determine the concentrations of metals in the solution. The dithiothreitol (DTT) consumption rate was calculated using a spectrophotometer at a wavelength of 412 nm. Results: The mean concentrations of PM2.5 at the brick kiln site and roadside reported are 509.3 ± 32.3 μg/m3 and 467.5 ± 24.9 μg/m3, and the average OC/EC ratio is 1.9 ± 0.4 and 2.1 ± 0.1. primary organic carbon (POC) contributed more to OC than secondary organic carbon (SOC), which indicated the dominance of primary combustion sources. The anion equivalent (AE) to cation equivalent (CE) ratio indicated that PM2.5 is acidic at both sites due to the dominance of NO3− and SO42−. The DTT consumption rate normalized by PM2.5 mass (DTTm) and DTT consumption rate normalized by air volume (DTTv) of PM2.5 at the roadside samples are higher than at the brick kiln site due to the higher contribution of ionic species to the mass of PM2.5. Carbonaceous species of PM2.5 at both sampling sites are significantly correlated with DTTv of PM2.5, while metallic species behaved differently. The incremental lifetime cancer risk (ILCR) values (lung cancer) of As and Cr at both sampling sites, while the ILCR value of Cd at the roadside samples is exceeding the permissible limits for adults and children. The lifetime average daily dose (LADD) value for adults is higher than that for children, indicating that children are less vulnerable to metals. Conclusion: The concentration of PM2.5 at both sampling sites were exceeding the permissible limits of Pakistan’ National Environmental Quality Standard (NEQS) and posing risk to the health of the local population. The POC and SOC contribution to OC at the brick kiln site and roadside in Lahore were 84.6%, 15.4% and 84.4%, 15.6%. POC at both sampling sites were the dominant carbon species indicating the dominance of primary combustion sources. The residence of Lahore poses the lung cancer risk due to Cr, As, and Cd at both sampling sites. The results of this study provide important data and evidence for further evaluation of the potential health risks of PM2.5 from brick kiln site and road side in Pakistan and formulation of efficient air-pollution control measures.

Objective: To find out any association of ABO and Rh blood groups with gestational diabetes mellitus (GDM). Study Design: Comparative cross-sectional study. Place and Duration of Study: Pathology and Gynecology departments of Combined Military Hospital-Lahore from Jun to Sep 2020. Methodology: A total of 250 patients of GDM and 250 pregnant ladies with normal glucose tolerance (NGT) were included in the study. The Oral glucose tolerance test (OGTT) was performed with a 75g glucose load, and results were interpreted according to the American Diabetes Association 2020 criteria. Blood glucose samples were taken in sodium fluoride containers in fasting conditions, 1 hour and 2 hours post-glucose load. Samples for blood groups were obtained in plain and EDTA containers. Blood glucose samples were analyzed using the Hexokinase method's automated chemistry analyzer, Cobas c-501. ABO and RhD blood grouping were carried out by forward and reverse techniques in the blood bank of CMH Lahore. Results: A total of 250 (50%) pregnant ladies with OGTT suggestive of Gestational Diabetes Mellitus and 250 (50%) pregnant ladies with OGTT suggestive of Normal Glucose Tolerance (NGT) were included in the study. No statistically significant difference was observed with ABO and RhD blood groups among them (p=0.944). Conclusion: There is no significant association found between ABO and RhD blood groups with Gestational Diabetes Mellitus.

OBJECTIVES To study the outcome of surgeries performed for retinal detachments associated with chorioretinal colobomas. METHODOLOGY This retrospective study enrolled 16 eyes of 16 patients with retinal detachments associated with chorioretinal colobomas. Standard 3-ports 23-gauge Pars plana vitrectomy (PPV) with silicon oil tamponade and endo-laser was performed along the rim of colobomas in all cases. In 4 (25.0%) eyes, buckling was performed with 240 silicon bands and vitrectomy to support the vitreous base. In 12 eyes (75.0%), the natural lens was removed with phaco / lensectomy and intra-ocular lens implantation. RESULTS Out of 16 eyes, the retina was re-attached in 15 eyes (93.75%) after the first operation. One eye needed a second surgery, which was successful anatomically after the second surgery. Heterocyclic amines and acetamide derivatives are known for their chemotherapeutic potential. Hence, in the present study, morpholine was taken as a principal product and novel morpholine derivatives were designed, formulated, characterized, and screened for the mechanism of inhibition of carbonic anhydrase and their anticancer potential. In addition, in vitro inhibition of hypoxia-inducible factor-1 (HIF-1) protein was also investigated. Results revealed that compounds 1c, 1d, and 1h possessed significant inhibitory activities against carbonic anhydrase with IC50 of 8.80, 11.13, and 8.12 μM, respectively. Interestingly, the carbonic anhydrase inhibitory activity of compound 1h was comparable with that of standard acetazolamide (IC50 7.51 μM). The compounds 1h and 1i significantly inhibited the proliferation of ovarian cancer cell line ID8 with IC50 of 9.40, and 11.2 μM, respectively while the standard cisplatin exhibited an IC50 8.50 μM. In addition, compounds 1c, 1b, 1h and 1i also exhibited significant inhibitory effects on HIF-1α. In conclusion, we report first time the biological potential of morpholine based compounds against ovarian cancer and HIF-1α that may serve as lead molecules for drug discovery.

OBJECTIVES To compare the post-operative (post-op) surgically induced mean astigmatism in patients undergoing 20-gauge- and 23-gauge vitrectomy. METHODOLOGY Sixty patients were enrolled. Relevant information was recorded in a Performa for each patient. Patients were randomly grouped into A (20-gauge) and B (23-gauge) groups. Astigmatism was recorded by a single technician using the same keratometer for all patients. A single surgeon operated on all patients. Post-operatively, all patients were re-examined after 1 week and 6 weeks, and their surgically induced astigmatism was recorded. RESULTS Of 60 patients, 58.6% were males, and 41.4% were females, with a mean age of 53.77 ± 9.63 years. The post-operative astigmatism difference between the two vitrectomy procedures was investigated. In the 20-gauge group, with 30 patients, the mean post-operative astigmatism difference was 2.21 D (±0.66 SD), with a standard error mean of 0.12. In the 23-gauge group, with 30 patients, the mean post-operative astigmatism difference was 0.74 D (±0.44 SD), with a standard error mean of 0.08. The degrees of freedom (df) were 58, and the standard error of the difference was 0.145. The t-value was calculated at 10.1504, and the p-value was found to be 0.001, indicating a significant difference. CONCLUSION The observation that the 20-gauge group displayed a notably higher mean difference in astigmatism than the 23-gauge group suggests a potential association between the choice of vitrectomy procedure and the extent of astigmatism alteration in post-operative outcomes.

Microplastics (MPs) are emerging environmental contaminants pervasive in the environment. Studies have revealed that the majority of airborne MPs are in the form of fiber. Inhaled atmospheric MPs adversely impact human health, causing inflammation and oxidative stress. Atmospheric MPs may also carry hazardous chemical pollutants in addition to heavy metals. Many research studies have been conducted about MPs’ presence in the environmental components, however questions about the presence and identification of atmospheric MPs remained unanswered. Based on the mentioned research gaps, this study examined the sources and abundance of MPs in the atmosphere and their generation, accumulation, and fate. The paper further studies and evaluates current methods for identifying and assessing airborne MPs and their impact on human health and the environment. According to the study findings, there are flaws and complexity in MPs assessment and technical procedures, which further challenges data comparability and dependability. In order to ensure data comparability and reliability, the abundance of atmospheric MPs needs to be expressed using a unified and standard methodology. Innovative and well-established identification methods, strategies to minimize the impacts of MPs, and appropriate steps to reduce this global pollution are required for the current state of MPs. It also provides an overview of atmospheric MPs' characteristics, sources, and airborne transmission and deposition mechanisms. Advances in bacterial degradation, sunlight-driven photocatalysts, fuels, and biodegradable plastics could revolutionize future studies on reducing plastic pollution. Future studies should include more research on the various aspects of atmospheric MPs. Despite several research studies conducted on the presence and identification of MPs in various environmental components, many questions regarding MPs in the atmosphere and from the perspectives of COVID-19 and One Health still need to be answered. This study will help the key stakeholders and policymakers in safeguarding public health and environment from the threats posed by airborne MPs.

Bioassay based fractionation of methanolic extract of Berberis baluchistanica (Berberidaceae), used traditionally for internal injuries, led to the isolation of known compounds (1–4). The structure of these compounds was elucidated by different spectroscopic analysis and available literature data. Antidiabetic and antioxidant potentials of B. baluchistanica fractions and isolated compounds were evaluated using in vitro alpha- amylase and DPPH assays. The isolated compounds were identified as obamegine (1), pakistanine (2), 8-oxyberberine (3) and baluchistine (4). Obamegine was reported from many other species of this genus but it is first time isolated from B. baluchistanica in present study. Moreover, in vitro pakistanine (2) was found as bioactive lead molecule for hypoglycemic (IC50:40.26 µg/ml) and antioxidant (IC50:14.15 µg/ml) activities compared to acarbose (IC50:33.68 µg/ml) and ascorbic acid (IC50:0.41 µg/ml). To the best of our knowledge, no previous data were available for these biological activities. Additionally, in silico antidiabetic and antioxidant activity of pakistanine against two proteins, α-amylase (-9.7 kcal/mol) and tyrosinase (-8.7 kcal/mol) are reported here for the first time. The molecular docking binding interactions authenticate and support the above-mentioned activities and are helpful in predicting the mechanism of action of pakistanine (2).

The present study was conducted to fabricate and characterize mucilage-based polymeric networks of Aloe vera for controlled drug release. Aloe vera mucilage was used to develop a polymeric network via the free-radical polymerization method using potassium persulphate as the initiator, N′ N′-Methylene bisacrylamide as the crosslinker, and acrylamide as the monomer. Using varying concentrations of Aloe vera mucilage, crosslinker, and monomer, we developed different formulations. Swelling studies were conducted at pH 1.2 and 7.4. Concentrations of polymer, monomer, and crosslinker were optimized as a function of swelling. Porosity and gel content were calculated for all samples. FTIR, SEM, XRD, TGA, and DSC studies were conducted for the characterization of polymeric networks. Thiocolchicoside was used as a model drug to study the in vitro release in acidic and alkaline pH. Various kinetics models were applied by using a DD solver. Increasing content of monomer and crosslinker swelling, porosity, and drug release decreased while gel content increased. An increase in Aloe vera mucilage concentration promotes swelling, porosity, and drug release of the polymeric network but decreases gel content. The FTIR study confirmed the formation of crosslinked networks. SEM indicated that the polymeric network had a porous structure. DSC and XRD studies indicated the entrapment of drugs inside the polymeric networks in amorphous form. The analytical method was validated according to ICH guidelines in terms of linearity, range, LOD, LOQ, accuracy, precision, and robustness. Analysis of drug release mechanism revealed Fickian behavior of all formulations. All these results indicated that the M1 formulation was considered to be the best polymeric network formulation in terms of sustaining drug release patterns.

AIM: To assess the effectiveness of low vision devices in visual rehabilitation of people with age-related macular degeneration(AMD).METHODS: Forty-four consecutive people with AMD were assessed in low vision clinic. Data about uncorrected and best corrected visual acuity, refractive errors, types of low vision aids(LVAs)and reading speed prior and with low vision devices were collected and analyzed.RESULTS: Among 44 people with mean age 73±10.8 years suffering from AMD, 36(82%)were male. 29(67%)participants had distance visual acuity(DVA)of 1.0-1.6 LogMAR in the better-seeing eyes while 36(82%)had near visual acuity(NVA)less than 3.2 m. Mean improvement in DVA with low vision devices was 0.67±0.27 LogMAR(P=0.000). With LVAs 42(95%)participants achieved NVA of 1 m or better whereas only two(4.5%)participants had NVA of 1 m without LVAs. With the provision of adequate LVAs, mean reading speed was improved from 2.9±4.78 words per minute(wpm)to 71.31±29.96 wpm among the group of participants who could read(P

In this work, series of pH-responsive hydrogels (FMA1–FMA9) were synthesized, characterized, and evaluated as potential carrier for oral delivery of an antiviral drug, acyclovir (ACV). Different proportions of β-cyclodextrin (β-CD), chitosan (CS), methacrylic acid (MAA) and N′ N′-methylenebis-acrylamide (MBA) were used to fabricate hydrogels via free radical polymerization technique. Fourier transform infrared spectroscopy confirmed fabrication of new polymeric network, with successful incorporation of ACV. Scanning electron microscopy (SEM) indicated presence of slightly porous structure. Thermal analysis indicated enhanced thermal stability of polymeric network. Swelling studies were carried out at 37 °C in simulated gastric and intestinal fluids. The drug release data was found best fit to zero-order kinetics. The preliminary investigation of developed hydrogels showed a pH-dependent swelling behavior and drug release pattern. Acute oral toxicity study indicated no significant changes in behavioral, clinical, or histopathological parameters of Wistar rats. Pharmacokinetic study indicated that developed hydrogels caused a significant increase in oral bioavailability of ACV in rabbit plasma as compared to oral suspension when both were administered at a single oral dose of 20 mg kg−1 bodyweight. Hence, developed hydrogel formulation could be used as potential candidate for controlled drug delivery of an antiviral drug acyclovir.

Objective: To highlight the presentation and outcomes of Aortofemoral bypass surgery at a tertiary care hospital. Study Design: Prospective observational study. Place and Duration of Study: Vascular Surgery department, Combined Military Hospital Lahore, from Jan 2015 to Dec 2019. Methodology: A total of 23 patients between 50-80 years of age presenting with critical limb ischemia due to Aortoiliac occlusive arterial disease type C or D of Trans-Atlantic inter Society Consensus (TASC) II classification confirmed on CT Angiogram, were included in the study. The clinical presentation, co-morbid illnesses, history of smoking, obesity, post operative patency of graft, wound infection, amputation rates, and mortality were documented on a structured proforma. All patients were followed up at 1, 6, 12 and 24 months of surgery. Outcomes were assessed in terms of graft patency, wound infection, limb survival, amputations and mortality. Data was analyzed by SPSS version 23.0. Results: The mean age of patients was 71.04 ± 7.29 years. Most patients presented with rest pain (56.5%). The frequency of smokers was 78.3% while 34.8% patients had both diabetes mellitus and ischemic heart disease. Hyperlipidemia was present in 30.4% patients. Two patients (8.7%) developed superficial surgical site infection. The limb salvage rate was 95.7% and the mortality rate was 4.3%. Conclusion: Aortofemoral bypass surgery is a safe and highly effective treatment modality for the management of aortoiliac occlusive arterial disease in experienced hands. Limb salvage can be successfully achieved in more than 95% cases.

The strength of subgrade layer plays an important role in achieving a sound and economic pavement design. To explore novel methods for improving the engineering properties of weak subgrade soils, this research study used bentonite, a naturally occurring pozzolanic material and compared its performance with commercially available cementitious materials such as cement and lime. For this purpose, these pozzolanic and cementitious additives were added in different percentages to representative weak subgrade soils for studying different parameters such as Optimum Moisture Content (OMC), Maximum Dry Density (MDD), California Bearing Ratio (CBR) and Resilient Modulus (MR). The experimental results revealed that the addition of cement lowered the OMC and increased MDD substantially compared to lime and bentonite. The CBR of soil samples enhanced by almost 19 times for 12 % cement addition, followed by lime, which enhanced CBR by 15 times for 15 % addition. The addition of bentonite and lime also enhanced the MR values by almost 3 and 7 times the control specimen for 15 % bentonite and 8% lime addition, respectively. These results indicate that; similar to cement and lime, addition of bentonite can improve the mechanical properties of weak soils by reducing its plasticity and swell potential.

The science of hydrological modeling has continuously evolved under the influence of rapid advancements in software and hardware technologies. Starting from simple rational formulae for estimating peak discharge and developing into sophisticated univariate predictive models, accurate conversion of rainfall into runoff and the assessment of inherent uncertainty has been a prime focus for researchers. Therefore, alternative data-driven methods have gained widespread attention in hydrology. Moreover, scientists often couple conventional machine learning models with data pre-processing techniques, i.e., wavelet transformation (WT), to enhance modelling accuracy. In this context, this research work attempts to explore the latent linkage between rainfall and runoff in Pothohar region of Pakistan by developing a novel linkage of five streamline techniques of machine learning, including single decision tree (SDT), decision tree forest (DTF), tree boost (TB), multilayer perceptron (MLP), and gene expression modeling (GEP), with a more sophisticated variant of WT, i.e., maximal overlap discrete wavelet transformation (MODWT), for boundary correction of the transformed components of timeseries data. This study also implements these machine learning models in a stand-alone mode for a more comprehensive comparative analysis of performances. Furthermore, the study uses a combined-basin approach that divides Pothohar region into two basins to compensate for the complex topographic division of the study area. The results indicate that MODWT-based DTF outperformed other stand-alone and hybrid models in terms of modeling accuracy. In the first scenario, considering the Bunha-Kahan River basin, MODWT-DTF yielded the highest NSE (0.86) and the lowest RMSE (220.45 mm) and R2 (0.92 at lag order 3 (Lo3)) when transformed with daubechies4 (db4) at level three. While in the Soan-Haro River basin, MODWT-DTF produced the highest accuracy modeling at lag order 4 (Lo4) (NSE = 0.88, RMSE = 21.72 m3/s, and R2 = 0.91). The highly accurate performance of 3- and 4-days lagged models reflects the temporal consistency in hydrological response of the study area. The comparison of simple and hybrid model performance indicates up to a 55% increase in modeling accuracy due to data pre-processing with wavelet transformation.

The pentacyclic triterpenoids (PTs) of plant origin are reputed to restrain prostate cancer (PCa) cell proliferation. This study aims to assess 3-epifriedelinol (EFD) isolated from aerial part of Ipomoea batatas against PCa and its potential mechanism, in vitro and in vivo. Molecular docking affirms good binding affinity of the compound with target proteins exhibiting binding energy of −7.9 Kcal/mol with BAX, −8.1 Kcal/mol (BCL-2), −1.9 Kcal/mol (NF-κB) and −8.5 Kcal/mol with P53. In the MTT assay, EFD treatment (3–50 µM) showed a significant (p < 0.05 and p < 0.01) dose and time dependent drop in the proliferative graph of DU145 and PC3, and an upsurge in apoptotic cell population. EFD displayed substantial IC50 against DU145 (32.32 ± 3.72 µM) and PC3 (35.22 ± 3.47 µM). According to Western blots, EFD administration significantly enhanced the cleavage of caspases and PARP, elevated BAX and P53 and decreased BCL-2 and NF-κB expression, thereby triggering apoptosis in PCa cells. When male Sprague Dawley rats were intoxicated with Bisphenol A (BPA), an apparent increase in prostate mass (0.478 ± 0.08 g) in comparison to control (0.385 ± 0.03 g) indicates prostatitis. Multidose treatment of EFD (10 mg/kg) significantly reduced prostate size (0.404 ± 0.05 g). EFD exhibited substantial curative potential in vivo, as hematological, hormonal and histopathological parameters have been significantly improved. Reduced peroxidation (TBARS), and suppression of inflammatory markers i.e., NO, IL-6 and TNF-α, signposts substantial antiinflammatory potential of the compound. Overall, EFD has shown better binding affinity with target molecules, acceptable ADMET profile, potent antiproliferative and apoptotic nature and significant reduction in inflamed prostate mass of rats. The present study demonstrates acceptable physicochemical and pharmacokinetic properties of the compound with excellent drugable nature, hence EFD in the form of standardized formulation can be developed as primary or adjuvant therapy against PCa and toxins-induced gonadotoxicity.

Background: Pericardiectomy procedure is commonly indicated for constrictive pericarditis (CP). One of the most important clue to diagnosis of CP is the signs and symptoms of right-sided heart failure that are not completely explained by presence of pulmonary disease or left-ventricular failure. The aim of this study was to assess clinical data presentation and early operative outcomes of pericardiectomy performed for symptomatic chronic constrictive pericarditis who had failed maximal medical therapy. Methodology: A retrospective analysis was carried out of consecutive constrictive pericarditis adult age patients who underwent pericardiectomy procedure from 2002 to 2012. The patients who had intraoperative diagnosis of other then constrictive pericarditis were excluded from further analysis. A total 51 patients were selected for this study. The preoperative assessment, intraoperative findings and postoperative course and outcome such as hospital stay, prolonge ICU stay, post-operative CPV etc. were evaluated. Results: 43 males and 8 females were include in the study among them 41% gave history of previous (TB) and 35% were pediatric patients (teens, as shown less than 19 years). 8 patients had effusive type pericarditis. The preoperative findings revealed dyspnea 54%, Chest Pain 45% and persistent cough 31%. Furthermore, 43% patients had ascites and 41% with pleural effusion. The mean preoperative central venous pressure (CVP) was 20.5 respectively. The post-operative outcome was short hospital stay, the post-operative patients CVP was estimated 13.5 and the mean drop of CVP due to pericardiectomy was estimated 7 respectively. Conclusion: The conclusive findings of our study revealed that among the patients of symptomatic chronic tuberculous CP, the timely pericardiectomy attempt can effectively improve the hemodynamics of the patient. The early pericardiectomy can also reduce the morbidity and mortality rate of these patients.

Background: Posterior capsular opacification (PCO) is the most frequent complication of cataract surgery. The objective of this study was to determine the frequency of acute rise of intraocular pressure (IOP) following Neodymium-Yttrium Aluminum Garnet (Nd:Yag) laser posterior capsulotomy. Materials & Methods: This cross-sectional study was conducted in the Department of Ophthalmology, HMC, Peshawar, Pakistan, from July-December 2016. Sample size was 325 selected by consecutive sampling technique. Inclusion criteria were all patients having significant PCO after uneventful cataract surgery with intra ocular lens implant. Demographic variables were sex, age groups and age in years. Research variable was presence of acute rise of IOP following Yag laser capsulotomy. Pre-laser assessment included slit lamp examination and recording of IOP with Goldmann applanation tonometer by a single ophthalmologist. IOP was measured 3 hours after Nd: Yag laser capsulotomy. Rise in IOP was labeled when IOP was ≥5 mmhg from baseline. Mean ±SD was used to express quantitative variables like age. Categorical variables were analyzed as frequency and percentage. Goodness-of-fit was applied to compare the frequency of acute rise in IOP between sample and population. SPSS-20 was used for data entry and its analysis. Results: Out of 325 patients, 170(52.3%) were men and 155(47.7%) were women. The mean age of the sample was 44.92 ±23.843 years. A total of 107(32.9%) patients were 20-39 years old whereas 218(67.1%) were 40-70 years. IOP was raised in 61 (18.8%) patients while in 264(81.2%) patients it was not raised. The frequency of acute rise in IOP after Yag laser capsulotomy was significantly different in sample compared to population. Conclusion: Men were more than women, especially of old age. The frequency of acute rise in IOP after Yag laser capsulotomy was significantly different in sample compared to population.

The high-altitude region of Asia is prone to natural resource degradation caused by a variety of natural and anthropogenic factors that also threaten the habitat of critical top predator species, the snow leopard (Panthera uncia). The snow leopard’s landscape encompasses parts of the twelve Asian countries and is dominated by pastoral societies within arid mountainous terrain. However, no investigation has assessed the vulnerability and pathways towards long-term sustainability on the global snow leopard landscape scale. Thus, the current study reviewed 123 peer-reviewed scientific publications on the existing knowledge, identified gaps, and proposed sustainable mitigation options for the longer term and on larger landscape levels in the range countries. The natural resource degradation in this region is caused by various social, economic, and ecological threats that negatively affect its biodiversity. The factors that make the snow leopard landscapes vulnerable include habitat fragmentation through border fencing, trade corridor infrastructure, non-uniform conservation policies, human–snow leopard conflict, the increasing human population, climatic change, land use and cover changes, and unsustainable tourism. Thus, conservation of the integrated Socio-Ecological System (SES) prevailing in this region requires a multi-pronged approach. This paper proposes solutions and identifies the pathways through which to implement these solutions. The prerequisite to implementing such solutions is the adoption of cross-border collaboration (regional cooperation), the creation of peace parks, readiness to integrate transnational and cross-sectoral conservation policies, a focus on improving livestock management practices, a preparedness to control human population growth, a readiness to mitigate climate change, initiating transboundary landscape-level habitat conservation, adopting environment-friendly trade corridors, and promoting sustainable tourism. Sustainable development in this region encompasses the political, social, economic, and ecological landscapes across the borders.

Abstract Research was conducted to identify a series of neuropsychological tests to assess the ability to drive. Driving performance of young and old UK drivers was modeled through multiple linear regression and univariate logistic regression tools. The UFOV3 test (i.e. the third subtest of the UFOV test) had comparatively high discriminating ability in separating poor-drivers from not-poor-drivers, with 92.86% of the drivers correctly classified; the UFOV3 test resulted in a Sensitivity of 62.5%. Age and a composite cognitive measure were also found to be sound discriminators of poor-drivers and not-poor-drivers with 91.07% and 89.28% of the drivers correctly classified respectively; both age and the composite cognitive measure resulted in a Sensitivity of 50%. It was found that the commonly recommended Clock Drawing Test and the Trail Making-B test were insignificant predictors of driving ability. Results suggest that for a score greater than 220 on the UFOV3 test, the driver may be further evaluated by a driving specialist to ascertain questionable driving behavior. Also, drivers above the age of 77 were more susceptible to exhibiting unusual driving behavior; if such drivers have UFOV3 scores greater than 220 it would be more appropriate to evaluate driving behavior through a driving specialist.

Ischemic stroke is categorized by either permanent or transient blood flow obstruction, impeding the distribution of oxygen and essential nutrients to the brain. In this study, we examined the neuroprotective effects of compound A3, a synthetic polyphenolic drug product, against ischemic brain injury by employing an animal model of permanent middle cerebral artery occlusion (p-MCAO). Ischemic stroke induced significant elevation in the levels of reactive oxygen species and, ultimately, provoked inflammatory cascade. Here, we demonstrated that A3 upregulated the endogenous antioxidant enzymes, such as glutathione s-transferase (GST), glutathione (GSH), and reversed the ischemic-stroke-induced nitric oxide (NO) and lipid peroxidation (LPO) elevation in the peri-infarct cortical and striatal tissue, through the activation of endogenous antioxidant nuclear factor E2-related factor or nuclear factor erythroid 2 (Nrf2). In addition, A3 attenuated neuroinflammatory markers such as ionized calcium-binding adapter molecule-1 (Iba-1), cyclooxygenase-2 (COX-2), tumor necrotic factor-α (TNF-α), toll-like receptors (TLR4), and nuclear factor-κB (NF-κB) by down-regulating p-JNK as evidenced by immunohistochemical results. Moreover, treatment with A3 reduced the infarction area and neurobehavioral deficits. We employed ATRA to antagonize Nrf2, which abrogated the neuroprotective effects of A3 to further assess the possible involvement of the Nrf2 pathway, as demonstrated by increased infarction and hyperexpression of inflammatory markers. Together, our findings suggested that A3 could activate Nrf2, which in turn regulates the downstream antioxidants, eventually mitigating MCAO-induced neuroinflammation and neurodegeneration.

Islam being a complete code of life encompasses all aspects of a person’s personal and social life. Islam considers those as Muslims who submit to the will of Allah Almighty in all aspects of life. Islam is not merely a matter of private life and its worship system is not restricted only to a set of rituals, but has pervasive social consequences and develops a strong sense of moral society based on system of rules around center of universal concept of justice. Therefore, Islam proposed institutions with relation to justice, governance, cooperation and solidarity for achieving high economic growth and development. Achievement of development and growth of economy is fast in a society or country if it has developed human capital (human resource). Human capital is developed when education is imparted and facilities of health and sound life are provided to human beings. According to Islamic principles, if human beings cannot afford proper education in the society (or other facilities such as health care) then those who are well off endow their properties in Zakat, Awqaf (plural of waqf) and Charities for their better development and nourishment. These properties when endowed as Awqaf are deployed for propagation of education (knowledge, skills, training etc.) by establishing Madrassas (schools, maktabs, colleges, universities etc.) libraries, translating books, and conducting research. In Islamic society there were many Awqaf founded for establishing Madrassas. This paper is dedicated to discuss the development of human capital through education funded by Islamic Awqaf by reviewing literature.
