Background In medical residency, performance observations are considered an important strategy to monitor competence development, provide feedback and warrant patient safety. The aim of this study was to gain insight into whether and how supervisor-resident dyads build a working repertoire regarding the use of observations, and how they discuss and align goals and approaches to observation in particular. Methods We used a qualitative, social constructivist approach to explore if and how supervisory dyads work towards alignment of goals and preferred approaches to performance observations. We conducted semi-structured interviews with supervisor-resident dyads, performing a template analysis of the data thus obtained. Results The supervisory dyads did not frequently communicate about the use of observations, except at the start of training and unless they were triggered by internal or external factors. Their working repertoire regarding the use of observations seemed to be primarily driven by patient safety goals and institutional assessment requirements rather than by providing developmental feedback. Although intended as formative, the institutional test was perceived as summative by supervisors and residents, and led to teaching to the test rather than educating for purposes of competence development. Conclusions To unlock the full educational potential of performance observations, and to foster the development of an educational alliance, it is essential that supervisory dyads and the training institute communicate clearly about these observations and the role of assessment practices of- and for learning, in order to align their goals and respective approaches.

Muris JWM, Damoiseaux RAMJ, Van Dijk N. Bijwerkingen en valkuilen van EBM: trap er niet in! Huisarts Wet 2017;60(11):548-50. Huisartsen zijn echte evidence-based zorgverleners. Systematische reviews en richtlijnen helpen ons bij het nemen van beslissingen – evidence-based medicine (EBM) heeft ons veel opgeleverd. Door de sterke nadruk op de wetenschappelijk onderbouwing is de kans echter groot dat de wensen en situatie van de specifieke patiënt die tegenover ons zit en onze eigen klinische ervaring te weinig aandacht krijgen. Uitkomsten uit onderzoek kunnen daarnaast een te grote invloed krijgen op ons handelen en de afrekening door zorgverzekeraars te sterk bepalen. Ook kan de industrie klinisch irrelevant onderzoek gebruiken voor financieel gewin. Deze ontwikkelingen hebben weerstand tegen EBM opgeroepen. Het is daarom van groot belang om tijdens de opleiding het eigenlijke perspectief van EBM centraal te stellen: het gebruik van evidence met inachtneming van de wens en situatie van de patiënt, en de klinische expertise van de arts. Behalve bevragen, zoeken en wegen van evidence moeten we de vertaalslag naar de praktijk benadrukken. Hoe kunnen we de valkuilen en bijwerkingen van een te beperkte toepassing van EBM ontwijken?

Deze aandoeningen, die soms met ‘verkoudheid’ of ‘griep’ betiteld worden, doen zich regelmatig – en vaak epidemisch – voor; bij kinderen (drie- tot viermaal per jaar) vaker dan bij volwassenen (een- tot tweemaal per jaar). Bij verkoudheid of griep is er vrijwel uitsluitend sprake van een virusinfectie. Slechts een deel van de verwekkers is bekend: influenzavirussen, para-influenzavirussen, adenovirussen en rinovirussen. Zo veroorzaken rinovirusinfecties vooral ontstekingsverschijnselen van het neusslijmvlies. Zij geven meestal weinig algemene ziekteverschijnselen. Adenovirusinfecties beginnen met keelpijn en neusirritatie en worden vaak vergezeld van bronchitis, terwijl koorts en algemene malaise als regel vanaf het begin aanwezig zijn. Infecties door influenzavirus uiten zich meestal door ernstige algemene symptomen (koorts, spierpijn, ‘doodziek’), terwijl lokale ziekteverschijnselen (keelpijn, hoest) op de achtergrond staan of zelfs achterwege kunnen blijven. Virale luchtweginfecties doen zich vooral voor in het najaar (vanaf september) en in het voorjaar (tot april). Zowel meteorologische (temperatuurwisselingen) als sociale (schoolgaan) omstandigheden lijken een rol te spelen.

Contains fulltext : 50510.pdf (Publisher’s version ) (Closed access) AIM: To assess whether exposure to house dust mite (HDM) allergens hampers a tapering off of inhaled corticosteroid (ICS) dosage in HDM-sensitive asthma patients. METHODS: Asthma patients sensitised to HDM allergens and using ICS were selected from general practices for this observational study. Dust samples from bed mattresses were taken to assess exposure ('no', 'low', 'intermediate' or 'high') to HDM allergens with a semi-quantitative test (Acarex). Patients were trained to use a self management plan to adjust the dose of ICS according to symptoms and peak flow. The observation period was three months. RESULTS: Outcomes from 123 patients were analysed. Within the 'no' and 'low' HDM exposure groups the proportion of patients who increased the dosage of their ICS medication was significantly lower than the proportion who tapered off or remained on the same dose of ICS. The group with high exposure to HDM allergens had the highest proportion of patients who increased their dose of ICS (p = 0.055). CONCLUSIONS: High exposure to HDM allergens seems to coincide with the use of higher dose ICS treatment in asthma patients sensitised to HDM allergens.
