Abstract

Introduction: Testicular cancer is among the most common malignancies in men under the age of 50 years. Most testicular symptoms are linked to benign diseases. Men's awareness of testicular diseases and testicular self-examination behaviours are suboptimal. In this pilot feasibility study and process evaluation we examine the feasibility of conducting a future definitive randomised controlled trial (RCT) to test the effect of the Enhancing Men's Awareness of Testicular Diseases using Virtual Reality intervention (E-MATVR) compared to the Enhancing Men's Awareness of Testicular Diseases using Electric information control (E-MATE). The study protocol is registered on ClinicalTrials.gov (NCT05146466). Methods: Male athletes, engaged in Gaelic games, and aged 18 to 50 years were included. Recruitment was via FacebookTM, XTM (formerly TwitterTM), and posters. Participants were individually randomised to either E-MATVR or E-MATE. Data were collected at baseline (T0), immediately post-test (T1), and three months post-test (T2) using surveys. Qualitative interviews were conducted with participants and researchers. Results: Data were collected from 74 participants. Of those, 66 were retained. All E-MATVR participants and most E-MATE participants (n = 33, 89.2%) agreed/strongly agreed that the device was easy to use and that they were engaged to learn by the device. Most E-MATVR participants (n = 34, 91.9%) and all E-MATE participants agreed/strongly agreed that the time it took them to complete the intervention was reasonable. All 74 participants were extremely satisfied/somewhat satisfied with their overall participation in the study. E-MATVR was described as interactive, easy, fun, and close to real life. Initial difficulty using VR equipment, nausea, and technical issues were identified as challenges to engaging with E-MATVR. Recommendations were made to make VR more accessible, shorten the survey, and incorporate more interactivity. Across all participants, mean testicular knowledge scores (range 0–1) increased from 0.4 (SD 0.2) at T0 to 0.8 (SD 0.2) at T1. At T2, overall mean scores for participants were 0.7 (SD 0.2). Mean knowledge scores did not differ by trial arm at any timepoint. At T2, all E-MATVR participants and 29/32 E-MATE participants (90.6%) reported purposefully examining their testes within the past three months. Conclusion: Findings are promising, highlighting the feasibility of using VR to promote young athletes' awareness of testicular diseases. Considering the strengths, limitations, and lessons learned from this study, some modifications are required prior to conducing an RCT. These include but are not limited to shortening survey questions, incorporating more interactivity and visual content, and targeting more heterogenous male-dominated environments. 27. Economic evaluations for health technologies with an evolving evidence base : a case study of transcatheter aortic valve implantation

Abstract

The primary aim of this thesis is to investigate the challenges in conducting economic evaluations for medical devices with evolving evidence bases. While economic evaluations for capital projects and medicines are well established in theory and practice, the same cannot be said for novel medical devices. New medical devices are often expensive and rely on scarce evidence for efficacy and cost. This increases uncertainty surrounding their clinical and cost effectiveness. In addition, as fewer formal procedures exist for evaluating devices relative to medicines, evidence bases are weak and health technology assessment agencies are reluctant to make rapid decisions. To address these issues a continuous iterative framework developed and proposed for economic evaluations of medical devices. In this thesis, using Transcatheter Aortic Valve Implantation (TAVI) as a case study, an iterative economic evaluation, employing Bayesian techniques, is developed to investigate how the challenges associated with medical devices can be overcome to produce an efficient and informative economic evaluation. This study is the first to investigate these challenges and identify solutions while conducting an economic evaluation early in a device’s life cycle, using the proposed continuous iterative framework. The consideration of Access with Evidence Development schemes to overcome these challenges and balance access with evidence generation for expensive and novel medical devices, with evolving evidence, is another important contribution of the thesis. Transcatheter Aortic Valve Implantation (TAVI) is a novel treatment for severe Aortic Stenosis for operable and inoperable patients. The iterative economic evaluation concludes that TAVI can be considered cost effective for inoperable patients compared to medical management. There is little value in commissioning new research for continued data collection for this group. However, the continued collection of evidence via the UK TAVI registry as indicated in the National Institute of Clinical Excellence (NICE) guidelines will ensure up to date evidence is available to inform future decisions regarding TAVI in this patient group. For operable patients, the iterative model could not conclude that TAVI was cost effective compared to Aortic Valve Replacement (AVR). However, additional evidence of improved outcomes from TAVI should enhance its cost effectiveness for these patients. The Bayesian value of information analysis indicates that further information on short and long term probability, resource and quality of life parameters is most valuable and the optimal research design for collecting such information is a registry.

Abstract

This article proposes a conceptual framework to explain the adoption and diffusion of a national human resource development (NHRD) standard. NHRD standards are used by governments to promote training and development in organizations and increase the professionalization of practices used by organizations. Institutional theory suggests that adoption and diffusion of HRD practices can be explained using a number of institutional concepts such as structural equivalence, social cohesion, and normative and mimetic isomorphism. Social cohesion and structural equivalence create normative and mimetic pressure, which results in increased adoption and diffusion. We highlight other organizational characteristics that explain adoption and diffusion, and also the implications for both theory and research. (Contains 1 figure.)

Abstract

Background: Older people are among the most vulnerable patients in acute care hospitals. The hospitalisation process can result in newly acquired functional or cognitive deficits termed hospital associated decline (HAD). Prioritising fundamental care including mobilisation, nutrition, and cognitive engagement can reduce HAD risk. Aim: The Frailty Care Bundle (FCB) intervention aims to implement and evaluate evidence-based principles on early mobilisation, enhanced nutrition and increased cognitive engagement to prevent functional decline and HAD in older patients. Methods: A hybrid implementation science study will use a pragmatic prospective cohort design with a pre-post mixed methods evaluation to test the effect of the FCB on patient, staff, and health service outcomes. The evaluation will include a description of the implementation process, intervention adaptations, and economic costs analysis. The protocol follows the Standards for Reporting Implementation Studies (StaRI). The intervention design and implementation strategy will utilise the behaviour change theory COM-B (capability, motivation, opportunity) and the Promoting Action on Research Implementation in Health Services (i-PARIHS). A clinical facilitator will use a co-production approach with staff. All patients will receive care as normal, the intervention is delivered at ward level and focuses on nurses and health care assistants (HCA) normative clinical practices. The intervention will be delivered in three hospitals on six wards including rehabilitation, acute trauma, medical and older adult wards. Evaluation: The evaluation will recruit a volunteer sample of 180 patients aged 65 years or older (pre 90; post 90 patients). The primary outcomes are measures of functional status (modified Barthel Index (MBI)) and mobilisation measured as average daily step count using accelerometers. Process data will include ward activity mapping, staff surveys and interviews and an economic cost-impact analysis. Conclusions: This is a complex intervention that involves ward and system level changes and has the potential to improve outcomes for older patients.
