6. Early detection of heart failure using in-patient longitudinal electronic health records.

Abstract

Heart Failure (HF) is common, with worldwide prevalence of 1%-3% and a lifetime risk of 20% for individuals 40 years or older. Despite its considerable health economic burden, techniques for early detection of HF in the general population are sparse. In this work we tested the hypothesis that a simple Transformer neural network, trained on comprehensive collection of secondary care data across the general population, can be used to prospectively (three-year predictive window) identify patients at an increased risk of first hospitalisation due to HF (HHF). The model was trained using routinely-collected, secondary care health data, including patient demographics, A&E attendances, hospitalisations, outpatient data, medications, blood tests, and vital sign measurements obtained across five years of longitudinal electronic health records (EHRs). The training cohort consisted of n = 183,894 individuals (n = 161,658 age/sex-matched controls and n = 22,236 of first hospitalisation due to HF after a three-year predictive window). Model performance was validated in an independent testing set of n = 8,977 patients (n = 945 HHF patients). Testing set probabilities were well-calibrated and achieved good discriminatory power with Area Under Receiver Operating Characteristic Curve (AUROC]) of 0.86, sensitivity of 36.4% (95% CI: 33.33%-39.56%), specificity of 98.26% (95% CI: 97.95%-98.53%), and PPV of 69.88% (95% CI: 65.86%-73.62%). At Probability of HHF ≥ 90% the model achieved 100% PPV (95% CI: 96.73%-100%) and sensitivity of 11.7% (95% CI: 9.72%-13.91%). Performance was not affected by patient sex or socioeconomic deprivation deciles. Performance was significantly better in Asian, Black, and Mixed ethnicities (AUROC 0.932–0.945) and in the 79–86 age group (AUROC 0.889). Abstract

Children and young people who live in out-of-home care (OoHC) can display aggressive behaviours when their emerging self-regulation and executive functioning systems are overwhelmed. Consequently, their carers can experience incidents that may cause some carers' ongoing stress symptoms. Early intervention with occupational health supports is important; however, including a specific trauma treatment such as Eye-Movement Desensitisation and Reprocessing (EMDR) is unusual. EMDR supports the resolution of stress symptoms and helps carers promote a stable home environment. Stability contributes to calm home environments and incident reduction. The purpose of this mixed methods study was to better understand the inclusion of EMDR into a workforce wellbeing model and explore changes in incident numbers in houses, staff retention, and time-loss injury. Interviews with senior managers and focus groups with therapeutic practitioners from two Australian states were conducted. Descriptive statistics for administrative data were calculated after a quantitative audit. Several programs including EMDR and occupational health policies were working towards enhanced workplace safety. A reduction in the monthly average incident rate from 11.9 (2019–2020) to 6.9 (2022–2023) was identified. Offering an effective treatment to staff that promotes wellbeing, retention, and stability in care homes may be a critically important policy recommendation for organisations with staff at risk of traumatic incidents. IMPLICATIONS Eye -Movement Desensitisation and Reprocessing (EMDR) is an effective trauma therapy that works for residential out-of-home care staff. EMDR enhances carers' emotional wellbeing and capacity when working with the young people in their care. Abstract

Aims: Alternative models of cardiac rehabilitation (CR) are required to improve CR access and uptake. Rehabilitation EnAblement in CHronic Heart Failure (REACH-HF) is a comprehensive home-based rehabilitation and self-management programme, facilitated by trained health professionals, for people with heart failure (HF) and their caregivers. REACH-HF was shown to be clinically effective and cost-effective in a multi-centre randomized trial. The SCOT:REACH-HF study assessed implementation of REACH-HF in routine clinical practice in NHS Scotland. Methods and results: A mixed-method implementation study was conducted across six regional Health Boards. Of 136 people with HF and 56 caregivers recruited, 101 people with HF and 26 caregivers provided 4-month follow-up data, after participating in the 12-week programme. Compared with baseline, REACH-HF participation resulted in substantial gains in the primary outcome of health-related quality of life, as assessed by the Minnesota Living with Heart Failure Questionnaire (mean difference: −9.8, 95% CI: −13.2 to −6.4, P < 0.001). Improvements were also seen in secondary outcomes (PROM-CR+; EQ-5D-5L; Self-Care of Heart Failure Index (SCHFI) domains of maintenance and symptom perception; Caregiver Contribution to Self-Care domains of symptom perception and management). Twenty qualitative interviews were conducted with 11 REACH-HF facilitators, five supporting clinicians, and four national stakeholders. Interviewees were largely positive about REACH-HF, considering it to have 'filled a gap' where centre-based CR was not an option. Key issues to support future roll-out were also identified. Conclusion: Our findings support wider roll-out of REACH-HF as an alternative to centre-based models, to improve CR access and uptake for people with HF. Graphical Abstract [ABSTRACT FROM AUTHOR]

In many countries, official guidance promotes alcohol abstinence to women during, and also before, pregnancy, on the basis of concern about Fetal Alcohol Spectrum Disorder (FASD). Guidance has moved away from reference to a ‘choice to drink’, claiming absence of evidence about safety of even ‘low level’ drinking as a justification. Scholarship drawing on sociologies of risk and uncertainty has drawn attention to problems with precautionary thinking in this area of policy making, including for women’s autonomy. We build on these insights to assess a more recent type of UK guidance. This is directed not to women advising them to abstain, but instead it is about women, and tasks health professionals with managing the risk pregnant women’s behaviour is deemed to present. Using qualitative discourse analysis, we assess one such example, developed in Scotland, called SIGN 156. We contextualise SIGN 156 first through discussion of the relevant literature, making particular use of Ruhl’s considerations of the meaning of risk and the social conditioning of choice, and second through an account of developments in UK Government advice in recent years. We show that SIGN 156 builds on a policy context where a precautionary approach is explicit, but we furthermore detail how this approach innovates the guidance and practice field. SIGN 156 expands the meaning of risk and uncertainty and so justifies ‘routine’ monitoring and screening, generating the case for an expanded form of surveillance of pregnant women. We conclude with a critical commentary on the implications of this case for analyses of risk and uncertainty, and power.

Abstract

This study presents an empirical test of Coyne, Thompson, and Whiffen's statement that the Depressive Experiences Questionnaire does not measure the psychoanalytic constructs it is supposed to measure. Convergence was evaluated between the Depressive Experiences Questionnaire's Dependency and Self-criticism scales and clinicians' ratings on hysterical/anaclitic and obsessional/ introjective personality styles in a sex-balanced sample of 56 Flemish outpatients. In line with expectations from Blatt's research paradigm, convergence was observed between the Dependency scale and ratings on hysterical/anaclitic style in the full sample as well as female and male samples separately; convergence between the Self-criticism scale and ratings on obsessional/introjective style was only observed for men. This study yielded no evidence for divergence of the Depressive Experiences Questionnaire and Blatt's psychoanalytic theory. Abstract

Background: \ud Microvascular dysfunction plays an important role in the pathogenesis of heart failure with preserved ejection fraction (HFpEF). However, no mechanistic link between systemic microvasculature and congestion, a central feature of the syndrome, has yet been investigated.\ud \ud Objectives: \ud This study aimed to investigate capillary–interstitium fluid exchange in HFpEF, including lymphatic drainage and the potential osmotic forces exerted by any hypertonic tissue Na+ excess.\ud \ud Methods: \ud Patients with HFpEF and healthy control subjects of similar age and sex distributions (n = 16 per group) underwent: 1) a skin biopsy for vascular immunohistochemistry, gene expression, and chemical (water, Na+, and K+) analyses; and 2) venous occlusion plethysmography to assess peripheral microvascular filtration coefficient (measuring capillary fluid extravasation) and isovolumetric pressure (above which lymphatic drainage cannot compensate for fluid extravasation).\ud \ud Results: \ud Skin biopsies in patients with HFpEF showed rarefaction of small blood and lymphatic vessels (p = 0.003 and p = 0.012, respectively); residual skin lymphatics showed a larger diameter (p = 0.007) and lower expression of lymphatic differentiation and function markers (LYVE-1 [lymphatic vessel endothelial hyaluronan receptor 1]: p < 0.05; PROX-1 [prospero homeobox protein 1]: p < 0.001) compared with control subjects. In patients with HFpEF, microvascular filtration coefficient was lower (calf: 3.30 [interquartile range (IQR): 2.33 to 3.88] l × 100 ml of tissue–1 × min–1 × mm Hg–1 vs. 4.66 [IQR: 3.70 to 6.15] μl × 100 ml of tissue–1 × min–1 × mm Hg–1; p < 0.01; forearm: 5.16 [IQR: 3.86 to 5.43] l × 100 ml of tissue–1 × min–1 × mm Hg–1 vs. 5.66 [IQR: 4.69 to 8.38] μl × 100 ml of tissue–1 × min–1 × mm Hg–1; p > 0.05), in keeping with blood vascular rarefaction and the lack of any observed hypertonic skin Na+ excess, but the lymphatic drainage was impaired (isovolumetric pressure in patients with HFpEF vs. control subjects: calf 16 ± 4 mm Hg vs. 22 ± 4 mm Hg; p < 0.005; forearm 17 ± 4 mm Hg vs. 25 ± 5 mm Hg; p < 0.001).\ud \ud Conclusions: \ud Peripheral lymphatic vessels in patients with HFpEF exhibit structural and molecular alterations and cannot effectively compensate for fluid extravasation and interstitial accumulation by commensurate drainage. Reduced lymphatic reserve may represent a novel therapeutic target.
