Abstract

Background: Hypertension remains one of the most important modifiable risk factors for stroke and heart disease. Anti-hypertensive medications are effective, but are often not used to maximum benefit. Sub-optimal dosing by prescribers and challenges with medication-taking for patients remain barriers to effective blood pressure control. Objectives: We aimed to systematically develop a theory-based complex intervention to support General Practitioners (GPs) and people with hypertension to maximise medication use to control blood pressure. Methods: We used the three-phase Behaviour Change Wheel (BCW) as the overarching intervention development framework. Collective Intelligence methodology was used to operationalise the stakeholder input to Phases 2 and 3 of the BCW. This took the form of a Collective Intelligence workshop with 19 stakeholders from diverse backgrounds including lived experience, general practice, nursing, pharmacy and health psychology. Techniques such as barrier identification, idea-writing and scenario-based design were used to generate possible intervention options. Intervention options were then selected and refined using the Acceptability, Practicability, Effectiveness, Affordability, Side-effects and Equity (APEASE) criteria and guidance from the MIAMI Public and Patient Involvement Panel. Results: The finalised MIAMI intervention consists of both GP and patient supports. GP supports include a 30-minute online training, information booklet and consultation guide (drop-down menu) embedded within the patient electronic health system. Patient supports include a pre-consultation plan, website, and a structured GP consultation with results from an Ambulatory Blood Pressure Monitor and urine chemical adherence test. The intervention components have been mapped to the intervention functions of the BCW and Behaviour Change Technique Ontology. Conclusion: Collective Intelligence offered a novel method to operationalise stakeholder input to Phases 2 and 3 of the BCW. The MIAMI intervention is now at pilot evaluation stage. Abstract

N-alkylated C2-symmetric amino acid amide derivatives were shown to catalyse the Michael addition of 2-hydroxy-1,4-napthoquinone to β-nitrostyrene, achieving a maximum ee of 44%. The corresponding trifluoroacetic acid salts also catalysed the aldol reaction between 4-nitrobenzaldehyde and hydroxyacetone, leading to the formation of predominantly syn-aldol products in up to 55% ee. Aspects of the solvent dependence of the aldol reaction and the H-bonding of the catalyst were investigated. [ABSTRACT FROM AUTHOR]

Abstract

Synthetic analogues of marine sponge guanidine alkaloids showed in vitro antiparasitic activity against Leishmania (L.) infantum and Trypanosoma cruzi. Guanidines 10 and 11 presented the highest selectivity index when tested against Leishmania. The antiparasitic activity of 10 and 11 was investigated in host cells and in parasites. Both compounds induced depolarization of mitochondrial membrane potential, upregulation of reactive oxygen species levels, and increased plasma membrane permeability in Leishmania parasites. Immunomodulatory assays suggested an NO-independent effect of guanidines 10 and 11 on macrophages. The same compounds also promoted anti-inflammatory activity in L. (L.) infantum-infected macrophages cocultived with splenocytes, reducing the production of cytokines MCP-1 and IFN-γ. Guanidines 10 and 11 affect the bioenergetic metabolism of Leishmania, with selective elimination of parasites via a host-independent mechanism.

35. From the Headlines to the Frontlines: The Teacher Shortage and Its Implications for Recruitment Policy.

Abstract

This report offers insight into how schools can find quality teachers to staff their classrooms by examining how districts responded to the teacher shortage over the period 1999 to 2002. It presents results of an 18-month study that analyzed data at 3 levels: a nationally representative database; a survey of 110 human resource directors across the country; and in-depth interviews with district and school administrators in 7 regions. The report disaggregates the national numbers associated with the problem and examines how districts in different parts of the country perceived its impact. It also identities the range of policies districts used to respond to the problem. Finally, the study attempts to determine the factors that constrained or facilitated district efforts to more effectively meet the demand for teachers. The analyses of the 1999-2002 shortage crisis suggest a two-fold challenge for policymakers working on teacher recruitment. First, because of the uneven impact of the shortage problem, governments need to approach teacher recruitment strategically, identifying policies that best match the problems their schools or district are facing. Second, it is equally important that districts and states identify and remove institutional barriers that thwart policy implications and constrain innovation. Abstract

Although ethical custom has long recognized man’s responsibility to the environment, the contribution of traditional economic entrepreneurship to the sustainability challenge has been limited. Indeed, it can be shown to have had a negative impact at times and although new business models have been introduced, addressing environmental, humane and social issues, questions have been raised about whether entrepreneurship and sustainability are compatible. Accordingly, this chapter proposes a new business model that integrates or harmonizes these four more traditional entrepreneurship models currently applied independently. The model is founded on general systems thinking and the principle of harmony. It is based on a case study of real-life commercial start-up operation, SEKEM Holding in Egypt. The case, which is based on secondary data and non-participant observation, is discussed in detail as is the resultant proposed Harmonious Entrepreneurship model. A definition is provided together with three further case examples that exemplify and demonstrate the model in different geographical and sectoral contexts. Each is based on a “bleeding edge,” innovative technological solution to the problem being addressed and the study concludes that: for entrepreneurship to address the sustainability challenge successfully a new entrepreneurship paradigm is needed that abandons the Friedman doctrine of being about making as much money as possible; the paradigm should incorporate systems thinking and operate both ethically and in accordance with the harmony principle, ensuring that profit, people and planet are harmonized; and the model can be implemented simultaneously, and not incrementally as previous research has suggested.
