1. Evolution of current and pressure driven instabilities in relativistic jets

Abstract

Instabilities in relativistic magnetized jets are thought to be deeply connected to their energy dissipation properties and to the consequent acceleration of the non-thermal emitting relativistic particles. Instabilities lead to the development of small scale dissipative structures, in which magnetic energy is converted in other forms. In this paper we present three-dimensional numerical simulations of the instability evolution in highly magnetized plasma columns, considering different kinds of equilibria. In fact, the hoop stresses related to the azimuthal component of magnetic field can be balanced either by the magnetic pressure gradient (force-free equilibria, FF) or by the thermal pressure gradient (pressure-balanced equilibria, PB) or by a combination of the two. FF equilibria are prone to current-driven instabilities (CDI), while PB equilibria are prone to pressure-driven instabilities (PDI). We perform a global linear stability analysis, from which we derive the different instability properties in the two regimes, showing that PDI have larger growth rates and are also unstable for high wavenumbers. The numerical simulations of the non-linear instability evolution show similar phases of evolution in which the formation of strong current sheets is followed by a turbulent quasi steady-state. Abstract

This work shows that bulk ionic liquids (ILs) and their water solution can be conveniently investigated by synchrotron-based UV resonance Raman (UVRR) spectroscopy. The main advantages of this technique for the investigation of the local structure and intermolecular interactions in imidazolium-based ILs are presented and discussed. The unique tunability of synchrotron source allows one to selectively enhance in the Raman spectra the vibrational signals arising from the imidazolium ring. Such signals showed good sensitivity to the modifications induced in the local structure of ILs by i) the change of anion and ii) the progressively longer alkyl chain substitution on the imidazolium ring. Moreover, some UVRR signals are specifically informative on the effect induced by addition of water on the strength of cation-anion H-bonds in IL-water solutions. All of these results corroborate the potentiality of UVRR to retrieve information on the intermolecular interactions in IL-water solutions, besides the counterpart obtained by employing on these systems the spontaneous Raman scattering technique., Comment: 10 pages, 5 figures

Abstract

We report on the confirmation and mass determination of a transiting planet orbiting the old and inactive G7 dwarf star HD219666 (Mstar = 0.92 +/- 0.03 MSun, Rstar = 1.03 +/- 0.03 RSun, tau_star = 10 +/- 2 Gyr). With a mass of Mb = 16.6 +/- 1.3 MEarth, a radius of Rb = 4.71 +/- 0.17 REarth, and an orbital period of P ~ 6 days, HD219666b is a new member of a rare class of exoplanets: the hot-Neptunes. The Transiting Exoplanet Survey Satellite (TESS) observed HD219666 (also known as TOI-118) in its Sector 1 and the light curve shows four transit-like events, equally spaced in time. We confirmed the planetary nature of the candidate by gathering precise radial-velocity measurements with HARPS@ESO3.6m. We used the co-added HARPS spectrum to derive the host star fundamental parameters (Teff = 5527 +/- 65 K, log g = 4.40 +/- 0.11 (cgs), [Fe/H]= 0.04 +/- 0.04 dex, log R'HK = -5.07 +/- 0.03), as well as the abundances of many volatile and refractory elements. The host star brightness (V = 9.9) makes it suitable for further characterisation by means of in-transit spectroscopy. The determination of the planet orbital obliquity, along with the atmospheric metal-to-hydrogen content and thermal structure could provide us with important clues on the formation mechanisms of this class of objects., Comment: Accepted for publication in A&A

Abstract

The strikingly anisotropic large-scale distribution of matter made of an extended network of voids delimited by sheets, themselves segmented by filaments, within which matter flows towards compact nodes where they intersect, imprints its geometry on the dynamics of cosmic flows, ultimately shaping the distribution of galaxies and the redshift evolution of their properties. The (filament-type) saddle points of this cosmic web provide a local frame in which to quantify the induced physical and morphological evolution of galaxies on large scales. The properties of virtual galaxies within the Horizon-AGN simulation are stacked in such a frame. The iso-contours of the galactic number density, mass, specific star formation rate (sSFR), kinematics and age are clearly aligned with the filament axis with steep gradients perpendicular to the filaments. A comparison to a simulation without feedback from active galactic nuclei (AGN) illustrates its impact on quenching star formation of centrals away from the saddles. The redshift evolution of the properties of galaxies and their age distribution are consistent with the geometry of the bulk flow within that frame. They compare well with expectations from constrained Gaussian random fields and the scaling with the mass of non-linearity, modulo the redshift dependent impact of feedback processes. Physical properties such as sSFR and kinematics seem not to depend only on mean halo mass and density: the residuals trace the geometry of the saddle, which could point to other environment-sensitive physical processes, such as spin advection, and AGN feedback at high mass., Comment: 28 pages, 27 figures, submitted to MNRAS

Abstract

The role of the cosmic web in shaping galaxy properties is investigated in the GAMA spectroscopic survey in the redshift range $0.03 \leq z \leq 0.25$. The stellar mass, $u - r$ dust corrected colour and specific star formation rate (sSFR) of galaxies are analysed as a function of their distances to the 3D cosmic web features, such as nodes, filaments and walls, as reconstructed by DisPerSE. Significant mass and type/colour gradients are found for the whole population, with more massive and/or passive galaxies being located closer to the filament and wall than their less massive and/or star-forming counterparts. Mass segregation persists among the star-forming population alone. The red fraction of galaxies increases when closing in on nodes, and on filaments regardless of the distance to nodes. Similarly, the star-forming population reddens (or lowers its sSFR) at fixed mass when closing in on filament, implying that some quenching takes place. Comparable trends are also found in the state-of-the-art hydrodynamical simulation Horizon-AGN. These results suggest that on top of stellar mass and large-scale density, the traceless component of the tides from the anisotropic large-scale environment also shapes galactic properties. An extension of excursion theory accounting for filamentary tides provides a qualitative explanation in terms of anisotropic assembly bias: at a given mass, the accretion rate varies with the orientation and distance to filaments. Abstract

The mass, accretion rate and formation time of dark matter haloes near proto-filaments (identified as saddle points of the potential) are analytically predicted using a conditional version of the excursion set approach in its so-called "upcrossing" approximation. The model predicts that at fixed mass, mass accretion rate and formation time vary with orientation and distance from the saddle, demonstrating that assembly bias is indeed influenced by the tides imposed by the cosmic web. Starved, early forming haloes of smaller mass lie preferentially along the main axis of filaments, while more massive and younger haloes are found closer to the nodes. Distinct gradients for distinct tracers such as typical mass and accretion rate occur because the saddle condition is anisotropic, and because the statistics of these observables depend on both the conditional means and their covariances. The theory is extended to other critical points of the potential field. The response of the mass function to variations of the matter density field (the so-called large scale bias) is computed, and its trend with accretion rate is shown to invert along the filament. The signature of this model should correspond at low redshift to an excess of reddened galactic hosts at fixed mass along preferred directions, as recently reported in spectroscopic and photometric surveys and in hydrodynamical simulations. Abstract

Axicabtagene ciloleucel showed efficacy for relapsed/refractory large B-cell lymphomas (LBCL), including primary mediastinal B-cell lymphomas (PMBCL); however, only few PMBCLs were reported. Aim was to evaluate efficacy and safety of axicabtagene ciloleucel in patients with PMBCL compared to those with other LBCL, enrolled in the Italian prospective observational CART-SIE study. PMBCLs (n = 70) were younger, with higher percentage of bulky and refractory disease, compared to other LBCLs (n = 190). Median follow-up time for infused patients was 12.17 months (IQR 5.53,22.73). The overall (complete + partial) response rate (ORR,CR + PR) after bridging was 41% for PMBCL and 28% for other LBCL, p = 0.0102. Thirty days ORR was 78% (53/68) with 50% (34) CR in PMBCL, and 75% (141/187) with 53% (100) CR in other LBCL, p = 0.5457. Ninety days ORR was 69% (45/65) with 65% (42) CR in PMBCL, and 54% (87/162) with 47% (76) CR in other LBCL; progressive disease was 21% in PMBCL and 45% in other LBCL, p = 0.0336. Twelve months progression-free survival was 62% (95% CI: 51–75) in PMBCL versus 48% (95% CI: 41–57) in other LBCL, p = 0.0386. Twelve months overall survival was 86% (95% CI: 78–95) in PMBCL versus 71% (95% CI: 64–79) in other LBCL, p = 0.0034. All grade cytokine release syndrome was 88% (228/260); all grade neurotoxicity was 34% (88/260), with 6% of fatal events in PMBCL. Non-relapse mortality was 3%. In conclusion, PMBCLs achieved significantly better response and survival rates than other LBCLs.

Abstract

Instabilities in relativistic magnetized jets are thought to be deeply connected to their energy dissipation properties and to the consequent acceleration of the non-thermal emitting relativistic particles. Instabilities lead to the development of small-scale dissipative structures, in which magnetic energy is converted in other forms. In this paper we present three-dimensional numerical simulations of the instability evolution in highly magnetized plasma columns, considering different kinds of equilibria. In fact, the hoop stresses related to the azimuthal component of magnetic field can be balanced either by the magnetic pressure gradient (force-free equilibria, FF) or by the thermal pressure gradient (pressure-balanced equilibria, PB) or by a combination of the two. FF equilibria are prone to current-driven instabilities (CDI), while PB equilibria are prone to pressure-driven instabilities (PDI). We perform a global linear stability analysis, from which we derive the different instability properties in the two regimes, showing that PDI have larger growth rates and are also unstable for high wavenumbers. The numerical simulations of the non-linear instability evolution show similar phases of evolution in which the formation of strong current sheets is followed by a turbulent quasi-steady state. PDI are however characterized by a faster evolution, by the formation of smaller scale dissipative structures and larger magnetic energy dissipation. [ABSTRACT FROM AUTHOR]

Abstract

We generalize the treatment of inflationary perturbations to deal with the non-Markovian colored noise emerging from any realistic approach to stochastic inflation. We provide a calculation of the power-spectrum of the gauge-invariant comoving curvature perturbation to first order in the slow-roll parameters. Properly accounting for the constraint that our local patch of the Universe is homogeneous on scales just above the present Hubble radius, we find a blue tilt of the power-spectrum on the largest observable scales, in agreement with the WMAP data which show an unexpected suppression of the low multipoles of the CMB anisotropy. Our explanation of the anomalous behaviour of the lower multipoles of the CMB anisotropies does not invoke any ad-hoc introduction of new physical ingredients in the theory., Comment: 9 pages, 2 figures

Abstract

Opioid maintenance treatment (OMT) is the most widespread therapy for both females and males opioid addicts. While many studies have evaluated the OMT impact on men’s sexuality, the data collected about the change in women’s sexual functioning is still limited despite the fact that it is now well-known that opioids - both endogenous and exogenous - affect the endocrine system and play an important role in sexual functioning. The present study aims to determine how OMT with buprenorphine (BUP) or methadone (MTD) affects sexual health in women; examining also any possible emerging correlation between sexual dysfunction (SD), type of opioid and patients’ mental health. This multi-center study case recruited 258 female volunteers attending Italian public Addiction Outpatients Centers that were stabilized with OMT for at least 3 months. SD was assessed with the Arizona Sexual Experience Scale. The twelve-item General Health Questionnaire was used to assess participants’ mental health conditions. The results show that 56.6% of women receiving OMT for at least 3 months presented SD without significant differences between MTD e BUP groups. The majority of the subjects with SD have a poorer quality of intimate relationships and worse mental health than the average. To the best of our knowledge, the present study is the largest report on the presence of SDs in women as a side effects of MTD and BUP used in OMT. Since SDs cause difficulties in intimate relationships, lower patients’ quality of life and interfere with OMT beneficial outcomes, we recommend that women undertaking an opioid therapy have routine screening for SD and we highlight the importance to better examine opioid-endocrine interactions in future studies in order to provide alternative potential treatments such as the choice of opioid, opioid dose reduction and hormone supplementation.

Abstract

We conducted a registry analysis including adult acute myeloid leukemia (AML) patients in remission who had received thiotepa, busulfan, and fludarabine (TBF) or treosulfan-based (Treo) conditioning for haplo-hematopoietic stem cell transplant (HSCT) with posttransplant cyclophosphamide (PTCy) between 2010 and 2020. A total of 1123 patients met the inclusion criteria (968 received TBF and 155 received Treo). A 1:1 matched-pair analysis was performed on 142 TBF and 142 Treo patients. In the Treo group, 68% of patients received treosulfan at a dose ≥36 g/m2 and 54% of patients received a second alkylator (thiotepa or melphalan). We observed a trend toward increased incidence of grade II-IV acute (a) graft-versus-host disease (GVHD) at 180 days in the TBF group compared with Treo (29% versus 20%; P = 0.08), while incidence of grade III-IV aGVHD was not statistically different. Similarly, the incidence of chronic (c) GVHD was not statistically different in the 2 groups. Incidence of nonrelapse mortality at 2 years was 19% in TBF and 14% in Treo (P = 0.4). Relapse incidence at 2 years was not statistically different in the 2 groups (16% and 18% in TBF and Treo, respectively; P = 0.9). Leukemia-free survival, overall survival, and GVHD-free, relapse-free survival was 65% versus 68% (P = 0.6), 73% versus 76% (P = 0.5), and 54% versus 53% (P = 0.8) in TBF versus Treo, respectively. In conclusion, we did not find a significant difference between the 2 conditioning in the present study; Treo and TBF represent 2 valid alternative regimens for haplo-HSCT with PTCy for AML in remission.

Conventional speech and language therapy includes each formal intervention for improving language and communication abilities. In the chronic stage after stroke, it is effective compared to no treatment, but its effect size is small. We present a new language training approach for the rehabilitation of patients with aphasia based on a brain-computer interface system. The approach exploits its capacity to provide feedback time-locked to a brain state. Thus, it implements the idea that reinforcing an appropriate language processing strategy may induce beneficial brain plasticity.In our approach, patients perform a simple auditory target word detection task while their EEG was recorded. The constant decoding of these signals by machine learning models generates an individual and immediate brain-state dependent feedback. It indicates to patients how well they accomplish the task during a training session, even if they are unable to speak.Results obtained from a proof-of-concept study with 10 stroke patients with mild to severe chronic aphasia (age range: 38–76 years) are remarkable. First, we found that the high intensity training (30 hours, four days per week) was feasible, despite a high word presentation speed and unfavorable stroke-induced EEG signal characteristics. Second, the training induced a sustained recovery of aphasia, which generalized to multiple language aspects beyond the trained task. Specifically, all tested language assessments (Aachen Aphasia Test, Snodgrass & Vanderwart, Communication Activity Log) showed significant medium to large improvements between pre- and post training, with a standardized mean difference of 0.63 obtained for the Aachen Aphasia Test, and five patients categorized as non-aphasic at post-training assessment. Abstract

Objective: The present manuscript aims to describe an international, electronic-based, user-friendly and interoperable patient registry for monogenic autoinflammatory diseases (mAIDs), developed in the contest of the Autoinflammatory Diseases Alliance (AIDA) Network. Methods: This is an electronic platform, based on the Research Electronic Data Capture (REDCap) tool, used for real-world data collection of demographics, clinical, laboratory, instrumental and socioeconomic data of mAIDs patients. The instrument has flexibility, may change over time based on new scientific acquisitions, and communicate potentially with other similar registries; security, data quality and data governance are corner stones of the platform. Results: AIDA project will share knowledge and expertise on mAIDs. Since its start, 118 centers from 24 countries and 4 continents have joined the AIDA project. Fifty-nine centers have already obtained the approval from their local Ethics Committees. Currently, the platform counts 337 users (122 Principal Investigators, 210 Site Investigators, 2 Lead Investigators, and 3 data managers). The Registry collects baseline and follow-up data using 3,748 fields organized into 21 instruments, which include demographics, patient history, symptoms, trigger/risk factors, therapies, and healthcare information for mAIDs patients. Conclusions: The AIDA mAIDs Registry, acts both as a research tool for future collaborative real-life studies on mAIDs and as a service to connect all the figures called to participate. On this basis, the registry is expected to play a pivotal role in generating new scientific evidence on this group of rare diseases, substantially improving the management of patients, and optimizing the impact on the healthcare system. Abstract

The outcome of refractory/relapsed (R/R) acute leukemias is still dismal and their treatment represents an unmet clinical need. However, allogeneic transplantation (allo-HSCT) remains the only potentially curative approach in this setting. A prospective study (GANDALF-01, NCT01814488; EUDRACT:2012-004008-37) on transplantation with alternative donors had been run by GITMO using a homogeneous myeloablative conditioning regimen with busulfan, thiotepa and fludarabine while GVHD prophylaxis was stratified by donor type. The study enrolled 101 patients; 90 found an alternative donor and 87 ultimately underwent allo-HSCT. Two-year overall survival of the entire and of the transplant population (primary endpoint) were 19% and 22%, without significant differences according to disease, donor type and disease history (relapsed vs refractory patients). Two-year progression-free survival was 19% and 17% respectively. The cumulative incidences of relapse and non-relapse mortality were 49% and 33% at two years. Acute grade II-IV and chronic GVHD occurred in 23 and 10 patients. Dose intensification with a myeloablative two-alkylating regimen as sole strategy for transplanting R/R acute leukemia does seem neither to improve the outcome nor to control disease relapse. A pre-planned relapse prevention should be included in the transplant strategy in this patient population.

Abstract

Purpose: Prevention of chemotherapy-induced nausea and vomiting (CINV) is particularly challenging for patients receiving highly emetogenic preparative regimens before autologous stem cell transplantation (ASCT) due to the daily and continuous emetogenic stimulus of the multiple day chemotherapy. While studies have shown effective prevention of CINV during the conditioning phase with NK1 receptor antagonist (NK1RA)-containing regimens, there have been no studies evaluating antiemetic use during chemomobilization prior to ASCT. Methods: This multicenter, open-label, phase IIa study evaluated the efficacy of every-other-day dosing of NEPA administered during chemomobilization in patients with relapsed-refractory aggressive non-Hodgkin’s lymphoma. Eighty-one patients participated. Results: Response rates were 77.8% for complete response (no emesis and no rescue use), 72.8% for complete control (complete response and no more than mild nausea), 86.4% for no emesis, and 82.7% for no rescue use during the overall phase (duration of chemomobilization through 48 h after). NEPA was well tolerated with no treatment-related adverse events reported. Conclusion: NEPA, administered with a simplified every-other-day schedule, show to be very effective in preventing CINV in patients at high risk of CINV undergoing to chemomobilization of hematopoietic stem cells prior to ASCT.

Abstract

Background: Hydroxychloroquine (HCQ) was proposed as potential treatment for COVID-19. Objective: We set-up a multicenter Italian collaboration to investigate the relationship between HCQ therapy and COVID-19 in-hospital mortality. Methods: In a retrospective observational study, 3,451 unselected patients hospitalized in 33 clinical centers in Italy, from February 19, 2020 to May 23, 2020, with laboratory-confirmed SARS-CoV-2 infection, were analyzed. The primary end-point in a time-to event analysis was in-hospital death, comparing patients who received HCQ with patients who did not. We used multivariable Cox proportional-hazards regression models with inverse probability for treatment weighting by propensity scores, with the addition of subgroup analyses. Results: Out of 3,451 COVID-19 patients, 76.3% received HCQ. Death rates (per 1,000 person-days) for patients receiving or not HCQ were 8.9 and 15.7, respectively. After adjustment for propensity scores, we found 30% lower risk of death in patients receiving HCQ (HR=0.70; 95%CI: 0.59 to 0.84; E-value=1.67). Secondary analyses yielded similar results. The inverse association of HCQ with inpatient mortality was particularly evident in patients having elevated C-reactive protein at entry. Conclusions: HCQ use was associated with a 30% lower risk of death in COVID-19 hospitalized patients. Within the limits of an observational study and awaiting results from randomized controlled trials, these data do not discourage the use of HCQ in inpatients with COVID-19.

Abstract

Prophylactic low molecular weight heparin (pLMWH) is currently recommended in COVID-19 to reduce the risk of coagulopathy. The aim of this study was to evaluate whether the antinflammatory effects of pLMWH could translate in lower rate of clinical progression in patients with COVID-19 pneumonia. Patients admitted to a COVID-hospital in Rome with SARS-CoV-2 infection and mild/moderate pneumonia were retrospectively evaluated. The primary endpoint was the time from hospital admission to orotracheal intubation/death (OTI/death). A total of 449 patients were included: 39% female, median age 63 (IQR, 50–77) years. The estimated probability of OTI/death for patients receiving pLMWH was: 9.5% (95% CI 3.2–26.4) by day 20 in those not receiving pLMWH vs. 10.4% (6.7–15.9) in those exposed to pLMWH; p-value = 0.144. This risk associated with the use of pLMWH appeared to vary by PaO2/FiO2 ratio: aHR 1.40 (95% CI 0.51–3.79) for patients with an admission PaO2/FiO2 ≤ 300 mmHg and 0.27 (0.03–2.18) for those with PaO2/FiO2 > 300 mmHg; p-value at interaction test 0.16. pLMWH does not seem to reduce the risk of OTI/death mild/moderate COVID-19 pneumonia, especially when respiratory function had already significantly deteriorated. Data from clinical trials comparing the effect of prophylactic vs. therapeutic dosage of LMWH at various stages of COVID-19 disease are needed.
