1. Sensory and compression evaluation of facial powders obtained by different manufacturing processes

Abstract: The aim of this study was to evaluate different manufacturing processes of compact powders and establish consumer preferences through sensory analysis. Four compact powders were formulated with the following treatments: micronization, hammer mill, addition of raw materials that modify the sensory, and simple mixture. The powders were compressed, passing the tests for glazing, detachment of color and drop test, and later forwarded to sensory evaluation by 45 volunteers. In the compression tests, drop test, glazing and detachment of color, the micronized sample obtained the best results showing the reduction of particle size influences the packaging of powders and improves the compression of the product, but in the sensory analysis the sample obtained by simple mixture was the preference of most of the volunteers. It was felt that, despite the best compaction results are obtained for samples micronized, the preference of the volunteers is still linked to older manufacturing processes where the resulting powder shows more coarse and form a thicker layer on the skin.

Abstract: To elucidate the responsible mechanisms of increased slope of minute ventilation relative to carbon dioxide production (VE/VCO(2)) during exercise after acute myocardial infarction without overt signs of heart failure, patients who had an acute myocardial infarction were examined after participating in a three month supervised exercise training programme.Exercise testing, hypercapnic CO(2) chemosensitivity measurement (rebreathing method), and pulmonary function test were repeated at entry and after three months in 50 acute myocardial infarction patients with neither symptoms nor signs of heart failure who completed the training programme. Ten patients who performed initial inhospital training served as controls.Age, peak oxygen uptake, left ventricular ejection fraction, CO(2) chemosensitivity, respiratory parameters (percentage of predicted normal vital capacity (%VC), forced expiratory volume in one second, and carbon monoxide transfer factor (%TLCO)) were all significantly correlated with VE/VCO(2) slope. Multivariate regression analysis showed that age (beta = 0.29, p = 0.01), %TLCO (beta = -0.27, p = 0.01), and CO(2) chemosensitivity (beta = 0.49, p0.001) were independent determinants of VE/VCO(2) slope. After three months, there was no significant change in these parameters in the control group. Peak oxygen uptake, %TLCO, and %VC and attenuation in CO(2) chemosensitivity increased significantly in the training group. The VE/VCO(2) slope decreased marginally (p = 0.11). The changes in VE/VCO(2) slope were correlated only with those in CO(2) chemosensitivity (r = 0.50, p0.001).After acute myocardial infarction, exercise hyperventilation is seen in association with aging, enhanced hypercapnic CO(2) chemosensitivity, and reduced TLCO, even in the absence of overt heart failure. The correlation of VE/VCO(2) attenuation after training with the reduction in CO(2) chemosensitivity suggests that exercise training may reduce increased VE/VCO(2) slope, at least partially by reducing CO(2) chemosensitivity.

Abstract: For a robot, recognizing its location in a real environment is important in terms of visual navigation, etc. This paper proposes a memory-based self-localization method based on the use of omnidirectional images. In the proposed method, information that is constant relative to rotation around the axis of an omnidirectional image sensor is first extracted by generating autocorrelation images from omnidirectional images containing global information that is useful for self-localization. Next, eigenspaces are formed from the generated autocorrelation images, and self-localization is performed by searching for the learning images that are closest to the input image in the eigenspace. Additionally, performing self-localization in two stages (general and local) results in robust self-localization. Lastly, experiments were conducted using time-series omnidirectional images that were actually captured both indoors and outdoors to verify the effectiveness of the proposed method. © 2003 Wiley Periodicals, Inc. 6. Physiological and pathological responses of TU waves to class Ia antiarrhythmic drugs

Abstract: Abnormal repolarization associated with torsades de pointes is expressed as QT prolongation. The physiological response to class Ia antiarrhythmic drugs is also reflected in prolongation of the QT interval. However, the essential difference between pathological and physiological prolongation is not clear. The purpose of this investigation was to differentiate between pathological and physiological changes in the repolarization waves of surface electrocardiograms (ECG) induced by class Ia drugs. In 18 patients without a history of torsades de pointes or syncope (control group), TU waves were compared before and after the administration of class Ia drugs (physiological response). In eight patients with torsades de pointes induced by class Ia drugs (torsades de pointes group), the TU waves at torsades de pointes were compared with those before drug administration (pathological response). In the control group, although the QTc (measured in lead II and corrected for heart rate by Bazett's formula) was increased significantly (0.40 +/- 0.04 to 0.44 +/- 0.05 s, P < 0.001), the U-amp (amplitude of the U wave measured in a precordial lead where the T and U waves were clearly differentiated) remained unchanged. In the torsades de pointes group, however, the QTc was increased (0.42 +/- 0.04 to 0.54 +/- 0.07 s, P < 0.02); the U-amp was also increased, significantly (0.09 +/- 0.07 to 0.27 +/- 0.18 mV, P < 0.05). Thus, enlargement of the U wave may help to differentiate between the physiological and pathological responses to class Ia drugs.

Abstract: Two sets of PCR primers are constructed to clone the cytochrome P450 structural gene, including putative promoter and terminator structures, and its adjacent genetic loci in Campylobacter lari isolates. The putative open reading frames (ORFs) of the P450 genes from 11 C. lari isolates (n=5 for urease-negative (UN) C. lari; n=6 urease-positive thermophilic campylobacters [UPTC]) examined consisted of 1365 or 1371 bases (455 or 457 amino acid residues), differing from those of the other thermophilic campylobacters (1359 [453] for C. jejuni and C. upsaliensis; 1368 [456] for C. coli). Each of the putative ORFs from the 11 isolates examined was also shown to carry start and stop codons and ribosome binding sites. Two putative promoter structures, consisting of sequences at the -35- and -10-like regions were also identified upstream of the ORFs. A single copy of the P450 gene in the genome was identified with UN C. lari JCM2530(T) and UPTC CF89-12, based on Southern blot hybridisation analysis. In addition, when reverse transcription polymerase chain reaction (RT-PCR) analyses were carried out, the transcription of the P450 structural gene in C. lari organisms in vivo was confirmed. The transcription initiation site for the gene was also determined. High nucleotide sequence similarities (95.2-98.8%) of the full-length P450 structural gene were shown with each of the 12 C. lari isolates. The UN C. lari and UPTC organisms showed similar findings with the neighbour-joining method, based on the sequence information of the P450 structural gene.

Abstract: In order to evaluate the electrophysiological effects of the combined administration of disopyramide and mexiletine on anterograde accessory pathway conduction, we studied 11 patients with Wolff-Parkinson- White (WPW) syndrome. Disopyramide was first infused intravenously at 1 mg kg−1for 5 min followed by a continuous infusion at 0.025 mg kg−1 min−1. After the infusion rate of disopyramide was decreased to 0.004 mg kg−1 mm−1 to maintain a lower serum concentration, mexiletine was infused at 2mg kg−1 for 5 min followed by a continuous infusion at 0.008mg kg−1 min−1 (protocol 1). The shortest atrial paced cycle length with 1:1 anterograde accessory pathway conduction (shortest CL with 1:1) and the anterograde effective refractory period of the accessory pathway (ERP) were measured: (1) before drug administration, (2) after disopyramide was given alone, and (3) after disopyramide and mexiletine were combined at lower concentrations. The same protocol was repeated with the order of drug administration reversed (mexiletine followed by disopyramide) in all patients (protocol 2). Both disopyramide and mexiletine lengthened the shortest CL with 1:1 and the ERP to the same degree. The combination of disopyramide and mexiletine at lower concentrations further lengthened the shortest CL with 1:1. Abstract: BACKGROUND Several recent experimental and clinical studies have shown that early afterdepolarizations (EADs) are important in the genesis of QTU prolongation and ventricular tachyarrhythmias (VTs) in patients with long QT syndrome. On the other hand, sympathetic stimulation is well known to contribute to the genesis of QTU prolongation and VTs in patients with congenital long QT syndrome. The present study was performed to examine the influence of isoproterenol on the genesis of EADs and on the action potential durations and QTU intervals in patients with congenital long QT syndrome. METHODS AND RESULTS We recorded monophasic action potentials (MAPs) with a contact electrode during right atrial pacing at a constant cycle length of 500 msec before and after continuous isoproterenol infusion (1 microgram/min). MAPs were obtained from the right and left ventricular endocardium in six patients with congenital long QT syndrome (LQT group, 18 recording sites) and in eight control patients (control group, 19 recording sites). Although no EADs were recorded from either group during the control state, MAP duration at 90% repolarization (MAPD90) was significantly longer in the LQT group (n = 18) than in the control group (n = 19) (275 +/- 36 versus 231 +/- 22 msec; p less than 0.0005). Isoproterenol induced EADs in four of the six LQT patients (five of 18 recording sites) but not in the eight control patients (zero of 19 recording sites). The appearance of EADs in the LQT group was associated with an increased amplitude of the late component of the TU complex, and the corrected QT (QTc) interval was prolonged by isoproterenol from 543 +/- 53 to 600 +/- 30 msec 1/2 (n = 6; p less than 0.05). Isoproterenol also prolonged the MAPD90 from 275 +/- 36 to 304 +/- 50 msec in the LQT group (n = 18; p less than 0.005), whereas it shortened the MAPD90 from 231 +/- 22 to 224 +/- 25 msec in the control group (n = 19; p less than 0.05). Moreover, isoproterenol increased the dispersion of MAPD90 (difference between the longest MAPD90 and the shortest MAPD90 in each patient) from 30 +/- 5 to 62 +/- 35 msec in the LQT group (n = 6; p = 0.08), whereas it did not change the dispersion of MAPD90 in the control group (n = 8; 25 +/- 14 versus 27 +/- 14 msec). CONCLUSIONS These results suggest that patients with congenital long QT syndrome have primary repolarization abnormalities and that EADs induced by isoproterenol play an important role in the exaggeration of these repolarization abnormalities.

Abstract: Atmospheric-pressure MOVPE growth of InP has been performed using the monovalent cyclopentadienyl indium (C5H5In)/PH3/H2 reaction system at a temperature of 570–640°C. Because of the Lewis basicity of the monovalent C5H5In, gas-phase polymerization with PH3 was not observed. The atmospheric reaction system was also found to provide an InP layer less-contaminated with carbon. In addition to chemical characterization of the new In precursor, electrical evaluation of the resultant InP layer has been carried out on the fundamental properties, such as mobility. A room-temperature mobility of 3900 cm2/V·s was found for unintentionally doped InP with a carrier concentration of 1.1 × 1015 cm-3. Excellent doping uniformity for a Si-doped InP layer was also revealed by the C-V technique using a reliable Schottky contact to InP. The results obtained show that the Lewis base monovalent C5H5In is a promising In precursor for atmospheric-pressure MOVPE growth of InP and related compounds.

Abstract: Inappropriate shock therapy from an implantable cardioverter defibrillator (ICD) is very painful for patients and may lead to a poor quality of life (QOL) and/or proarrhythmic effects. Excessively aggressive and specific programming of the ICD detection parameters with the aim of avoiding inappropriate therapy may result in a loss of its sensitivity (and in the worst case even to fatal underdetection of VF). Both sensitivity and specificity of the ICD’s diagnostic functions are expected to be improved by atrial sensing, which provides a more complex algorithm for arrhythmia detection based on both ventricular and atrial activity [1–3]. On the other hand, the addition of an atrial lead may produce several adverse effects, such as extension of the operating time, higher risk of lead dislodgement, cardiac tamponade, and occlusion of the subclavian vein.

Abstract: Objective: To investigate the relation between the wall motion abnormalities and sodium channel abnormalities in cases of the Brugada syndrome. Design: Consecutive prospective case‐control study in a single hospital. Setting: Tertiary referral centre. Patients: 13 consecutive patients with Brugada syndrome and 13 age and sex matched control subjects. Interventions: Each subject underwent electron beam computed tomography (EBT) and a 12 lead ECG before and after disopyramide injection. Main outcome measures: QRS width and the magnitude of ST segment elevation in the 12 lead ECG; wall motion by EBT. Results: After disopyramide, EBT revealed deterioration of focal wall motion abnormalities in the right ventricular outflow tract region in eight of the 13 patients (62%). Prolongation of the QRS width after disopyramide injection in lead V2, which usually reflects the electrical activity in right ventricular outflow tract region, was greater in these eight patients (p < 0.01) than in the other five patients, in whom wall motion did not change after disopyramide. The degree of augmentation of ST segment elevation did not differ significantly between the two groups Conclusions: The deterioration of wall motion abnormalities in the right ventricular outflow tract region after disopyramide suggests the presence of functional abnormalities of the sodium channel. Some patients with Brugada syndrome may have arrhythmogenic substrates with abnormal responses to sodium channel blockers.

Abstract: The interval from the Q onset to the T wave apex (Q-aT) is known to be heart rate dependent. this study investigated the relation between Q-aT and R-R interval in the case where the T waves have complicated morphology. Holter ECGs recorded from one normal subject and three patients with long QT syndrome were used for the investigation. It is found that when the T wave is monophasic, Q-aT is well correlated to Q-T interval. On the other hand, if the T wave is biphasic or bipolar, the correlation between Q-aT1 (the interval from Q onset to the first T maximum) and conventional Q-T is poor, but Q-aT2 (the interval from Q onset to the second T maximum) is well correlated with conventional Q-T. This study suggests that by taking T wave morphology into consideration, Q-aT is possible to be used for the investigation of ventricular repolarization duration. >

Abstract: Aims Differences in the sensitivity of the genotype of the congenital long QT syndrome to sympathetic stimulation have been suggested. This study compared the influence of sympathetic stimulation on continuous corrected QT (QTc) intervals between LQT1, LQT2 and LQT3 forms of the congenital long QT syndrome. Methods and Results We recorded a 12-lead electrocardiogram continuously before and after bolus injection (0·1μg.kg−1) of epinephrine followed by continuous infusion (0·1μg.kg−1 min−1) in 12 LQT1, 10 LQT2, 6 LQT3, and 13 control patients. The QT intervals and previous RR intervals of all beats were measured semi-automatically, and the QTc intervals of all beats were calculated by Bazett's method. Abstract: A 54-year-old woman began to notice numbness and motor weakness in the lower extremities in July 1999. These symptoms rapidly progressed and she could not walk any more. When she admitted to our hospital, she showed peripherally dominant, moderate motor weakness, drop foot and loss of superficial and deep sensation in the lower extremities, but only slight weakness in the hands. Cranial nerves were intact. Deep tendon reflexes were all absent. Nerve conduction velocities were reduced and cerebrospinal fluid protein was elevated. VEGF was greatly increased in serum (1,850 pg/ml), which has been found to be increased exclusively in patients with Crow-Fukase syndrome. Many characteristic manifestations of the syndrome except polyneuropathy are well explained to be resulted from the abnormal production of VEGF. She did not, however, exhibit any constellation of Crow-Fukase syndrome such as edema, skin change, organomegaly, bone lesions or M-proteinemia. Steroid therapy improved her symptoms and lessened the levels of serum VEGF and cerebrospinal fluid protein. This case indicated that overproduction of VEGF could induce polyneuropathy rather than the other symptoms of Crow-Fukase syndrome, and that a polyneuropathy associated with increased VEGF might exist.

Abstract: A 48-year-old female presented with headache and limitation of upward gaze. She had a history of total gastrectomy for gastric adenocarcinoma 2 years previously. Computed tomography with contrast medium and T1-weighted magnetic resonance (MR) imaging with gadolinium showed ring-like enhancement of a solitary mass in the pineal region with obstructive hydrocephalus. T2-weighted MR imaging showed the tumor as hypointense. This MR imaging finding complicated the preoperative diagnosis, but malignancy was suspected from the medical history. The tumor was subtotally resected via the occipital transtentorial approach with a rigid endoscope. Histological examination of the surgical specimen revealed adenocarcinoma cells with extensive coagulation necrosis, which might have contributed to the hypointensity on T2-weighted MR imaging. Correct diagnosis of metastatic adenocarcinoma based only on MR imaging may be difficult in such cases, but metastatic adenocarcinoma of the pineal region must be considered in the differential diagnosis of pineal tumors.

Abstract: Ogawa, M., et al.: Acute Effects of Different Atrial Pacing Sites in Patients with Atrial Fibrillation: Comparison of Single site and Biabrial Pacing. It has been reported that atrial single site or biatrial pacing can suppress the occurrence of AF. However, its mechanism remains unclear. The study population included 32 patients with AF (n = 20: AF group), or without paroxysmal AF (n = 12: control group). The mechanism and efficacy of atrial pacing were investigated by electrophysiological studies to determine which was more effective for suppressing AF induction; single site pacing of the right atrial appendage (RAA) or distal coronary sinus (CS–d), or biatrial (simultaneous RAA and CS–d) pacing. In the AF group, AF inducibility was significantly higher with RAA extrastimulus during RAA (12/20; P < 0.0001) or biatrial paced drive (7/20; P < 0.01) than during CS–d paced drive (0/20). In the control group, AF was not induced at any site paced. In the AF group, the conduction delay and other parameters of atrial vulnerability significantly improved during CS–d paced drive. The atrial recovery time (ART) at RAA and CS–d was measured during each basic pacing mode. ART was defined as the sum of the activation time and refractory period, and the difference between ARTs at RAA and CS–d was calculated as the ART difference (ARTD). The ARTD was significantly longer during RAA pacing in the AF group than in control group (155.0 ± 32.8 vs 128.8 ± 32.9 ms, P < 0.05). In the AF group, ARTDs during biatrial (52.0 ± 24.2 ms) and CS–d pacing (51.7 ± 26.0 ms) were significantly shorter than ARTD during RAA pacing. The CS–d paced drive was more effective for suppressing AF induction than biatrial or RAA paced drive by alleviating conduction delay. CS–d and biatrial pacing significantly reduced ARTD compared with RAA pacing.

Abstract: 5'-DFUR is a pro drug of 5-FU, which is known to be converted by thymidine phosphorylase (dThdPase). A recent pre-clinical study revealed that CPA upregulates dThdPase activity specifically in tumor cells. Furthermore, clinical trials have shown significant response rates in breast cancer patients, when using the chemotherapy combination of 5'-DFUR, CPA and MPA. The purpose of this study was to examine the efficacy of this regimen as a pain reduction therapy for breast cancer patients with bone metastasis. Ten patients who had bone metastasis with restricted ADL were included in the study. All of the patients had had previous exposure to such standard chemotherapy as CAF, CMF, taxol and oral 5-FU administration. The patients were administered daily oral doses of 5'-DFUR at 800-1,200 mg, CPA at 200 mg and MPA at 400-800 mg for two weeks as induction therapy, followed by two weeks rest (one to two cycles). Daily dose of 800 mg of 5'-DFUR, 100 mg of CPA, 400-800 mg of MPA was continuously administered thereafter. The main findings included a significant decrease in pain in eight patients, which continued for more than 6 months. In five patients, the effect lasted more than one year. As the pain decreased, the patients' QOL was improved. Hematological toxicity of more than grade 3 was observed in three patients but only during the induction therapy. One patient had pulmonary thrombosis and required hospitalization. In conclusion, oral administration of 5'-DFUR/CPA/MPA is well tolerated and useful in reducing pain.

Abstract: Aims This study was designed to determine, using electron-beam CT, whether there are morphological abnormalities in patients with the Brugada syndrome and to elucidate the relationship between those abnormalities and arrhythmogenesis. Methods and Results Twenty-six consecutive patients with the Brugada syndrome and 23 age- and gender-matched control subjects (controls) were evaluated for morphological abnormalities using electron beam CT. Electron beam CT demonstrated morphological abnormalities of the right ventricle in 21 (81%) of 26 patients, but in only two (9%) of 23 controls. The sites of morphological abnormalities were the right ventricular outflow tract area in 17 patients and the inferior wall of the right ventricle in four patients. Of the seven patients with monoform premature ventricular contractions recorded only in the acute phase, four of the five patients with premature ventricular contractions from the right ventricular outflow tract area had morphological abnormalities in the right ventricular outflow tract area, and the other two patients with premature ventricular contractions from the inferior wall of the right ventricle had morphological abnormalities in the inferior wall of the right ventricle. Conclusion The sites of morphological abnormalities detected by electron beam CT in patients with the Brugada syndrome were related to the origins of premature ventricular contractions recorded only in the acute phase, which may trigger ventricular fibrillation. These morphological abnormalities may be related to arrhythmogenic substrates in patients with the Brugada syndrome.

Abstract: The clinical usefulness of QT dispersion in 12-lead ECG has been controversial in identifying subjects at risk for sustained ventricular tachycardia (VT) in patients with idiopathic dilated cardiomyopathy (DCM). We hypothesized that increasing the spatial resolution of the ECG improves the accuracy of risk stratification. The purpose of this study was to test the ability of recovery time dispersion measured from 87-lead body surface potential mapping (BSPM) to identify patients at risk for sustained VT in idiopathic DCM.We obtained 87-lead BSPM and 12-lead ECG in 33 patients with idiopathic DCM (15 patients with a history of sustained VT [VT(+) group] and 18 patients without a history of sustained VT [VT(-) group]) and in 20 normal control subjects. We measured the corrected QT dispersion and corrected recovery time dispersion from 12-lead ECG (QTc-12 dispersion and RTc-12 dispersion, respectively) and 87-lead BSPM (QTc-87 dispersion and RTc-87 dispersion, respectively). Signal-averaged ECG also was recorded in 25 patients. Neither the QTc-12 nor QTc-87 dispersion discriminated between the VT(+) and VT(-) groups patients. The VT(+) group patients had a larger but insignificant RTc-12 dispersion than the VT(-) group patients. In contrast, the RTc-87 dispersion was significantly larger in the VT(+) group patients than in the VT(-) group patients (236 +/- 39 msec vs 184 +/- 28 msec, P0.001). Receiver operating curve analysis indicated that the RTc-87 dispersion was as good as late potentials in predicting susceptibility to sustained VT; its sensitivity, specificity, and negative predictive value were 73%, 76%, and 76%, respectively (cutoff value 200 msec). RTc-87 dispersion200 msec combined with positive late potentials provide high sensitivity (92%) and high negative predictive value (88%) for sustained VT.The RTc-87 dispersion is a useful tool to identify subjects at risk for sustained VT in patients with idiopathic DCM.

Abstract: Aims Brugada syndrome is considered to be a distinctive subgroup of idiopathic ventricular fibrillation. Identification of the circadian pattern of ventricular fibrillation would contribute to the elucidation of its underlying pathophysiology, but this pattern remains unknown in patients with Brugada syndrome. Methods and Results A total of 12 consecutive Brugada syndrome patients (46±14 years, all male) who underwent implantation of an implantable cardioverter–defibrillator were studied. The distribution of the time of ventricular fibrillation detection was examined and classified into four 6-hour time periods of the day. The mean follow-up period following implantation was 777±535 days. In six out of the 12 patients, ventricular fibrillation occurred during follow-up. The data logs revealed that ventricular fibrillation was detected 30 times (range, 3–9). Abstract: To test the hypothesis that local or diffuse wall motion abnormalities in the right ventricle in patients with arrhythmogenic right ventricular dysplasia (ARVD) may induce the ST-segment elevation in response to exercise, we examined exercise electrocardiograms in patients with ARVD. In 17 patients with ARVD, who demonstrated right ventricular wall motion abnormalities without organic coronary lesions, we conducted a treadmill exercise test. Significant exercise-induced ST-segment elevation (ESTE) was defined as a 0.1 mV or more ST-segment elevation at J point. ESTE was observed in 11 patients (65%). It manifested most frequently in right-sided precordial leads. Severe right ventricular asynergy was seen in all but one (91%) among 11 with ESTE, whereas it was seen only in two (33%) among six without ESTE (P < .05). The maximal magnitude of ESTE inversely correlated with right ventricular ejection fraction (r = −0.58, P < .05). ESTE was seen in two thirds of ARVD patients, helping us noninvasively diagnose ARVD. The fact that ventricular wall motion abnormalities could cause ESTE in the absence of organic coronary lesions suggested the critical role of mechanical factors in the genesis of ESTE.

Abstract: Background —Arrhythmogenic right ventricular dysplasia (ARVD) is characterized by local or diffuse wall motion abnormalities in the right ventricle (RV), associated with recurrent ventricular tachycardia (VT) of RV origin. Brain natriuretic peptide (BNP) was first isolated from a porcine brain extract. In humans, BNP is expressed predominantly in the ventricles of failing hearts, and its expression has been observed primarily in myocytes in the interstitial fibrous area in dilated cardiomyopathy. We hypothesized that BNP is increasingly secreted from the residual myocytes within the atrophic tissue in patients with ARVD. Methods and Results —Plasma BNP levels were measured in 17 patients with ARVD, 12 patients with idiopathic RV outflow tract tachycardia (RVOT), and 120 control subjects. We performed cardiac catheterization, RV endomyocardial biopsy, electron- beam CT, and biventricular endomyocardial mapping in the ARVD patients. There was a significant increase in plasma BNP levels in the ARVD patients compared with the RVOT patients and control subjects (61.4±59.6 pg/mL versus 8.3±5.5 pg/mL and 9.3±5.8 pg/mL; P r =−0.588, P =0.025) and with the fractionated-area scores ( r =0.705, P =0.005). Light microscopic immunohistochemistry showed strong BNP immunoreactivity in residual myocytes with fibrofatty replacement. Conclusions —These results suggest that plasma BNP levels were not increased in RVOT patients but were increased in ARVD patients, and that the increased BNP levels indicate the severity of both the RV dysfunction and the arrhythmogenic substrate.

Abstract: To clarify the etiology of the prepotentials (PPs) between the end of P wave and the onset of QRS complex (P'-R segment) in signal averaged electrocardiogram (SAECG), SAECG was recorded in 26 healthy volunteers and 15 patients with idiopathic left ventricular tachycardia (VT) and electrophysiologic study (EPS) was done in those 15 VT patients. The PPs which had three components (PP1, PP2, PP3) were recorded in 14 of the 26 volunteers (53.8%) and 10 of the 15 VT patients (66.7%). The intervals from the PPs to the onset of QRS complex were 45.4 +/- 6.6, 30.6 +/- 5.6, 16.0 +/- 4.7 ms, respectively. These intervals in SAECG were very similar to the intervals from the electrograms of His bundle, right bundle, Purkinje network to the onset of QRS complex in EPS. The PPs in SAECG were thought to represent the electrical activity of His bundle, right bundle and Purkinje network.

Abstract: Results: Out of 664159 women there were 22938 patients with hyperlipidaemia (3.5%) and 9312 patients with breast cancer. Out of the hyperlipidaemia patients, 530 patients developed breast cancer (2.3%) compared with 8782 patients developing breast cancer without hyperlipidaemia (1.4%). A logistic regression model accounting for time from first presentation to development of breast cancer showed that the presence of hyperlipidaemia increases the outcome of breast cancer by 1.64 times (95% C.I. 1.50-1.79). Conclusions: Whilst we appreciate numerous limitations of our methods, coupled with the main findings of the recent basic science research, our analysis further augments the case for the role of cholesterol in the development of breast cancer. Our data from a large clinical relevant sample further strengthens the argument to prospectively trial statins in the management of breast cancer.

31. [A case of allergic bronchopulmonary candidiasis improved with steroid inhalation]

Abstract: A 49-year-old woman was admitted to hospital because of productive cough and dyspnea. She had been well two months before admission, when she developed an attack of asthma. Chest roentgenogram taken on admission revealed numerous shadows of inhomogeneous density in both lungs. Laboratory findings showed leukocytosis with eosinophilia (25%), high IgE level in serum and positive RAST score to Candida albicans. A diagnosis of allergic bronchopulmonary candidiasis was made by these laboratory data and clinical course. The patient was treated successfully by oral administration of methylprednisolone and inhalation of amphotericin B, but she had a relapse of the disease on cessation of steroid medication. Inhalation of beclomethasone dipropionate and procaterol hydrochloride was commenced. Thereafter, pulmonary infiltration and clinical symptoms improved after three weeks.

32. [Cerebellopontine angle lipoma--case report]

Abstract: Intracranial lipomas are rare. Cerebellopontine angle lipomas are extremely rare. We report a case of right cerebellopontine angle lipoma. A 47-year-old woman has been suffering from right trigeminal neuralgia for about 1 year. Neurological examination showed right 5th and 8th cranial nerve dysfunction. MRI demonstrated the high intensity mass lesion at right cerebellopontine angle. The lesion was demonstrated as negative intensity in an opposed image by using Dixon's technique. CT showed the lesion as a very low density area (-104 Hounsfield units). Subtotal removal of the tumor was performed following right suboccipital craniectomy. Pathological diagnosis was lipoma. We review the literature on this rare tumor and discuss the clinical symptoms, radiological findings and management. We suggest that the operation should be prudently performed for the case with only hearing disturbance because there is a high possibility that hearing disturbance will worsen.

Abstract: The aim of the present study was to determine the mechanisms whereby apo A-1 retards cholesterol crystal nucleation in human gallbladder bile. Ten human gallbladder bile samples were sterilely collected from patients with gallstones at surgery, subsequently applied on gel permeation chromatography to separate various lipid particles. A non-micellar fraction coeluted with apo A-1 was ultrastructurally characterized by transmission electron microscopy (TEM). As results, that fraction containing very little or no apo A-1, 400-600 A in diameter, showed rapid transformation, i.e. rouleau formation, multilamellar formation and consequently microcrystal formation, whereas those rich in apo A-1, 200-400 A in diameter, showed very little transformation for 7 days. Those data indicated that apo A-1 stabilizes non micellar fraction by forming an apo A-1 and lipid complexed particle, resulting in prolonged cholesterol crystal nucleation.

37. Neuropeptides stimulate production of interleukin-1β, interleukin-6, and tumor necrosis factor-α in human dental pulp cells.

Abstract: Objective:Orthodontic tooth movement causes inflammatory reactions in the periodontal membrane and dental pulp. It has been reported that substance P (SP) and calcitonin gene-related peptide (CGRP), both sensory neuropeptides, are manifested in the dental pulp of rats during experimental tooth movement, suggesting that they might be involved in the dental pulp inflammation during orthodontic tooth movement. However, the relationships between neuropeptides and pro-inflammatory cytokines have not been fully elucidated. Materials and methods:Human dental pulp (HDP) fibroblasts were prepared from 6 healthy young volunteers (3 males, 3 females; 15–25 years old) during the course of orthodontic treatment. HDP cells were incubated for 24 h in fresh medium containing 2% FCS in the presence of various concentrations of CGRP (10 –12 to 10 –4 M) and SP (10 –12 to 10 –4 M), and the levels of interleukin (IL)-1ß, IL-6, and tumor necrosis factor (TNF)-a present in the media were determined using commercially available high-sensitivity enzyme-linked immunosorbent assay kit. Results:We examined the effects of stimulation by these neuropeptides on the production of inflammatory cytokines in HDP fibroblasts, and found that the levels of IL-1ß, IL-6, and TNF-a increased in a time- and concentration-dependent manner. However, the neuropeptides did not act synergistically to increase cytokine secretion in HDP cells or significantly modify LPS-induced cytokine production by HDP cells. Conclusions:Our results suggest that human pulp fibroblasts may be involved in the progress of inflammation in pulp tissue during orthodontic tooth movement, as they produced large amounts of IL-1ß, IL-6, and TNF-a following stimulation with neuropeptides. Abstract: The study objective was to assess the widespread applicability of ST/HR slope for the modified Bruce exercise test using a computerized electrocardiogram (ECG); compare the usefulness of the ST/HR slope with standard ECG criteria in detection of coronary artery disease (CAD) and identification of three-vessel or left main CAD; and then develop a new, modified ST/HR score (MSHS) for improving the diagnostic accuracy of ST/HR slope. The studies were retrospective and prospective in design, conducted in referral-based cardiology clinics at a national cardiovascular center. A selected sample of 142 patients underwent exercise ECG and coronary angiography, as did a normal control group of 402 patients who were apparently free from CAD. Sixty three other patients who underwent coronary angiography were also studied prospectively. No limitations of medical treatment were exacted for the test except digitalis treatment. Linear regression analysis, from which ST/HR slope was derived, was done with seven measurements of HR and ST displacement at 60 ms from J point in leads aVF and V 5 during 6 min before the end of exercise. MSHS was derived from a multiple regression model with peak HR (% maximum HR), ST index (ST depression + ST slope), and ST/HR slope. Although the usual ST criteria (sensitivity = 63 percent, specificity = 73 percent), ST index (71 percent, 80 percent), and ST/HR slope (70 percent, 97 percent) were equally accurate in detection of CAD, MSHS showed significantly improved sensitivity (88 percent) with similar specificity (81 percent). In identification of three-vessel or left main CAD, when compared with other criteria, ST/HR slope and MSHS provided improved diagnostic accuracy: sensitivity (74 and 78 percent, respectively), specificity (88 and 93 percent) and overall test accuracy (85 and 89 percent). The improved accuracy of ST/HR slope and MSHS was prospectively validated in 63 other patients. ST/HR slope was applicable to computerized ECG data for the standard treadmill test, and showed improved accuracy in detection of three-vessel or left main CAD. Abstract: Electrophysiologic studies were performed in 16 patients 11 to 45 years old (mean 33 years) with idiopathic sustained (lasting more than 5 min) ventricular tachycardia (VT) originating from the left ventricle. Endocardial mapping during VT showed that the earliest site of activation was at the apical inferior portion of the left ventricle in 14 patients whose QRS morphology during VT showed a right bundle branch block pattern and left-axis deviation, but at the apical anterosuperior portion of the left ventricle in two patients whose QRS morphology during VT showed a right bundle branch block and right-axis deviation. Single programmed ventricular stimulation induced VT in 13 patients, and rapid ventricular pacing induced VT in the remaining three patients. Rapid ventricular pacing terminated VT in all patients. The relationship between the coupling interval and the echo interval was inverse in all eight patients with a wide VT inducible zone. Entrainment was recognized in three of six patients. The initiation of VT by constant pacing depended on the number of pacing beats but not the duration of pacing in all four patients tested. Intravenous verapamil terminated the VT in 13 of 14 patients. Long-term oral verapamil was also effective in all five patients who required long-term oral therapy for their symptoms associated with VT. In conclusion (1) idiopathic left ventricular tachycardia has unique electrocardiographic, electrophysiologic, and electropharmacological properties, (2) the electrophysiologic characteristics suggest that the mechanism is reentry, and (3) verapamil is effective in both the short- and long-term treatment of VT.

Abstract: Mononuclear cell infiltration of the central nervous system and ganglioneuritis are characteristic histopathological findings of proventricular dilatation disease (PDD) caused by parrot bornavirus (PaBV) infection. The purpose of this study was to clarify the link between the degree of inflammatory lesions and the distribution of the virus antigen in naturally PaBV-infected parrots. Pathological examination was performed on 18 PaBV-infected birds identified by reverse transcriptase-PCR. Dilatation of the crop, proventriculus, and ventriculus was observed in all 18 (100%) birds, and dilation of the right ventricle of the heart was observed in 14/18 (78%) birds. Cases were classified based on the scores for the distribution and degree of histological lesions into neural type, with severe brain lesions, digestive type, with severe gastric lesions, or nervous/digestive type, with severe lesions in both the brain and ventriculus. The PaBV immunohistological score correlated with the inflammatory lesion scores. Ganglioneuritis, myocarditis, and myocardial degeneration were frequently observed in the heart. Interestingly, macroscopic and microscopic lesions and virus antigen were detected in the hearts of all three histological types. The present study showed that parrots naturally infected with PaBVs can be grouped into three types based on the lesion distribution, and heart failure is an important symptom in PaBV-infected parrots.
