La cholangite sclérosante primitive (CSP) est une pathologie rare dont la prévalence est estimée à 10/100 000 en Europe du Nord. Le cas présenté est celui d’un jeune incorporé de 23 ans, asymptomatique, présentant une perturbation isolée et chronique des tests hépatiques. En réitérant et élargissant les examens paracliniques, le diagnostic d’une CSP associée à une rectocolite hémorragique pancolique est posé. Un traitement par acide ursodésoxycholique et des dérivés de 5ASA sont débutés. L’intérêt d’effectuer une enquête exhaustive devant toute anomalie chronique et isolée des tests hépatiques chez un jeune patient, même asymptomatique et sans aucun antécédent, est souligné. L’association significative entre rectocolite hémorragique (RCH) et CSP nécessite la recherche systématique d’une maladie inflammatoire chronique intestinale (MICI) lors du diagnostic de CSP. Le risque oncogénique de cette association pathologique est rappelé.

Resume Introduction La selectivite des inhibiteurs de la cyclo-oxygenase de type 2 (anti-Cox2) a fait chuter la toxicite gastro-duodenale des anti-inflammatoires non steroidiens. Les effets sur la muqueuse colique sont peu connus. Observation Une colite aigue ulcero-hemorragique chez une patiente de 38 ans, indemne de tout passe digestif, etait observee au 2e jour d’un traitement par celecoxib (200 mg/j) prescrit pour une sciatalgie. Discussion L’imputabilite intrinseque du celecoxib est tres vraisemblable dans cette observation, par combinaison des criteres chronologiques et semiologiques. L’imputabilite extrinseque est discutee a partir des quelques donnees bibliographiques disponibles qui concernent essentiellement le rofecoxib. Conclusion Cette observation qui constitue, a notre connaissance, le premier cas de colite ulcero-hemorragique lie au celecoxib, confirme la toxicite colique des anti-Cox2 et impose de ne pas meconnaitre cette nouvelle etiologie de colite aigue. La communication des effets indesirables coliques des anti-Cox2 s’impose en raison de leur recente commercialisation.

Effects on colic mucosa are little known.A case history is described of a 38-year-old women, without digestive past, who presented with hemorrhagic ulcerated acute colitis beginning 2 days after starting celecoxib (200 mg/d) prescribed for sciatica.Intrinsic imputability of celecoxib is very probable in this case report by combination of chronological and semiological criteria. Extrinsic imputability is discussed, starting from the available bibliographical data which relate primarily to rofecoxib.This observation, which constitutes, as far as we know, the first case report of hemorrhagic ulcerated colitis related to celecoxib, confirms the colic toxicity of anti-Cox2 and identify a new cause of acute colitis. Report of colic side effects with Cox2 inhibitors is required because of their new marketing.

We report the case of a woman who had been operated upon for a recurrent meningioma of the falx cerebri which, 22 years after it was discovered, produced pulmonary metastases. Metastases from meningiomas are extremely rare; they affect the lungs in 60% of the cases or, less frequently, the liver or lymph nodes. The factors predictive of secondary dissemination are the site of the tumour, the quality of excision, the histological type (haemangiopericytic meningioma) and the presence of cell necrosis, hypercellularity and strong mitotic activity.
