Resume Les suicides par arme a feu constituent au niveau international et en France une part tres importante des deces par suicide. La prevention des conduites suicidaires represente une priorite nationale de sante publique, en particulier en ce qui concerne la reduction de l'acces aux moyens potentiellement letaux. A ce titre, et comme travail prealable a une etude clinique et therapeutique en psychiatrie de liaison, nous avons examine les differentes etudes publiees au sujet de la disponibilite du moyen arme a feu et le risque de suicide, tout d'abord chez l'adolescent puis chez l'adulte. Nous avons egalement etudie les effets de certaines reglementations en matiere de detention et d'acquisition d'armes sur les taux de suicide. D'une part, 18 etudes sur 19 recensees rapportent qu'il existe une correlation positive entre la disponibilite d'une arme a feu et le taux de suicide par arme a feu. En ce qui concerne le taux global de suicide, les resultats sont plus contrastes puisque seules dix etudes sur 19 montrent une correlation positive entre la disponibilite d'une arme a feu et ce taux global. Par ailleurs, 15 etudes sur 16 recensees montrent une diminution du taux de suicide par arme a feu suite a la mise en place d'une legislation plus stricte en matiere de detention d'arme. Cette revue de la litterature montre donc tout l'interet de la reduction de l'acces a des moyens letaux tels que les armes a feu dans le cadre de la prevention du suicide, ainsi que la necessite d'une reflexion autour de la mise en place d'une legislation restrictive en matiere d'acquisition et de detention des armes a feu comme point de depart a une veritable politique de prevention des conduites suicidaires.

The psychosocial rehabilitation and the quality of life of schizophrenics have constituted the main concern in the taking care of these patients since their return into our society. Thus, the emergence of atypical neuroleptics allows us to imagine very interesting and important perspectives concerning these 2 concepts. The adaptation to social and professional life, and the quality of life have been rarely compared according to the antipsychotic therapeutic used (classical neuroleptics or atypical neuroleptics). As many authors underline it, there are still very few studies undertaken concerning this subject because of the diverse methodological and ethical limitations implied. That is why we established a transversal study of the psychosocial adaptation and of the quality of life of 2 stabilized schizophrenic groups. These 2 different groups are: the NC group under classical neuroleptics on the one hand, and the NA group under atypical neuroleptics on the other hand. The main target of this study is to observe the influence of the atypical neuroleptics independently of the other factors. Three evaluation scales were used: the PANSS, the psychosocial skill scale (EAPS) of G. Darcourt and the French translation by P. Martin of the Functional Statut Questionnaire (FSQ), and a collection of the clinical and therapeutic socio-demographic data. 4. Les états dépressifs de la post-adolescence. Résultats d’une enquête menée chez 1521 étudiants de l’université de Poitiers

Connaissant les liens entre depression et adolescence, la frequence des etats depressifs et la forte prevalence du suicide chez les 15-24 ans, nous nous sommes interesses a une population de jeunes adultes, etudiants a l'universite de Poitiers. Dans une approche descriptive et analytique, nous avons interroge plus de 1500 etudiants de 18 a 24 ans dans differentes UFR, par l'intermediaire d'un autoquestionnaire. Dans ce but, nous souhaitions estimer la prevalence des syndromes depressifs dans cette population et definir des facteurs associes. Notre outil d'evaluation reprenait l'echelle CESD, les criteres DSM-IV d'episode depressif majeur, ainsi que des items sur la consommation de produits toxiques, les idees de suicide, les antecedents d'abus sexuel et d'eventuels symptomes psychotiques. Les resultats montrent une forte prevalence des syndromes depressifs chez l'etudiant avec, au-dela de l'episode depressif majeur, d'autres entites « subsyndromiques » en faveur d' une approche dimensionnelle du trouble. Nous retrouvons les syndromes depressifs associes a une consommation abusive de toxiques d'autant plus importante que la depression est intense. Enfin, les mauvaises relations familiales, qu'elles soient entre les parents ou entre l'etudiant et les parents, sont elles aussi associees a la prevalence de la depression.

La decouverte d'un cancer marque souvent l'entree dans un lourd et long processus de soins : interventions chirurgicales, chimiotherapies, radiotherapie. Si le corps doit affronter de rudes epreuves, la psyche n'est pas epargnee. De nombreuses etudes ont montre une incidence accrue de troubles depressifs chez les patients cancereux. Pourtant, ces troubles restent sous-evalues dans l'ensemble en soins generaux. Une etude prospective realisee au CHU de Poitiers s'est interessee a une population de patientes atteintes de cancer du sein, prise en charge a un stade precoce de la maladie. Le cadre est celui offert par la psychiatrie de liaison et le but, l'etude de l'incidence des troubles anxieux et depressifs durant la phase du traitement adjuvant. Chacune des cinq evaluations successives proposees comporte un autoquestionnaire, 1'HADs, et un entretien structure, le MINI reprenant les criteres du DSM-IV. Les resultats de cette etude confirment la forte prevalence des troubles depressifs mais egalement anxieux dans cette population au pronostic pourtant favorable. Parallelement, l'evaluation longitudinale des troubles plaide en faveur d'une symptomatologie psychiatrique evolutive et fluctuante en fonction du stade du traitement rendant peu operant l'utilisation des systemes criteriologiques classiques. Enfin, l'echelle HADs apparait comme un moyen interessant, facilement utilisable par le personnel soignant non psychiatrique, et adaptee au milieu cancerologique, pour favoriser le depistage et l'orientation des patientes vers l'equipe de psychiatrie de liaison.

Resume La loi n° 2002-303 du 4 mars 2002 est consideree en France comme une etape fondamentale du droit medical francais, veritable pont entre le passe et l’avenir des relations entre personnes malades et medecins. Elle temoigne d’une evolution sociologique manifeste des rapports medecin-patient et nous fait passer d’une medecine paternaliste a une pratique medicale consumeriste qui fait place au malade en tant que consommateur et acteur de ses soins. Cette loi est une loi proteiforme qui fait evoluer les droits reconnus au malade, le consentement ou le secret professionnel. Elle etablit l’acces direct au dossier du patient et impose l’information de la personne malade. La loi du 4 mars 2002 est une loi generaliste qui traite de toute pratique medicale, sans specificite pour la psychiatrie. Seuls quelques points concernant l’hospitalisation du malade mental y sont precises. Elle prone une democratie sanitaire et cherche a developper la qualite du systeme de sante. Son apport essentiel sera surement le dispositif mis en place pour l’indemnisation de l’alea therapeutique.

International audience; La schizophrénie est souvent associée à une méconnaissance du trouble sévère et persistante. Ce déficit d'insight est corrélé à l'hypofrontalité mais indépendant du pronostic de la maladie ou du quotient intellectuel. L'insight cognitif est défini comme la différence entre la capacité de réflexion sur soi et la certitude dans cette réflexion. Cette capacité est trouvée diminuée dans la schizophrénie mais augmentée en cas de dépression. Ainsi, la schizophrénie avec comorbidité dépressive est associée à un plus haut niveau d'insight. Les auteurs discutent que la prise de conscience de la maladie psychotique peut être vécue comme traumatique, constituant un facteur de risque de dépression et de passage à l'acte suicidaire.

The main target of this study is to observe the influence of the atypical neuroleptics independently of the other factors. Three evaluation scales were used: the PANSS, the psychosocial skill scale (EAPS) of G. Darcourt and the French translation by P. Martin of the Functional Statut Questionnaire (FSQ), and a collection of the clinical and therapeutic socio-demographic data. Concerning the psychosocial capacities, the total results revealed one significant difference (p-value0.01) between the two groups. The NA group showed a better psychosocial adaptation (80 +/- 10.89 versus 72 +/- 13.39). Besides 4 key-domains are statistically different, but are always in favour of the NA group; the domains are family life (p=0.01), social life (p=0.0026), presentation (p=0.003) and housing and food (0.029). These observations incite to modify our cure. The analysis of the total score of the FSQ did not reveal a statistically significant difference between both groups but this total score seems high in both groups, conveying an important feeling of satisfaction about the quality of life of our sample of schizophrenics. Literature data also abound along the same lines. Indeed, authors underline that patients clearly prefer to live outside rather than in the hospital. Besides, this study allows to confirm the literature data while studying some domains (such as sexuality) rarely brought to light in studies. As a conclusion, we can say that our sample is representative of a population of schizophrenics, and in spite of the difficulties of methodology inherent to this type of study, we can make a comparison between our 2 groups thanks to the stu-died data. Data in which differences could be found, did not show any influence on the results of scales, thus, they cannot be considered as a confounding factor. The NA group showed a better psychosocial adaptation. Furthermore the FSQ did not reveal a difference between both groups. It seems important to remind that a prospective study, conducted for several months and with several evaluations, is essential to confirm the results obtained on both types of treatment.

In an effort to continue the pioneering work of Harary in USA and Flament in France, we have undertaken to develop, on an experimental basis, a formalized theory of systems of beliefs and their modifications. This theory uses the psycho-social concepts of theories of cognitive consistency and of the tools of discrete mathematics, such as rewriting and intervals within graphs. The axioms and rewriting rules are elaborated from experimental data, and we demonstrate that the system we have built has the property of termination. This result is in accordance with experimental observations that show that every subject having an inconsistent system of beliefs (i.e., one containing contradictions) makes this system evolve towards consistency to reach a simple, consistent reference framework.

- Abstract
Introduction: Despite extensive testing, the efficacy of low-frequency (1 Hz) repetitive transcranial magnetic stimulation (rTMS) of temporo-parietal targets for the treatment of auditory verbal hallucinations (AVH) in patients with schizophrenia is still controversial, but promising results have been reported with both high-frequency and neuronavigated rTMS. Here, we report a double-blind sham-controlled study to assess the efficacy of high-frequency (20 Hz) rTMS applied over a precise anatomical site in the left temporal region using neuronavigation., Methods: Fifty-nine of 74 randomized patients with schizophrenia or schizoaffective disorders (DSM-IV R) were treated with rTMS or sham treatment and fully evaluated over 4 weeks. The rTMS target was determined by morphological MRI at the crossing between the projection of the ascending branch of the left lateral sulcus and the superior temporal sulcus (STS)., Results: The primary outcome was response to treatment, defined as a 30% decrease of the Auditory Hallucinations Rating Scale (AHRS) frequency item, observed at 2 successive evaluations. While there was no difference in primary outcome between the treatment groups, the percentages of patients showing a decrease of more than 30% of AHRS score (secondary outcome) did differ between the active (34.6%) and sham groups (9.1%) (P = .016) at day 14., Discussion: This controlled study reports negative results on the primary outcome but demonstrates a transient effect of 20 Hz rTMS guided by neuronavigation and targeted on an accurate anatomical site for the treatment of AVHs in schizophrenia patients.

Background: Patients with schizophrenia display significant working memory and executive deficits. In patients with obsessive-compulsive disorder (OCD), several studies suggest that working memory dysfunction may be one of the causes of compulsive checking behaviors. Hence, this study aimed at assessing whether patients with schizophrenia were impaired on an image comparison task used to measure checking behaviors, and whether the origin and profile of impairment on this task was different between schizophrenia and OCD., Methods: Eye movement recordings were used to assess the checking behavior of 24 patients with schizophrenia and 24 control participants who had to decide whether two images were different or identical. The verbal and visuo-spatial components of participants' working memory were measured using the reading span and backward location span tests., Results: Compared to controls, patients with schizophrenia had reduced working memory spans and showed excessive checking behavior when comparing the two images. However, the intensity of their checking behavior was not significantly related to their working memory deficits., Conclusions: Several recent studies demonstrated that the excessive checking behaviors displayed by patients with OCD were related to working memory dysfunction. The absence of a relationship between the excessive checking behavior of patients with schizophrenia and their working memory deficits suggests that checking behaviors do not have the same origin in the two disorders., (Copyright © 2014. 17. Successful treatment of pudendal neuralgia with tricolumn spinal cord stimulation: case report.

Background and Importance: There is large variation in the success of decompressive surgery for pudendal neuralgia (PN), the most chronic, disabling form of perineal pain. We attempt to determine whether spinal cord stimulation using new-generation multicolumn leads could form part of the treatment algorithm for refractory PN., Clinical Presentation: A man with PN that was unresponsive to conventional treatment demonstrated a neuropathic component and had a negative response to nerve infiltrations (so he was not indicated for decompressive surgery) and a positive response to perianal transcutaneous electrical nerve stimulation and was implanted with a 16-contact surgical lead at the level of conus medullaris, allowing multicolumn stimulation. Using transverse combinations, it was possible to obtain 100% paresthesia over the perineal area without unwanted dorsal root stimulation. Perineal and radicular pain was successfully relieved for up to 12 months (80% and 60% reduction in the visual analogue scale scores, respectively), with an improvement in all quality of life domains and a reduction in drug consumption., Conclusion: Spinal cord stimulation using a 16-contact lead may be a viable therapeutic option for patients with refractory PN for whom decompressive surgery is contraindicated.

- Abstract
Twiddler's or twist syndrome is the twisting of pulse generators around themselves. It may result from mechanical manipulation that can induce the malfunction of the device. In this case, twiddler's syndrome resulted from compulsive checking of the device. The implantable cardioverter-defibrillator (ICD) triggered the development of an obsessive compulsive disorder (OCD). Two invasive procedures were required to replace the ICD. Psychiatric intervention prevented the recurrence of twiddler's syndrome in this patient for more than 2 years. We believe that preimplant psychiatric assessment should be the rule.

Besides 4 key-domains are statistically different, but are always in favour of the NA group; the domains are family life (p=0.01), social life (p=0.0026), presentation (p=0.003) and housing and food (0.029). These observations incite to modify our cure. The analysis of the total score of the FSQ did not reveal a statistically significant difference between both groups but this total score seems high in both groups, conveying an important feeling of satisfaction about the quality of life of our sample of schizophrenics. Literature data also abound along the same lines. Indeed, authors underline that patients clearly prefer to live outside rather than in the hospital. Besides, this study allows to confirm the literature data while studying some domains (such as sexuality) rarely brought to light in studies. As a conclusion, we can say that our sample is representative of a population of schizophrenics, and in spite of the difficulties of methodology inherent to this type of study, we can make a comparison between our 2 groups thanks to the stu-died data. Data in which differences could be found, did not show any influence on the results of scales, thus, they cannot be considered as a confounding factor. The NA group showed a better psychosocial adaptation. Furthermore the FSQ did not reveal a difference between both groups. It seems important to remind that a prospective study, conducted for several months and with several evaluations, is essential to confirm the results obtained on both types of treatment.
