The heavy burden of meningococcal infection is associated not only with life-threatening complications in the acute period and high mortality in invasive forms of the disease, but also with severe consequences in survivors, who are not recorded in our country.The aim of study: to analyze clinical manifestations, complications of the acute period and outcomes of invasive forms of meningococcal disease in children in various regions of the Russian Federation.Materials and methods: an analysis of data from 1327 inpatient medical records of children with an invasive meningococcal infection from 14 regional centers of the Russian Federation for 2012-2021 was carried out (28.3% of cases of the disease in children in the represented federal districts).Results: it was found that young children predominated among the patients – the median was 27.4 (10.7-70.4) months. Complications of the acute period, often combined, were observed in 47.6% of cases. The development of septic shock was noted in 30.4%, Waterhouse-Friderichsen syndrome in 6.6%, carditis in 2.9%, cerebral edema in 15.7%, arthritis in 1.4% of cases; the formation of hydrocephalus, subdural effusion, sensorineural hearing loss in 1.8%, 0.6%, 1% of children, respectively. The presence of soft tissue necrosis requiring surgical intervention was noted in 3.5% of cases. Mortality rate was 10.1%. At the time of discharge from the hospital, 30% of children had complications associated with meningococcal infection: organ dysfunction/ failure in 13.2% of patients (severe in 1.3%), cerebral insufficiency in 19.6%; severe psycho-neurological deficits, sensorineural hearing loss, problems associated with the need for orthopedic/surgical interventions accounted for 0.7%, 0.6% and 0.8%, respectively.Conclusion. Considering the epidemiological features of meningococcal infection – the risk of a sharp increase in morbidity in short periods of time, the life-threatening nature of the disease itself, it is necessary to remain alert to these risks and take all possible measures to prevent the disease using all available means, the most effective of which is vaccine prevention.

Introduction. Rotavirus infection is the leading cause of acute gastroenteritis in young children. Due to the lack of etiotropic treatment of viral gastroenteritis, the interest of scientists and practitioners in the use of antiviral drugs is increasing. Studies of domestic authors have proven that the low molecular weight interferon inductor – meglumine acridonacetate has antiviral, immunoregulatory and anti-inflammatory activity.Aim. Evaluation of the efficacy and safety of the antiviral drug meglumine acridonacenate in the treatment of acute gastroenteritis of rotavirus etiology in children aged 4–7 years in a hospital setting.Materials and methods. A prospective analysis of cases of treatment with meglumine acridonacetate for moderately severe rotavirus gastroenteritis in preschool children (n = 29, group I) was carried out. The comparison group consisted of patients receiving standard therapy (n = 31, group II). The presence of rotavirus infection was confirmed by the detection of the pathogen antigen in the feces. Meglumine acridonacetate was prescribed according to the scheme, parenterally, every other day.Results and discussion. The effectiveness of therapy with an antiviral drug on the 3rd day of treatment of moderately severe rotavirus gastroenteritis was 79.3% (p < 0.05). The elimination of the main clinical symptoms of the disease was noted on days 2–3: a decrease in the symptoms of intoxication, fever, a significant decrease in the duration of vomiting and diarrhea (p < 0.05), a reduction in the pathogen elimination period by 2.67 days (p < 0.01), meglumine acridonacetate was well tolerated, no side effects of the drug were detected.Conclusions. Meglumine acridonacetate can be recommended for the complex treatment of moderate rotavirus infection from the first days of the disease.

Introduction. Currently, the coronavirus infection pandemic caused by the SARS-CoV-2 virus continues around the world. Research data from domestic and foreign authors indicate that the kidneys are a target organ for a new infection, lesions vary from proteinuria and hematuria to acute kidney injury.Aim of the study – to determine the frequency and nature of kidney damage in children with confirmed coronavirus infection.Materials and methods. A retrospective and prospective analysis of cases of confirmed COVID-19 infection in children (n = 441) admitted to the Samara Regional Children’s Infectious Diseases Hospital from March 2020 to July 2021 was carried out. SARSCoV-2 RNA was detected in all patients by a one-step reverse transcription reaction combined with a polymerase chain reaction. The changes in the kidneys that occurred in 57 children were studied. The research results were processed using the Statistica 7.0 software (StatSoft, USA).Results. The involvement of the kidneys in the infectious process was detected in every 8 children with COVID-19 (12.9%), more often in the form of isolated urinary syndrome, the detection rate of which correlated with the severity of the course of coronavirus infection: in severe cases, proteinuria was detected in 31.6% of patients, hematuria – in 21%, acute kidney injury – in 10.5%, diabetic nephropathy – in 5.3%. Kidney damage was combined with damage to the respiratory and gastrointestinal tract, characterized by rapid recovery of urine output and azotemia parameters without special renal therapy. A clinical case of the onset of nephrotic syndrome that developed 2 weeks after suffering a coronavirus infection is described.Conclusions. Children with COVID-19 require kidney function monitoring for early detection and correction in case of impairment. Patients with isolated urinary syndrome in the acute period require long-term observation in order to detect latent renal pathology.

Introduction. Tonsillopharyngitis often accompanies the course of acute respiratory infections in children (up to 12-15%). The use of systemic antibacterial drugs for viral etiology of the disease has not been shown, therefore, the interest of researchers in the use of herbal products is increasing. In numerous clinical trials conducted around the world, the liquid extract of the roots of Pelargonium sidoides EPs® 7630 has been shown to be effective and safe in the treatment of acute respiratory infections, acute tonsillopharyngitis, rhinosinusitis, acute bronchitis in adults and children.Objective of the study: assessment of the efficacy and safety of the drug EPs® 7630 (Umckalor) in acute tonsillopharyngitis of non-streptococcal etiology in children 2-6 years old on an outpatient basis.Materials and methods. A prospective analysis of cases of treatment with Umckalor for acute tonsillopharyngitis of non-streptococcal etiology against the background of acute respiratory infection in children 2-6 years old (n = 23, group 1) was carried out.The comparison group (2) consisted of children receiving standard therapy (n = 22). Systemic antibiotics and antiviral drugs were not used in all patients. The study was conducted over 6 months (2019-2020). The research results were processed using the Statistica 7.0 software (StatSoft, USA).Results and discussion. Therapy with Umckalor in children with acute tonsillopharyngitis contributed to the elimination of the main clinical symptoms of the disease on the 4th day of treatment (efficiency was 69.9%, p < 0.05). The most persistent symptom was hyperemia of the tonsils and oropharynx. The period of convalescence occurred 2 days earlier than in the children of group 2 (p < 0.05). Umckalor was well tolerated, no side effects of the drug were identified.Conclusions. The drug Umckalor can be recommended for monotherapy for 7 days in patients with acute non-streptococcal tonsillopharyngitis on an outpatient basis with mild and moderate disease.

Objective: to conduct a comparative assessment of the effectiveness of immunomodulatory drugs in infectious mononucleosis of mixed etiology (EBV and CMV) in children, using the principles of evidence-based medicine.Materials and methods: a comprehensive comparative assessment of the effectiveness of immunomodulatory drugs in children with infectious mononucleosis EBV+CMV-etiology in 3 groups: group I-20 patients receiving recombinant interferon α-2β, group II-20 children receiving interferon – meglumin inducer acridonacetate and group III-20 children treated with a synthetic immunomodulator-inosin pranobex. The comparison group (IV) consisted of 20 children who received basic therapy.Results: Analysis of key intervention indicators for the outcome «fever Duration less than 5 days» showed the highest effectiveness of recombinant interferon α-2β, less effective were meglumina acridonacetate and inosine pranobex. According to the outcomes «Reduction of lymph nodes by 2 or more times» and «Reduction of the liver and spleen by 2 or more times», data on the advantage of meglumin acridonacetate were obtained. The recombinant interferon α-2β was the second most effective. All the studied drugs do notsignificantly affect the severity of the hematological syndrome. The expediency of using immunomodulatory therapy in infectious mononucleosis of EBV+CMV-etiology is shown, the deviation from the norm of the integral outcome indicator is significantly higher in the group of children who were on symptomatic therapy. The overall effectiveness of the studied immunomodulatory drugs with all clinical outcomes does not have statistically significant differences.Conclusion: the obtained data allow us to rationally approach the choice of immunomodulatory therapy, taking into account the effectiveness of clinical outcomes and the overall effectiveness of the drug.

Objective: to study the features of the clinical and epidemiological characteristics of whooping cough in children in the Samara region.Materials and methods: 389 cases of pertussis in the Samara region for 2015–2016 were analyzed.Results: it is shown that in spite of 95–98% vaccination coverage, in recent years there has been an increase in the incidence of whooping cough. Seasonality of morbidity remains. Among the children observed, the youngest children were not vaccinated against pertussis. The clinical picture of the disease remains typical with the classic course of catarrhal and spasmodic periods. Moderately severe forms of the disease predominate. Complications were noted mainly in unvaccinated children of the first year of life. The most frequent complications were pneumonia and apnea. There is a hypodiagnosis of pertussis in outpatient conditions. Infection often occurs under the mask of ARVI, while the sensitivity of the bacteriological method of diagnosis is zero. Of the methods for confirming the diagnosis, the most reliable is ELISA and PCR.Conclusion: these epidemiological and clinical features of pertussis current testify to the need to further improve methods of early diagnosis, especially express methods, etiopathagenetic treatment, specific prevention, antiepidemic measures in the foci of infection.

Objective. To study the peculiarities of trichinosis on the material of the group flashes.Materials and methods. Described and analyzed outbreak of trichinosis in 14 adolescents aged 13 to 18 years, were hospitalized in pediatric infectious ward of the city of Samara.Results. The source of infection – infected meat badger. The severity of illness depend on the amount of meat eaten, that is, the infectious dose of the pathogen. In severe cases, the incubation period of trichinosis was minimal (7 days), and if less severe – the maximum (from 19,5 to 21 days). Mild course also contributed to the early administration of antiparasitic therapy. Most of the children the disease begins with fever and dyspepsia, half recorded rash.At the height of the disease is predominant myalgias, half of cases – swelling. The peculiarity of the current severe trichinosis were pronounced toxic-allergic syndrome, generalized edema, myalgia and myasthenia gravis, up to a full adinamii, organ destruction: infectious-toxic kidney, cardiomyopathy, fulminant hepatitis, polyserositis with ascites, hydrothorax 3 weekdisease. The temperature in most of the patients wore a doublewavelength character with rises in the disease onset and disease for 2–3 weeks. It noted the duration of the clinical symptoms. The longest was myalgic, half of the patients remained at the time of discharge from the hospital. The features of laboratory parameters (leukocytosis, eosinophilia, ALT, AST, CRP) in various forms of the disease. The diagnosis was confirmed by serology (IgM, Ig G to Trichinella spiralis) in different phases of the disease. All patients received a specific antiparasitic therapy albendazolom secondary therapeutic dosage for 10 days, as well as detoxication, allergen and symptomatic therapy is indicated.Conclusion. Thus, health education among the population and will be a true prevention of trichinosis.

Studied the comparative efficacy of probiotics with different composition of strains in the complex treatment of acute intestinal infection in 89 children with functional disorders and chronic gastrointestinal tract. Conducted a dynamic study of the intestinal microflora bacteriological method and gas-liquid chromatography with the definition of short-chain fatty acid content of the level of carbohydrates in the feces and stool data. Set different dates for stopping diarrhea and features state of the intestinal ecosystem indicators after treatment in patients receiving comprehensive probiotic containing bifidobacteria and enterococcus, or probiotic containing lactobacillus.

Discribe the case of severe flow of а trichinosis in a girl 15 years old. The symptoms are an acute to x ico-allergological syndrome, generalized swelling myalgia and myasthenia with the full adynamia, infectios-toxicology kidney, an acute hypoalbuminemia with development polyserositis, adsence eosinophilia.

This paper presents results, which were obtained during investigation of 106 children with acute intestinal infections in order to determine the presence of cryptosporidia and to evaluate their intestinal microbial status. The cryptosporidia's oocysts were found in 31 of children (29,2 %) that were mainly under the 3 age old. At the same time the authors found the dysbacteriosis of the second and. third, degrees in 92,5 % of patients. Six patients (5,7 %) became cryptosporidium negative and improved their intestinal microbial status. The obtained results showed that lactobacterias containing probiotics are effective in treatment of children with acute intestinal infections for normalizing of intestinal biocenosis and. to eradicate cryptosporidia.
