Abstract

Mobility is considered one of the main reasons for the COVID-19 spread. Predicting the effect of control measures on mobility is essential to apply effective decisions. This work proposes an AI-based model for mobility changes prediction. The proposed CNN-LSTM with Autoregression has achieved the best results compared to other investigated models. Results show that the proposed model can predict the effect of precaution control measures on future community mobility with minimum loss. The mean absolute error over all countries in the study is 5.3. For Egypt and Saudi Arabia, the model achieved an MAE loss of 4.6 and 3.7 consecutively.

3. Deep neural network approach for arabic community question answering

Abstract

In this paper, we approach the Arabic community question answering problem. We integrated different types of similarity features, in addition to exploring the effect of using preprocessing. Moreover, we developed a novel deep neural network ensemble model that outperformed the previously achieved performance. Our ensemble model benefits from the semantic and lexical similarity features. Also, our ensemble model uses the recent advances in language models using BERT model. Our proposed model produced a MAP value of (62.9%), an AvgRec value of (86.6%), and an MRR value of (68.86%).

Abstract

Objectives Paediatric cancer is a leading cause of death for children. Children in low-income and middle-income countries (LMICs) were four times more likely to die than children in high-income countries (HICs). This study aimed to test the hypothesis that the COVID-19 pandemic had affected the delivery of healthcare services worldwide, and exacerbated the disparity in paediatric cancer outcomes between LMICs and HICs.Design A multicentre, international, collaborative cohort study.Setting 91 hospitals and cancer centres in 39 countries providing cancer treatment to paediatric patients between March and December 2020.Participants Patients were included if they were under the age of 18 years, and newly diagnosed with or undergoing active cancer treatment for Acute lymphoblastic leukaemia, non-Hodgkin’s lymphoma, Hodgkin lymphoma, Wilms’ tumour, sarcoma, retinoblastoma, gliomas, medulloblastomas or neuroblastomas, in keeping with the WHO Global Initiative for Childhood Cancer.Main outcome measure All-cause mortality at 30 days and 90 days.Results 1660 patients were recruited. 219 children had changes to their treatment due to the pandemic. Patients in LMICs were primarily affected (n=182/219, 83.1%). Abstract

The high cost of establishing a network experimental lab obstructs researchers from fulfilling and validating their research proposal. Remotely accessible testbeds overcome this difficulty by allowing researchers to access the testbed and the attached expensive wireless devices through the Internet. In this paper, we introduce WiPi as a low-cost networking testbed that can be utilized remotely and supports large-scale experiments. WiPi is implemented from the available off-the-shelf computing nodes, such as standard laptops and Raspberry Pis, with the goal to be affordable to many institutions, especially in developing countries. Multiple features, including users' isolation, disk protection, ease of user experience, power efficiency, multiple application domains, efficient disk utilization, and resource pooling, are implemented as part of the testbed. The interface and functionality of WiPi target three different levels of researchers in terms of their research experience: Expert, users with no prior knowledge with ns3, and novice. Besides, WiPi can combine simulation, emulation, and experimentation over real devices in the same experiment to further support larger-scale experiments. The web interface of the testbed allows researchers to partition and map a virtual network with a large number of virtual nodes to a physical network with a limited number of real nodes. Evaluation results show that WiPi can be utilized by a wide range of researches and can support different networking applications. Furthermore, it can reduce the execution time of large-scale experiments by almost 40%, highlighting its suitability as a low-cost, large-scale remotely-accessible network testbed.

Abstract

Mobility is considered one of the main reasons for the COVID-19 spread. Predicting the effect of control measures on mobility is essential to apply effective decisions. This work proposes an AI-based model for mobility changes prediction. The proposed CNN-LSTM with Autoregression has achieved the best results compared to other investigated models. Results show that the proposed model can predict the effect of precaution control measures on future community mobility with minimum loss. The mean absolute error over all countries in the study is 5.3 . For Egypt and Saudi Arabia, the model achieved an MAE loss of 4.6 and 3.7 consecutively.

SUMMARYBackgroundChronic respiratory diseases are considered a significant cause of morbidity and mortality worldwide, although data from Africa are limited. This study aimed to determine the prevalence and determinants of chronic respiratory diseases in Khartoum, Sudan.MethodsData was collected from 516 participants, aged≥40, who had completed a questionnaire and undertook pre- and post-bronchodilator spirometry testing. Trained field workers conducted questionnaires and spirometry. Survey-weighted prevalence of respiratory symptoms and spirometric abnormalities were estimated. Regression analysis models were used to identify risk factors for chronic lung diseases.ResultsUsing the NHANESIII reference equations, the prevalence of Chronic Airflow Obstruction (CAO) was 10%. The main risk factor was older age 60-69 years (Odds ratio 3.16, 95% Confidence Interval 1.20 – 8.31). Lower education, high body mass index and a history of tuberculosis were also identified as significant risk factors. The prevalence of a low forced vital capacity (FVC) using NHANES III was 62.7% [SE 2.2] and 11.3% [SE 1.4] using locally derived values.ConclusionThe prevalence of spirometric abnormality mainly (low FVC); was high suggesting that chronic respiratory disease is of substantial public health importance in urban Sudan. Strategies for the prevention and control of these problems are needed.

Abstract

ObjectivesPaediatric cancer is a leading cause of death for children. Children in low-income and middle-income countries (LMICs) were four times more likely to die than children in high-income countries (HICs). This study aimed to test the hypothesis that the COVID-19 pandemic had affected the delivery of healthcare services worldwide, and exacerbated the disparity in paediatric cancer outcomes between LMICs and HICs.DesignA multicentre, international, collaborative cohort study.Setting91 hospitals and cancer centres in 39 countries providing cancer treatment to paediatric patients between March and December 2020.ParticipantsPatients were included if they were under the age of 18 years, and newly diagnosed with or undergoing active cancer treatment for Acute lymphoblastic leukaemia, non-Hodgkin’s lymphoma, Hodgkin lymphoma, Wilms’ tumour, sarcoma, retinoblastoma, gliomas, medulloblastomas or neuroblastomas, in keeping with the WHO Global Initiative for Childhood Cancer.Main outcome measureAll-cause mortality at 30 days and 90 days.Results1660 patients were recruited. 219 children had changes to their treatment due to the pandemic. Patients in LMICs were primarily affected (n=182/219, 83.1%). Relative to patients with paediatric cancer in HICs, patients with paediatric cancer in LMICs had 12.1 (95% CI 2.93 to 50.3) and 7.9 (95% CI 3.2 to 19.7) times the odds of death at 30 days and 90 days, respectively, after presentation during the COVID-19 pandemic (pConclusionsThe COVID-19 pandemic has affected paediatric oncology service provision. It has disproportionately affected patients in LMICs, highlighting and compounding existing disparities in healthcare systems globally that need addressing urgently. However, many patients with paediatric cancer continued to receive their normal standard of care. This speaks to the adaptability and resilience of healthcare systems and healthcare workers globally.

Abstract

IntroductionChildhood cancer is a leading cause of death. It is unclear whether the COVID-19 pandemic has impacted childhood cancer mortality. In this study, we aimed to establish all-cause mortality rates for childhood cancers during the COVID-19 pandemic and determine the factors associated with mortality.MethodsProspective cohort study in 109 institutions in 41 countries. Inclusion criteria: children ResultsAll-cause mortality was 3.4% (n=71/2084) at 30-day follow-up, 5.7% (n=113/1969) at 90-day follow-up and 13.0% (n=206/1581) at 12-month follow-up. The median time from diagnosis to multidisciplinary team (MDT) plan was longest in low-income countries (7 days, IQR 3–11). Multivariable analysis revealed several factors associated with 12-month mortality, including low-income (OR 6.99 (95% CI 2.49 to 19.68); pConclusionsChildren with cancer are more likely to die within 30 days if infected with SARS-CoV-2. However, timely treatment reduced odds of death. This report provides crucial information to balance the benefits of providing anticancer therapy against the risks of SARS-CoV-2 infection in children with cancer.

Abstract

Action anticipation in egocentric videos is a difficult task due to the inherently multi-modal nature of human actions. Additionally, some actions happen faster or slower than others depending on the actor or surrounding context which could vary each time and lead to different predictions. Based on this idea, we build upon RULSTM architecture, which is specifically designed for anticipating human actions, and propose a novel attention-based technique to evaluate, simultaneously, slow and fast features extracted from three different modalities, namely RGB, optical flow, and extracted objects. Two branches process information at different time scales, i.e., frame-rates, and several fusion schemes are considered to improve prediction accuracy. We perform extensive experiments on EpicKitchens-55 and EGTEA Gaze+ datasets, and demonstrate that our technique systematically improves the results of RULSTM architecture for Top-5 accuracy metric at different anticipation times., Accepted to EPIC@ICCV 2021

Abstract

Objective: Evaluate the impact of demographic and socio-economic factors on diabetic control (Hba1c) among patients in an outpatient setting. Methods: A prospective study from January 2015 to May 2017 involving 211 patients with diabetes mellitus managed at an outpatient endocrinology clinic. Serial measurements of HbA1c were collected at baseline, 3 and 6 months. HbA1c was treated as a continuous variable and presented as mean ± standard deviation. Baseline HbA1c level was compared among dichotomized socio-economic variables (married vs. not married; employed vs. unemployed; health literacy (using the REALM survey) and educational level using t-test. A multivariable linear regression model was used to examine the association between Hb1Ac control and patient demographic and socio-economic characteristics. Results: Among study participants, 96% were African American, 63.5% female, with mean age of 53 ± 14.7) and baseline mean Hb1Ac of 8.80 ± 2.30. In univariate analysis, higher baseline Hb1Ac was observed in patients with REALM score below 7 vs. above 7, (9.30%±2.36 vs. 8.49% ± 2.27, p-value = 0.01), unemployed vs. unemployed (9.17%± 2.46 vs. 8.56%± 2.23, p-value = 0.08), and high school or lower vs. college and above (9.08% ± 2.36 vs. 8.7%± 2.28, p-value = 0.10). Single and divorced patients had higher baseline HbA1C (8.94 ± 2.4 and 8.90 ± 2.4 respectively) compared to their married counterpart (8.65%±2.4)Adjusting for baseline Hb1Ac level, gender, race marital status, employment status and educational level, multivariable regression analysis showed that higher educational level was significantly associated with change in HbA1c values. Patients with college education and above showed significant reduction in HbA1C (regression coefficient -0.75, 95% CI -1.27 to -0.230, p=0.005) compared to those with high school attainment and lower. Conclusion: In this prospective study involving predominantly African Americans, higher educational attainment was independently associated with better glycemic control. Disclosure K.M. Abstract

Pituitary stalk interruption syndrome (PSIS) is a congenital disorder of the pituitary gland. Symptoms at presentation may vary widely as this disease presents along a spectrum which includes; ectopic posterior pituitary, interrupted pituitary stalk or aplasia and hypoplasia of the pituitary gland. It is a heterogeneous disorder in terms of radiologic and clinical presentation. It can present clinically as an isolated pituitary hormone deficiency (most common being growth hormone deficiency) or as multihormonal deficiencies. CASE PRESENTATION Patient is a 34-year-old woman with history of primary amenorrhea who was evaluated by a gynecologist and was prescribed oral contraceptive pills which lead to her having a menstrual bleed for the first time in her life. She denied any difficulty with smell. She had undergone normal psychomotor milestones and highest level of education was high school. She had normal puberty with normal pubic and axillary hair growth, normal breast development but no menarche. Of note, patient has a short stature, height is 4 feet and 11 inches, and her biological parents are of normal adult height On evaluation, patient had normal am cortisol, prolactin and thyroid function tests. IGF-1 was significantly low for her age, FSH and LH were inappropriately low for her low estradiol level suggesting hypogonadotropic hypogonadism. Patient subsequently had an MRI of the pituitary and DXA scan. MRI findings were suggestive of ectopic neurohypophysis. DXA scan showed significant reduction in bone mineral density for age. Patient is currently being treated with hormonal replacement which is the main modality of treatment for ectopic neurohyphysis. She will need long term follow up as disease progression to pan-hypopituitarism is common. CONCLUSION PSIS is a rare syndrome with different phenotypic presentation depending on when the diagnosis is made; therefore, adequate follow up is indicated as the disease can progress from a single hormonal deficiency to pan-hypopituitarism.

Abstract

Hyperprolactinemia and multiple sclerosis (MS) have a direct relationship and hyperprolactinemia may precede clinical signs of MS as a heralding manifestation of disease. Prolactin has significant pro-inflammatory effects in addition to its lactotrophic properties and can also lower the body’s immune tolerance, inducing autoimmunity. High levels of prolactin have been thought to contribute to the inflammation of multiple sclerosis. However, elevated levels of prolactin, especially in pregnant women, can be protective for MS patients and induce remission. Prolactin is neuroregenerative and stimulates the precursors for oligodendrocytes, the cells responsible for myelination. Our hypothesis is that an elevated prolactin level detected during an MS flare should not be treated with dopamine agonist, but rather allowed to decrease as the MS improves with treatment. Case Presentation A 24 year old woman with a history of marijuana use is referred to our clinic for elevated prolactin levels associated with galactorrhea for 3 months duration. In addition to marijuana use, patient was also sexually active and having regular menses, with menarche at age 11 years old. On physical exam, the patient was found to have bilateral nipple discharge with stimulation, and visual fields were intact to confrontation. At the time of referral, the patient’s prolactin was 92.3 ng/dL (4.8−23.3 ng/mL) TSH was normal, and pregnancy test negative.An MRI showed multiple areas of enhancement compatible with active demyelination, concerning for multiple sclerosis. The pituitary gland was enlarged, without evidence of adenoma. A follow up prolactin level was 101 ng/dL and upon further discussion, patient also admitted to some “funny feeling” and weakness in her right hand and a feeling of being “off balance” diagnosed as a left ear infection. Patient was advised to seek urgent treatment for multiple sclerosis. She was admitted, where she was seen by neurology and diagnosed with relapsing remitting multiple sclerosis. She was initially treated with a course of IV methylprednisolone. She was discharged after this course and followed with neurology as an outpatient. For a few months our patient went into remission and her prolactin improved to 24 ng/dL. A few months later, she had a significant increase in her prolactin to 71.5 ng/dL accompanied by evidence of disease progression on MRI and symptoms of weakness and falls. Neurologists changed her medication from Copaxone to Tecfidera and patient improved clinically and has not had any further flares. Notably, she never received any dopaminergic agent to treat her prolactin level, which improved significantly.Our case illustrates that prolactin may be a disease marker in the acute phase of MS and can be restorative. Further more it will improve when the MS is treated and we should not use any dopamine agonist.

Abstract

The high cost of establishing a network experimental lab obstructs researchers from fulfilling and validating their research proposal. Remotely accessible testbeds overcome this difficulty by allowing researchers to access the testbed and the attached expensive wireless devices through the Internet. In this paper, we introduce WiPi as a low-cost networking testbed that can be utilized remotely and supports large-scale experiments. WiPi is implemented from the available off-the-shelf computing nodes, such as standard laptops and Raspberry Pis, with the goal to be affordable to many institutions, especially in developing countries. Multiple features, including users' isolation, disk protection, ease of user experience, power efficiency, multiple application domains, efficient disk utilization, and resource pooling, are implemented as part of the testbed. The interface and functionality of WiPi target three different levels of researchers in terms of their research experience: Expert, users with no prior knowledge with ns3, and novice. Besides, WiPi can combine simulation, emulation, and experimentation over real devices in the same experiment to further support larger-scale experiments. The web interface of the testbed allows researchers to partition and map a virtual network with a large number of virtual nodes to a physical network with a limited number of real nodes. Evaluation results show that WiPi can be utilized by a wide range of researches and can support different networking applications. Furthermore, it can reduce the execution time of large-scale experiments by almost 40%, highlighting its suitability as a low-cost, large-scale remotely-accessible network testbed.

Abstract

BACKGROUND: Steroid cell tumor is a rare sex cord stromal tumor. There are 3 types including steroid cell tumor not otherwise specified (NOS), stromal luteoma and leydig cell tumors. Steroid cell tumor (NOS) is the most common of all the subtypes. About 75% of the steroid cell tumor (NOS) are secretory. They can secrete androgens and estrogens. In a few cases, cortisol and renin secretion have been reported. The patient’s clinical features depend on the hormone secreted. CASE PRESENTATION Here, we report a case of a 22-year-old woman who was seen at an outpatient clinic for hirsutism, irregular menstrual bleeding, and progressive weight gain. Examination revealed androgenic facial hair growth, clitoromegaly and obesity. Initial differentials on presentation were PCOS, ovarian or adrenal pathology. Initial LH, FSH values were normal. Lab investigation showed elevated testosterone, DHEAS and 17-OH progesterone levels. Baseline labs showed Androstenedione of 3345 ng/dl (41-262 ng/dl), DHEAS of 595.5 ug/dl (110- 431.7 ug/dl), 17-OH progesterone was 2394 ng/dl (follicular: 15-70 ng/dl, Luteal: 35-290 ng/dl), total and free testosterone was 558 ng/dl (premenopausal; 10-55ng/dl) and 33.0 pg/ml, (0.8-1.4pg/ml), respectively. Patient subsequently had an ACTH stimulation test which showed an increase in 17-OH progesterone from 384 ng/dl to 657 ng/dl and a repeat showed an elevation from 204 to 322 ng/dl, ruling out a late onset CAH. She was then sent for pelvic sonogram which showed the presence of a left ovarian mass 4.9x 4.8x 4.8 cm and a pelvic CT scan confirmed a left ovarian mass measuring 6.8x5.5x 5.5 cm and an unremarkable right ovary. She underwent a laparoscopic unilateral salpino-oophorectomy and histology confirmed the diagnosis of an ovarian steroid cell tumor (NOS). Post-surgery, the androgen levels returned to normal. The patient is now being followed for recurrence. CONCLUSION Women who present with virilism should be evaluated adequately with comprehensive history taking and physical examination along with appropriate biochemical tests and imaging studies to confirm tumor location and plan for surgery.

Abstract

Neuroblastoma (NBL) is the most common malignant solid extracranial tumour in children. It accounts for about 8%-10% of overall childhood malignancies in developed countries and only 1%-3% in developing countries. We aim to study the clinical presentation and outcome of NBL in children treated at the National Cancer Institute (NCI), University of Gezira, Wad Madani, Sudan. This is a retrospective, descriptive, hospital-based study, where patients' records were reviewed from the years 2004-2015. During this period, 38 paediatric patients were treated in the Paediatric Oncology Unit at the NCI, accounting for about 4% of the overall childhood malignancies. Of them, 24 (63%) were males and 14 (37%) were females with a ratio of 1.7: 1. The age at presentation ranged between 2 months and 7 years with a mean age at the presentation of 3 years. Twenty-nine (76%) of them were classified as high-risk disease, no one received high-dose chemotherapy, 33(87%) died and only 5 (13%) achieved complete remission. This study reflects the poor outcome of NBL among Sudanese children which can be explained by the late presentation of the patients, lack of the diagnostic modalities and lack of the sophisticated treatment modalities for high-risk NBL.

Abstract

Retinoblastoma (RB) is a rare and unique cancer that affects the eyes of very young children. There are few reports on RB in Sudan.We performed a retrospective study of data from patients diagnosed with retinoblastoma between January 1999 and December 2009 at the National Cancer Institute in Gezira (NCI-Gezira).Of the 519 cases of childhood cancer treated at NCI-Gezira during the study period, 25 (4.8%) were retinoblastoma. Of these 25 patients with retinoblastoma, there were 13 boys and 19 cases were unilateral. The median age at diagnosis was 36 months (range, 8-60 months). The disease was localized in 9 patients, regional in 5 patients, and metastatic in 11 patients. The most frequent symptoms were enlarged eye (n = 14) and leukocoria (n = 8). Nine patients (36%) have been lost to follow-up; 9 were alive at last follow-up (7 in remission, 2 progressed); and 7 have died (5 from disease and 2 from unrelated causes). Twenty-two eyes were enucleated in 16 patients (6 bilateral and 10 unilateral). Pathologic examination of the enucleated eyes could only be completed in 11 patients. Diagnostic imaging in the form of computerized tomography scans or ultrasonography of the brain and orbit was done for 10 patients (40%).Although these findings are not surprising, and similar to reports from other developing countries, we hope our work will provide a foundation for strategies to improve outcome for retinoblastoma in our center such as proper training, public awareness, team approach, and twinning.

Abstract

Cervical cancer is one of the commonest female cancers in Sudan due to the absence of HR-HPV screening and vaccination programs. The aim of this study was to screen for the existence of cervical cancer related herpes viruses (HPV, EBV, CMV and EBV) among Sudanese women with cervical cancer. This was analytical case control study conducted in Khartoum state-Sudan during period from October 2013 to June 2016. One hundred and eighty paraffin-embedded tissue blocks were included in this study. Of the 180 tiaaue's blocks, 98 blocks were from patients with cervical squamous cell carcinoma (case group) and 82 were from cervical tissue's blocks malignancy-free (control group). Screening for herpes viruses (HPV, EBV, CMV and EBV) were performed using Immunohistochemistry (IHC) techniques. HR-HPV infection was detected in 28.6% of the cases, and 2.4% of the controls. Association between HR-HPV and cervical carcinoma was found to be statistically significant P < 0.0001. CMV infection was detected in 15.3% of the cases and 1.2% of the controls with statistically significant difference, the P < 0.0001. HSV-2 revealed positivity in 6.1% of the cases and in 1.2% of the controls with no statistically significant correlation. EBV was positive in 2% of the cases, while none of the control was found positive. HR-HPV and EBV are prevalent etiological factors that contribute to the etiology of cervical carcinoma in Sudan. 31. Predicting Milk and Soft Drink Consumption Among Female Adolescents Using the Theory of Planned Behavior

Abstract

A total of 756 female adolescents, aged 13 to 18 years, from five public high schools in Antelope Valley Union High School District located in North Los Angeles County participated in this study between February 1999 and March 1999. The ethnic composition was 53.3% White/Anglo Americans, 19.3% Hispanics or Latinos and 10.2% Black/African Americans. The constructs of the Theory of Planned Behavior (attitude, subjective norm, and perceived behavioral control and their underlying beliefs) were assessed to determine the predictors of low-fat milk and regular soda consumption. A cross-sectional design with group-administered questionnaires was utilized. Most participants (89.6%, n = 636) reported that they currently drink milk; 64.3% (n = 456) of all participants reported drinking reduced-fat milk and nonfat milk. Most participants 96.3% (n = 681) reported that they currently drink soda; 84.3% (n = 596) reported drinking regular soda. Attitude, subjective norm, and perceived behavioral control explained 66% of the variation in the intention to drink low-fat milk and 64% of the intention to drink regular soda. The strongest predictor for both drinks was attitude, followed by perceived behavioral control and subjective norm. The primary expected outcomes that predicted a positive attitude towards drinking low-fat milk were enjoyment of taste and feeling healthy. Weaker predictors were having more energy and not expecting stomachache or gas upon its consumption. Normative beliefs of students about what parents thought about milk use predicted subjective norm. Perceived behavioral control was predicted by availability of low-fat milk at home. The primary expected outcomes that predicted a positive attitude towards drinking regular soda were enjoyment of taste, quenching of thirst, becoming hyper or having a sugar rush, and feeling healthy.
