Abstract

There seems to be an increase in the rate of autoimmune hypothyroidism among atomic bomb survivors. Autoimmune hypothyroidism is decreased activity of the thyroid gland due to an abnormal immune response. Researchers questioned and examined 2,587 men and women who had survived the atomic bombing of Nagasaki. Thyroid diseases were identified in 447 (17.3%) of subjects. Of these 447, 260 did not indicate a history of thyroid disease. In most forms, this condition afflicted more women than men. Some types of thyroid disease seem to be related to the level of radiation exposure. These include cancer, tumors, and autoimmune hypothyroidism. Some rates of thyroid disease were higher only in women. These include a solid, small, rounded mass in the thyroid and a thyroid mass consisting of tissue that could not be diagnosed., Objective. - To elucidate the current thyroid disease status for the Nagasaki Adult Health Study cohort of the Radiation Effects Research Foundation. Design. - Survey study. Setting. - Nagasaki, Japan. Participants. - Cohort members of the Nagasaki Adult Health Study who received biennial health examinations from October 1984 to April 1987 (n=2856). A total of 2587 subjects remained after exclusion of persons exposed in Hiroshima or in utero and those who were not in Nagasaki at the time of the bombing. Thyroid radiation dose by the dosimetry system established in 1986 was available for 1978 of the 2587 subjects. Main Outcome Measures. - Thyroid diseases were diagnosed using uniform procedures including ultrasonic scanning. The relationship of the prevalence of each thyroid disease with thyroid radiation dose, sex, and age was analyzed using logistic models. Results. - A significant dose-response relationship was observed for solid nodules, which include cancer, adenoma, adenomatous goiter, and nodules without histological diagnosis, and for antibody-positive spontaneous hypothyroidism (autoimmune hypothyroidism) but not for other diseases. The prevalence of solid nodules showed a monotonic dose-response relationship, yet that of autoimmune hypothyroidism displayed a concave dose-response relationship reaching a maximum ([+ or - ]SE) level of 0.7[+ or -]0.2 Sv. Conclusions. - The present study confirmed the results of previous studies by showing a significant increase in solid nodules with dose to the thyroid and demonstrated for the first time a significant increase in autoimmune disease among atomic bomb survivors. A concave dose-response relationship indicates the necessity for further studies on the effects of relatively low doses of radiation on thyroid disease. (JAMA. 1994;272:364-370)

Abstract

Measuring fractions of the blood protein alpha-fetoprotein may be useful in distinguishing between liver cancer and liver cirrhosis and in predicting the development of liver cancer among patients with cirrhosis. Of 76 patients with liver cirrhosis and elevated levels of alpha-fetoprotein, 33 developed liver cancer within an average period of about three years. The proportion of alpha-fetoprotein L3 was significantly higher among the 33 at the time of tumor detection than the initial value among patients who did not subsequently develop liver cancer. Also, the average proportion of alpha-fetoprotein P4+P5 was higher among patients with cirrhosis and liver cancer than among those with cirrhosis but no liver cancer. Among the patients with elevated proportions of either alpha-protein L3 or P4+P5 at the time of tumor detection, the concentration of the protein first rose three to 18 months before tumors were ultimately detected.

Abstract

The HLA DRB1*08032 gene in people with Graves disease may be associated with an increased risk of developing agranulocytosis in response to the drug methimazole. Graves disease is a thyroid disease often treated with methimazole and agranulocystosis is the shortage of granulocytes, resulting in disorders of the mucous membranes. Researchers analyzed granulocyte DNA from 24 people with Graves disease and agranulocytosis caused by methimazole, 68 people with Graves disease and no agranulocytosis, and 525 healthy people. HLA class II genes that were significantly associated with agranulocytosis were DRB1*08032 and DRB1*0501. Among the groups of people with Graves disease, DRB1*08032 was found in 54% of those with agranulocytosis, but only 22% of those without agranulocytosis. These results suggest that such cases of agranulocytosis are caused by an autoimmmune attack on granulocytes by native T cells.

Abstract

The synovial fluid of rheumatoid arthritis patients appears to contain an abnormally high number of CD8 cells. CD8 cells are components of the immune system. Synovial fluid acts as a joint lubricant. Samples of synovial fluid were taken from 15 patients with RA and 10 patients with osteoarthritis (OA). Some RA patients had blood samples drawn simultaneously. The subtypes of CD8 cells were characterized. Synovial fluid from RA patients had greater concentrations of certain CD8 subtypes than blood from the same patient or synovial fluid from OA patients. The attraction of these cells to the joint-lining membrane and their function as killer cells in the inflammatory reaction suggest that these cells play a role in tissue damage to the joint lining.

Abstract

Background. The development of hepatocellular carcinoma (HCC) is associated closely with cirrhosis. In the present study, the cumulative risk of HCC in patients with cirrhosis was investigated. Methods. A total of 401 patients were registered from April 1977 and followed for a mean of 4.4 years. Of 401 patients, 255 640%) were tested for hepatitis B surface antigen (HBsAg) and antibody (anti-) to the hepatitis C virus (HCV); 87 (340%) patients were positive for HBsAg but were negative for anti-HCV (hepatitis B virus [HBV] group), 126 (49%) were negative for HBsAg but were positive for anti-HCV (HCV group), 10 (4%) were positive for both and 32 (13%) were negative for both (non-B non-C group). Results. By the end of March 1993, HCC was diagnosed in 127 (31.6) patients. The cumulative risk of HCC in the HCV group was slightly higher than that in HBV group (P = 0.3, 5-year risk: 36.9 versus 21.2%). In contrast, the rate was significantly lower in the non-B non-C group than in the HBV or HCV groups (P < 0.05 and P < 0.01, respectively, 5 year risk: 12.4%). Conclusions. These results suggest that not only HBV infection but also HCV infection increase the risk for HCC in patients with cirrhosis. Cancer 1994; 74:2234-8.

Abstract

Interleukin-4 (IL-4) may rapidly increase and enhance the adhesion of T-cells to synovial cells via the VLA-4/VCAM-1 (very late antigen 4/vascular cell adhesion molecule 1) pathway. This adhesion interaction may determine immune responses or the retention of inflammatory cells in the synovial fluid of rheumatoid arthritis (RA) patients. A study of six Japanese RA patients analyzed their synovial fluid at the time of surgery for total knee replacement. IL-4 was found to rapidly increase T-cell adhesion to synovial fibroblasts as it increased VCAM-1 expression on the surface of the fibroblasts. IL-4 and IL-1-beta combined did not change the expression of ICAM-1 and VCAM-1 on synovial fibroblast surfaces. Anti-VLA-4-alpha monoclonal antibody totally blocked T cell adhesion to IL-4-stimulated synovial cells. It is suggested that the VLA-4/fibronectin pathway and the VLA-4/VCAM-1 pathway mediate T cell adhesion to IL-4-stimulated synovial cells.
