1. Heterologous Booster Dose with CORBEVAX following Primary Vaccination with COVISHIELD Enhances Protection against SARS-CoV-2

Abstract

Despite effective vaccination programs, waning immunity in the vaccinated populations and the emergence of variants of concern posed a risk of breakthrough infections. A booster dose was demonstrated to provide substantially increased protection against symptomatic disease and hospitalization. We aimed to evaluate immune memory and the efficacy of reducing the rate of SARS-CoV-2 infection post heterologous booster with CORBEVAX after primary vaccination with two doses of COVISHIELD. SARS-CoV-2 S1/S2 spike IgG and RBD-specific antibody responses were elicited with both booster vaccines, with a greater response in individuals receiving heterologous booster. T and B memory responses were increased with booster dose, whereas B memory needed a longer duration to develop in individuals who received a homologous booster (90 days) in comparison to a heterologous booster (30 days). RBD-specific B memory and antibody-secreting (non-memory) B lymphocytes were enhanced with both boosters; however, the duration of response was longer with the heterologous booster compared to the homologous, indicating greater protection with the heterologous booster. The rate of infection 14 days after administration of the heterologous booster was comparatively lower than that of the homologous booster, with the symptoms being much less or asymptomatic.

Abstract

Emerging variants of SARS-CoV-2 with increased transmissibility or immune escape have been causing large outbreaks of COVID-19 infections across the world. As most of the vaccines currently in use have been derived from viral strains circulating in the early part of the pandemic, it becomes imperative to constantly assess the efficacy of these vaccines against emerging variants. In this hospital-based cohort study, we analysed clinical profiles and outcomes of 1161 COVID-19 hospitalized patients (vaccinated with COVISHIELD (ChAdOx1) or COVAXIN (BBV-152), n = 495 and unvaccinated n = 666) in Hyderabad, India between April 24th and May 31st 2021. Viral genome sequencing revealed that >90% of patients in both groups were harbouring the Delta variant (Pango lineage B.1.617.2) of SARS-CoV-2. Abstract

ObjectivePancreatic islet β-cells are factories for insulin production; however ectopic expression of insulin is also well recognized. The gallbladder is a next-door neighbour to the developing pancreas. Here, we wanted to understand if gallbladders contain functional insulin-producing cells.DesignWe compared developing and adult mouse as well as human gallbladder epithelial cells and islets using immunohistochemistry, flow cytometry, ELISAs, RNA-sequencing, real-time PCR, chromatin immunoprecipitation and functional studies.ResultsWe demonstrate that the epithelial lining of developing, as well as adult mouse and human gallbladders naturally contain interspersed cells that retain the capacity to actively transcribe, translate, package, and release insulin. We show for the first time that human gallbladders also contain functional insulin-secreting cells with the potential to naturally respond to glucose in vitro and in situ. Notably, in a NOD mouse model of type 1 diabetes, we observed that insulin-producing cells in the gallbladder are not targeted by autoimmune cells. Conclusion: In summary, our biochemical, transcriptomic, and functional data in human gallbladder epithelial cells collectively demonstrate their potential for insulin-production under pathophysiological conditions, and open newer areas for type 1 diabetes research and therapy.Significance of the studyWhat is already known about this subject?Developing pancreas and gallbladder are next-door neighbours and share similar developmental pathways.Human Gallbladder-derived progenitor cells were shown to differentiate into insulin-producing cells.What are the new findings?Gallbladder epithelium contains interspersed cells that can transcribe, translate, package and secrete insulin.Insulin-producing cells in the gallbladder are not destroyed by immune cells in an animal model of type 1 diabetes (T1D).Our studies demonstrating the absence of insulin splice variants in human gallbladder cells, and higher splice forms in human islets, suggest a potential mechanism (via defective ribosomal products) in escaping islet autoimmunity.How might it impact clinical practice?Deciphering mechanisms of protection of insulin-producing cells from immune cells in the gallbladder could help in developing strategies to prevent islet autoimmunity in T1D.

Abstract

Background/Aims: Information on pediatric inflammatory bowel disease (PIBD) and very early onset IBD (VEOIBD) are sparse in India, where IBD is emerging. We aimed to evaluate characteristics of VEOIBD and later onset PIBD (LO-PIBD) in India. Methods: We performed retrospective analysis of a large, prospectively maintained IBD registry. PIBD was divided in to VEOIBD ( < 6 years) and LO-PIBD (6--17 years). Demographic data, disease characteristics and treatment were compared between the PIBD groups and with other Asian/Western studies as well as the adult patients of the registry. Results: Of 3,752 IBD patients, 292 (7.8%) had PIBD (0--17 years) (175 Crohn's disease [CD], 113 ulcerative colitis [UC], 4 IBD-undifferentiated; 22 VEOIBD [7.5%], and 270 LO-PIBD [92.5%]). VEOIBD patients had more severe disease compared to LO-PIBD in both UC (P= 0.003) and CD (P< 0.001). Familial IBD was more common in VEOIBD (13.6%) compared to LO-PIBD (9.2%). Ileal disease (L1) was an independent risk factor for diagnostic delay in pediatric CD. Diagnostic delay ( > 6 months) was significantly lower in VEOIBD (40.9%) than in LO-PIBD (78.8%) (P< 0.001). Compared to other Asian and Western studies, extensive UC (72.5%) and complicated CD (stricturing/penetrating: 42.7%) were relatively more common. Perianal CD was relatively less frequent (7.4%). PIBD had a significantly higher number of complicated and ileal CD and extensive UC comparison to adult cohort of the registry. Conclusions: VEOIBD has more aggressive phenotype than LO-PIBD. Disease appears distinct from other Asian and Western studies and adult onset disease, with more complicated CD and extensive UC. [ABSTRACT FROM AUTHOR]

Abstract

Background and Aims Precise optical diagnosis of colorectal polyps could improve the cost-effectiveness of colonoscopy and reduce polypectomy-related complications. We conducted this study to estimate the diagnostic performance of visual inspection alone (WLI + NBI) and of EndoBRAIN (endocytoscopy-computer-aided diagnosis [EC-CAD]) in identifying a lesion as neoplastic or nonneoplastic using EC in real-world scenario. Methods In this observational, prospective, pilot study, a total of 55 polyps were studied in the patients aged more than or equal to 18 years. EndoBRAIN is an artificial intelligence (AI)-based system that analyzes cell nuclei, crypt structure, and vessel pattern in differentiating neoplastic and nonneoplastic lesion in real-time. Endoscopist assessed polyps using white light imaging (WLI), narrow band imaging (NBI) initially followed by assessment using EC with NBI and EC with methylene blue staining. The sensitivity, specificity, positive predictive value, negative predictive value, and accuracy of endoscopist and EndoBRAIN in identifying the neoplastic from nonneoplastic polyp was compared using histopathology as gold-standard. Results A total of 55 polyps were studied, in which most of them were diminutive (36/55) and located in rectum (21/55). The image acquisition rate was 78% (43/55) and histopathology of the majority was identified to be hyperplastic (20/43) and low-grade adenoma (16/43). EndoBRAIN identified colonic polyps with 100% sensitivity, 81.82% specificity (95% confidence interval [CI], 59.7–94.8%), 90.7% accuracy (95% CI, 77.86–97.41%), 84% positive predictive value (95% CI, 68.4–92.72%), and 100% negative predictive value. The sensitivity and negative predictive value were significantly greater than visual inspection of endoscopist. The diagnostic accuracy seems to be superior; however, it did not reach statistical significance. Specificity and positive predictive value were similar in both groups. Conclusion Optical diagnosis using EC and EC-CAD has a potential role in predicting the histopathological diagnosis. The diagnostic performance of CAD seems to be better than endoscopist using EC for predicting neoplastic lesions.
