3. Turnover Intention among Staff Who Support Older Adults Living Alone in Japan: A Cross-Sectional Study.

Abstract

Older adults in Japan traditionally rely on family members for care. However, the growing number of older people living alone has increased staff burden in community general support centers (CGSCs), which provide care for older people in Japan. This study aimed to identify factors linked to turnover intention among CGSC staff. Of 1002 staff invited to participate, 283 completed a survey (response rate: 28.2%). Logistic regression analysis of 183 valid questionnaires examined demographic, job-related, and attitudinal factors associated with turnover intention. The results showed that the perceived difficulty of supporting older adults living alone was the main predictor of turnover intention. Staff reporting high perceived difficulty levels were more likely to consider leaving. Negative attitudes toward supporting this population also increased turnover risk. These findings highlight a pressing need for targeted interventions to strengthen the ability of CGSC staff to manage the challenges of caring for isolated older individuals. Addressing staff perceptions and providing essential training and resources could reduce turnover rates and increase the quality of care for this vulnerable group. [ABSTRACT FROM AUTHOR]

5. Effect of the 2024 Noto Peninsula earthquake on outpatient chemotherapy among cancer survivors in Japan: a retrospective study.

Abstract

Background: The study aim was to elucidate the effect of the 2024 Noto Peninsula earthquake on outpatient chemotherapy treatment of cancer survivors at Kanazawa Medical University Hospital (KMUH), Japan. Methods: Medical and nursing records for January 4–31, 2024, from KMUH were retrospectively collected, and data for 286 participants were analyzed. Results: Of the 286 participants, 95.1% were able to attend their first scheduled appointment. Of the 12 (4.2%) who could not attend because of the earthquake, 7 (58.3%) rescheduled their appointments. A total of 8 participants (2.8%) were unable to attend their second scheduled appointment in January, despite being able to attend their first appointment; 3 (37.5%) of these participants reported that they were unable to attend their appointments because of the effect of the earthquake. Chemotherapy was not administered to 53 (18.5%) participants who did attend, mainly owing to neutropenia, progressive disease, rash, and anemia. Evacuation information was available for 25 participants (8.7%); of these, 8 (28.6%) evacuated to their homes, 7 (25.0%) to public shelters, and 4 (14.3%) to apartments near the hospital. Disaster status information was obtained from 62 participants (21.7%), and indicated experiences such as home damage, water outages, and relying on transportation assistance from family to attend appointments. Conclusions: Most cancer survivors receiving chemotherapy at KMUH were able to maintain outpatient visits. However, a few could not attend because of the earthquake. Further studies are needed to provide more detailed information on the effect of disasters on cancer survivors and the potential factors underlying non-attendance at medical appointments. Abstract

Severe heavy rains caused by linear precipitation systems are occurring more frequently in Japan owing to climate change, and residents are being asked to evacuate more often. The purpose of this study was to identify factors associated with the willingness of people with mental health illness (PMHI) in group homes to disclose their illness when being evacuated. Participants were PMHI living in group homes in Japan. We conducted an original anonymous self-administered questionnaire based on previous research. Valid data from 119 people were analyzed. Factors associated with the willingness to disclose illness to supporters upon evacuation were "I can imagine living in a public shelter" (Odds Ratio [OR] 4.50, 95% Confidence Interval [CI]: 1.78–11.43), and "I socialize with neighbors" (OR 5.63, 95% CI: 1.74–18.22). Managers of group homes should encourage PMHI to imagine life in an evacuation zone by increasing opportunities for disaster training and for interaction with local residents. People who are less likely to socialize with neighbors should be especially careful, as they may not be able to disclose their illness, and those who support evacuees should pay special attention to these people. Abstract

Aim: The aim of this study was to clarify the actual status of management of triggers ofbehavioral and psychological symptoms of dementia (BPSD) among care managers andthe factors that affect the implementation of management.Methods: An anonymous self-administered questionnaire was conducted for care managersfrom 4000 in-home long-term care support providers throughout Japan. The followingitems were surveyed: gender, basic qualifications, years of work experience with basicqualifications and care manager, cognitive symptoms, underlying illness and physical andmental conditions, living ability, living environment, social participation and life history,precautions when creating care plans, and difficulties in support. Univariate analysis wasconducted according to whether the management of BPSD triggers was implemented.Binary logistic regression analysis was conducted with the items that showed significantdifferences as independent variables, and the implementation of trigger management as thedependent variable.Results: An analysis was conducted on 832 care managers with the basic qualifications ofa certified care worker and nurse. Of these, 430 (53.6%) managed triggers, while 402 (46.4%)did not. Compared to those who didn’t manage triggers, a significantly higher percentageof those who did manage triggers assessed their cognitive symptoms, physical and mentalstatus, living ability, living environment and social participation, and adopted precautionswhen creating care plans. Factors affecting the implementation of trigger managementincluded the patient’s strengths, connection with local residents, promotion of familymembers’ understanding of dementia, consideration of abuse prevention, cooperation withdoctors, and basic qualifications.Conclusions: Half of the respondents implemented trigger management, which wasaffected by the list of precautions when creating care plans, and basic qualifications.Promoting care plans that include support, which increases the self-efficacy of peoplewith dementia, building stable relationships with local residents and family members,and cooperation with doctors, may lead to the implementation of trigger management. Inparticular, the necessity of promoting trigger management among certified care workerswas suggested.

Abstract

Deinstitutionalization of psychiatric care has been associated with increased homelessness, crime, and suicide, partly owing to insufficient, adequate, and accessible community resources. Therefore, appropriate resource placement is a key deinstitutionalization issue. The study's aim was to identify residential group homes for people with mental illness in Kochi Prefecture, Japan, and the social resources necessary for social reintegration using a geographic information system (GIS). Everyday living areas (ELAs), as defined by the Japanese Community-Based Integrated Care System for People with Mental Illness (CICSM), were assessed using ELA location simulations. We used GIS to determine the spatial distribution of group homes, visiting nursing stations, psychiatric hospitals, daycare centers, and employment support offices. Following the CICSM definition of ELAs, we identified areas that people with mental illness could reach within 30 min on foot/by bicycle and counted the number of social resources in them. The ELA location simulation results suggest that policymakers should avoid uniform distribution of ELAs according to the CICSM definition. Establishing ELAs in suburban areas requires careful consideration of the available community resources, number of people with mental illness, existing support systems, and feasibility of the location.
