Abstract

Tumor markers such as carbohydrate antigen 19-9 (CA19-9) are generally useful in ruling out malignancy of hepatic cysts. The patient was a 72-year-old man who had a ruptured liver cyst in the right liver, which had been noted since he was 67 years old at another hospital. The initial laboratory tests demonstrated elevated CA19-9 (193 784.3 U/mL). We made the diagnosis with a simple ruptured liver cyst from magnetic resonance imaging and cytological examination of ascites, and laparoscopic fenestration with drainage of the abdominal fluid was performed. Pathological diagnosis of the resected wall cyst was non-parasitic simple hepatic cyst with acute inflammation and hemorrhage. The patient's serum levels of CA19-9 were 164.0 U/mL on postoperative day 23. The follow-up abdominal computed tomography scan performed 2 months later did not any finding of tumor. Abstract

Pancreatic ductal adenocarcinoma (PDAC) is associated with a poor prognosis, and it has a recurrence rate of >70%, even in resectable cases. The treatment strategy for recurrent PDAC involves systemic chemotherapy, with gemcitabine (GEM) monotherapy historically serving as the standard of care. The present study describes the case of a patient with PDAC and postoperative liver metastases that maintained clinical complete remission (cCR) for >7 years following GEM monotherapy. A 63-year-old woman with upper abdominal pain was diagnosed with resectable PDAC and underwent pancreaticoduodenectomy. The patient was treated with GEM + S-1 as adjuvant chemotherapy for 6 months. Multiple liver metastases were detected 15 months post-operation and the patient was administered GEM alone. After 12 cycles, computed tomography showed cCR and GEM monotherapy was discontinued after 15 cycles. The patient has had no signs or symptoms of recurrence >7 years after the first recurrence. In addition, the present study analyzed PDAC resection specimens from four patients, including this case, to determine the expression levels of hENT1 protein in the tumor tissues. hENT1 is a transmembrane protein that acts as a nucleoside transporter and is a major mediator of GEM uptake into human cells. In the present case, hENT1 staining exhibited low frequency and weak positivity in the central region, whereas a strong positive reaction was observed in nearly all cell membranes at the invasive front of the cancer. The location, intensity, and frequency of hENT1 staining varied among cases. In conclusion, the efficacy of GEM may be predicted prior to treatment by evaluating hENT1 expression. Abstract

Simple Summary: In pancreatic cancer, recurrence rates after surgery remain high. The ability to identify patients at risk of early recurrence before surgery will contribute to the selection of treatment strategies. We examined the value of preoperative dual time point (DTP) 18F-fluorodeoxyglucose positron emission tomography/computed tomography fusion imaging (FDG PET/CT) as a predictor of early recurrence in or the outcomes of patients with pancreatic cancer. The results showed that DTP FDG PET/CT may effectively predict relapse in patients, and the combination of SUVmax1 and ΔSUVmax% identified early recurrent patient groups more precisely than SUVmax1 alone. We examined the value of preoperative dual time point (DTP) 18F-fluorodeoxyglucose positron emission tomography/computed tomography fusion imaging (FDG PET/CT) as a predictor of early recurrence or the outcomes in patients with pancreatic cancer. Standardized uptake values (SUVs) in DTP FDG PET/CT were performed as preoperative staging. SUVmax1 and SUVmax2 were obtained in 60 min and 120 min, respectively. ΔSUVmax% was defined as (SUVmax2 − SUVmax1)/SUVmax1 × 100. The optimal cut-off values for SUVmax parameters were selected based on tumor relapse within 1 year of surgery. Optimal cut-off values for SUVmax1 and ΔSUVmax% were 7.18 and 24.25, respectively. The combination of SUVmax1 and ΔSUVmax% showed higher specificity and sensitivity, and higher positive and negative predictive values for tumor relapse within 1 year than SUVmax1 alone. Relapse-free survival (RFS) was significantly worse in the subgroups of high SUVmax1 and high ΔSUVmax% (median 7.0 months) than in the other subgroups (p < 0.0001). The multivariate Cox analysis of RFS identified high SUVmax1 and high ΔSUVmax% as independent prognostic factors (p = 0.0060). DTP FDG PET/CT may effectively predict relapse in patients with pancreatic cancer. The combination of SUVmax1 and ΔSUVmax% identified early recurrent patient groups more precisely than SUVmax1 alone. 本研究では，国外の学校と交流学習に取り組んだことのある生徒を対象に，（a）普段の生活におけるマルチメディア利用，（b）国際交流学習時における静止画や動画の利用についてアンケート調査を行い，交流学習におけるマルチメディアの利用とその課題について検討した．その結果，生徒は，普段の生活において，携帯電話のカメラ機能やデジタルカメラを問題なく利用していた．とりわけ，携帯電話のカメラ機能をデジタルカメラよりも頻繁に使用し，携帯電話で撮った写真をメールに添付して知り合いに送る傾向があった．くわえて，国際交流学習において，（ 1 ） 繰り返し内容を確認する場合，動画を見るのと文章を読むのでは，動画を好む傾向があった．また，（ 2 ）インターネットで検索した写真をメッセージに添付する傾向があることが分かった．さらに，（ 3 ） 生徒は相手のトピックを知る，自身のトピックを伝えるのに，写真や動画（リンク）を用いることが有用であると認識していたものの，実際は，写真を活用し，動画のリンクはあまり利用していなかった．また，生徒が気軽に使える機器の用意や，写真や動画をインターネット上で簡単に検索できる仕組みを検討する必要があることが分かった．, In this paper, the use of multimedia in international exchange learning and its issues were discussed, based on findings from a questionnaire. In the questionnaire, the following two topics were asked participants who have experiences to communicate with students in abroad as exchange learning: (a) the use of multimedia (e.g. a cell phone camera and digital camera) in daily life; (b) the use of a still image and video image in international exchange learning. As a result, the students used the cell phone camera and digital camera without any difficulties in daily life. They displayed a tendency to use the cell phone camera functions more than the digital camera, and they were likely to send the picture, which they took using the cell phone camera, to their friend(s). With regard to international exchange learning, (1) when repeatedly checking contents, the students were more likely to watch the movie than read the text.; (2) they tended to attach pictures from the Internet in a message; and (3) they understood that the use of a still image and/or video (a link) could be helpful in international exchange learning. However, the students were more likely to use still images rather than videos when communicating with partner students. To sum, the following items are necessary in international exchange learning : a device, such as a digital camera, that is simple for students to use, and a system that makes it easy to search for relevant still images and/or videos over the Internet.

Abstract

本論文では，遠隔交流学習で用いられるICT（情報通信技術）のうち，左右反転した自己像と遠隔地の相手像とを同一画面上に重畳表示することで，まるで同じ空間にいるかのように対話できる「超鏡」を取り上げ，遠隔学習の授業設計をするにあたって重要である学習環境の理解を目指す研修について，Webサイトの活用を提案するとともに評価を行った．その結果，Webサイトが「超鏡」の理解に有効であることが確認された．加えて，Webサイトと「超鏡」の体験とで理解される内容に違いがあり，Webサイトのみでも理解できる特性と，体験することでより会得できる特性があることが明らかになった．これらを踏まえ，「超鏡」の学習環境の理解の支援について，Webを活用した情報提供と，手軽に「超鏡」を体験できる研修の複合システムを提案した．今後は，本論文を通じて提案した支援システムを構築し，効果を検証したい．, This paper focuses on HyperMirror—one of the distance learning environments—and discusses the course of lectures on HyperMirror. HyperMirror is a special type of videoconferencing system. In HyperMirror, everyone appears in the same video image; in other words, everyone appears as if they are in the same room. The characteristics of HyperMirror effect on distance learning. In addition, it is important to understand the characteristics of HyperMirror in order to plan the lesson utilizing HyperMirror. In this paper, first, we developed a Web site that provides information about HyperMirror and explains its characteristics. Second, we implemented a course of lecture combined with visiting the Web site and the experience of HyperMirror. To begin with, participants in this combined course visited the Web site and obtained information about HyperMirror. Next, they experienced HyperMirror. According to the questionnaire that the participants answered before visiting the Web site and after the experience, it was clear that the Web site was useful for understanding the characteristics of HyperMirror. However, some characteristics of HyperMirror were well understood by only the Web site, the others were understood better by the experience of HyperMirror. On the basis of these results, we believe that not only information about HyperMirror but also experience of HyperMirror should be available on the web.

39. Thermodynamics of the Thermal Denaturation of Acid Molten Globule State of Cytochrome cIndicate a Reversible High-Temperature Oligomerization Process

Abstract

In this study, we performed differential scanning calorimetry (DSC) and pressure perturbation calorimetry (PPC) analysis of the thermal transition of cytochrome cfrom an acidic molten globule (MG) state with the protein concentrations of 0.5–18.2 mg/mL. DSC profiles were highly reversible and showed clear protein-concentration dependence, indicating that reversible oligomerization occurred accompanying the thermal transition from the MG state. The DSC and PPC data required at least a six-state model (MG1 ⇄ MG2 ⇄ D ⇄ 1/2I2⇄ 1/3I3⇄ 1/4I4) including three new oligomeric states: dimer (I2), trimer (I3), and tetramer (I4) in addition to the three monomeric states previously characterized. Dynamic light scattering confirmed the oligomerization during the thermal transition. The partial specific volumes of these oligomeric states were found to be smaller than those of the monomeric states, MG2 and D, indicating dehydration of hydrophobic surface or hydration of released anions may occur with the reversible oligomerization.

Abstract

Phenylalanine dehydrogenase produced by Bacillus badius IAM 11059 was purified from the crude extract of B. badius to homogeneity, as judged by disc gel electrophoresis. The enzyme has an isoelectric point of 3.5 and a relative molecular mass, Mr, of 310000–360000. The enzyme is composed of identical subunits with an Mr 41000–42000. The substrate specificity of the enzyme in the oxidative deamination reaction was high for L-phenylalanine, but rather low in the reductive amination reaction, with phenylpyruvate, p-hydroxyphenylpyruvate, and 2-oxohexanoate. The gene for the enzyme was cloned into Escherichia coli with plasmid pBR322 as a vector. The enzyme was expressed in high level in E. coli. The enzyme produced by E. coli transformant was purified to homogeneity and shown to be identical to that of B. badius IAM 11059 with respect to the specific activity, Mr, subunit structure and amino acid composition.
