U prospektivno kliničko ispitivanje uključeni su bolesnici koji su doživjeli neočekivani srčani zastoj (NZS) tijekom 2003. godine u KB Split, lokalitet Firule. Iz medicinske dokumentacije i anketnog listića zabilježene su demografske karakteristike, etiologija i način prezentacije NZS, te vrijeme, mjesto, način i uspješnost reanimacije. Zabilježeno je 120 NZS od 32861 hospitaliziranih. To predstavlja 12, 5% od ukupno 962 umrla bolesnika. Reanimirano je 96 (80%), a otpušteno kući 27 (22, 5%) bolesnika s NZS. U Klinici za unutarnje bolesti taj učinak je 20%, u Jedinici koronarne skrbi je 29, 2%, a u ostalim klinikama KB Split samo 8% (χ 2=4, 9 ; p=0, 03). Osvjedočenih NZS je bilo 92 (76, 7%) od kojih je 26 (28, 3%) preživjelo i otpušteno kući. Od 25 (20, 8%) neosvjedočenih NZS, samo je jedna osoba živa napustila bolnicu (4% ; χ 2=6, 5 ; p0, 05). Preživljavanje ovisi o tipu zastoja srca, osvjedočenosti NZS i dobi bolesnika, dok o spolu i vremenu pojave NZS ne ovisi.

The Outpatient Hypertension Clinic in Split, Croatia, established in 1975, published its first operative studies more than 20 (Acta Med Salin 1980 ; 9:33-44), and then 10 years ago (Liječ Vjesn 1993 ; 115:30-4). In the following 10 year period (1993-2003) 10, 000 patient data were analyzed from the protocols. In this period the female sex was preponderant (50.3% vs. earlier 33.7% ; p0.05), and had grade I/II hypertension (57.2% vs. 58.0% ; p>0.05). The eye-ground changes in most current patients were either nonexistent (42.9%) or minimal (up to grade II 56.8% of the examinees). The significant, grade III/IV changes marginally increased in the last 10 years (0.75% vs. 0.65% ; p=0.51). The aim of this study was to test the hypothesis that prolonged treatment of mild to moderate hypertension with low-dose thiazide diuretics or beta blockers does not induce any of the major untoward biochemical changes, such as hypertriglyceridemia, hypercholesterolemia, hyperuricemia and electrolyte imbalances. The effect of these drugs was analyzed in 100 outpatients (52 males and 48 females), aged 52.0 +/-7.9 years with mild to moderate hypertension in a prospective 6-month study. After an appropriate > work-up the patients were randomized to either 25 mg chlorthalidone (40 pts), 120 mg propranolol (30 pts) or 2 mg bopindolol (30 pts). A significant reduction by approximately 10% in systolic and diastolic blood pressure was recorded in all the groups. At the end of the 6th month in the chlorthalidone group trigycerides increased to 3.0+/-2.1 mmol/l from 2.8+/-1.6 mmol/l, while cholesterol after an initial increase to 6.6+/-1.6 from 6.4+/-1.6 mmol/l returned to the baseline level. Uricemia and serum potassium decreased by 4%. The body weight was reduced from 86.1+/-13.4 to 83.8+/-13.4 kg. There was no change in serum glucose level. In the propranolol group, as expected, heart rate decreased by 20% but there were no significant changes in glucose or potassium plasma concentrations. Triglycerides did not change significantly, while cholesterol after a small increase returned to the initial levels. Similar results were obtained in the bopindolol group, apart from the triglycerides which increased significantly (to 2.5+/-1.1 from 2.2+/-0.4 mmol/l), probably because of the lower baseline concentration. We conclude that in prolonged teratment chlorthalidone, propranolol and bopindolol do not induce significant untoward biochemical changes that alone might increase cardiovascular risk.
