1. Prevalence of Anxiety and Depression among Caregivers of Cancer Patients - A Case Study in a Public Hospital in Malaysia.

Abstract

INTRODUCTION: Anxiety and depression are common mental illnesses among cancer caregivers. Most caregivers often sacrifice a lot of time, energy and their own physical and emotional needs. MATERIALS AND METHOD: A cross sectional study was conducted among 156 caregivers of cancer patients who provided treatment and moral support to cancer patients at the Oncology clinic of Sultan Ismail Hospital (SIH), Johor Bahru, Malaysia. The Hospital Anxiety Depression Scale (HADS) was used to assess the depression and anxiety levels among the respondents. Data was analysed using Statistical Package for Social Sciences version 17.0 software. The P value of less than 0.5 was taken a significant. RESULTS: In general, there was moderate relationship between cancer caregivers and patients in term of emotional, care and patient treatment, but in some circumstances, caregivers were also exposed to risk during care period of cancer patients. There was a significant relationship with moderate correlation between factors of taking care of emotion of patients (p<0.05, r=0.403), their diseases (p<0.05, r=0.456) and treatment (p<0.05, r=0.600) with the level of anxiety. There was a strong correlation between caring and anxiety in the item of lack of attention on patient demand (p<0.05, r=0.91) and not angry to patient (p<0.83). CONCLUSION: The awareness regarding the anxiety and depression among caregivers need to be increased among health professionals such as physicians and nurses. 3. Toxic Multinodular Goitre (Thyrotoxicosis with Hyperthyroidism) Induced Cardiomyopathy: A Case Report

Abstract

Introduction: Toxic multinodular goitre, first described by H.S. Plummer in 1913, is unremitting and often develops slowly, with more subtle symptoms than Graves’ disease. Cardiac symptoms such as tachycardia, heart failure, or arrhythmia and atrial fibrillation are most frequent. Here we describe a case who presented with symptoms of thyroid enlargement and heart failure. Case report: A 48-year old female presented to us with complaints of dysphagia, hoarseness of voice, breathlessness on exertion and palpitations since one month ago. Furthermore, the patient gave history of swelling of the neck which was initially pea sized and gradually increased to the current size over a period of two months. On examining the swelling of the neck, thyroid gland appears enlarged, firm with multiple nodules. Two-dimensional echocardiography revealed a dilated left ventricle with generalized hypokinesia. Computed Tomography of the neck suggested enlarged thyroid gland (12.1 cm × 6.5 cm) with heterogenous architecture, and thyroid gland encircling the trachea for approximately 270 degree with mass effect. Thyroid scan showed multinodular goitre with multiple hyperfunctioning nodules of both lobes and warm nodules only in left lobe. The patient was diagnosed as multinodular goitre with cardiomyopathy. The patient was treated medically with methimazole, propranolol, aspirin, ramipril, sustained release urodeoxycholic acid, rosuvastatin, pantoprazole and multivitamin. The patient underwent near total thyroidectomy with radioactive iodine ablation, as and when required. Conclusion: Patients with toxic multinodular goitre very frequently present with cardiovascular symptoms, which when identified and treated early can reduce the morbidity significantly.

5. Weil’s Disease (Fulminating Leptospirosis) Complicating as Acute Respiratory Distress Syndrome

Abstract

Leptospiroisis presents with a wide range of clinical symptoms, ranging from mild disease symptoms to life threatening complications. The clinical features and routine laboratory findings are not specific, and therefore a high index of suspicion must be maintained for the diagnosis. We documented a case of a patient, who presented with high grade fever, diarrhea, vomiting, had icterus on examination. Initial laboratory investigations revealed low platelet count, along with raised liver enzymes. IgM leptospirosis came positive and patients received broad spectrum antimicrobials and supportive fluid therapy. During admission, patient developed sudden onset breathlessness, which was diagnosed as acute respiratory distress syndrome after doing immediate arterial blood gases. The patient received noninvasive ventilation with positive airway pressure, from which the patient eventually recovered. Most of the cases of leptospirosis are uneventful with complete recovery. However, a minority of these cases may end with life threatening complications. A vaccine for human use is not available in most countries. Therefore, the prevention involves reducing exposure, either by avoiding situations that pose a risk of transmission or using protective clothing.

6. Conn’s Syndrome Secondary to Adrenal Adenoma

Abstract

Primary hyperaldosteronism is not as uncommon as we thought, and by recent estimates the prevalance may be as high as 11% among hypertensives. We present a case of a 33 years old male patient came with a complaint of headache in occipital area & weakness in both lower limb since 4 weeks. The patient was a diagnosed case of hypertension and was already on tablet Atenolol (25 mg) once a day. At the time of admission, the patient was found to have a pulse of 96/min and blood pressure of 170/100 mm of Hg. Power was 3/5 for both right and left lower limbs. At the time of admission, serum potassium was 2.8. As a result, the patient was shifted to tablet Metroprolol and Amlodipine (50 + 5 mg) once a day. We also added tablet Cilnidipine (10 mg) and Telmisartan (80 mg) once a day and syrup Potklor thrice a day with water. The patient was sent for renal artery doppler, urine for metanephrines and vanillylmandelic acid, serum aldosterone, plasma aldosterone/plasma renin activity ratio (PAC/PRA ratio), urinary aldosterone, urinary cortisol and adrenocorticotrophic hormone level and computed tomography (CT) abdomen to rule out hyperaldostronism. Plasma aldosterone/plasma renin activity ratio, urinary aldosterone was high and CT abdomen revealed a solitary nodule in the left adrenal gland measuring 1.2 × 1.0 cm in maximum transverse diameter, which was suggestive of adrenal adenoma. Diagnosis of Conn’s syndrome secondary to adrenal adenoma was made. The patient was advised tablet Aldactone (25 mg) once a day and serum electrolytes were repeated which showed normalization with normal blood pressure.

Abstract

In this paper, we present a new method for quantifying the uncertainty of economic projections due to uncertainty in future oil prices. Traditionally, the petroleum industry has employed what are known as “hockey stick” price forecasts, i.e., monotonically increasing price profiles, in economic calculations to evaluate investment opportunities. Calculations are often run using most-likely, optimistic, and pessimistic price profiles in an attempt to quantify the uncertainty in the resulting economic indicators. These conventional hockey stick methods significantly underestimate uncertainty because they do not reproduce the volatility inherent in oil prices. Stochastic methods that attempt to model price volatility have been used successfully and indicate that there is considerably more uncertainty in oil and gas development projects than has been previously recognized. 14. Effects of N-acetyl-cysteine and acetylsalicylic acid on the tonsil bacterial biofilm tissues by light and electron microscopy.

Abstract

OBJECTIVE: The present study aimed to investigate the effects of the bacterial biofilm formation on the tonsil surface exposed N-acetyl-cysteine (NAC) and acetylsalicylic acid (ASA) of patients undergoing tonsillectomy by light and electron microscopy. The general process of biofilm formation comprises adhesion of free-living or planktonic bacteria to a surface, which subsequently develop into micro-colonies and form a biofilm. Based on studies that have shown the presence of biofilms in common sites of chronic infections, it has become clear that bacteria may persist on mucosal surfaces through formation of biofilms. PATIENTS AND METHODS: Ten patients between 4 and 39 years of age (mean, 11.9 ± 11.2 years). In all cases, periodic acide Schiff (PAS) staining was found to be an accurate predictor of the presence or absence of biofilm using light microscopy as a control standard. Therapeutic doses of NAC and ASA were identificated as the effective on the tonsil bacterial biofilm using light and electron microscopy. RESULTS: Biofilm formation was detected on all samples. Tonsils removed from patients with ASA-10 had showed higher-grade inhibitory effect at the biofilm formation than the other group (p ≤ 0.0001). The correlation was found between drug dose and decrease at the biofilm formation. CONCLUSIONS: In chronic or recurrent tonsillitis patients, decrease on the tonsils surface biofilm formation may be associated with ASA dose. 15. Inverted Hockey Stick Method Quantifies Price Uncertainty in Petroleum Investment Evaluation.

Abstract

In this paper, we present a new method for quantifying the uncertainty of economic projections due to uncertainty in future oil prices. Traditionally, the petroleum industry has employed what are known as “hockey stick” price forecasts, i.e., monotonically increasing price profiles, in economic calculations to evaluate investment opportunities. Calculations are often run using most-likely, optimistic, and pessimistic price profiles in an attempt to quantify the uncertainty in the resulting economic indicators. These conventional hockey stick methods significantly underestimate uncertainty because they do not reproduce the volatility inherent in oil prices. Stochastic methods that attempt to model price volatility have been used successfully and indicate that there is considerably more uncertainty in oil and gas development projects than has been previously recognized. However, many operators do not use stochastic methods for modeling oil prices, most likely because they require more time and effort to implement than conventional methods. The Inverted Hockey Stick method presented herein is similar to conventional methods in that only three price realizations are run to quantify uncertainty. However, the high and low cases are designed to better capture the range of possible future price paths. Uncertainty ranges for economic indicators predicted by the new method are comparable to 70–95% probability ranges predicted by the stochastic bootstrap method, significantly greater than the 32–42% ranges predicted by conventional methods. This new method can be easily incorporated into existing economic modeling systems. Recognition of the greater uncertainty in oil and gas investment opportunities, both upside as well as downside, should improve investment decision making. [ABSTRACT FROM AUTHOR]

16. Lifetime Lamb and Wool Production of Targhee or Finn-Dorset-Targhee Ewes Managed as a Farm or Range Flock

Abstract

Lifetime (5 years) lamb and wool production from 207 straightbred Targhee (T) and 474 1/4-Finn 1/4-Dorset 1/2-Targhee (FDT) crossbred ewes managed in a range or farm flock system were evaluated for ewe fertility, prolificacy, lamb birth weight, lamb survival, lamb weaning weight, ewe fleece weight, and total lamb weight weaned per ewe exposed. Data included 2620 ewe exposures, 2384 parturitions, 4638 lambs born and 3498 lambs weaned. Ewes were born in 1984 through 1987 and lambed the first time as 2-yr-olds. The basic model included main effects of ewe breed, management system, and age of ewe. Lamb birth type and lamb sex were added for lamb birth weight, weaning weight and survival analyses. Expressed as mean annual values ewe fertility rate was similar (90. 6 vs 89.3%, P = .2755) between the FDT and T ewes. Range flock ewes had a higher (P < .01) fertility rate than farm flock ewes (93.3 vs 86.5%). Targhee ewes had heavier lambs at birth (5.1 vs 4.6 kg, P < .01) and weaning (25.8 vs 25.4 kg, P = .0521) than FDT ewes. Finn-Dorset-Targhee ewes had heavier (P < .01) lamb weaning weights (26.2 vs 25.0 kg) and had higher lamb survival (74.9 vs 70.7%) than Tewes. Range flock ewes had higher (P < .01) lamb survival rates than farm flock ewes (75.4 vs 70.2%). Over the 5 yr period of the study for ewes entering the experiment, FDT ewes had higher (P < .01) cumulative number of lambs born (7.42 vs 5.70), number of lambs weaned (5.48 vs 4.38) and total lamb weight weaned (131.7 vs 110.5 kg) per ewe exposed than Tewes. Targhee ewes produced more (P < .01) wool (16.1 vs 13.6 kg) than the FDT ewes. Farm flock ewes had produced more wool (15.2 vs 14.5 kg, P < .01) than range flock ewes. Ewes in the range flock had a higher cumulative number of lambs born (6.74 vs 6.38, P < .05), number of lambs weaned (5.39 vs 4.48, P < .05) and total lamb weight weaned (135.8 vs 106.5 kg, P < .01) per ewe exposed than farm flock ewes. Percent of ewes remaining in the flock at the end of the study was higher (P < .01} for the range flock (53.3 vs 40.5%) and FDT ewes (48.5 vs 43.0%} compared to the farm flock and T ewes, respectively. Incorporation of Finnsheep and Dorset breeding increased the reproductive performance in both management systems but decreased wool production. Finn cross ewes produced equally as well under range management as in a farm flock situation.

Abstract

This study examines the effects of milled Tribulus terrestris (TT), Avena sativa (AS), white ginseng (WG) and triple-combination (TC) powders on sexual dysfunction parameters - such as serum total cholesterol, free testosterone levels and histopathological changes in testicular tissue - in rats fed a high-cholesterol diet. The study's animal matenal consisted of 42 male Wistar albino rats weighing 200-210 g divided into six groups. Group I was fed normal pellet feed, while the remaining groups were fed pellet feed containing 2% cholesterol. Group III, IV, V, and VI also received 0.6 g/kg/day of TT, 0.3 g/kg/day of AS, 0.2 g/kg/day of WG and 0.55 g/kg/day of TC (7.5% TT, 3.75% AS, 2.5% WG), respectively. After 90 days, the rats were sacrificed and blood and testicular tissue samples obtained. Serum total cholesterol and free testosterone levels were measured, and the Johnsen testicular biopsy score (JTBS) was calculated by a histopathological examination of testicular tissue samples. The high-cholesterol diet in Group II significantly caused increase in total cholesterol level and decrease in JTBS as compared to Group I. Although the groups' free testosterone levels were not statistically significant, WG and TC significantly prevented total cholesterol increase. TC significantly increased the JTBS compared to TT, AS and WG alone. Thus, it was concluded that TC might be particularly efficient for improving male sexual dysfunction.

Abstract

The aim of this study was to investigate seminal oxidant-antioxidant activity in idiopathic and varicocele infertility in men.Total anti-oxidant capacity (TAC), total oxidant status (TOS), paraoxonase (PON1), aryl esterase (ARE), and total thiol levels (TTL) were measured in seminal plasma with an autoanalyzer. The TOS/TAC ratio was determined as the oxidative stress index (OSI). A histopathological evaluation of the sperm was performed in the andrology laboratory of the hospital. Number, motility, morphology, volume, pH, and leukocytes were evaluated in all samples according to World Health Organization criteria. The three study groups were as follows: G1, males with idiopathic infertility; G2, males with varicocele infertility; and G3, normal healthy males (had fathered a child in the last 2 years). Each group was composed of 36 men (age, 25 - 40 years). The Rel Assay Diagnostics kit was used to determine the levels of the parameters. The study was conducted according to the principles of the declaration of Helsinki and was approved by Sakarya University Medicine Faculty Ethic Committee (e.n: 16214662/050.01.04/07). Statistical significance was assumed if p0.05. All statistical evaluations were performed using SPSS (version 20.0 for Windows; SPSS, Inc., Chicago, IL, USA).No differences were detected between the mean values of antioxidant parameters among the three groups (Kruskal-Wallis test). The p-values of the test parameters (TAC, TOS, PON1, ARE, TTL, OSI) are respectively: 0.494, 0.548, 0.068, 0.151, 0.202, 0.873. The antioxidant parameters of all subjects were compared using the MannWhitney U-test in both groups as fertile (G3) and infertile (G1 + G2). The PON1 levels in infertile subjects were significantly higher than those in fertile subjects. There was a statistically significant difference (p = 0.042). The other antioxidant parameters had no statistically significant difference (p0.05). The ARE was not performed in group 3 (control) due to a methodological problem.PON1 levels in infertile subjects were significantly higher than those of fertile subjects.

Abstract

Research on disease detection by leveraging machine learning techniques has been under significant focus. The use of machine learning techniques is important to detect critical diseases promptly and provide the appropriate treatment. Disease detection is a vital and sensitive task and while machine learning models may provide a robust solution, they can come across as complex and unintuitive. Therefore, it is important to gauge a better understanding of the predictions and trust the results. This paper takes up the crucial task of skin disease detection and introduces a hybrid machine learning model combining SVM and XGBoost for the detection task. The proposed model outperformed the existing machine learning models - Support Vector Machine (SVM), decision tree, and XGBoost with an accuracy of 99.26%. The increased accuracy is essential for detecting skin disease due to the similarity in the symptoms which make it challenging to differentiate between the different conditions. In order to foster trust and gain insights into the results we turn to the promising field of Explainable Artificial Intelligence (XAI). Abstract

Background: Aging results in many changes in health status, body composition, muscle strength, and, ultimately, functional capacity. These changes coincide with significant alterations in the endocrine system, such as insulin-like growth factor-1 (IGF-1) and IGF-binding proteins (IGFBPs), and may be associated with many symptoms of aging. The objectives of this study is to investigate the potential influence of different types of exercise, such as resistance training and aerobic training, on IGF-1 and IGFBP-3 levels in postmenopausal women., Methods: Medline, Scopus, and Google Scholar databases were systematically searched up to November 2023. The Cochrane Collaboration tool was used to assess the risk of bias and the quality of the studies. The random-effects model, weighted mean difference (WMD), and 95% confidence interval (CI) were used to estimate the overall effect. Between-study heterogeneity was assessed using the chi-squared and I 2 tests., Results: Seventeen studies were included in the present systematic review and 16 studies were included in the meta-analysis. The pooled results from 16 studies (21 trials) with 1170 participants examining the impact of exercise on IGF-1 concentration showed a significant increase in IGF-1, and the pooled results among six studies (trials) showed a significant decrease in IGFBP-3 concentration (730 participants). In addition, resistance training and aerobic training had a significant effect on increasing IGF-1 concentration post-exercise compared with placebo., Conclusion: Based on this meta-analysis, Women who have completed menopause and followed an exercise routine showed changes in IGF-1 and IGFBP-3 levels that can indirectly be associated with risk of chronic age-related conditions., (© 2024. 21. Long term outcomes of percutaneous coronary intervention vs coronary artery bypass grafting in patients with diabetes mellitus with multi vessels diseases: A meta-analysis.

Abstract

Aims: Long term cardiovascular outcome comparison of multivessel coronary disease among patients with diabetes mellitus (DM) undergoing percutaneous coronary intervention (PCI) or coronary artery bypass graft (CABG) is limited. The objective of this study was to compare the long-term cardiovascular outcome PCI vs CABG among DM patients with multivessel disease., Method and Results: Online databases were explored to identify studies that compared cardiovascular outcomes between PCI and CABG among patients with DM. The primary outcome was all-cause mortality. Secondary outcomes included major adverse cardiovascular and cerebrovascular events (MACCE), myocardial infarction (MI), rate of revascularization, cardiac death, and cerebrovascular accident (CVA). A total of 27 studies with 37,091 (PCI n = 19,838 (53 %) and CABG n = 17,253 (47 %)) patients were included. The mean age was 64 ± 5.9 years for PCI group and 63.8 ± 5.3 years for CABG group; and, predominantly male (71.22 % vs 74.29 %) for PCI and CABG respectively. The most common comorbidity was hypertension (64.35 % vs 62.88 %) in both PCI and CABG respectively. Compared with CABG, PCI group had a higher odds of overall all-cause mortality (OR 1.18, 95 % CI 1.02-1.37, p = 0.03), MACCE (OR 1.52, 95 % CI 1.31-1.75, p = 0.00), MI (OR 1.85, 95 % CI 1.46-2.36, p = 0.00), repeat revascularization (OR 3.08, 95 % CI 2.34-4.05, p = 0.00) and cardiac death (OR 1.27, 95 % 1.02-1.59, p = 0.04), while CVA (0.57, 95 % CI 0.37-0.86, p = 0.01) was higher with CABG., Conclusion: Diabetic patients with multivessel coronary artery disease have worse outcomes undergoing PCI as compared to CABG. However, CVA was significantly higher with CABG. Abstract

Background: Although various dietary patterns have been indicated to be associated with the resting metabolic rate [RMR], limited data are available in this field. This study was therefore focused on the association between dietary patterns and resting metabolic rate among participants with overweight and obesity., Methods: This cross-sectional study was conducted on 304 women with overweight or obesity (BMI ≥ 25 kg/m 2 ), aged 18-50. Anthropometric assessments, physical activity and biochemical measurements were assessed. RMR was also measured by means of indirect calorimetry. Dietary intake of participants was evaluated by 147-item semi-quantitative food frequency questionnaire [FFQ]., Results: There was a significant association between higher adherence to the healthy dietary pattern [HDP] and RMR (P = 0.05), intakes of protein (P = 0.003), minerals (P = 0.001) as well as fat free mass [FFM] (P = 0.002), bone mineral content (P = 0.001), skeletal muscle mass (P = 0.001), soft lean mass (P = 0.002) and visceral fat area (P = 0.05). Also, there was a considerable association between higher adherence to the unhealthy dietary pattern [UHDP] and fasting blood sugar [FBS] (P = 0.05). Using multinomial logistic regression has been shown that the medium adherence to the HDP was marginally significant with decreased resting metabolic rate [Dec. RMR] group in crude model (OR: 0.54; 95% CI: 0.28-1.05, P = 0.07). After controlling for various confounders such as age, FFM, physical activity, and energy intake, the association between Dec. RMR group and the lowest quartile of the HDP (OR: 0.36; 95% CI: 0.14-0.91, P = 0.03) became significant as well as the association between Dec. RMR group and medium adherence to the HDP (OR: 0.42; 95% CI: 0.18-0.97, P = 0.04). The medium adherence to the UHDP in crude model was also significant with increased resting metabolic rate [Inc. Abstract

Aim: RBP4, Vaspin and omentin-1 are adipokines, which play an important role in the development of obesity-related complications. The main aim of this study was to investigate the effects of different kinds of fat intake on adipokine levels in obese women., Methods: A total of 272 obese women (BMI ≥ 30) were included in the current cross-sectional study, according to the inclusion and exclusion criteria. Body composition was measured using a body composition analyzer. For the measurement of retinol binding protein 4 (RBP4), vaspin and omentin-1 serum concentrations, an enzyme-linked immunosorbent assay (ELISA) method was used. Dietary intake was assessed using a 3-day 24-h dietary recall., Results: Statistically significant differences were found between polyunsaturated fatty acid (PUFAs) and linoleic acid intake and vaspin and omentin-1 levels. In addition, there were found statistically significant relationships between cholesterol, monounsaturated fatty acid (MUFAs) and total fat intakes with omentin-1 levels. Also, RBP4 and vaspin levels were different significant with eicosapentaenoic acid (EPA) and docosahexaenoic acid (DHA) intake. Background: curcumin consumption may have a protective effect against exercise-induced muscle damage (EIMD) through stabilization of the cell membrane via inhibition of free radical formation. Evidence supporting a protective role of curcumin after physical activity induced muscle injury in humans, however, it is inconsistent., Methods: Medline, Scopus, and Google scholar were systematically searched up to May 2020. The Cochrane Collaboration tool for assessing the risk of bias was used for assessing the quality of studies. Random effects model, weighted mean difference (WMD), and 95% confidence interval (CI) were used for estimating the overall effect. Between-study heterogeneity was assessed using the chi-squared and I 2 statistic., Results: The results revealed a significant effect of curcumin supplementation on reducing creatine kinase (CK) (weighted mean difference [WMD] = -48.54 IU.L -1 ; 95% CI: -80.667, -16.420; p = .003) and muscle soreness index decrease (WMD = -0.476; 95% CI: -0.750, -0.202; p = .001). Moreover, a subgroup analysis resulted in a significant decrease in CK concentrations and muscle soreness index, according to follow-ups after exercise, dose of curcumin, duration of studies, exercise type, train status and study design., Conclusions: The current evidence revealed a efficacy of curcumin in reducing CK serum levels and muscle soreness index among adults. Objective: Mental health, sleep quality, and dietary intake are interlinked. Impairment of mental health and low sleep quality may contribute to obesity through the consumption of diets high in energy density. Nevertheless, it is not clear whether dietary energy density (DED) influences mental health. This study aimed to examine the association of DED with mental health indices, including depression, anxiety, stress, and sleep quality in women with overweight/obesity., Results: There was a decreasing trajectory in serum triglyceride across quartiles of DED (from Q1 to Q4) in the crude analysis and also after adjustment for age, BMI, and physical activity After adjustment for age, BMI, and physical activity, subjects in the highest quartile of DED had higher systolic and diastolic blood pressure. DED was significantly associated with increased odds of stress in the crude (OR = 2.15, 95% CI 1.01-4.56, p = 0.04) and adjusted model for age, BMI, and physical activity (OR = 2.56, 95% CI 1.13-5.79, p = 0.02). No significant relationship was observed between DED and depression, anxiety and sleep quality. In conclusion, current study shows preliminary evidence of an association between DED and stress.

Abstract

Background: Recent studies have shown that depression is inversely correlated with high protein and low fat intake and positively correlated with vitamin D-binding protein (VDBP). Therefore, the aim of this study was to examine the interaction between protein/fat dietary patterns and VDBP genotypes with regard to the risk of depression in apparently healthy adults who have not been diagnosed with any chronic disease., Methods: In this study, 265 individuals (126 males and 139 females) aged 18-55 years were recruited from the communities of central and west Tehran based on convenience sampling. Body composition was measured with a body composition analyzer and depression symptoms were categorized as normal, moderate depression, or severe depression using the Depression Anxiety Stress Scales 21 (DASS-21) questionnaire. Dietary patterns were determined by a semiquantitative food frequency questionnaire to assess typical food intake during the 12-month period. Blood samples were collected from and biochemical measurements performed on all participants. An analysis of two polymorphisms (rs7041 and rs4588) in the GC gene, which encodes VDBP, was performed by polymerase chain reaction-restriction fragment length polymorphism., Results: A statistically significant association was found between depression and diet (p = 0.03) after having categorized the participants into three groups: a high-protein/low-fat (HP/LF) group, a moderate-protein/moderate-fat (MP/MF) group, and a low-protein/high-fat (LP/HF) group. Moreover, the findings demonstrated that depression was related to both the rs7041 and the rs4588 polymorphism (p = 0.05 and p = 0.02, respectively). We next used multinomial logistic modeling to investigate the risk of depression. A significant interaction was observed between HP/LF diet and the rs7041 polymorphism in the moderate- and severe-depression groups (β = -0.30, p = 0.05, and β = -0.48, p = 0.01, respectively)., Conclusion: This study showed that an HP/LF diet interacts with the rs7041 polymorphism, with T allele carriers having a greater prevalence of moderate and severe depression., (© 2018 S. Abstract

The Polio Eradication and Endgame Strategic plan outlines the phased removal of oral polio vaccines (OPVs), starting with type 2 poliovirus-containing vaccine and introduction of inactivated polio vaccine in routine immunization to mitigate against risk of vaccine-associated paralytic polio and circulating vaccine-derived poliovirus. The objective includes strengthening routine immunization as the primary pillar to sustaining high population immunity. After 2 years without reporting any wild poliovirus (July 2014-2016), the region undertook the synchronized switch from trivalent OPV (tOPV) to bivalent OPV (bOPV) as recommended by the Strategic Advisory Group of Experts on Immunization. Consequently the 47 countries of the World Health Organization (WHO) African Region switched from the use of tOPV to bOPV within the stipulated period of April 2016. Planning started early, routine immunization was strengthened, and technical and financial support was provided for vaccine registration, procurement, destruction, logistics, and management across countries by WHO in collaboration with the United Nations Children's Fund (UNICEF) and partners. National commitment and ownership, as well as strong coordination and collaboration between UNICEF and WHO and with partners, ensured success of this major, historic public health undertaking., (© The Author 2017. Abstract

Background: The aim of this study was to investigate seminal oxidant-antioxidant activity in idiopathic and varicocele infertility in men., Methods: Total anti-oxidant capacity (TAC), total oxidant status (TOS), paraoxonase (PON1), aryl esterase (ARE), and total thiol levels (TTL) were measured in seminal plasma with an autoanalyzer. The TOS/TAC ratio was determined as the oxidative stress index (OSI). A histopathological evaluation of the sperm was performed in the andrology laboratory of the hospital. Number, motility, morphology, volume, pH, and leukocytes were evaluated in all samples according to World Health Organization criteria. The three study groups were as follows: G1, males with idiopathic infertility; G2, males with varicocele infertility; and G3, normal healthy males (had fathered a child in the last 2 years). Each group was composed of 36 men (age, 25 - 40 years). The Rel Assay Diagnostics kit was used to determine the levels of the parameters. The study was conducted according to the principles of the declaration of Helsinki and was approved by Sakarya University Medicine Faculty Ethic Committee (e.n: 16214662/050.01.04/07). Statistical significance was assumed if p < 0.05. All statistical evaluations were performed using SPSS (version 20.0 for Windows; SPSS, Inc., Chicago, IL, USA)., Results: No differences were detected between the mean values of antioxidant parameters among the three groups (Kruskal-Wallis test). The p-values of the test parameters (TAC, TOS, PON1, ARE, TTL, OSI) are respectively: 0.494, 0.548, 0.068, 0.151, 0.202, 0.873. The antioxidant parameters of all subjects were compared using the MannWhitney U-test in both groups as fertile (G3) and infertile (G1 + G2). The PON1 levels in infertile subjects were significantly higher than those in fertile subjects. There was a statistically significant difference (p = 0.042). The other antioxidant parameters had no statistically significant difference (p > 0.05). The ARE was not performed in group 3 (control) due to a methodological problem., Conclusions: PON1 levels in infertile subjects were significantly higher than those of fertile subjects.

Abstract

Objective: Infertility is described as the inability of a couple to conceive after 1 year of unprotected intercourse. Male factor is assumed to be responsible for 50% of cases of infertile couples. It is a common clinical problem, affecting approximately 13-15% of couples worldwide. In this study we aimed to investigate the effects of hypoosmotic swelling (HOS) and anti-HOS tests on sperm membrane integrity at the ultrastructural level., Study Design: Twenty-nine infertile and 10 fertile men were included in this study. The fertile and infertile subjects were classified according to Kruger and WHO criterion. All semen samples were examined and ana-yzed based on WHO guidelines. Sperm viability was determined by using the eosin Y staining method. After HOS and anti-HOS tests wore applied, the samples were evaluated at the ultrastructural level., Results: Normal structural features of all regions of sperm were observed in sections of sham normospermia. Some histopathological changes wore seen in HOS and anti-HOS group sections., Conclusion: The HOS procedure was found not to cause degenerative changes in the sperm ultrastructure. The anti-HOS procedure can be applied in normospermic and oligospermic groups.

Abstract

Tamoxifen is a triphenylethylene derivative commonly used in the treatment of breast cancer The Wistar rats (9 weeks old, 180-200 g body weight) used in these trials were divided into two groups of 20 animals each (control and experimental group). Animals of the experimental group were given drinking water containing 200 μg/kg tamoxifen citrate for a period of 30 day. At the end of exposure, body and uterus weights were measured. There was no statistical difference in the uterus weights between the control and treated groups (p < 0,05). Squamous metaplasia, characterized by the presence of stratified squamous epithelium on the lumenal surface of the uterus and the lining of the luminal surface of uterine glands was seen in some animals that had received tamoxifen. Ultrastructural changes were revealed in the form of vacularization of cell cytoplasm and The cytoplasm contained areas of low electron density. Not only was the cytoplasm full with intense crystalized and degeneration in mitochondria.
