Summary Background & aims COVID-19 patients present a high hospitalization rate with a high mortality risk for those requiring intensive care. When these patients have other comorbid conditions and older age, the risk for severe disease and poor outcomes after ICU admission are increased. The present work aims to describe the preliminary results of the ongoing NUTRICOVID study about the nutritional and functional status and the quality of life of adult COVID-19 survivors after ICU discharge, emphasizing the in-hospital and discharge situation of this population. Methods A multicenter, ambispective, observational cohort study was conducted in 16 public hospitals of the Community of Madrid with COVID-19 survivors who were admitted to the ICU during the first outbreak. Preliminary results of this study include data retrospectively collected. Malnutrition and sarcopenia were screened at discharge using MUST and SARC-F; the use of healthcare resources was measured as the length of hospital stay and requirement of respiratory support and tracheostomy during hospitalization; other study variables were the need for medical nutritional treatment; and patients’ functional status (Barthel index) and health-related quality of life (EQ-5D-5L). Results A total of 176 patients were included in this preliminary analysis. Most patients were male and older than 60 years, who suffered an average (SD) weight loss of 16.6% (8.3%) during the hospital stay, with a median length of stay of 53 (27–89.5) days and a median ICU stay of 24.5 (11–43.5) days. At discharge, 83.5% and 86.9% of the patients were at risk of malnutrition and sarcopenia, respectively, but only 38% were prescribed medical nutrition therapy. In addition, more than 70% of patients had significant impairment of their mobility and to conduct their usual activities at hospital discharge. Conclusions This preliminary analysis evidences the high nutritional and functional impairment of COVID-19 survivors at hospital discharge and highlights the need for guidelines and systematic protocols, together with appropriate rehabilitation programs, to optimize the nutritional management of these patients after discharge.

Resolution of parenteral nutrition-associated liver disease has been identified in infants receiving SMOFlipid™ or a 100% fish oil lipid emulsion (FOLE). However, the effect of FOLE is unknown when the previous emulsion received is a mixed lipid emulsion containing fish oil. This observational pilot study reports data regarding the use of Omegaven™ after the diagnosis of cholestasis while receiving SMOFlipid™. We conducted a retrospective review of medical charts of neonates in which a partially fish oil-based lipid emulsion was replaced by a fish oil lipid emulsion at 1 g/kg/day due to cholestasis. Thirty-eight infants (92.1% preterm, being 44.7% born below 28 weeks’ gestation), received FOLE. Birth weight was 1390 (743.0; 2298) grams. The age that cholestasis diagnosed was 15.0 (10.0; 24.8) days. The fish oil emulsion was administered for 38.5 (11.2; 51.8) days. In 73.7% (28/38) of the neonates, the cholestasis was resolved. In 34.2% (13/38), resolution happened before FOLE discontinuation. In addition, in the rest of the neonates (15) in whom cholestasis resolved, resolution occurred after FOLE discontinuation. Nine of the neonates died. In conclusion, the use of a 100% fish oil-based emulsion in neonates afflicted with cholestasis developed while on a partially fish oil-based emulsion is associated with a bilirubin decrease.

Disease-related malnutrition increases health care costs in relation to a longer hospital stay, higher incidence of infectious and non-infectious complications, greater need of treatment, increase in readmissions, more prolonged stay in the intensive care unit and/or the need of referral to continuing care centers at discharge. Prolonged parenteral nutrition (PN) leads to liver damage. Recent interest has focused on the lipid component of PN. A lipid emulsion based on w-3 fatty acids decrease conjugated bilirubin. A mixed lipid emulsion derived from soybean, coconut, olive, and fish oils reverses jaundice. Here we report the reversal of cholestasis and the improvement of enteral feeding tolerance in 1 infant with intestinal failure-associated liver disease. Treatment involved the substitution of a mixed lipid emulsion with one containing primarily omega-3 fatty acids during 37 days. Growth and biochemical tests of liver function improved significantly. This suggests that fat emulsions made from fish oils may be more effective means of treating this condition compared with an intravenous lipid emulsion containing soybean oil, medium -chain triglycerides, olive oil, and fish oil.La nutrición parenteral prolongada produce daño hepático. Recientemente se ha comunicado el efecto de las emulsiones lipídicas intravenosas basadas exclusivamente en ácidos grasos omega-3 en la resolución de la colestasis. Lo mismo se ha observado con el uso de emulsiones lipídicas mixta derivadas del aceite de soja, coco, oliva y pescado. Comunicamos la desaparición de colestasis y mejoría de la tolerancia enteral en un niño con enfermedad hepática asociada a nutrición parenteral. El tratamiento consistió en sustituir una emulsión lipídica mixta por otra que contenía de forma exclusiva aceite de pescado durante 37 días. El crecimiento y los datos bioquímicos de función hepática mejoraron de forma significativa. Este caso sugiere que emulsiones lipídicas intravenosas a partir de aceite de pescado pueden ser mas eficaces para tratar la colestasis si se comparan con emulsiones mixtas.

For the last 20 years, most adult patients following home parenteral nutrition (HPN) under the care of the Clinical Nutrition and Dietetics Dept. of the La Paz University Hospital (Unidad de Nutrición Clínica y Dietética del Hospital Universitario La Paz), Madrid, Spain, have received their nutrition formula via a catering system (Nutriservice) responsible for its preparation and home distribution.To assess the clinical characteristics, quality of life and degree of satisfaction with the care received, of patients undergoing HPN who received their nutrition formulae via the named catering service.The characteristics of the patients who received HPN via this service between 1992 and 2013 were retrospectively collected. Those patients still undergoing treatment completed a quality of life questionnaire, plus a survey of their satisfaction with the catering system and the care provided by our department.Seventy eight patients were examined; 57.7% were men. The mean age of the patients was 53.1±14.3 years. The most common underlying disease was cancer, both in advanced stage (33.8% of all cases) and in stages in which active treatment was being provided (34%). The most common indication for HPN was intestinal obstruction (46%). The median duration of HPN treatment was 96 [1-5334] days. The most common complication was catheter infection (72%). For the analysis of the results, the patients were divided depending on disease type: those with advanced stage cancer, those with cancer under active treatment and with a better prognosis, and those with non-oncological disease. The patients undergoing active oncological treatment believed the quality of their lives to have been improved by the Nutriservice catering system, and rated the care received by our department positively.In our experience, patients with cancer are those who most commonly receive HPN, especially those with advanced disease. Given the seriousness of their conditions, and the complexity surrounding the use of HPN, catering systems appear to offer a means of improving their quality of life.La mayoría de los pacientes adultos con nutrición parenteral domiciliaria (NPD) que dependen de la Unidad de Nutrición Clínica y Dietética del Hospital Universitario La Paz, reciben desde hace más de 20 años la fórmula de nutrición a través de un sistema de catering que elabora y distribuye directamente al domicilio del paciente.Evaluar las características clínicas, calidad de vida y grado de satisfacción con la atención recibida de los pacientes con nutrición parenteral domiciliaria que recibieron las fórmulas de nutrición mediante el sistema de catering: Nutriservice.Se recogen retrospectivamente las características de los pacientes que utilizaron fórmulas de nutrición parenteral en su domicilio a través de Nutriservice, desde el año 1992 hasta la actualidad. Los pacientes en tratamiento activo realizaron un cuestionario de calidad de vida y encuesta de satisfacción sobre el sistema de catering y la atención prestada por nuestra Unidad.Se registraron 78 pacientes, el 57,7% eran hombres y la edad media 53,1±14,3 años. La patología de base más frecuente fue el cáncer, tanto en estadios avanzados (34%) como en tratamiento activo (34%). El motivo principal de indicación fue la obstrucción intestinal (46%).La duración del soporte nutricional tuvo una mediana de 96 días (1-5334). Las complicaciones más frecuentes fueron las infecciones relacionadas con el catéter (72%). Se analizaron los resultados, clasificando a los pacientes según la enfermedad de base: cáncer en estadio avanzado, cáncer en tratamiento activo y mejor pronóstico y patología no oncológica. Los pacientes en tratamiento activo consideraron afectada su calidad de vida y valoraron positivamente el servicio ofrecido por Nutriservice y la atención recibida de la Unidad de Nutrición.En nuestra experiencia, los pacientes oncológicos son los que con mayor frecuencia tienen nutrición parenteral domiciliaria, siendo muy frecuente su indicación en estadios avanzados de la enfermedad. A la gravedad de la patología de base se une la complejidad de la NPD. Los sistemas de catering podrían ser una alternativa eficaz para mejorar la calidad de vida en algunos de estos pacientes.
