Repetitive movements and the speed of upper limbs increase the risk of musculoskeletal disorders. This study aimed to analyse the risk of shoulder injuries in repetitive tasks by evaluating the humerus angle, angular velocity, and angular acceleration during simulated chicken wing cutting. The study was conducted in a laboratory simulating a real environment. Thirty-six healthy right-handed volunteers were assessed using an electromagnetic tracking device, TrakSTAR, integrated with MotionMonitor™ (Innovative Sports Training, Inc. Chicago, IL) and software to collect 3-D kinematic data developed in the research centre. The equipment measured the angles performed by the upper limbs during the entire movement. The humerus angles were automatically transformed into angular velocity (°·s−1) and angular acceleration (°·s−2). Maximum angular velocities were 27.39°·s−1 (men, right humerus) and 22.39°·s−1 (women, left humerus), both below the safe limit. Maximum accelerations were 25.32°·s−2 (men, left side) and 28.94°·s−2 (women, left side); safety values for these accelerations are not established. Monotony is a risk factor, especially for the dominant side. Future studies should evaluate risk factors simultaneously in repetitive tasks. Repetitiveness exceeds the safe limit according to the OCRA method.

A large proportion of annual production of worldwide greenhouses gases results from the use of internal combustion engines. This experimental work evaluates the influence of dual-fuel operation on the overall emissions of a low-displacement compression-ignition engine based on operational conditions such as torque, rotational speed, and load. Hydroxy gas is used as a supplementary gaseous fuel while using pure diespel or palm oil biodiesel as baseline fuels. The CO, CO2, NOx, and HC emissions levels were carefully characterized through experimental measurements and statistical analysis. The influence of hydroxy enrichment was also examined on the engine's fuel consumption. The study incorporates an in-house hydroxy generator to store and supply the gas in the intake air system using an electrolyzer. The results demonstrated that the ANOVA analysis provides accurate predictions compared to experimental measurements with less than 5% relative error. The use of hydroxy reduces the SFC by up to 25%, which represents an economic advantage of dual-fuel operation, additionally it decreases CO, HC, and CO2 emissions. However, with hydroxy enrichment, NOx emissions levels escalate at medium and high loads. Overall, hydroxy enrichment demonstrates to be a robust alternative from an environmental and economic perspective. Future research will be focused on evaluating the biodiesel – hydroxy dual operation, broadening the spectrum of biodiesel concentration percentages, and selecting different raw materials for biofuel production.

ABSTRACT Mixing acerola pulp with whey, a by-product of the dairy industry, led to this study, whose aim was the development of a powdered beverage mix of acerola pulp, whey and maltodextrin (DE20). To determine the proportions of the mixture, a simplex-centroid mixture design was used. Different proportions of the mixture were dehydrated in a spray dryer at 175 °C. The results were evaluated in relation to hygroscopicity, moisture and ascorbic acid. Higher proportions of whey and pulp increased hygroscopicity and moisture, while maltodextrin reduced these parameters. An increase in pulp content raised the levels of ascorbic acid. The proportions (m m-1) of 50% acerola pulp, 25% whey and 25% maltodextrin resulted in significantly lower hygroscopicity (8.25 g 100 g-1) and moisture (1.66%), and appreciable levels of ascorbic acid (1492.49 g 100 g-1). The results indicated that the powder obtained shows good stability after 75 days of storage, with 9.20%, 1.79% and 1146.13 g 100 g-1 of hygroscopicity, moisture and ascorbic acid respectively.

Background: The Children's Hospital Oakland Hip Evaluation Scale is a disease-specific tool for the clinical and functional assessment of the hip in sickle cell disease. Objectives: To translate the tool into Brazilian Portuguese and evaluate the interobserver and test–retest reliability. Methods: Eighteen patients diagnosed with sickle cell disease and a mean age of 49 ± 11.9 years participated in the study. The scale was applied by two evaluators who did not speak to each other regarding their understanding of the tool and who had no prior training. Interobserver and test–retest reliability of individual items and of the total score were evaluated using the intraclass correlation coefficient and the Bland–Altman method. Results: When the overall score for each hip was considered, the test–retest intraclass correlation coefficient score for the right hip was 0.95 (0.89–0.98) and for the left hip it was 0.96 (0.91–0.98). Considering all assignments (total score), the score was 0.96 (0.90–0.98). The test–retest intraclass correlation coefficient varied from 0.76 to 1 for 18 of the 27 items (excellent) and from 0.53 to 0.75 for nine items (moderate). When the overall score for each hip was considered, the interobserver intraclass correlation coefficient for both hips was 0.94 (0.86–0.98). Considering all assignments, the total score was 0.94 (0.86–0.98). The interobserver intraclass correlation coefficient varied from 0.48 to 0.75 for 18 out of 27 items (moderate) and varied from 0.77 to 1 for the remaining nine items (excellent). Conclusion: The results demonstrate that the Brazilian version of the Children's Hospital Oakland Hip Evaluation Scale presented adequate interobserver and test–retest reliability and that the version can be used to evaluate clinical function in sickle cell disease patients, producing consistent, standardized and reproducible results. The Motor Activity Log (MAL) assesses the spontaneous use of the most affected upper limb with the amount of use (AOU) and quality of movement (QOM) scales during daily activities in real environments in individuals with chronic stroke. Objectives: This study translated the testing manual into Portuguese and assessed the inter-rater and test-retest reliabilities of the MAL, based upon the Brazilian manual version. Methods: The inter-rater reliability was evaluated by comparing the results of two examiners, and the test-retest reliability was tested by comparing the results of two evaluations, repeated one-week apart with 30 individuals with chronic hemiparesis (55.8±15.1 years). Results: The intra-class correlation coefficients (ICCs) for the total scores were adequate for both the inter-rater (0.98 for the AOU and 0.91 for QOM) and test-retest reliabilities (0.99 for both scales). Conclusions: The results suggested that the MAL was reliable to evaluate the spontaneous use of the most affected upper limb after stroke.

Studies have demonstrated that phosphodiesterase 5 (PDE5) inhibition is associated with right ventricle (RV) functional improvement in patients with primary pulmonary hypertension. This study aims to demonstrate the immediate impact of Sildenafil, a PDE5 inhibitor, on RV function, measured by cardiovascular magnetic resonance (CMR), in patients with heart failure (HF).We conducted a randomized double-blind controlled trial.diagnosis of HF functional class I-III; left ventricle ejection fraction < 35%. Patients underwent CMR evaluation and were then equally randomly assigned to either 50 mg of Sildenafil or Placebo groups. One hour following drug administration, they were submitted to a second scan examination.26 patients were recruited from a tertiary reference center in Brazil and 13 were allocated to each study group. The median age was 61.5 years (50-66.5 years). Except for the increase in RV fractional area change following the administration of sildenafil (Sildenafil [before vs. after]: 34.3 [25.2-43.6]% vs. 42.9 [28.5-46.7]%, p = 0.04; Placebo [before vs. after]: 28.1 [9.2-34.8]% vs. 29.2 [22.5-38.8]%, p = 0.86), there was no statistically significant change in parameters. There was no improvement in left ventricular parameters or in the fractional area change of the pulmonary artery. This study demonstrated that a single dose of Sildenafil did not significantly improve RV function as measured by the CMR.ClinicalTrials.gov NCT01936350.

BRAIN-derived neurotrophic factor , *CONSTRAINT-induced movement therapy , *MATRIX metalloproteinases , *AEROBIC exercises , *IRISIN - Abstract
BackgroundObjectiveMethodsResultsConclusionIntegrating aerobic exercise (AE) into rehabilitation programs for post-stroke individuals could enhance motor recovery and cardiovascular health by increasing brain-derived neurotrophic factor (BDNF) and the myokine irisin. Chronic stroke survivors typically exhibit elevated matrix metalloproteinase-9 (MMP-9) activity, which is negatively correlated with steps and time in medium cadence, although the impact of AE on this biomarker remains unclear. To evaluate the effect of high-intensity AE training prior to modified constraint-induced movement therapy (mCIMT) on BDNF and irisin concentration, and on MMP-2 and MMP-9 activity in chronic post-stroke individuals and to associate these results with functional improvements.Nine participants received AE combined with mCIMT for two weeks, while the control group (
n = 7) received mCIMT alone. Manual dexterity and functional capacity were assessed before and after the intervention. Serum samples were analyzed for BDNF, irisin, MMP-2 and MMP-9.There were no significant main effects of assessment, group or interaction on molecular biomarkers. However, the AE group had a significant increase in MMP-9 activity post-intervention (p = .033;d = 0.67). For the Box and Block Test, there were significant main effects of assessment (F [1, 14] = 33.27,p = .000,η p2 = 0.70) and group (F [1, 14] = 5.43,p = .035,η p2 = .28). No correlations were found between biomarkers and clinical assessments.AE prior to mCIMT did not influence circulating BDNF and irisin levels but did induce an acute rise in MMP-9 activity, suggesting potential effects on cardiovascular remodeling in this population. AbstractPurposeMethodsResultsConclusions\nImplications for rehabilitationTo identify the content and the constructs measured in the Lower Extremity Motor Activity Log (LE-MAL) based on the International Classification of Functioning, Disability and Health (ICF) framework and to determine whether the instrument items fit within the ICF mobility domain.Concepts and constructs measured within each scale were linked to the best-matched ICF categories and classified using established linking rules. Two independent researchers determined the initial linkages. A final consensus was reached with the other researchers. The agreement was analysed through the agreement percentage and the kappa coefficient. The subscales assess performance through information about need or dependency, personal and environmental factors, and appraisal of the items. Thirteen concepts were identified in LE-MAL: two concepts for the Assistance Scale are covered in the environmental factors component; 10 concepts for the Functional Performance Scale where all items are covered in the mobility domain; one concept for the confidence scale is covered in the body functions component. The general construct of LE-MAL assessed is performance in the ICF mobility domain and provides additional information about the mental function, environmental factors, independence, appraisal, and personal factors. The study highlighted the conceptual connection between the LE-MAL and the ICF framework. The use of the lower extremities influences the quality and independence of gait in neurological populations. No studies have analysed the Lower Extremity Motor Activity Log (LE-MAL) content with a theoretical framework.Theoretical framework linkage complements the content validity of the LE-MAL.Linking with the International Classification of Functioning, Disability and Health (ICF) aids clinics in report writing and treatment planning. Abstract Introduction : The Graded Wolf Motor Function Test (GWMFT) was developed as a modification of the Wolf Motor Function Test (WMFT), designed to address moderate-to-severe upper-extremity motor impairment, consequent to a stroke or brain injury, by combining time and quality of movement measures in both isolated movements and functional tasks. Objectives : To translate and adapt the GWMFT form and instructions manual to Brazilian Portuguese and evaluate the inter-rater reliability. Materials and methods : Ten individuals, mean age 53.2 ± 11.39 (range: 28-72) years and a mean time since stroke onset of 82.5 ± 85.83 (16-288) months participated in the study. After translation and cultural adaptation, two independent evaluators, based on the instructions manual information, administered GWMFT. Video observations were used to rate the time and the compensatory movements in the Functional Ability Scale (FAS). Intra-class Correlation Coefficients (ICCs) and Bland-Altman plots were calculated to examine the inter-rater reliability for performance time and FAS. Results : The translated and adapted version obtained a total ICC inter-rater time 0.99 (0.95-1.00), showing less reliability in the task of lifting a pen, with ICC = 0.71 (- 0.15-0.93). The ICC of the total FAS was 0.98 (0.92-0.99) and the task of elbow extension has shown the lowest ICC rate = 0.83 (0.31-0.96). Conclusion : The GWMFT scale reliability proved to be appropriate to evaluate the paretic upper limb in individuals with chronic hemiparesis post severe stroke.

Dementia -- Complications and side effects -- Physiological aspects -- Diagnosis - Abstract
Objective. The purpose of this study was to analyze the relative and absolute reliability of assessment tests addressing body structure and function and activity in older adults with dementia. Methods. Medline, Embase, Web of Science, The Cochrane Library, and Scielo were searched from inception until March 2021. Two independent reviewers performed the selection process based on titles, abstracts, and full text. Reliability studies of assessment tests in older adults with dementia were included. Methodological quality of the studies was evaluated using the COnsensus-based Standards for the selection of health Measurement INstruments Risk of Bias checklist. Relative reliability was analyzed using the intraclass correlation coefficient (ICC) interpreted based on Munro classification. Absolute reliability was analyzed using the minimal detectable change (MDC) and standard error of measurement. Results. Fifteen studies involving a total of 560 older adults with dementia were included. Nineteen assessment tests were identified: 13 addressing body structure and function (muscle strength, postural balance, cardiorespiratory fitness) and 6 addressing activity (walking and mobility). Studies determined test-retest and interrater reliability. Fifteen studies evaluated relative reliability using the ICC, with values ranging from no or small correlation to very high correlations. Ten studies evaluated absolute reliability using the MDC or standard error of measurement or both. Conclusion. Relative reliability of the assessment tests for body structure and function and activity was high to very high based on ICCs, demonstrating good reproducibility. Regarding absolute reliability, the analysis of the MDC values revealed the need for substantial change to determine that a real change had occurred. Future investigations should consider the type of dementia and standardization of verbal encouragement during the assessment. Impact. This review identified the good reproducibility of assessment tests of body structure and function (muscle strength, postural balance, cardiorespiratory fitness) and activity (walking and mobility) domains in older adults with dementia. Clinically important values may differ when older adults with dementia of diverse etiologies are analyzed together and older adults specifically with Alzheimer disease. Identifying the type of dementia, analyzing types of dementia separately, and standardizing verbal commands during the execution of tests is of considerable clinical importance for this population of older adults. Keywords: Alzheimer Disease, Dementia, Disability and Health, International Classification of Functioning, Reproducibility of Results, Introduction The number of older adults with dementia is expected to increase exponentially in the upcoming years, with Alzheimer disease the most common form of dementia. To link the items from shoulder-specific Patient Reported Outcome Measures (PROMs) to the International Classification of Functioning, Disability and Health (ICF) domains and categories, and to determine if the items fit into the ICF framework. The Brazilian versions of the Oxford Shoulder Score (OSS), Shoulder Pain and Disability Index (SPADI), Simple Shoulder Test (SST) and Western Ontario Rotator Cuff Index (WORC) were linked to the ICF by two researchers independently. Agreement between raters was determined by calculating the Kappa Index. Fifty-eight items from the PROMs were linked to eight domains and 27 categories of ICF. The PROMs covered components of body functions, activities, and participation. Components of body structure and environmental factors were not covered by any of the PROMs. There was substantial agreement between raters when linking the OSS (Kappa index = 0.66), SPADI (Kappa index = 0.92), SST (Kappa index = 0.72) and WORC (Kappa index = 0.71). WORC and SST were the PROMs that covered the highest number of ICF domains (seven and six, respectively). However, SST is short and may be less time consuming in a clinical assessment. Clinicians can benefit from this study to decide which shoulder-specific PROM may be more adequate according to the clinical demand. Western Ontario Rotator Cuff Index was the shoulder-specific Patient Reported Outcome Measure (PROM) that best showed a broader view of functionality through the International Classification of Functioning, Disability and Health (ICF) domains that can influence shoulder pain and disability. Simple Shoulder Test seems to be the most recommended shoulder-specific PROM considering the number of domains covered by the ICF and the clinical evaluation time consumption. Shoulder Pain and Disability Index fails to provide a broader view of functioning through other ICF domains that may influence shoulder pain and disability. [ABSTRACT FROM AUTHOR]

This study aimed to assess the effect of the Lower Extremity Constraint-Induced Movement Therapy (LE-CIMT) intervention on the intra-limb hip-knee coordination and its variability in walking in chronic post-stroke individuals considering the lenses of the dynamic systems approach to motor behavior. Twelve chronic post-stroke individuals received the LE-CIMT intervention for 10 days following all protocol’s recommendations and designed according to individuals’ needs. Before, three days, and 30 days after the intervention, angular kinematics in the sagittal plane of the trunk, hip and knee of both extremities were analyzed. We analyzed joint angles for swing and stance phases of walking, and the intra-limb coordination of hip-knee joints through the vector coding technique. For separate joint angular kinematics, we found differences between affected extremities but no effects of the intervention. However, we observed changes in intra-limb coordination pattern as a function of increased variability after intervention and increased stability at follow-up. These changes follow the predicted pattern from the dynamic systems approach to motor behavior. The immediate decreased stability of the initial pattern could be due to the active exploration of space solution offered by the LE-CIMT and, posteriorly, the stability increased after finding new movement solutions in the real-life environment.

Objective To investigate the effects of resistance exercise (RE) on body structure and function, activity, and participation in individuals with Parkinson Disease (PD) in the mild to moderate stages. Data Sources Medline, Embase, Web of Science, The Cochrane Library, Lilacs, and PEDro were searched from inception until June 2020 using the terms “Parkinson Disease,” “Exercise,” “Resistance Training,” “Muscle Strength,” “Cardiorespiratory Fitness,” “Postural Balance,” “Gait,” and “Quality of Life.” Study Selection We included studies conducted in individuals with PD involving RE compared with a control group. Two independent reviewers performed the selection process based on titles, abstracts, and full-text reading. In total, 270 individuals with PD were included from 10 selected studies. Data extraction Two reviewers independently extracted characteristics related to participants, intervention and control types, and results. The PEDro scale was used to assess the methodological quality, and the level of evidence was analyzed and synthesized using the Grading of Recommendation, Assessment, Development, and Evaluations approach. Data Synthesis The level of evidence for body structure and function was low and without effect for lower limb muscle strength; very low and with effect for upper limb muscle strength, cardiorespiratory fitness, and postural balance; and very low and without effect for flexibility after RE training. For activity, the evidence was very low and with effect for gait and very low and without effect for mobility. For participation (ie, quality of life) the evidence was very low and without effect. Conclusions Although the level of evidence was low to very low, RE was shown to promote improvements in body structure and function (upper limb muscle strength, cardiovascular function, postural balance) and activity (gait). In contrast, RE did not significantly improve participation (quality of life). However, based on the present findings, the practice of RE can be recommended for individuals with PD in the mild to moderate stages.

Most measures to assess the lower extremity (LE) after a neurological injury assess the lower limb motor capacity in the laboratory or clinical settings as the lower-extremity motor subscale of Fugl Meyer Assessment (FMA-LE). The LE Motor Activity Log (LE-MAL) measures the use of paretic LE in real-life conditions, which for many researchers and stakeholders is considered an important goal of rehabilitation. Investigate the reliability, validity, floor, and ceiling effects of the Brazilian version of the LE-MAL. Cross-sectional study to examine the reliability and validity of LE-MAL in patients with chronic stroke. The translation and adaptation of LE-MAL was carried out for the Brazilian version. After this stage, to ensure the inter-rater reliability two raters (LCF, DBM) applied the translated version of LE-MAL in the 21 selected participants. Subsequently, the concurrent validity was calculated by the correlation with the FMA-LE. The translation and retro-translation of the LE-MAL was approved to the Brazilian context. A floor effect was not observed, and there was no ceiling effect for the LE-MAL total scores. The Inter-rater reliability ranged between 0.80 and 0.86. The Bland–Altman analysis showed difference of LE-MAL scores ranged from 0.8 to 1.6. The Cronbach's alpha coefficients for LE-MAL and subscales were high ranging between 0.86 and 0.80. There was a significant moderate correlation between the LE-MAL and FMA-LE (Pearson correlation 0.55 (p =.009)). The Brazilian version LE-MAL is valid and reliable to assess real-world use of the paretic LE of individuals with chronic stroke. [ABSTRACT FROM AUTHOR]

This research analyzes community water supply organizations, their effects (symbolic, political and technical), as well as possibilities of articulation and complementarity with public management. For this purpose, three community water service organizations were selected in the city of Villavicencio, Colombia. Located where the eastern mountain range of the Colombian Andes meets the region known as Llanos Orientales, in the center of the country, Villavicencio has 531,275 inhabitants and has undergone strong urban and socioeconomic transformations in the last twenty years. The main provider of water supply services in the city is the Empresa de Acueducto y Alcantarillado de Villavicencio (EAAV), a public company that has been criticized for various technical and administrative deficiencies, such as extended service cuts. In this context, a multiplicity of private and community providers have emerged in the urban area, challenging the idea that the universal and industrial water distribution model is a natural monopoly and the only - and ideal - way for water supply in the cities. Through an action research methodology, this work, carried out between 2017 and 2022, employed various qualitative methodological procedures such as participant observation, semi-structured interviews, community censuses and participatory workshops. Guided by the political ecology approach, the "commons" perspective of the Theory of Collective Action (Ostrom, 2011), and Dardot and Laval's (2010, 2015) arguments about "the common" as a political process, this research raises three main considerations. First, it questions the technical relevance and social effects of the "big system" of water infrastructure networks. Second, although community water organizations also present conflicts and difficulties, they can be recognized as a flexible and more democratic supply model, where community organizations have also given rise to broader social movements that question the principles - normative and ideological - that underpin the water services management in Colombia. Third, and as a general proposal of this research, it suggests the possibility of articulating public action with community water supply strategies at a local scale, forging new paradigms of water service management that are more democratic, more sustainable and more relevant to the reality of the cities.

The occurrence of Alzheimer's disease (AD) can exert a negative impact in social participation in affected older adults. The purpose of this study was to investigate whether social participation in older adults with AD is associated with disease stage and cognitive function as well as the quality of life and depressive symptoms in their caregivers. A correlational, cross-sectional study was conducted in 40 older adults with AD (28 women and 12 men) and 40 caregivers (30 women and 10 men). Social participation was assessed using the 'social participation' domain of the Activities of Daily Living Questionnaire. Disease stage was determined using the Clinical Dementia Rating scale and cognitive function was assessed using Addenbrooke's Cognitive Examination. Quality of life and depressive symptoms in the caregivers were evaluated using the Quality of Life Assessment Scale on Alzheimer's Disease and Beck Depression Inventory respectively. The older adults with AD had a mean percentage of 59.4% on the social participation domain and a mean score of 49.0 for cognitive function. The caregivers had mean scores of 39.1 for quality of life and 9.9 for depressive symptoms. The stepwise backward multiple linear regression model indicated that the predictors analysed together explained 48% of the variability in social participation among older adults with AD. Therefore, lower social participation among older adults with AD is associated with more advanced stages of the disease and cognitive decline in these individuals as well as a lower perception of quality of life and greater levels of depressive symptoms in their caregivers.

Mixing acerola pulp with whey, a by-product of the dairy industry, led to this study, whose aim was the development of a powdered beverage mix of acerola pulp, whey and maltodextrin (DE20).

In the Paris agreement about climate change, it was established that greenhouse gas emissions must be reduced by 0% in order to avoid irreversible damage, so the world economy based on fossil fuels must undergo a transformation towards an economy clean, safe and sustainable. As a result, it has been established that the only alternative that guarantees 0% emissions of polluting gases is the economy of hydrogen. Countries such as the Netherlands, the United States, and the European Union have projected policy towards 2050 that expects to meet the objectives set out in the Paris agreement. In this document shows the different alternative fuels that are currently being proposed to mitigate polluting emissions, as well as the overview of these fuels in Colombia, and how, from the different renewable energy sources that this country has, could venture economically and politically towards economy based on the use of hydroxy and hydrogen as a raw material and energy source, showing its possible applications. The foregoing will allow to identify the potential of application of these fuels in the energy matrix of Colombia and define policies to accelerate their implementation. Keywords: Environmental Pollution, Energy Policy, Alternative Fuels, Hydrogen, Renewable Energy.JEL Classifications: L78, L90, O31, Q20DOI: https://doi.org/10.32479/ijeep.8395

Background Children with cerebral palsy (CP) often have difficulty with activities that require the upper extremities secondary to deficits in strength and range of motion, spasticity, and poor timing and coordination of movement. This study aimed to identify and compare timing and coordination of the trunk and upper extremity in children with and without CP during a functional task. Methods Eighteen children, N = 9 with CP and N = 9 with typical development were enrolled. Participants were seated in a standard chair and instructed to drink from a glass placed at a distance of 75% of available arm length. The task was divided into 3 Phases: 1) reaching to the glass, 2) transporting the glass to the mouth, and 3) returning the glass to the table. The spatiotemporal and angular variables were analyzed with 3D kinematics of movement using a 4-camera Qualysis Motion System. Findings Children with CP demonstrated poor upper extremity timing and coordination. Despite significant trunk displacement used as a compensation in Phase 1, children with CP demonstrated a significantly lower mean velocity and velocity peak during Phases 2 and 3; and demonstrated less straight motion which required more time and movement units in all phases. Interpretation Children with CP demonstrated poor upper extremity timing and coordination when drinking (even when they successfully completed the task) measured by more trunk displacement, slower, less straight movements, and more movement units. Current rehabilitation strategies could consider training speed and use functional tasks that require different strategies across multiple phases.

The increasing prevalence of dementia demands innovative solutions; however, existing technological products often lack tailored support for individuals living with this condition. The Living Lab approach, as a collaborative innovation method, holds promise in addressing this issue by actively involving end-users in the design and development of solutions adapted to their needs. Despite this potential, the approach still faces challenges due to its lack of recognition as a research methodology and its absence of tailored guidelines, particularly in dementia care, prompting inquiries into its effectiveness. This narrative review aims to fill this gap by identifying and analysing digital health Living Labs focusing on dementia solutions. Additionally, it proposes guidelines for enhancing their operations, ensuring sustainability, scalability, and greater impact on dementia care. Fifteen Living Labs were identified and analyzed. Based on trends, best practices, and literature, the guidelines emphasize user engagement, interdisciplinary collaboration, technological infrastructure, regulatory compliance, transparent innovation processes, impact measurement, sustainability, scalability, dissemination, and financial management. Implementing these guidelines can enhance the effectiveness and long-term impact of Living Labs in dementia care, fostering new collaborations globally.

This study determined the ratio between glenohumeral and three-dimensional scapular motion during arm elevation and lowering in 91 individuals without shoulder pain. Scapular kinematics were assessed using an electromagnetic tracking device. Individuals performed 3 repetitions of elevation and lowering of the arm in the sagittal plane. Two-way ANOVAs (interval: 30-60°, 60-90°, 90-120° x phase: elevation and lowering) and paired t-tests were used for data analysis. For scapular internal/external rotation, lesser scapular internal rotation contribution was found during the 60-90° interval as compared to the 90-60° interval. Lesser scapular external rotation was identified in the 60-30° interval of arm lowering. The ratio was greater during arm elevation (1.89) compared to lowering (1.74) across the entire motion arc. For scapular upward rotation, greater upward rotation contribution was observed during arm elevation at the 30-60° interval, and less scapular downward rotation contribution in the final range of arm lowering. For scapular tilt, lesser scapular posterior tilt contribution during arm elevation was observed compared to arm lowering. The ratios between glenohumeral elevation/lowering and each individual scapulothoracic motion showed either differences between intervals and/or between elevation and lowering during specific intervals in healthy individuals.

Objective The purpose of this study was to analyze the relative and absolute reliability of assessment tests addressing body structure and function and activity in older adults with dementia. Methods Medline, Embase, Web of Science, The Cochrane Library, and Scielo were searched from inception until March 2021. Results Fifteen studies involving a total of 560 older adults with dementia were included. Nineteen assessment tests were identified: 13 addressing body structure and function (muscle strength, postural balance, cardiorespiratory fitness) and 6 addressing activity (walking and mobility). Studies determined test–retest and interrater reliability. Fifteen studies evaluated relative reliability using the ICC, with values ranging from no or small correlation to very high correlations. Ten studies evaluated absolute reliability using the MDC or standard error of measurement or both. Conclusion Relative reliability of the assessment tests for body structure and function and activity was high to very high based on ICCs, demonstrating good reproducibility. Regarding absolute reliability, the analysis of the MDC values revealed the need for substantial change to determine that a real change had occurred. Future investigations should consider the type of dementia and standardization of verbal encouragement during the assessment. Impact This review identified the good reproducibility of assessment tests of body structure and function (muscle strength, postural balance, cardiorespiratory fitness) and activity (walking and mobility) domains in older adults with dementia.
