Abstract

Deficiency of Complement Factor H Related (CFHR) plasma proteins and Autoantibody Positive Hemolytic Uremic Syndrome (DEAP-HUS) is a subtype of atypical hemolytic uremic syndrome, known to be associated with significant morbidity. Its pathogenesis is linked to the production of IgG autoantibodies against complement factor H, a regulator of the alternative complement pathway. The binding of the autoantibodies to the C terminal of complement factor H interferes with its regulatory function, leading to increased activation of the alternative complement pathway and consequent endothelial cellular damage. Early diagnosis and initiation of appropriate therapy is reported to lead to favorable outcomes. Institution of plasma exchange therapy within 24 h of diagnosis has been shown to rapidly lower antibody levels, leading to clinical improvement. Adjunctive immunosuppression therapy suppresses antibody production and helps in maintaining long-term clinical remission of the disease. Abstract

Background: Regulatory organizations have acknowledged the need for systematic rules for understanding development as a result of the large increase in concerns and criticism regarding the quality and pharmaceutical products. Bilastine is a second generation antihistamine medication. Generally, it is used for treatment of allergic rhino conjunctivitis and urticaria (hives). Objectives: The current study outlines the methodical design and validation of a reversed-phase high-performance liquid chromatographic method for the estimation of Bilastine in bulk drugs using AQbD approach. Materials and Methods: Using Box Behnken design, the critical method parameters were methodically optimized. Risk estimation matrix was performed and Critical Analytical Attributes, Critical Method Attributes were correlated to identify risk factors of method development. A reverse phase column in isocratic elution mode with mobile phase NaH2PO4 buffer and methanol of different ratio and flow rate 1 mL/min was set for RP-HPLC method development. Results: Chromatographic separation was accomplished on INTERSIL C8 column. The optimized and predicted data from JMP PRO 14 software consist of mobile phase 0.1N NaH2PO4 (60%): Methanol (40%), pumped at a flow rate of 1 mL/min gave the higher desirability function of 77%. LOD and LOQ are, respectively, 0.005 mcg/mL and 0.016 mcg/mL. The Rt of Bilastine was discovered to be 1.894 min. The created method was approved and validated in accordance with ICH Q2 (R1) recommendations. Conclusion: The chosen models were determined to be significant with p<0.05. The validation parameter findings were within the permitted range. Forcefully testing the drug's stability under various stress situations revealed considerable degradation in the presence of heat. Abstract

BackgroundCurrent therapeutic strategies to effectively treat antibody-mediated rejection (AMR) are insufficient. Thus, we aimed to determine the benefit of a therapeutic protocol using bortezomib for refractory C4d + AMR in pediatric kidney transplant patients.MethodsWe examined seven patients with treatment-refractory C4d + AMR. Immunosuppression included antithymocyte globulin or anti-CD25 monoclonal antibody for induction therapy with maintenance corticosteroids, calcineurin inhibitor, and anti-metabolite. Estimated glomerular filtration rate (eGFR) calculated by the Schwartz equation, biopsy findings assessed by 2013 Banff criteria, and human leukocyte antigen (HLA) donor-specific antibodies (DSA) performed using the Luminex single antigen bead assay were monitored pre- and post- bortezomib therapy.ResultsSeven patients (86 % male, 86 % with ≥6/8 HLA mismatch, and 14 % with pre-formed DSA) age 5 to 19 (median 15) years developed refractory C4d + AMR between 1 and 145 (median 65) months post-transplantation. All patients tolerated bortezomib. One patient had allograft loss. Of the six patients with surviving grafts (86 %), mean pre-bortezomib eGFR was 42 ml/min/1.73 m(2) and the mean 1 year post-bortezomib eGFR was 53 ml/min/1.73 m(2). Five of seven (71 %) had improvement of histological findings of AMR, C4d staining, and/or acute cellular rejection. Reduction in HLA DSAs was more effective for class I than class II.ConclusionsBortezomib appears safe and may correlate with stabilization of eGFR in pediatric kidney transplant patients with refractory C4d + AMR.

Abstract

BackgroundExposure to gadolinium-based contrast agents (GBCA) in patients with chronic kidney disease (CKD) has been associated with the development of a potentially fatal disorder, nephrogenic systemic fibrosis (NSF). Although contrast-enhanced computed tomography (CT) is an alternative to magnetic resonance imaging (MRI), it carries the risk of radiation exposure and further reduction of residual renal function. Therefore we sought to assess the feasibility of ferumoxytol as an alternative to GBCA for contrast-enhanced MR angiography (MRA) in a pediatric cohort with CKD. Ferumoxytol is a parenteral iron supplement that contains ultrasmall superparamagnetic iron oxide (USPIO) and is a potent relaxivity agent for MRI.MethodsWe describe the MRI findings in ten pediatric patients who needed detailed vascular mapping. Ferumoxytol (4 mg/kg) was administered intravenously for contrast-enhanced MRA. The patients tolerated the procedure without complications.ResultsResulting studies were highly diagnostic and were pivotal in guiding patient management. The images were notable for clear delineation of multiple vascular occlusions.ConclusionsGiven the concerns associated with the use of GBCAs in renal failure, ferumoxytol is an excellent alternative contrast agent in pediatric end stage renal disease (ESRD) patients. Future studies are needed in order to further evaluate safety and efficacy of ferumoxytol in this patient population.

Abstract

River Ganga, one of the largest perennial rivers, conserves the cultural heritage of the Indian sub-continent, supporting as the nectar of life to millions residing on the banks of the river basin. With unlimited use, the river also receives a large amount of untreated wastewater discharged from different industrial, commercial and residential complexes, unbalancing the nutrient concentration at many points along the stretch. The current study assesses water samples collected from 20 monitoring locations and the Ganga River System in the Himalayan region, evaluating the potential risk of heavy metal pollution. The concentration of Zn, Pb, Mn, Fe, Cu, Al, Ni, Cd, Mg and Co is determined by atomic absorption spectrophotometer (AAS). Fe, Cd and Mg's observed values exceeded various standards (WHO, BIS and USEPA) for drinking water. The pollution assessment index (heavy metal pollution index: HPI) and multivariate analysis (principal component analysis: PCA and cluster analysis: CA) were implemented to identify the intensity of pollution and its sources. The seasonal values of HPI were found 88.69, 90.32, 88.53 and 84.96 in winter, summer, monsoon and post-monsoon seasons. The index value on all monitoring locations varied between the range of PCA outcomes for three factors and explained 72.1% of the variance, indicating natural and man-made activities as responsible metal abundance factors in the river system. The hazard quotient (HQ) and hazard index (HI) are used to assess non-cancer health risks to humans. The seasonal evaluation recorded HQ < 1 for all the metals and HI > 1 was found for the entire study period. This study contributes to various conservation initiatives for the River Ganga System with factual datasets and characterization of various sampling locations using heavy metal indexing while sharing common masses' findings for people participation at the regional level. Abstract

Objective: One of the declining medicinal plant species is Coptis teeta Wall, found solely in eastern parts of India and China. The major chemical constituents present in the plant are berberine, pentadecanoic acid, copisine, palmatine, columbamine, epiberberineand jateorrhizine. The aim of present study is screening the phytoconstitutes in methanolic extract of roots by GC-MS analysis and isolating the pentadecanoic acid, aligned with DNA gyrase subunit B protein target involved in bacterial infection by carrying out molecular docking. Methods: Each concentrate of Coptis teeta was evaluated for antibacterial action by plate dissemination technique against gram-positive and gram-negative microorganisms.GC-MS was performed for the identification of various phytochemical constituents present in methanolic extracts of Coptis teeta.Out of various chemical constituents,pentadecanoic acid was isolated and characterised by IR, 1HNMR and MASS spectroscopy.The isolated pentadecanoic acid was screened against bacterial strains such as Staphylococcus aureus, Staphylococcus epidermidis, Pseudomonas aeruginosa and Escherichia colihave shown good antibacterial activity. Results: Column chromatography was performed for separation andGC-MS showed the presence of pentadecanoic acid by a significant spot on methanolic extract, characterized by IR, NMR and MASS. The methanolic extract and isolated compoundhas shown good antibacterial activity.Protein-ligand docking was achieved between the protein PDBID: 1KZM and pentadecanoic acid which showed a good affinity indicating its potent inhibiting action. Conclusion:In binding studies, ligand showed an interaction with amino acids residue His 170, Ala 140 and Ser 246 of 1KZM which indicated the inhibition of bacterial growth. [ABSTRACT FROM AUTHOR]

Abstract

The Pandemic COVID-19 outbreak has significantly affected all sections of life, including a substantial reduction in economic development and production, from industrial activities to tourism and automobile congestion. During this phase, the maximum human activities were restricted, but COVID-19 came out as a blessing for the environment. Globally reported that all the environmental variables have improved since the pandemic outbreak, including water and air quality and water quality while minimizing the restrictions for wildlife even in urban areas. India has always been a hotspot of pollution, with rising air quality index (AQI) readings in all large cities due to its vast population, traffic congestion, and polluting industries. However, after the lockdown announced during the pandemic, air quality started improving, and Other environmental factors, such as the water quality of rivers, started to improve. This paper reviewed the studies conducted to define the improvement in India's air and water quality during the lockdown period. Different tools such as remote sensing technologies and onsite real-time monitoring are used in many studies to monitor India's air and water quality during this period.

Abstract

Background Kidney transplantation is the treatment of choice for end-stage renal disease. However, since pediatric patients have long projected life-years, it is also optimal for them to get well-matched transplants to minimize long-term sensitization. In North America, pediatric kidney transplantation is largely dependent upon the use of deceased donor organs, making it challenging to identify timely, well-matched transplants. Pediatric recipients may have willing living donors who are either HLA- or ABO-incompatible (ABOi); therefore, one solution is to utilize ABOi transplants and paired exchange programs to enhance HLA matching and living donation. Case-diagnosis/treatment We adopted this approach for a highly sensitized patient with cPRA 90 %, who received a successful ABOi paired exchange transplant. Abstract

Accurate detection of hypertension is crucial for clinical management of pediatric chronic kidney disease (CKD). The 2017 American Academy of Pediatrics (AAP) clinical practice guideline for childhood hypertension included new normative blood pressure (BP) values and revised definitions of BP categories. In this study, we examined the effect of applying the AAP guideline’s normative data and definitions to the Chronic Kidney Disease in Children (CKiD) cohort compared with use of normative data and definitions from the 2004 Fourth Report on the Diagnosis, Evaluation, and Treatment of High Blood Pressure in Children and Adolescents.

Abstract

Background: Periodontal diseases are of microbial etiology and are globally causing loss of teeth in adult population. Many severe oral diseases have been recently associated to Herpes viruses, of which Epstein Barr Virus (EBV) and human cytomegalovirus (HCMV) have been indicated in the etiology of periodontal diseases. Aim: The purpose of the study was to compare the effect of EBV in different types of periodontal diseases namely acute gingivitis, chronic gingivitis, acute and chronic, localized and generalized aggressive (juvenile) periodontitis and apical periodontitis. Material and Method: 70 individuals were included in this study. Supragingival plaque and plaque from two deepest sites of the periodontal pockets were collected then stored at -70° c and prepared for nucleic acid extraction. For EBV detection, DNA were extracted from the plaque samples with the QIAamp DNA mini kit. Q-PCR was performed by targeting the non-polymorphic Epstein-Barr nuclear antigen-1 (EBNA-1) gene using Corbett Research 6000 Q-PCR instrument and Rotor gene 6000 software. Results: Overall prevalence of EBV in the disease group was 60% (27/45 patients) as compared to only 8% (4/25 people) in the normal population. The mean copy number of EBV DNA was found to be significantly higher in periodontitis (2234 ± 1811.34) when compared to gingivitis (554 ± 537.64, p = .001) and normal patients (370 ± 161.03, p < .001). Conclusion: Here, we found that the prevalence of EBV as well as copy number of EBV was significantly higher in periodontitis patients as compared to gingivitis patients or normal population.
