Introduction: In sub-Saharan Africa, many older people experience vision impairment (VI) and its adverse health outcomes. In this study, we examined separately the association between VI and each adverse health conditions (cognitive disorders, vision-related quality of life [VRQoL], and daily functioning interference [DFI]) among Congolese older people. We also explored whether VI had a significant effect on VRQoL components in our population. Method: We performed cross-sectional analyses on data from 660 Congolese people aged ≥65 years who participated in the 2013 survey of the EPIDEMCA population-based cohort study. VI was defined as having a near visual acuity Results: VI was not associated with cognitive disorders after adjustment for residence area (adjusted odds ratio = 1.7; 95% confidence interval [CI]: 0.6; 4.7), but it was associated with a low VRQoL score (adjusted β = −12.4; 95% CI: −17.5; −7.3) even after controlling for several covariates. An interaction between VI and age (p = 0.007) was identified, and VI was associated with DFI only among people aged >73 years (adjusted β = 0.5; 95% CI: 0.2; 0.8). Our exploratory analysis showed that all components of VRQoL decreased with a decrease in visual acuity (corrected p ≤ 0.05). Conclusion: VI was associated with poor VRQoL and high DFI. Residence area seems to play a confounding role in the association between VI and cognitive disorders. Our findings suggest that targeting interventions on vision could reduce DFI among older people and improve their well-being.

International audience; Objectives: Older people are vulnerable to mental and cognitive health issues. Studies reported that depressive or anxious older people are more likely to present or to develop dementia over time. The relations between these psychiatric disorders and dementia are poorly explored in sub-Saharan Africa. Therefore, we investigated the association between depression, anxiety and dementia among older people living in two Central Africa countries.Methods: A multicenter cross-sectional study was carried out between 2011 and 2012 in Republic of Congo (ROC) and in Central African Republic among people aged ≥65 years. An annual follow-up was performed only in ROC until 2015. Data were collected using a standardized questionnaire and participants underwent a short physical examination. Depression and anxiety symptoms were collected at baseline and were ascertained using a community version of the Geriatric Mental State (GMS-B3). Probable cases were defined as having a GMS-AGECAT score ≥3. Dementia diagnosis was performed by a neurologist according to DSM-IV criteria. Cross-sectional and longitudinal associations between depression, anxiety and dementia were investigated using logistic and competing-risk regression models respectively.Results: Sample for cross-sectional analyses was 1773 participants (median age: 72 [interquartile range: 67-77] years) and 1053 (59.4%) were female. In total, 650 (36.7%), 133 (7.5%) participants presented respectively depression and anxiety symptoms, and 135 (7.6%) participants were diagnosed with dementia. After accounting for several covariates, depression significantly increased the odds of dementia (adjusted Odds Ratio= 1.62; 95%CI: 1.05-2.50), while anxiety was not associated with dementia (aOR= 0.81; 95%CI: 0.40-1.65). The effects of both psychiatric disorders on dementia incidence within the Congolese participants will be presented in detail.Conclusions: Many Central African older people experience depression more than anxiety and are more likely to present dementia. Promoting and strengthening social support around depressive older people could help to delay or manage dementia in this population.

International audience; Introduction: Depression and anxiety are two very common psychiatric disorders in late-life. They are markers of poor quality of life and are strongly associated with death among older people. Yet, few studies on these comorbidities have been conducted in the African population. This study aims to present the epidemiology of depression and anxiety among older people in Central Africa.Method: A cross-sectional population-based study was carried out in Republic of Congo (ROC) and Central African Republic (CAR) between 2011 - 2012 among older people aged ≥ 65 years (EPIDEMCA study). Data were collected using a standardized questionnaire and participants underwent a brief physical examination. Depression and anxiety symptoms were ascertained using a community version of the Geriatric Mental State (GMS-B3) and the Automated Geriatric Examination for Computer Assisted Taxonomy diagnostic system (AGECAT), probable cases were defined as having a GMS-AGECAT level of 3 or more. Logistic regression models were used to investigate the association between potential risk factors collected and each symptom.Results: Overall 2002 participants were included in the EPIDEMCA study (500 in Brazzaville and 529 in Gamboma in ROC, 500 in Bangui and 473 in Nola (473) in CAR). Median age of the participants was 72 years [interquartile range: 68 – 78 years] and females were mostly represented (61.8%). Prevalence was 38.1% (95% Confidence Interval: 35.9% - 40.2%) for depression, 7.7% (95% CI: 6.5% - 8.9%) for anxiety and 5.7% (95% CI: 4.6% - 6.7%) for the co-occurrence of both disorders. For all three outcomes, prevalence was significantly higher among females and in rural areas. Only depression increased with age. International audience; Objective:It is recognized that some cardiovascular risk factors (CVRF) are involved in poor ocular health. The deleterious effects are potentially more pronounced among older adults. However, it is unknown whether CVRF areassociated independently or not to Visual Impairment (VI) among African older people while both (CVRF and VI) are verycommon in sub-Saharan Africa. Therefore, we investigated the association between CVRF and near VI amongCongolese older people.Methods: Participants were Congolese older people aged ≥ 65 years included in EPIDEMCA-FU (Epidemiology ofDementia in Central Africa - Follow-up) population-based cohort carried out from 2011 to 2015. Visual acuity wasassessed at 30 cm using a Parinaud chart. Near VI was defined as having a visual acuity < 20/40. A cohort design wasused taking all CVRF and covariates at baseline and visual acuity at 1st follow-up. Associations were investigated usingmultivariate logistic regression.Results: 549 participants were included in our analyses. Participants median age was 72 years [interquartile range: 68-78 years] and 331 (60.3%) were females. In total, 378 had near VI (328 had mild/moderate near VI, 38 had severe VI and12 were blind). Prevalence of near VI was 68.8% [95% Confidence Interval: 64.9% - 72.7%] and higher in males (71.1%;95%CI: 65.1% - 77.1%) than females (67.4%; 95% CI: 62.3% - 72.4%). Of the CVRF explored, we found that having highBMI ≥ 25 kg/m2 (adjusted Odds Ratio = 2.31 [95%CI: 1.25 – 4.28]), diabetes (aOR=2.14 [95%CI: 1.07 – 4.29]) andhypertension (aOR=1.65 [95%CI: 1.03 – 2.66]) were independently associated with near VI. However, after adjustmenton cognitive status, hypertension (full-adjusted OR=1.58 [95%CI: 0.97 – 2.58]) was no longer significantly associated withVI. There was no interaction between these CVRF. We found no significant association for alcohol consumption(p=0.541), smoking status (p=0.88), and indirect marker of poor diet (0.33) in our sample.Conclusion: Prevalence of near VI was high among Congolese older people. High BMI, diabetes, and hypertension wereindependently associated to near VI in this population. Our study suggests that these CVRF could represent targets for VIprevention. However, further studies are required to clarify underling mechanisms because these associations may bemediated by the presence of ocular conditions/eye diseases.

18. Cardiovascular Health and Near Visual Impairment Among Older Adults in the Republic of Congo: A Population-Based Study.

Background: Visual impairment (VI) and determinants of poor cardiovascular health are very common in Sub-Saharan Africa. However, we do not know whether these determinants are associated with VI among older adults in this region. This study aimed at investigating the association between the determinants of poor cardiovascular health and near VI among older adults living in Congo. Methods: Participants were Congolese adults aged 65 or older included in Epidemiology of Dementia in Central Africa—Follow-up population-based cohort. Near VI was defined as visual acuity less than 20/40 measured at 30 cm. Associations between determinants of poor cardiovascular health collected at baseline and near visual acuity measured at first follow-up were investigated using multivariable logistic regression models. Results: Among the 549 participants included, 378 (68.8%; 95% confidence interval [CI]: 64.9%–72.7%]) had near VI. Of the determinants of poor cardiovascular health explored, we found that having high body mass index of at least 25 kg/m
2 (odds ratio [OR] = 2.15; 95% CI: 1.25–3.68), diabetes (OR = 2.12; 95% CI: 1.06–4.25) and hypertension (OR = 1.65; 95% CI: 1.02–2.64) were independently associated with near VI. Conclusions: Several determinants of poor cardiovascular health were associated with near VI in this population. International audience; Introduction : Le vieillissement correspond à l’ensemble des processus physiologiques modifiant la structure et les fonctions de l’organisme à partir de l’âge mûr. Plus les personnes vieillissent, plus il y a apparition de fragilité physique et de polypathologie, dont les troubles cognitifs. L’objectif de cette étude était d’analyser l’association entre les troubles cognitifs et la fragilité physique.Méthodes : L’étude multicentrique, Epidemiology of dementia in Central Africa (EPIDEMCA) faite d’enquêtes transversales porte-à-porte auprès des personnes âgées de 65 ans et plus, a été menée entre novembre 2011 et décembre 2012 dans deux pays d’Afrique centrale, République du Congo et République centrafricaine. Un questionnaire sur la fragilité, SOF (Study of Osteoporotic Fractures) index, était intégrée lors de ces enquêtes avec des critères suivants : robuste (0 composante), pré fragile (1 composante) et fragile (≥ 2 et 3 composantes).Résultats : 1551 personnes âgées de 65 ans et plus étaient inclues, réparties en 394 à Brazzaville et 376 à Gamboma en République du Congo, puis 407 à Bangui et 374 à Nola en République centrafricaine. En RCA, les personnes âgées de 65 ans et plus présentant des troubles cognitifs de type Mild Cognitive Impairment (MCI) avaient deux fois plus le risque d’être fragiles 2,0 (1,1-3,9) avec p = 0,03, alors que le risque était multiplié par trois en cas de démences 3,0 (1,5-6,0) avec p = 0,002 au cours de l’analyse multivariée. Par contre en République du Congo, au cours de l’analyse uni variée, le risque était presque doublé quand les troubles cognitifs étaient de type MCI 1,9 (1,0-3,4) avec p = 0,05 et en cas de démences 2,2 (1,0-5,2) avec p = 0,06, mais aucune différence n’était statistiquement significative en analyse multivariée.Discussion : Les troubles cognitifs font partie des critères de causalité du syndrome de fragilité, au regard du processus physiopathologique du vieillissement cérébral. Les mécanismes comme les diminutions du poids du cerveau d’environ 2 %, du débit cérébral et des enzymes responsables de la synthèse des neurotransmetteurs sont incriminés.Conclusion : Les personnes âgées atteintes des troubles cognitifs et fragiles doivent être dépistées, traitées et suivies au long terme. L’information, l’éducation et la communication des populations ainsi que l’expertise gériatrique auront un impact sur la prise en charge

International audience; Introduction et but de l’étudeNous avons retrouvé précédemment que la formule de Chumlea pour les noirs américains, validée pour la prédiction de la taille à partir de la hauteur talon-genou (HTG), ne fournit une bonne prédiction (± 5 cm) de la taille que dans 67 % des cas chez les personnes âgées (PA) d’Afrique Centrale (AC). Aucune formule n’est validée pour cette population. Le but de ce travail était de créer et valider une formule prédictive de la taille utilisant la HTG dans une population de PA d’AC.Matériel et méthodesL’étude EPIDEMCA, a été réalisée chez des 1754 PA de plus de 65 ans au Congo et en République Centrafricaine. Les PA incluses ont bénéficié d’une évaluation nutritionnelle avec mesure de la taille (cm) et de la HTG (cm). Les formules prédictives ont été créées à la suite d’une régression linéaire multiple selon le sexe, l’âge (ans) et la HTG à partir d’un sous-échantillon randomisé (n = 877). La précision de cette nouvelle formule a été testée sur l’autre moitié (n = 877). Des tests de corrélation intraclasse et le pourcentage de bonne prédiction ± 5 cm par rapport à la taille mesurée étaient calculés.Résultats et analyse statistiqueL’âge moyen était de 72,9 ± 6,5 ans et la taille moyenne de 157,4 ± 9,2 cm. La sex-ratio H/F était de 0,6 dans les deux pays. Une formule ajustée sur le sexe (FAS) a été créée : taille (cm) = 72,75 + (1,86 × HTG)–(0,13 × âge) + 3,41 [si homme] ainsi que deux formules stratifiées sur le sexe (FSS), pour les hommes : taille (cm) = 82,77 + (1,76 × HTG)–(0,14 * âge [ans]) ; pour les femmes : taille (cm) = 74,09 + (1,78 × HTG)–(0,09 × âge). Le pourcentage de prédiction et le coefficient de corrélation intraclasse des deux formules dans la population test sont présentés dans le Tableau 1. Le pourcentage de bonne prédiction ± 5 cm était de 71,3 % et de 70,9 % pour les formules FAS et FSS, respectivement. Le pourcentage de bonne prédiction était significativement meilleur avec les formules FAS et FSS par rapport à la formule CPNA (p = 0,008 et p = 0,01, respectivement). L’indice de masse corporelle et le statut nutritionnel n’étaient significativement pas différents que ce soit en utilisant la taille mesurée ou les tailles prédites (Tableau 1).ConclusionLes formules créées semblent applicables pour l’évaluation de la taille par la HTG chez les PA d’AC dans environ 71 % des cas. Ces formules pourront être testées et validées dans d’autre pays d’Afrique pour l’estimation de la taille chez les PA afin d’étendre leur utilisation.

Objectives: Our study aimed at estimating the prevalence of neuropsychiatric symptoms and investigating associated factors among older adults living in two countries in Central Africa (Central African Republic [CAR] and Republic of Congo [ROC]).Methods: The EPIDEMCA multicentre population-based study was carried out in rural and urban areas of CAR and ROC between 2011 and 2012 among people aged 65 and over. After cognitive screening using the Community Screening Interview for Dementia, participants with low performances underwent neurological examination including the brief version of the Neuropsychiatric Inventory Questionnaire (NPI-Q). Multivariate logistic regression analyses were performed to identify factors independently associated with neuropsychiatric symptoms in this population.Results: NPI-Q data were available for 532 participants. Overall, 333 elderly people (63.7%) reported at least one neuropsychiatric symptom. The prevalence of neuropsychiatric symptoms was 89.9% (95% CI, 84.6-95.1) in participants with dementia, 73.4% (95% CI, 65.1-81.7) in participants with mild cognitive impairment (MCI), and 48.7% (95% CI, 42.9-54.6) in participants with no MCI nor dementia after neurological examination (P < 0.0001). The most common symptoms were depression, anxiety, and irritability. Participants living in Gamboma, with normal hearing and with friends in the community, were less likely to present neuropsychiatric symptoms. Physical disability, difficulties in eating, female sex, and dementia were significantly associated with neuropsychiatric symptoms.Conclusion: Neuropsychiatric symptoms are common among older people with neurocognitive disorders in CAR and ROC. Our results confirm those from previous studies in Nigeria and Tanzania. Nevertheless, knowledge of these symptoms remains limited in sub-Saharan Africa, hampering their appropriate management. International audience; Objective: The aim of our study was to estimate the prevalence of behavioral symptoms and associated factors in the elderly in the Central African Republic and the Republic of Congo.Methods: An analysis of baseline data was performed from the EPIDEMCA (Epidemiology of Dementia in Central Africa) multicenter population-based study carried out in rural and urban areas of the Central African Republic and the Republic of Congo between 2011 and 2012 in elderly aged 65 and over. Cognitive disorders were assessed in two phases. The first phase was cognitive screening using the Community Screening Interview in Dementia. Following cognitive screening, in the second phase, participants suspected of dementia underwent clinical assessment based on the abridged version of the neuropsychiatric inventory (NPI) which assesses 12 behavioral disorders in the elderly over the previous 30 days. Results: NPI data were available for 532 out of participants examined in the second phase of EPIDEMCA. Among the 532 elderly included in the analytic sample, 284 were cognitively normal, 130 were demented, 113 had Mild Cognitive Impairment (MCI) and 5 remained without diagnosis. The median age was 75 and most were female. At least one behavioral disorder was reported by 333 elderly (63.7%) with 16.6% reporting 1 or 2 behavioral disorders and 47.1% reporting 3 or more symptoms. In participants with no cognitive impairment, 48.7% reported at least one symptom. The prevalence of behavioral disorders was 73.4% in people with MCI and 89.9% in people with dementia. In our sample, the most frequent symptoms were depression/dysphoria (45.3%), anxiety (28.1%) and irritability (23.7%).Discussion: Behavioral disorders are common in this population. Our data are comparable to those reported in previous population-based studies in Nigeria and Tanzania. It would be important to carry out additional studies to better characterize behavioral disorders in others countries in Africa.

International audience; Objectives: The great age was long regarded as synonymous with wisdom in Africa. Nowadays, it is increasingly synonymous to: body that breaks down and senile dementia. Dementia is a public health problem in high-income countries and in low-and middle-income-countries. In the latter, where more than half of people with dementia (PWD) live, the disease is considered as an enigma by informal caregivers (IC) who sometimes abuse them and limit their access to health care because of pejoratives socio-cultural representations. The objective of the study was to analyze these representations could influence their access to health care and quality of life. Methods: An ethnological study was carried out in three months immersion in urban and rural areas to Congo. The non-directive interviews were conducted in local languages, associated by participant observations to analyze behaviors, gestures, discourses, to record phenomena that an interviewee could intentionally or not, fail to say and to control his statements. All data collected were analyzed at the time of their compilations. All non-directive interviews and participant observations were annotated and then transcribed. Before classifying these interviews into themes, a critical reading was made of them. Results: 36 life stories were conducted among IC. In urban area, 68.4% and 26.4% of PWD were treated respectively by syncretic church leaders (SCL) and traditional healers (TH). In rural area, 58.8% and 29.4% of PWD were treated respectively by TH and SCL. Only 5.3% of PWD was treated by general practitioner in urban area versus 11.8% by nurses in rural area. PWD were considered as possessed of evil spirits or demons, justifying the solicitation of to SCL and TH. The disease isn’t medicalized, the hospital isn’t a priority. Conclusion: Chance and ignorance have no place in the emergence of mental illnesses in traditional societies. Metaphysical causes are the most evoked, hence the inaccessibility of PWD to health care.

International audience; A multicenter population-based study was carried out in Central African Republic (CAR) and Republic of Congobetween 2011 and 2012 including both urban and rural sites in each country. Participants aged ≥65 years oldwere interviewed and participants in Congo were followed-up annually between 2012 and 2015 (EPIDEMCA-FUstudy). Qualitative interviews using an ethno-anthropological approach were also performed with peopleidentified with dementia and their relatives in order to investigate social representations, beliefs and stigma.ResultsOverall, 2,002 elderly were interviewed in both countries, including 473 in Nola (rural CAR), 500 in Bangui (urbanCAR), 529 in Gamboma (rural Congo) and 500 in Brazzaville (urban Congo). The crude prevalence of DSM-IVdementia was estimated at 8.5% (CI95%[6.1-11.3] in Nola, at 6.4% (CI95%[4.4-8.9]) in Bangui, 5.7% (CI95%[3.9-8.0] in Gamboma and at 6.6% (CI95%[4.6-9.1]) in Brazzaville. The crude incidence was estimated at 15.8(95% CI 10.2 – 23.3) per 1000 PY. Factors associated with either prevalent or incident dementia in thesestudies, as well as the results of the qualitative interviews, will be detailed during the presentation.ConclusionThe need for targeted social and health policies for older people, as well as strategies for dementia care, isevident and growing in sub-Saharan Africa (SSA).

OBJECTIVES: Neuropsychiatric symptoms are common in dementia. Limited data are available concerning their association with dementia in developing countries. Our aim was to describe the severity of neuropsychiatric symptoms among older people, evaluate the distress experienced by caregivers, and assess which neuropsychiatric symptoms were specifically associated with dementia among older adults in Central Africa. DESIGN: This study is part of the EPIDEMCA program, a cross‐sectional multicenter population‐based study. SETTING: The EPIDEMCA program was conducted from November 2011 to December 2012 in urban and rural areas of the Central African Republic and the Republic of the Congo. PARTICIPANTS: Participants were older people (≥65 y) included in the EPIDEMCA program who underwent a neuropsychiatric evaluation. The sample included overall 532 participants, of whom 130 participants had dementia. MEASUREMENTS: Neuropsychiatric symptoms were assessed with the brief version of the Neuropsychiatric Inventory including the evaluation of severity and associated distress. Diagnostic and Statistical Manual of Mental Disorders, 4th ed., Text Revision, criteria were followed to diagnose dementia. A logistic regression model was used to identify associated neuropsychiatric symptoms. RESULTS: The prevalence of neuropsychiatric symptoms was 89.9% (95% confidence interval = 84.6‐95.1) among people living with dementia. The overall median severity score for neuropsychiatric symptoms was 9 [interquartile range [IQR] = 6‐12], and the overall median distress score was 7 [IQR = 4‐10]. Overall median scores of both severity and distress were significantly increased with the number of neuropsychiatric symptoms, the presence of dementia, and dementia severity. Depression, delusions, apathy, disinhibition, and aberrant motor behavior were associated with dementia after multivariate analysis. CONCLUSION: This report is one of the few population‐based studies on neuropsychiatric symptoms among older people with dementia in Sub‐Saharan Africa and the first one evaluating the severity of those symptoms and distress experienced by caregivers. Individual neuropsychiatric symptoms were strongly associated with dementia in older people and require great attention considering their burden on populations. International audience; IntroductionAccording to the World Health Organization, dementia poses a public health problem in sub-Saharan Africa because of the growing number of elderly. But the perception of this condition is poorly documented in low-income countries.ObjectivesTo Identify and analyze the perceptions of dementia in different population groups more or less concerned. MethodA Cross sectional study was conducted following a general population survey on dementia between October and December 2008 in Bangui and February-March 2009 in Brazzaville. Five groups of subjects were involved. The stigmatization of people with dementia was assessed using scale consisting of 15 questions. The Stat View 5.0 and SAS 9.1.3 software were used for data analysis.ResultsThe subjects of the groups interviewed were dominated by women with the exception of nursing staff. There was no equivalent to the biomedical concept of dementia in the local languages of the two countries in which the study was conducted. The problems of inability to work (73.3 to 82.7%) and reduction in family income (55.7 and 79.0%) were reported by subjects with mild cognitive impairment. The emotional impact was dominated by concerns about the progression of cognitive problems (52.5 to 76.8%), sadness and anxiety (50.8 to 79.4%).ConclusionIn urban zones in Central Africa, we find that perceptions of dementia include traditional and magical-religious aspects, even among health care workers who supposed best placed to provide tailored information.

Background: There are a few validated tools capable of assessing the dimensions essential for the diagnosis of dementia and cognitive disorders in sub-Saharan Africa. Objectives: Our aim was to develop an adapted tool, the Central African – Daily Functioning Interference (DFI) scale. Methods: An initial 16-item scale of activity limitations and participation restrictions was completed by 301 participants with low cognitive performances to assess their level of DFI. A psychometric evaluation was performed using Item Response Theory. Results: A unidimensional 10-item scale emerged with a reasonable coverage of DFI (thresholds range: –1.067 to 1.587) with good item discrimination properties (1.397–4.076) and a high reliability (Cronbach's al pha = 0.92). The cutoff for detecting 96% of those with dementia was with a latent score ≥0.035 that corresponds to the LAUNDRY limitation. Conclusions: These results provide valuable support for the reliability and internal validity of an operational 10-item scale for DFI assessment used in Central Africa for the diagnosis of dementia in the elderly.
