The understanding of the nature of catecholamine-secreting tumors has changed significantly in recent years, affecting terminology and classification. Phaeochromocytoma/paraganglioma (PCC/PG) is a rare neuroendocrine tumor from chromaffin tissue that produces and secretes catecholamines. The incidence of PCC/PG is relatively low, with 2–8 cases per 1 million population per year; among patients with arterial hypertension, their prevalence is 0.2–0.6%. However, delayed diagnosis of PCC/PG is associated with a high risk of cardiovascular complications and a high mortality rate. The consensus presents the clinical manifestations of the disease with an emphasis on the course of arterial hypertension as the most common symptom in PCC/PG; modern ideas about the features of diagnosis, aspects of preoperative preparation, treatment, and follow-up of patients with PCC/PG are considered.

The problem of preservation and strengthening of student's health is one of the most actual for today. One of integrated indicators of the level of individual health is the indicator of biological age of the person. Research by definition of biological age of students is conducted. 63 young men and 45 girls have taken part in research at the age of 17-20 years. The biological age depending on a floor and level of impellent activity is estimated. Received results are compared with the data of researchers of other regions.

The protective properties of the coating applied to the surface of lithium-manganese spinel (LiMn2O4), using the eutectic composition of Li2O : B2O3 = 47 : 53 (wt.) with the melting point of 650°C, were studied. The content of the eutectic lithium borate varied from 1% to 10%. The electrochemical behavior of the obtained materials in the cathode half-cells of lithium-ion battery was studied at room temperature. It was shown that an abnormally large decrease in the specific capacity of lithium-manganese spinel took place simultaneously with the stabilizing effect. The side chemical reactions that occur between LiMn2O4 and the eutectic lithium borate during annealing while applying a protective layer were analyzed. The chemical stability of lithium-manganese spinel (LiMn2O4) and the manganese-containing solid solution with the layered structure, LiNi1/3Mn1/3Co1/3O2, with respect to enriched lithium borates, was compared.

Introduction. Allergy to birch pollen is a common allergic disease. The main allergen of birch pollen is Bet v 1. The Bet v 1 protein structure is the similar to proteins of hazel, alder and elm pollen, as well as fruits proteins (apples, apricots, peaches, cherries) and vegetables ones (carrots, celery). Therefore, children with an allergy to the Bet v 1 protein may experience oral allergy syndrome (OAS) and even anaphylaxis when eating these foods. Allergen-specific immunotherapy (ASIT) is the indication for the pollen allergy treatment.Presentation of the clinical case. A 11 years old child was observed with a diagnosis of bronchial asthma, allergic form, moderate persistent clinical course. Pollinosis. Allergic rhinitis. Allergic conjunctivitis. Allergy to birch pollen (Bet v 1). Cross food hypersensitivity: oral syndrome (PR-10 proteins) and anaphylaxis (hazelnuts, peanuts). Systemic manifestations of pollen allergy limit immunotherapy due to the possible development of adverse reactions.The child was prescribed sublingual immunotherapy (SLIT) with the medication “Staloral birch pollen allergen”. Baseline therapy was chosen for the child to achieve control of asthma and rhinitis, a hypoallergenic diet with the exclusion of significant food allergens was prescribed. Asthma and rhinitis exacerbation and OAS was observed during the 1st and 2nd treatment courses. Against the background of baseline therapy, SLIT was continued. The full maintenance dose was achieved during the 3rd SLIT course. Immunotherapy contributed to the reduction of symptoms of rhinitis and asthma, reduction of disease clinical course severity, reduction of baseline medications doses and OAS relief.

Introduction. In conditions of the Emergency Cardiology Department, a timely and differential diagnosis of myocardial pathology is especially important in the absence of visible focal changes and significant coronary artery stenosis. This group of patients includes recipients of a transplanted heart, when it is difficult to count on the high sensitivity of perfusion images alone. This can be explained by the diffuse, balanced distribution of ischemia.Objective. To present the possibilities of perfusion myocardial single photon emission computed tomography synchronized with electrocardiography for a detailed assessment of the functional condition of both ventricles of the transplanted heartMaterial and methods. We have presented three clinical case reports of the patients with different pathology of the transplanted heart who referred themselves to the emergency cardiology clinic and underwent electrocardiographysynchronized perfusion myocardial single photon emission computed tomography for diagnostic purposes.Results. In all the cases presented, the radionuclide study influenced making the diagnosis and changing the treatment tactics. In the first case, it was possible to identify focal myocardial changes, for which coronary angiography and percutaneous coronary intervention with thromboextraction were immediately performed. In the second case, the image analysis reflecting the function of the myocardium (polar maps of wall movement and systolic thickening) allowed us to note a low efficacy of treatment for the transplanted heart rejection. In the third clinical case, the initial single photon emission computed tomography suggested the inflammatory nature of changes in the myocardium, which was verified by the results of endomyocardial biopsy.Conclusion. In the presented clinical cases, the urgently performed electrocardiography-synchronized perfusion myocardial single photon emission computed tomography made it possible to perform invasive interventions in time and thereby contribute to the recovery of the graft functional state and patient's condition improvement.

Introduction. Histoplasmosis is not an endemic form of fungal infection in Russia; its sporadic cases are mainly associated with the import of the fungus from endemic countries. We consider it necessary to demonstrate a rare case of the disseminated histoplasmosis development in a kidney transplant recipient.Objective. Demonstration of a case of the disseminated histoplasmosis development in a kidney transplant recipient.Results. The clinical manifestations of the disease were described; the affected organs were macroscopically and histologically studied, which made it possible to diagnose disseminated histoplasmosis.Conclusions. This case prompts being on alert to potential occurrence of fungal infections, including non-endemic ones, in patients receiving immunosuppressive therapy.

Introduction. Chemotherapy in combination with targeted therapy (CT + TT) using a monoclonal antibody against epidermal growth factor receptor (EGFR) cetuximab and subsequent maintenance targeted therapy (CT + TT/TT) is the leading 1st line therapy of recurrent/metastatic head and neck squamous cell carcinoma to achieve objective response irrespective of programmed cell death-ligand 1 (pD-L1) expression level. However, often in clinical practice patient profile does not match characteristics of patients included in registration studies. Therapy selection is based on massive advancement of the tumor, low performance status of the patient, use of various chemotherapy regimes which often decreases therapy effectiveness. This creates a necessity of identification of clinical markers of effectiveness based on the drug's pharmacodynamics and mechanism of action.Aim. To analyze the effect of clinical characteristics, peripheral blood markers, and systemic inflammation on long-term results of CT + TT/TT with cetuximab in cancer of the mucosa of the head and neck.Materials and methods. The prospective observational study performed at the Oncology Department with a course on thoracic surgery of the Kuban State Medical University, included 52 patients with head and neck squamous cell carcinoma receiving CT + TT/TT between 2020 and 2023. Clinical characteristics and results of peripheral blood tests were retrospectively analyzed, indices of inflammatory reaction prior to treatment and 12-16 weeks after CT + TT/TT with cetuximab were calculated. Statistical analysis was performed using the med Calc ver. 20.218 and IBM SPSS Statistics 22 software.Results. CT + TT/TT with cetuximab significantly increased red blood cell count (RBC), lymphocyte-monocyte ratio (LMR), and decreased systemic inflammatory markers (SIM) (p 3.27 after 12-16 weeks of therapy (AUC = 0.685; 95 % CI 0.486-0.885; p = 0.0691). median survival of patients after the start of CT + TT/TT with cetuximab was 28 months (95 % CI 17-48), progression-free survival was 8 months (95 % CI 5-36). For RBC count >3.8 x 1012/L 12-16 weeks after the start of therapy, risk of progression decreased by 79 % (hazard ratio 0.21; 95 % CI 0.07-0.62; p = 0.0047). Introduction. Cigarette smoking along with alcohol consumption, infection with human papilloma (HPV) and the Epstein–Barr viruses, is one of the main risk factors for head and neck squamous cell carcinoma. It can also contribute to virus-associated carcinogenesis and affect the clinical and prognostic features of the tumor. Continued smoking at diagnosis of head and neck squamous cell carcinoma is a negative prognostic factor and contributes to reduced patient survival rates. Aim. To analyze the smoking status and the effect of etiopathogenesis on molecular and clinical features in patients with oropharyngeal squamous cell carcinoma (OpSCC). Materials and methods. In patients with OPSCC, the smoking status and the level of motivation to its cessation were clarified. Immunohistochemical analysis of tumor sections from paraffin blocks from the ThermoScentific immunohis-tosteiner was performed using monoclonal antibodies to estrogen receptors α (ERα), programmed death-ligand 1 (pD-L1), Bcl-2, p53 and p16. The percentage of tumor-infiltrating lymphocytes (TILs) in the stromal component of the tumor (hematoxylin and eosin staining) was also analyzed. Results. Of 113 patients with OpSCC, 80 % of patients were current smokers, including 75 % of patients with Hpv-positive tumor status. men with OpSCC were statistically significantly more likely to smoke than women (p = 0.001), as were men with Hpv-positive tumor (p = 0.015). Smoking patients were characterized by a larger size of the primary tumor (T3–4), including those with Hpv (p = 0.015). positive Hpv status of the tumor is associated with autoimmune diseases (autoimmune thyroiditis, psoriasis) (p ˂ 0.05). A high level of TILs is characteristic of smokers, including those with p16+-status (p ˂0.05), but not for all patients with this status. The positive expression of ERα is associated with p16+-status, and the latter is associated with the absence of p53 expression (p ˂0.05). During conservative therapy, tumor regression is associated with the primary tumor size T1–2, p16 + status, female sex, age less than 55 years, TILs level ≥ 57.5 %, pD-L1 expression levels on tumor cells ≥2.2 %, Bcl-2 ≥5 % and ERα ≥65 % (p ˂ 0.05). At the same time, quality of good predictive model for tumor regression showed a TILs level of ≥57.5 % (area under curve, AuC) 0.796; sensitivity 78.6 %; specificity 75 %) and ERα (AuC 0.725; sensitivity 65 %; specificity 83 %). The level of motivation to quit smoking ranged from 1 to 7 points according the 10-point motivation scale in 89 % of respondents. Only 10 patients were ready to quit smoking immediately (10 points on the motivation scale). Conclusion. Molecular heterogeneity of OPSCC was revealed, including in the group of p16+-tumors. Correlation of clinical and molecular parameters with response to conservative therapy can be used in planning of primary therapy and earlier use of surgical treatment. The level of motivation to quit smoking in patients with squamous cell carcinoma of the head and neck may be increased by drawing attention to the problem at each doctor’s appointment as well as by informing about how to quit smoking and therapeutic approaches. In unmotivated patients, the concept of reducing the harm of smoking can be used involving the replacement of cigarettes with electronic tobacco heating systems, eliminating the carcinogenic effect of its combustion products.

The progression of head and neck squamous cell cancer (HNSCC) may be influenced by the characteristics of the local and systemic immune response, including the peculiarities of systemic inflammatory response during surgical interventions. The dependence of the prognosis of cancer patients on the preoperative general somatic status and postoperative complications is demonstrated precisely through the prism of a systemic inflammation development promoting the progression of oncological process. It is necessary to study the interaction of a typical inflammatory reaction and the features of reparative processes to understand the correct treatment strategy for patients with oral cancer in the conditions of the existing oncological process. It was found that chronization of inflammation is associated with m2-macrophage polarization which contributes to the tumor progression. And systemic inflammation indices correlate with treatment results. It is also necessary to identify prognostic factors of postoperative complications that affect the chronization of inflammation. They may include the general somatic and nutritional status of patients, concomitant diseases and indices of systemic inflammation. It is obvious that improving the results of surgical treatment of patients with oral cancer can be influenced by various ways of managing the inflammatory response, including the peculiarities of performing the reconstructive interventions and improving the preoperative general somatic and nutritional status.

New concept of passive cooling of buildings is presented in this article.. In order to achieve that, it is planned to define three basic elements of the building concept – shape of building, sun movement and sustainability of the building. These parameters will serve as corner stone’s also further in development of building concept. In addition not only passive cooling elements are considered during concept elaboration process but also passive building basic principles which include building location, window placement on facades, room alignment, building constructions and high comfort level. Dynamic simulation software was used in order to perform calculations, and to make conclusions on most optimal window/wall proportions and passive cooling potential. Paper presents study on multi apartment buildings renovation specifics in Latvia and its influence on total energy consumption. The necessary reconstruction goals are defined to reach European Regional Development Fund project “A New Concept for Sustainable and Nearly Zero-Energy Buildings” Nr. 1.1.1.1/16/A/007 main targets.

In this article the possibility of heat accumulator use in one of the buildings of SPBSTU was analyzed. The use of this accumulator could lower the hot water demand. To complete the goal it was assumed that there is an additional café with defined working hours. Different kinds of heat collecting materials were studied: water, red brick, concrete and granite. Depending on charging time of the heat accumulator, the results of the heat gaining abilities of different materials were made. As a result it was calculated that single-phased accumulating materials should not be used as heat collectors in public buildings.

Currently, stem cells technology is an effective tool in regenerative medicine. Cell therapy is based on the use of stem/progenitor cells to repair or replace damaged tissues or organs. This approach can be used to treat various diseases, such as cardiovascular, neurological diseases, and injuries of various origins. The mechanisms of cell therapy therapeutic action are based on the integration of the graft into the damaged tissue (replacement effect) and the ability of cells to secrete biologically active molecules such as cytokines, growth factors and other signaling molecules that promote regeneration (paracrine effect). However, cell transplantation has a number of limitations due to cell transportation complexity and immune rejection. A potentially more effective therapy is using only paracrine factors released by stem cells. Secreted factors can positively affect the damaged tissue: promote forming new blood vessels, stimulate cell proliferation, and reduce inflammation and apoptosis. In this work, we have studied the anti-inflammatory and neuroprotective effects of proteins with a molecular weight below 100 kDa secreted by glial progenitor cells obtained from human induced pluripotent stem cells. Proteins secreted by glial progenitor cells exerted anti-inflammatory effects in a primary glial culture model of LPS-induced inflammation by reducing nitric oxide (NO) production through inhibition of inducible NO synthase (iNOS). At the same time, added secreted proteins neutralized the effect of glutamate, increasing the number of viable neurons to control values. This effect is a result of decreased level of intracellular calcium, which, at elevated concentrations, triggers apoptotic death of neurons. In addition, secreted proteins reduce mitochondrial depolarization caused by glutamate excitotoxicity and help maintain higher NADH levels. This therapy can be successfully introduced into clinical practice after additional preclinical studies, increasing the effectiveness of rehabilitation of patients with neurological diseases.

In the context of a significant reduction in international mobility for Russian scientists, the importance of domestic programs is increasing. This study aims to assess the scale of domestic academic mobility programs, create a profile of those interested in participating in such programs, and describe the outcomes of such participation using young candidates of sciences as an example. The study is based on the results of representative surveys conducted among Russian teachers, researchers, and graduate students as part of the Monitoring of the Economy of Education in June-October 2022, as well as in-depth interviews conducted with participants of Russian postdoc programs by the authors in June-September 2022. It was found that the level of domestic mobility for teachers and researchers is low: only 3% of them underwent long-term internships within the country in 2019-2021, and 3% of graduate students participated in Russian postdoc programs. The potential mobility level is also low: one-third of Russian teachers (30%) are not willing to change their place of residence for a new job. Participants of Russian postdoc programs highlight numerous positive effects of these programs, including significant growth in qualifications, acquisition of new competencies, and expansion of professional networks. However, the development of domestic academic mobility is hindered by various institutional barriers such as the closed academic environment and cautious attitude towards mobility, as well as lack of support during relocation.

Since the collapse of the Soviet Union and the opening of borders, numerous measures have been taken to internationalize Russian science. In particular, publications in journals that are listed in international scientific citation databases have become a point of closer focus. Universities and scientific organizations began inviting foreign professors and sending their own staff and students abroad on international internships. After the start of the Special Military Operation in Ukraine in 2022, Russian science faced numerous sanctions that entailed a reduction in international cooperation. However Russian scientists have accumulated 30 years of experience in active participation in international cooperation and academic mobility. Nevertheless, there is still a significant lack of understanding of how this experience will impact the scientific careers of Russian scientists going forward. In this article, the authors present the results of a study dedicated to exploring this issue. The theoretical framework for the analysis is based on such a concept as “the Three Careers of an Academic” (organizational, cognitive, and community careers) proposed by German experts in sociology of science Johan Gl?ser and Grit Laudel. The empirical data used in the study include materials from in-depth interviews with young scientists under the age of 39, collected in 2020–2021 as part of a project known as “International Mobility of Russian Young Researchers”, as well as the results of a sociological survey of 7,255 highly productive scientists conducted under the “Monitoring of education markets and organizations” in 2022. All respondents had spent more than three months abroad. The study revealed that international mobility has a comprehensive impact, noticeable in all three components of a scientific career. Acquired knowledge and skills, as well as involvement in international projects contribute to the development of a researcher’s cognitive career, while new professional connections promote advancement in the international scientific community. As for the “organizational dimension” of a career, mobility does not have a direct influence on career advancement. However, mobility often leads to personal changes, particularly influencing one’s determination to switch jobs and seek better conditions. These assessments are presented for the first time and can be useful in developing recommendations for science and technology policies regarding the revision of mobility support programs in light of the new geopolitical reality.

Based on the characteristics of the current state of the use of renewable energy sources (RES) in the world and in Russia, the role of solar energy in solving one of the urgent problems of the development of modern society - energy transition to the use of low-carbon energy sources is considered. The volumes of electricity produced and the reduction of CO2 emissions at photovoltaic stations operating in the regions of Russia at the beginning of 2022 were evaluated and analyzed. The Republic of Kalmykia and Altai were singled out, which have the largest shares of solar electricity in the energy balances, 35.7 and 23.2%, respectively. The problems of assessing the natural resource risks of using solar resources in the Russian Federation in connection with the climatic zoning of the territory are considered. The authors propose a method for assessing the degree of risk of using solar resources based on calculating the characteristics of the variability of solar radiation on the earth’s surface, assessing and mapping the level of natural resource risks. The levels of resource risks in the development of solar energy for the Orenburg and Astrakhan regions, leaders in the development of solar energy in Russia, were assessed.

This article delves into the proliferation of dacha as a second / temporary country residence for urbanites in Russia. The phenomenon is viewed from a socio-cultural and economic perspective, uncovering the reasons behind their popularity among Russian city dwellers­. These reasons are related to the geographical, historical, and economic features of the nation as well as to the evolution of rural areas and agriculture across various zones. The article analyzes the diversity of second homes, their types, quantities as well as ­preferences and activities of dacha dwellers (dachniks), their socio-economic composition­ and the challenges they face, contingent on the geographic location of these estates and the demand for them among specific urban groups. It is found that the interaction ­between dachniks who come from urban centers, local communities, and rural economies, as well as the distinctive facets of dacha life, vary markedly depending on the natural and socio-economic conditions, which are largely shaped by the remoteness of a place from cities. Special emphasis is put on the distribution and distinctiveness of dachas in the Non-Black Earth zone regions of central Russia, where their prevalence and significance are especially pronounced.

This review presents well-known surgical techniques aimed at the treatment of unilateral limbal insufficiency syndrome. Among the many available techniques, one should especially highlight the so-called “simple limbal epithelial transplantation”, which has proven to be an effective, safe and generally available method of treating unilateral limbal insufficiency syndrome (SLN). It should be noted that the risks of recurrence of SLN remain significant, and the mechanisms of engraftment of transplanted limbal epithelial stem cells on the recipient’s eye are not fully understood. The review also considers options for new promising approaches to the possible solution of existing problems in the transplantation of limbal epithelial stem cells.

The study data of the last two decades on primary and secondary immunodeficiency in congenital heart defects (CHD) as a cause of frequent infectious complications before and after cardiac surgery are presented. Based on screenings of various levels, data are provided on the greater severity of immunological disorders in critical and cyanotic CHD in conotruncal defects compared with those in septal defects and stenotic defects. Violations were more often related to T-cell function and immunoglobulin deficiency (especially the IgG and IgG4 subgroups). Various types of primary immunodeficiency were found in 13 genetic syndromes in combination with CHD. The review discusses the possibility of using the technique of quantitative determination of DNA TREC and KREC — by-products of maturation of T- and B-cell receptors, which allows us to judge the defects of the T- and B-cell links of the immune system to predict infectious complications in children with CHD. The data of our own study of 200 infants with CHD (in 5% of cases with syndromic forms of CHD) are presented, where a decrease in TREC was found in 23.5% of cases, including all infants with syndromic forms, more often with cyanotic and conotruncal CHD and in children admitted in critical conditions. In children with reduced TREC values, infectious complications in the postoperative period were observed significantly more often than in children with normal indicators (36 and 3.6%, respectively). The analysis of publications confirmed the importance of TREC and KREC screening for targeted preoperative preparation in order to reduce postoperative complications and reduce the risk of mortality in CHD.

The article considers the results of the study of self-esteem of adolescents prone to victim behavior. The relevance of the study is due to the increasing number of cases of victim behavior in adolescence and the emergence of its new forms, which determines the need for timely organization of preventive work with such students by an educational psychologist. The article highlights the provisions of domestic and foreign studies of self-esteem of adolescents prone to victim behavior, presents the results of empirical research conducted by the authors of the article. Its purpose was to establish the peculiarities of self-esteem of adolescents prone to victim behavior. The study used the method of testing and methods of mathematical statistics. The test subjects were 30 college students (males) aged 15–17 years old. The authors established the peculiarities of self-esteem of adolescents prone to victimization: insufficient differentiation, inconsistency, lower assessment of physical appearance and appearance and higher assessment of skills for labor paid activity.

Triazolo[4,3-a]pyrimidine is one of the promising structural fragments for the development of drugs, including anticancer drugs. This work is devoted to the synthesis of a number of new 2-arylhydrazone derivatives of thiazolo[3,2-a]pyrimidine, which are synthetic precursors for triazolo[4,3-a]pyrimidines. The crystal structure of 6-acetyl-7-methyl-5-phenyl-2-(2-phenylhydrazineylidene)-5H-thiazolo[3,2-a]pyrimidin-3(2H)-one was established by SCXRD. In the reduction reaction of the compound, the following system was used: vanadium(V) oxide, and sodium borohydride in ethanol at room temperature, which led to the formation of only one pair of diastereomers (1R*)-1-((5S*,6R*,7R*)-(1-(hydroxymethyl)-7-methyl-1,5-diphenyl-1,5,6,7-tetrahydro[1,2,4]triazolo[4,3-a]pyrimidin-6-yl)ethan-1-ol.

The article deals with modelling the translation process performed in the frameworks of a translation project in a team that consists of translators and experts from related fields. This type of collaborative activity is denoted by the term “cooperative translation” studied from the perspective of the integrative approach and synergetics. The cooperative translation of a photo album, which describes museum exhibits was chosen as the material for the study. The analysis of the operational processes of the project is presented by means of a matrix model in two variants: an organizational matrix and a responsibility matrix. Both models imply the intersection of horizontal and vertical alignment of management and communication levels with a description of the roles of participants, their position in the project matrix structure, characteristics of functional responsibilities and the degree of involvement in the project. The authors proposed a hypothesis about the effectiveness of the matrix format for visualizing operational processes in cooperative translation activities and adapting the communication flows between the participants, which is validated by empirical data. The findings confirm the practicality of the modelling and mapping methodology to describing cooperative activities within a translation project. The article indicates further directions that work should take including a deeper analysis of cooperative translation from a cognitive perspective with reference to embodied cognition of a related research area.
