Trabalho Final do Mestrado Integrado em Medicina apresentado à Faculdade de Medicina Introdução: Os inibidores de checkpoint imunológico demonstraram ser uma valiosa arma terapêutica no tratamento contra diversas neoplasias como o melanoma, cancro do pulmão de não pequenas e o carcinoma de células renais. No entanto, a sua utilização não está isenta de efeitos adversos diversos.Objetivo: Pretendemos avaliar os efeitos adversos endócrinos associados às diferentes classes de imunomoduladores.Metodologia: Esta revisão foi elaborada de acordo com os critérios PRISMA 2020. As bases de dados utilizadas para a pesquisa foram a PubMed e Scopus, entre Maio e Dezembro de 2021. Foram selecionadas revisões sistemáticas, escritas em inglês ou português, que avaliassem os efeitos adversos endócrinos da utilização da imunoterapia no tratamento de diversas neoplasias. A escala AMSTAR 2 foi utilizada para avaliar a qualidade metodológica.Resultados: Foram identificadas 127 revisões, tendo sido eliminadas 101 por não cumprirem os critérios de elegibilidade. Uma revisão foi eliminada por ter qualidade baixa após aplicados os critérios da escala AMSTAR 2. 25 meta-análises foram incluídas nesta revisão. A incidência global de hipotiroidismo variou entre 6.6-13.8%, do hipertiroidismo 2.9-4.54%, hipofisite 0.6-1.31%, insuficiência suprarrenal primária 0.6-0.7% e diabetes mellitus tipo 1 0.2%.Limitações: Apenas 5 revisões apresentaram incidências de efeitos adversos do Tremelimumab e 7 dos inibidores do PD-L1 pelo que não foi possível aferir os efeitos adversos a si associados e estabelecer comparações com outros imunomoduladores. A insuficiência suprarrenal demonstrou ser um efeito adverso raro, pelo que não permitiu concluir qual o imunomodulador mais associado a este efeito adverso e o uso concomitante de corticosteroides no tratamento de outros efeitos adversos poderá condicionar o seu diagnóstico. Outra limitação desta revisão foi a apresentação dos resultados de várias revisões em Odd Ratio e Risco Relativo, o que conduziu a uma diminuição do número dos resultados apresentados. Existem poucas revisões sistemáticas que avaliem diretamente os efeitos adversos endócrinos associados à imunoterapia pelo que é recomendado realizar no futuro uma nova revisão umbrella com meta-análise.Discussão: A incidência de hipo e hipertiroidismo encontram-se mais associada à utilização de inibidores do PD-1, a hipofisite à utilização de inibidores do CTLA-4 enquanto a diabetes mellitus tipo 1 se encontra mais associada aos inibidores PD-1 e PD-L1. A insuficiência suprarrenal primária é um efeito adverso raro pelo que não permite concluir a qual classe de imunomodulador se encontra mais associada. Quando utilizados em terapêutica combinada, as incidências de todos os EA endócrinos aumentam.Conclusão: Nesta revisão sistemática demonstrou-se que existe um maior risco de desenvolver um efeito adverso endócrino aquando da utilização dos inibidores imunológicos de checkpoint, sendo que a incidência de cada efeito adverso varia consoante as diferentes classes de imunomoduladores. As incidências de todos os efeitos adversos aumentam quando estes fármacos são utilizados em terapêutica combinada. Introduction: Immune checkpoint inhibitors have proven to be a valuable therapeutic weapon in the treatment against several neoplasms such as melanoma, non-small cell lung cancer and renal cell carcinoma. However, their use is not absent of adverse effects.Objective: We intend to evaluate the endocrine adverse effects associated with different classes of immunomodulators.Methodology: This review was conducted according to the PRISMA 2020 guidelines. The search occurred between May and December 2021 and the databases used were PubMed and Scopus. Systematic reviews, written in English or Portuguese, that evaluated the endocrine adverse effects of using immunotherapy in the treatment of various neoplasms were selected. The AMSTAR 2 scale was used to assess methodological quality.Results: 127 reviews were identified and 101 were eliminated for not meeting eligibility criteria. One review was eliminated for poor quality after applying the AMSTAR 2 scale criteria. 25 meta-analyses were included in this review. The overall incidence of hypothyroidism ranged from 6.6-13.8%, of hyperthyroidism from 2.9-4.54%, hypophysitis from 0.6-1.31%, primary adrenal insufficiency from 0.6-0.7% and type 1 diabetes mellitus incidence was 0.2%.Limitations: Only 5 reviews presented the incidence of adverse effects for Tremelimumab and 7 for PD-L1 inhibitors so it was not possible to draw a good conclusion about adverse effects on these drugs and compare them with other immunomodulators. Adrenal insufficiency proved to be a rare adverse effect, so it was not possible to conclude which immunomodulator was more associated with this adverse effect and the simultaneous use of corticosteroids in the treatment of other adverse effects may condition its diagnosis. Another limitation of this review was the inclusion of several reviews which results were reported in Odds Ratio and Relative Risk, this led to a decrease in the number of results presented. There are only a few systematic reviews concerning the adverse effects of immunotherapy directly on the endocrine glandes, therefore it is recommended further umbrella revisions with meta-analysis.Discussion: The incidence of hypo and hyperthyroidism is more related to the use of PD-1 inhibitors, hypophysitis with CTLA-4 inhibitors and type 1 diabetes mellitus with PD-1 and PD-L1 inhibitors. Primary adrenal insufficiency is a rare adverse effect, so it is not possible to conclude which class of immunomodulator it is most associated with. When used in combination therapy, the incidences of all endocrine adverse effects increase.Conclusions: This systematic review demonstrated that there is an increased risk of developing an endocrine adverse effect when using immune checkpoint inhibitors, with the incidence of each adverse effect varying according to the different class of immunomodulator used. The incidences of all adverse effects increase when these drugs are used in combination therapy.

The development of rectovaginal fistulae in patients with advanced pelvic malignancies can lead to significant morbidity and suffering. It represents a heavy burden on patients, dealing with local symptoms, psychological distress, and self-image issues, contributing to poor quality of life. It also represents a challenge for clinicians due to the need for multiple professional interventions, need for continuity of care and regular clinical reevaluation. Palliative care professionals play a central role in these cases. Nevertheless, literature in this specific context is lacking. We present a narrative review on this topic, focusing on conservative treatment with both pharmacological measures to control fistula effluent and local care to prevent complications; psychological and spiritual support addressing self-image, self-esteem, and sexuality issues.
