Patient and Public Involvement (PPI) in research should focus on the engagement of patients/carers in every stage of the research cycle. However, the inconsistent level of education and research experience may compromise this involvement and training and support are recommended. In Portugal, consistent training to empower patients in their health decisions, or to be involved in research is neither available nor adapted for the patients and public. Our main goal is to develop and deliver the first high-quality and comprehensive online educational programme for adult patients with asthma and their carers, based on patients’/carers’ needs and preferences. The project includes four tasks. In Task 1, we will identify and organise the most important topics on asthma and health research, based on the identification of educational needs of adult patients/carers with asthma and the expertise of asthma specialists, using Delphi methodology. In Task 2, we will design and implement a high-quality, free, online self-learning programme based on reliable sources of information and on patients’/carers’ preferences. In Task 3, we will assess its feasibility, satisfaction and impact. Finally, Task 4 will be centred on the management and dissemination. Patients and carers will be actively engaged in all stages of this project as co-researchers, including the writing of this protocol. By the end of this project, we expect to have implemented the first high-quality, free-of-charge, online self-learning programme on asthma and health research, adapted for adult patients with asthma and their carers. It will be available for all patients/carers not only from Portugal but also from all Lusophone countries. This programme will empower patients and carers to increase their knowledge about the disease, develop new skills and have the opportunity to learn about research. Ultimately, it will introduce the knowledge patients/carers need to represent themselves and influence health policies in Portugal successfully.

No âmbito da realização do estágio, do mestrado de Gestão Ambiental, foi desenhado e implementado um Sistema de Gestão Integrado de Qualidade e Ambiente, com o objetivo de alcançar a sua certificação segundo as normas ISO 9001:2015 e ISO 14001:2015.Este trabalho incide sobre um caso prático, em que, uma empresa com 17 anos, depois de um percurso penoso com uma quebra significativa do volume de negócios e perda de grande parte da sua estrutura organizacional, vê na certificação a alavanca necessária para voltar a entrar nos mercados que lhe foram vedados. Garantindo desta forma a recuperação do volume de negócios, consolidando a sua posição nestes mercados e talvez no futuro expandir-se para novas áreas geográficas. O sistema foi implementado tendo por base os princípios de gestão de qualidade e ambiente, adotando a abordagem por processos, que incorpora o ciclo PDCA (PLAN - DO - CHECK – ACT) de melhoria contínua, integrando o pensamento baseado no risco e o ciclo de vida dos produtos e serviços. No seguimento da auditoria externa realizada, a entidade certificadora constatou que o sistema de gestão está, globalmente, concebido, implementado e mantido de acordo com os requisitos das normas ISO 9001:2015 e ISO 14001:2015; e demonstra aptidão para, de uma forma consistente, cumprir os requisitos aplicáveis, atingir os objetivos e realizar a política da Organização. Tendo a Equipa Auditora recomendado à Direção da entidade certificadora a Decisão de Certificação. As part of the internship, of the Environmental Management Master, an Integrated Quality and Environment Management System has been designed and implemented, with the goal of achieving its certification, according to ISO 9001: 2015 and ISO 14001: 2015.This work focuses on a practical case, in which a 17-year-old company, after a painful journey with a significant drop in turnover and loss of a large part of its organizational structure, sees certification as the necessary lever to re-enter in the inaccessible markets. It would also ensure the turnover recovery, consolidating its position in these markets and perhaps in the future expanding into new geographic areas.The system has been implemented based on the principles of quality and environment management, adopting the process approach, which incorporates the PDCA (PLAN - DO - CHECK – ACT) cycle of continuous improvement, integrating risk-based thinking and the life cycle of products and services.Following the external audit carried out, the certifying entity found that the management system is, globally, designed, implemented and maintained in accordance with the requirements of ISO 9001: 2015 and ISO 14001: 2015; and demonstrates its ability to consistently comply with applicable requirements, achieve objectives and carry out the Organization's policy. The Audit Team recommended the Certification Decision to the Board of the certifying entity.

OBJECTIVE: To explore how cancer could be diagnosed in a more timely way.DESIGN: Grounded theory analysis of primary care physicians' free text survey responses to: 'How do you think the speed of diagnosis of cancer in primary care could be improved?'. Secondary analysis of primary care physician interviews, survey responses, literature.SETTING: Primary care in 20 European Örenäs Research Group countries.SUBJECTS: Primary care physicians: 1352 survey respondents (2013-2016), 20 Spanish and 7 Swedish interviewees (2015-2019).MAIN OUTCOME MEASURES: Conceptual explanation of how to improve timeliness of cancer diagnosis.RESULTS: Pluralistic task shifting is a grounded theory of a composite strategy. It includes task sharing - among nurses, physicians, nurse assistants, secretaries, and patients - and changing tasks with cancer screening when appropriate or cancer fast-tracks to accelerate cancer case finding. A pluralistic dialogue culture of comprehensive collaboration and task redistribution is required for effective pluralistic task shifting. Pluralistic task shifting relies on cognitive task shifting, which includes learning more about slow analytic reasoning and fast automatic thinking initiated by pattern recognition; and digital task shifting, which by use of eHealth and telemedicine bridges time and place and improves power symmetry between patients, caregivers, and clinicians. Financial task shifting that involves cost tracking followed by reallocation of funds is necessary for the restructuring and retraining required for successful pluralistic task shifting. A timely diagnosis reduces expensive investigations and waiting times. Also, late-stage cancers are costlier to treat than early-stage cancers. Timing is central to cancer diagnosis: not too early to avoid overdiagnosis, and never too late.CONCLUSIONS: We present pluralistic task shifting as a conceptual summary of strategies needed to optimise the timeliness of cancer diagnosis.Key pointsCancer diagnosis is under-researched in primary care, especially theoretically. Thus, inspired by classic grounded theory, we analysed and conceptualised the field:Pluralistic task shifting is a conceptual explanation of how the timeliness of cancer diagnosis could be improved, with data derived mostly from primary care physicians.This includes task sharing and changing tasks including screening and cancer fast-tracks to accelerate cancer case finding, and requires cognitive task shifting emphasising learning, and digital task shifting involving the use of eHealth and telemedicine.Financial task shifting with cost tracking and reallocation of funds is eventually necessary for successful pluralistic task shifting to happen.

A Tomografia Computorizada (CT) é actualmente uma ferramenta de diagnóstico amplamente utilizada em todo o mundo. Desde a sua primeira utilização no início dos anos 70 que a CT tem vindo a evoluir consideravelmente, registando-se actualmente cerca de 62 milhões de exames anuais apenas nos EUA. Apesar das suas múltiplas vantagens, especialmente em diagnóstico não-invasivo em casos de traumatismo, os exames de Tomografia Computorizada expõem os paciente a uma maior dose de radiação ionizante, quando comparados com Radiologia Convencional. Este facto, e os problemas de saúde que advêm de uma exposição excessiva, têm vindo a ser discutidos por várias comunidades de profissionais de radiologia mundialmente. As incertezas quanto aos efeitos biológicos de radiação de baixa dose, a relação risco radiológico-dose e as recentes descobertas sobre a dependência da radiosensibilidade com a idade, especialmente em crianças, tornam os exames de CT, especialmente pediátricos, alvo de preocupação e discussão. Na última década, várias campanhas de sensibilização sobre o assunto foram publicadas, prevenindo os profissionais de saúde sobre os riscos inerentes a uma exposição excessiva em crianças, particularmente radiosensíveis; estas campanhas promovem a redução de dose em exames de CT, advertindo que, ainda assim, é possível manter uma boa qualidade de imagem para diagnóstico. Actualmente, associações de profissionais de radiologia como o “American College of Radiology”, a “International Comission on Radiological Protection (ICRP)”, a campanha “Image Gently” e várias outras instituições tentam através de publicações aumentar a sensibilização da comunidade científica e não-científica para os efeitos prejudiciais da exposição excessiva em crianças de tenra idade. O principal objectivo é sensibilizar a comunidade médica para a redução de doses em exames de imagiologia médica, mantendo a qualidade de imagem necessária. Neste estudo, dados de exames pediátricos de CT de dois hospitais Portugueses foram recolhidos e analisados. Parâmetros como kV, mA, pitch, etc. foram comparados com os valores recomendados em estudos europeus semelhantes. Foi provado que, para algumas idades, e para certos tipos de exames, o CTDI (CT Dose Index) e o DLP (Dose Length Product) são mais elevados do que o recomnedado, levando a um aumento da exposição. Neste estudo procedeu-se também a medições em quatro equipamentos dos dois hospitais visitados; com um fantoma de PMMA e uma Câmara de Ionização, efectuou-se a medição de parâmetros dosimétricos de CT, como o CTDIvol. Conclui-se 9 que em alguns protocolos pediátricos definidos nos hospitais se utilizam parâmetros que tornam excessivos os valores de exposição, devendo se revistos e optimizados.
