Abstract

BackgroundAdolescent idiopathic scoliosis (AIS) is a condition resulting in spinal deformity and tissue adaptation of the paraspinal muscles. Although prior studies have demonstrated asymmetries in fiber type and other energetic features of muscle on the concave side of the curve, muscle morphology, architecture, and composition have not been evaluated. Therefore, the purpose of this study was to compare differences in paraspinal muscle microarchitecture and composition between concave and convex sides of a scoliotic curve in individuals with AIS.MethodsParaspinal muscle biopsies were obtained at the apex of the scoliotic curve in 29 individuals with AIS undergoing surgical deformity correction. Histological assays were performed to quantify fiber size, evidence of muscle degeneration and regeneration, and tissue composition (proportion of muscle, collagen, and fat). Differences between contralateral muscle samples were compared using two-tailed paired Student's t tests, and relationships between clinical characteristics (age and curve severity) and muscle characteristics were investigated using Pearson correlations.ResultsMuscle fibers were significantly larger on the convex side of the curve apex (P = .001), but were lower than literature-based norms for healthy paraspinal muscle. There were no differences in amount of degeneration/regeneration (P = .490) or the proportion of muscle and collagen (P > .350) between the concave and convex sides, but high levels of collagen were observed. There was a trend toward higher fat content on the concave side (P = .074). Larger fiber size asymmetries were associated with greater age (r = .43, P = .046), and trended toward an association with greater curve severity (r = .40, P = .069).ConclusionsThis study demonstrates that although muscle fibers are larger on the convex side of the scoliotic curve in AIS, muscles on both sides are atrophic compared to non-scoliotic individuals, and demonstrate levels of collagen similar to severe degenerative spinal pathologies. These findings provide insight into biological maladaptations occurring in paraspinal muscle in the presence of AIS.

35. Defining the three-dimensional deformity in slipped capital femoral epiphysis

Abstract

The purpose of this study was to define a novel angle measure (theta) characterizing true slipped capital femoral epiphysis (SCFE) deformity; use theta to differentiate between SCFE hips, contralateral unaffected hips, and normal hips; and to compare theta to the Southwick slip angle (SSA). 3DCT reconstructions of the pelvis and femurs were obtained and pelvic position was standardized. The center point and direction vector of the femoral epiphysis was determined. The femoral neck axis was defined. The angle between the femoral neck axis and epiphysis vector defined the 3D angle of deformity (theta). The 3D translation of the femoral epiphysis, measured as a percentage of femoral neck diameter, was measured in three planes. 42. Rigid segmental cervical spine instrumentation is safe and efficacious in younger children

Abstract

The utilization of cervical spine instrumentation in the young pediatric patient in not well reported. This study presents outcomes and complications of cervical spine instrumentation in patients who underwent cervical spine fusion surgery before age 10. Radiographic and clinical data were collected on all patients who underwent cervical spine surgery with instrumentation at a single institution between January 1, 2006 and March 21, 2015. Patients were ≤ years of age at the time of surgery with any cervical spine deformity/injury diagnosis. Patient demographics, details on cervical spine diagnosis, procedural data, imaging data, and post-operative follow up data were collected. Twenty children met the criteria and were included in the study with a mean follow-up of 10.6 months (3 mo to 2 years). Initial indication for cervical spine correction surgery included: deformity (7 cases), trauma (6 cases), instability (3 cases), stenosis (2 cases), rotary subluxation (1 case), and infection (1 case). Fifteen cases were treated with adult 3.5mm cervical spine instrumentation, 3 with wiring (1 sublaminar and 2 spinous process), and 2 with cannulated screws. Postop immobilization included 16 Halo fixation, 3 collars, and 1 CTO. Overall there were 5 complications related to the surgery. two patients who had wiring (1 sublaminar and 1 spinous process) developed a non-union and required revision surgery (1 with cannulated screws and 1 with 3.5mm segmental cervical spine instrumentation). One patient developed a postop infection that required incision and drainage. Five patients developed superficial pin infections for their Halo. Tow deformity patients experienced neurological complications that were likely unrelated to the cervical instrumentation. Conclusions: rigid segmental fixation can be safe and efficacious when used in pediatric cervical spine patients. Whether used with Halo or orthosis, patients experience minimal to no complications from the instrumentation and achieve successful fusion. Cervical spine wiring had a high risk of non-union requiring revision surgery. The incidence of wound infection was low with one in 20 cases.
