Diagnosis and endovascular treatment for ruptured wide neck aneurysms are challenges in clinics, particularly in developing countries. In the present study, we described a clinical case with a ruptured wide neck aneurysm in Vietnam treated by flow diverter stent and coil embolization. A 77-year-old- female patient had a right droopy eyelid for 2 months. The patient was admitted to hospital on the second day after being presented with a sudden-severe headache. Cerebral computed tomography (CT) and magnetic resonance imaging (MRI) were taken on the second day and fifth day after the onset of the headache. The results showed an aneurysm in the right internal carotid artery but no potential subarachnoid hemorrhage (SAH) was displayed. An uncoagulated blood was found in cerebrospinal fluid indicated by a lumbar puncture test. Digital subtraction angiography provided images with one wide-neck right internal carotid aneurysm. The patient was treated by flow diverter stent and coil embolization and the dual antiplatelet therapy with ticagrelor and aspirin at home. After 45 days, the patient did not face with any complication, no neurological symptoms, and the aneurysm was partially thrombosed indicated by MRI images. These results suggested that a lumbar puncture should be analyzed on the patient with brain aneurysm appeared a sudden severe headache and even no potential SAH on brain MRI or CT was found. The combination of flow diverter stent and coil embolization to treat cases with ruptured wide necked aneurysms should be considered in the future.

Lаrge аrteriovenous mаlformаtions (АVMs) аre chаllenges in mаnаgement becаuse of outcomes аnd аdverse аffects. Volume-stаged rаdiosurgery hаs been аn аppropriаte аpproаch when removаl resection аnd embolizаtion аre not recommended. А 53-yeаr-old gentlemаn wаs diаgnosed with а lаrge intrаcrаniаl АVM with persistent heаdаche аnd short-term seizure. Brаin mаgnetic resonаnce аnd аngiogrаph showed а bulky volume of АVM nidus. Removаl resection аnd embolizаtion were not recommended becаuse of high risk of аdverse аffects. The pаtient wаs treаted by volume-stаged rаdiosurgery. One yeаr post-treаtment, obliterаtion for right internаl cаrotid аrtery wаs completed. Volume-stаged rаdiosurgery is а potentiаl treаtment option for lаrge АVM with controlled аnd obliterаtion efficаcy, especiаlly to АVMs which аre not аppropriаte for removаl surgery аnd embolizаtion.

Аbstrаct Bаckgrоund Laparoscopic cyst excision and Roux-en-Y hepaticojejunostomy for treating congenital choledochal cysts has been proven to be efficacious in children, but its safety and efficacy in adult patients remain uncertain. This study aims to investigate the safety and effectiveness of laparoscopic choledochal cyst excision and Roux-en-Y hepaticojejunostomy in adults compared to those in children patients. Results There was no conversion to open surgery in both groups. The mean operative time and hospital duration stay in adults (253.4 min and 11.7 days, respectively) were longer in than those in children (214.7 min and 9.3 days, respectively). Intrаоperаtive blооd trаnsfusiоn wаs required in one adult and twо children. There was not reoperation due to bile leakage in adults compared to those in 2 children. The time frоm surgery tо drаinаge remоvаl wаs longer in adults (3.6 vs. 2.9 days). The outcome within three months of discharge was classified as good in 88.2 % of adults and 90.5% for children. There were nо stаtisticаlly significаnt differences in eаrly pоstоperаtive cоmplicаtiоns оr treаtment оutcоmes between the twо grоups. Cоnclusiоns Lаpаrоscоpic chоledоchаl cyst excisiоn fоllоwed by Rоux-en-Y hepаticоjejunоstоmy wаs sаfe аnd efficient in bоth аdults аnd children. Оperаtive time wаs lоnger in аdults thаn in children.

Pancreatic cysts in infants are rare. To the best of our knowledge, there are only 38 cases of pancreatic cysts detected among children under 2 years of age. To date, no pediatric case has been reported in Vietnam. Due to its limited incidence, the correct diagnosis and optimal management are still not clearly defined. Here, we present the successful excision of a true congenital pancreatic cyst in a newborn at our center. Literature on this topic was also reviewed.

Introduction: Gastrointestinal stromal tumors (GISTs) are the most common mesenchymal neoplasms of the gastrointestinal system. Histopathological examination takes an important part in confirming the subtypes of GISTs, to choose appropriate therapeutics for patients. This study aims to explore the histopathological characteristics and evaluate the relationship between malignant risk classification (according to Armed Forces Institute of Pathology criteria) and the histopathological features of GISTs in a cohort of Vietnamese patients. Methods: We reviewed 89 patients with primary GIST who underwent surgery between 2014 and 2019 at Hue Central Hospital, Vietnam. We investigated histopathological characteristics and immunohistochemical findings of all patients. Results: The average age was 55.9 ± 11.9 years. A tumor size of 2-5 cm accounted for 64.1%. The most common position was at the stomach which accounted for 48.5%. Among the subtypes of GIST, spindle cells were seen in 85.9% of patients; epithelial form 10.9%; multi-morphology (3.2%). 97.4% of the samples were positive for CD117, 61.5% of cases were positive for CD34; and no case was positive for Desmin. The rate of high-risk GIST was dominant (46.9%) as compared to the intermediate-risk (28.1%), low-risk (0.3%-2%), and very low-risk groups (4.7%). Conclusions: This study demonstrates the histopathological characteristics of GIST and emphasizes the significant rate of high-risk GIST.

Critical pulmonary valvar stenosis causes cyanosis and can lead to potentially lethal in newborn infants. Following initiation of Prostaglandin infusion, percutaneous balloon pulmonary valvuloplasty (BPV) is considered to be the treatment of choice. The percutaneous femoral venous approach is the most preferred route for BPV in most cardiac centers. We present the case of a 5-day-old female infant with critical pulmonary valvar stenosis who successfully underwent BPV through the femoral venous access. The narrowed pulmonary valve was significantly improved after intervention. This was the first case at our center that was undertaken BPV at the earliest postnatal day.

Background: Even though no age is free to be affected by appendicitis, it is extremely rare in newborn infants. Clinical presentations are usually nonspecific, abdominal imaging is not always diagnostic, and definitive diagnosis is difficult. We present a 15-day-old infant with acute perforated appendicitis that was diagnosed and treated in our pediatric center. Case report: This case presented at Neonatal Intensive Care Unit with an 18-h-history of irritability, vomiting, and abdominal distension. With high white blood cell count and C-reactive protein level, the baby was initially diagnosed with neonatal sepsis. The ultrasound performed on day 3 after hospitalization found peritonitis but not seen appendicitis. Postoperative diagnosis confirmed acute perforated appendicitis with peritonitis. The baby was well covered without complication. Conclusion: This case highlights that preoperative diagnosis of acute appendicitis in neonates is difficult due to nonspecific clinical presentation and the extreme rarity of the disease.

Background: Nurses' competencies are crucial in providing effective dementia care in healthcare settings for older people. Understanding nurses' current knowledge, attitudes and confidence in this area is essential for developing education programmes for healthcare professionals to improve patient care. The purpose of this study was to assess the knowledge, attitudes and confidence related to providing dementia care among nurses practicing in geriatric hospital wards and nursing homes in Hanoi, Vietnam. Methods: A total of 269 out of 313 (response rate was 86%) full‐time nurses working at six geriatric wards in hospitals and nursing homes in Hanoi were surveyed using three self‐administered questionnaires: the Dementia Knowledge Assessment Scale (DKAS), Dementia Attitude Scale (DAS) and the Confidence in Dementia Scale (CODE). Multiple regression models were constructed to identify factors associated with dementia care knowledge, attitudes and confidence. Results: The overall mean scores of nurse's knowledge, attitudes and confidence were 28.1 ± 8.0, 102.1 ± 13.4 and 28.3 ± 6.4, respectively. A positive correlation was reported between the knowledge and attitude scores and between the attitudes and confidence scores. Greater seniority (β: 0.29; 95% CI: 0.03–0.56) and having learned information through colleagues or experts (β: 3.02; 95% CI: 0.88–5.16) were associated with better dementia knowledge. A higher level of dementia training desirability was associated with increased knowledge (β: 0.74; 95% CI: 0.28–1.20) and favourable attitudes (β: 0.94; 95% CI: 0.15–1.74), whereas frequent exposure to dementia cases was associated with higher confidence (β: 3.56; 95% CI: 1.39–5.73) and more favourable attitudes (β: 3.96; 95% CI: 0.27–7.66). Conclusion: Our study highlights deficits in knowledge, low levels of social comfort in nurses' attitudes towards people with dementia and a lack of confidence in providing effective care among nurses practicing in healthcare settings for older adults in Hanoi, Vietnam. With the ageing of the population and with increasing numbers of persons living with dementia, our findings suggest the importance of improving the training of nurses to specifically address these deficits. Implications for Practice: Multidisciplinary consultation meetings need to be encouraged in the healthcare workplace setting as well as ensuring the presence of qualified counsellors for care teams working with older adults in non‐hospital settings. Training about non‐cognitive symptoms of dementia and demonstrating effective verbal and non‐verbal communication skills is critical and should be integrated into nurse's educational training. We developed a research model to explore the direct and indirect influences of top management support of an organization's environmental performance in the Vietnamese electric power sector. Results from a survey of 203 senior managers were used to test the hypotheses. The analysis showed that top management support directly influenced both green training and environmental performance. The results also provided evidence for the mediating effect of green training on the link between top management support and environmental performance. The implications of the results for theory and practice are discussed. We concluded with limitations and suggestions for future research. [ABSTRACT FROM AUTHOR]

Objectives: This study aimed to investigate the relationship between sarcopenia and frailty and examine factors associated with frailty among older patients with and without sarcopenia. Methods: This cross‐sectional study was conducted on older inpatients and outpatients in Vietnam. Participants aged 60 years or older were consecutively enrolled in the study. Sarcopenia was defined using the Asian Working Group for Sarcopenia (AWGS) 2019 criteria. Fried's frailty phenotype was applied to define frailty. Logistic regression models with frailty as the dependent variable were applied. Results: A total of 835 patients (mean age: 71.3 years, SD 8.4) were recruited. The overall prevalence of frailty was 17%. Among participants with and without frailty, 92% and 47% had sarcopenia, respectively. In unadjusted analysis, sarcopenia was significantly associated with increased frailty (OR 12.3, 95% CI 6.7–22.6) and remained significant after adjustment for sociodemographic factors (OR 6.3, 95% CI: 3.0–12.6) and for both sociodemographic and clinical factors (OR 5.4, 95% CI: 2.4–12.2). Among participants with sarcopenia, older age, inpatient status, having a high risk for falls, malnutrition and a history of hospitalisation in the last year were significantly associated with frailty. Among participants without sarcopenia, the factors associated with frailty were older age, inpatient status, low educational level, high risk of falls and malnutrition. Conclusions: Our study results highlighted that sarcopenia and frailty are two related but distinct geriatric syndromes. [ABSTRACT FROM AUTHOR]
