The frequencies and diversities of human leukocyte antigen (HLA) alleles and haplotypes are representative of ethnicities. Matching HLA alleles is essential for many clinical applications, including blood transfusion, stem cell transplantation, and tissue/organ transplantation. To date, the information about the frequencies and distributions of HLA alleles and haplotypes among the Kinh Vietnamese population is limited because of the small sample size. In this study, more than 3,750 cord blood units from individuals belonging to the Kinh Vietnamese population were genotyped using PCR sequence-specific oligonucleotide (PCR-SSO) for HLA testing. The results of the study demonstrated that the most frequently occurring HLA-A, -B, -C, and -DRB1 alleles were A*11:01 (25%), A*24:02 (12.3%), A*02:01 (11.2); A*03:03 (8.95%), A*02:03 (7.81%), A*29:01 (7.03%); B*15:02 (15.1%), B*46:01 (10.7%), B*58:01 (7.65%), B*38:02 (7.29%); C*08:01 (17.2), C*07:02 (16.2%), C*01:02 (15.2), C*03:02 (8.3%), C*15:05 (6.13); DRB1*12:02 (31.0%), DRB1*09:01 (10.47%), DRB1*15:02 (7.54%); DRB1*07:01 (6.68%), DRB1*10:01 (6.63%), respectively, with the highest allele diversity level observed in locus B (93 alleles). The most frequent haplotypes of two-locus combinations of HLA-A–B, HLA-A–C, HLA-A–DRB1, HLA-B–C, HLA-B–DRB1, and HLA-C–DRB1 haplotypes were A*11:01–B*15:02 (7.63%), A*11:01–C*08:01 (7.98%), A*11:01–DRB1*12:02 (10.56%), B*15:02–C*08:01 (14.0%), B*15:02–DRB1*12:02 (10.47%), and C*08:01–DRB1*12:02 (11.38%), respectively. In addition, the most frequent haplotypes of three- and four-locus sets of HLA-A–B–C, HLA-A–B–DRB1, HLA-A–C–DRB1, HLA-B–C–DRB1, and HLA-A–B–C–DRB1 were A*11:01–B*15:02–C*08:01 (7.57%), A*11:01–B*15:02–DRB1*12:02 (5.39%), A*11:01–C*08:01–DRB1*12:02 (5.54%), B*15:02–C*08:01–DRB1*12:02 (10.21%), and A*11:01–B*15:02–C*08:01–DRB1*12:02 (5.45%), respectively. This study provides critical information on the frequencies and distributions of HLA alleles and haplotypes in the Kinh Vietnamese population, accounting for more than 85% of Vietnamese citizens. It paves the way to establish an umbilical cord blood bank for cord blood transplantation programs in Vietnam.

BackgroundStevens-Johnson syndrome/toxic epidermal necrolysis (SJS/TEN) is a rare and life-threatening disease of the skin and mucosal surfaces. Although gastrointestinal manifestations in adults are potential prognostic factors for disease severity, there are limited data on such cases and their standard management in the pediatric population.Case PresentationWe herein report the case of an 8-year-old girl with a 1-year history of epilepsy, who presented with bilateral conjunctivitis and progressively widespread bullous, and pruritic eruption based on erythematous skin after administration of carbamazepine. A diagnosis of carbamazepine-induced TEN was made, and the drug was immediately discontinued. The result of genetic screening showed that the patient was positive for the HLA-B*15:02 allele. Then, her condition got worse by developing gastrointestinal involvement, including hematemesis and severe watery bloody diarrhea. A combination of the intravenous immunoglobulin and the appropriate dose of systemic steroids have contributed to a favorable outcome in this case. Multidisciplinary care of mucocutaneous involvement, supplemental nutrition, and fluid replacement was also critically warranted. This report aims to contribute to the current literature on TEN-related gastrointestinal manifestations in pediatrics and highlights the need for further investigations in determining the optimal treatment in such cases.ConclusionIn conclusion, we reported the successful treatment of TEN-related gastrointestinal manifestations in a pediatric patient, which should be critically considered in patients with SJS/TEN. Since it may significantly contribute to the poor prognosis of the illness, further investigations in determining standard management in such cases are necessary.

Allopurinol (ALP) is commonly used as a drug for gout treatment. However, ALP is known to cause cutaneous adverse reactions (CARs) in patients. The HLA-B*58:01 allele is considered a biomarker of severe CAR (SCAR) in patients with gout, with symptoms of Stevens Johnson syndrome, and with toxic epidermal necrolysis. However, in patients with gout and mild cutaneous adverse drug reactions (MCARs), the role of HLA-allele polymorphisms has not been thoroughly investigated. In this study, 50 samples from ALP-tolerant patients and ALP-induced MCARs patients were genotyped in order to examine the polymorphisms of their HLA-A and HLA-B alleles. Our results showed that the frequencies of HLA-A*02:01/HLA-A*24:02 and HLA-A*02:01/HLA-A*29:01, the dual haplotypes in HLA-A, in patients with ALP-induced MCARs were relatively high, at 33.3% (7/21), which was HLA-B*58:01-independent, while the frequency of these dual haplotypes in the HLA-A locus in ALP-tolerant patients was only 3.45% (1/29). The HLA-B*58:01 allele was detected in 38% (8/21) of patients with ALP-induced MCARs, and in 3.45% (1/29) of ALP-tolerant patients. Notably, although HLA-B*58:01 may be a cause for the occurrence of MCARs in patients with gout, this correlation was not as strong as that previously reported in patients with SCAR. In conclusion, in addition to the HLA-B*58:01 allele, the presence of the dual haplotypes of HLA-A*02:01/HLA-A*24:02 and/or HLA-A*02:01/HLA-A*29:01 in the HLA-A locus may also play an important role in the appearance of ALP-induced MCARs in the Vietnamese population. The obtained primary data may contribute to the development of suitable treatments for patients with gout not only in Vietnam but also in other Asian countries.

Pyrazinamide (PZA) is a key antibiotic in current anti-tuberculosis regimens. Although the WHO has stressed the urgent need to obtain data on PZA resistance, in high tuberculosis burden countries, little is known about the level of PZA resistance, the genetic basis of such resistance or its link with Mycobacterium tuberculosis families. In this context, this study assessed PZA resistance through the molecular analysis of 260 Vietnamese M. tuberculosis isolates. First-line drug susceptibility testing, pncA gene sequencing, spoligotyping and mycobacterial interspersed repetitive units-variable number of tandem repeats (MIRU-VNTR) typing were performed. Overall, the pncA mutation frequency was 38.1% (99 out of 260 isolates) but was higher than 72% (89 out of 123 isolates) in multidrug and quadruple-drug resistant isolates. Many different pncA mutations (71 types) were detected, of which 55 have been previously described and 50 were linked to PZA resistance. Among the 16 novel mutations, 14 are likely to be linked to PZA resistance because of their mutation types or codon positions. Genotype analysis revealed that PZA resistance can emerge in any M. tuberculosis cluster or family, although the mutation frequency was the highest in Beijing family isolates (47.7%, 62 out of 130 isolates). These data highlight the high rate of PZA resistance-associated mutations in M. tuberculosis clinical isolates in Vietnam and bring into question the use of PZA for current and future treatment regimens of multidrug-resistant tuberculosis without PZA resistance testing.Emerging Microbes & Infections (2017) 6, e86; doi:10.1038/emi.2017.73; published online 11 October 2017

Polyaniline titanium dioxide (PANi-TiO2) composite had improved electrochemical behavior by intercalation of varied carbon nanotubes (CNTs): from 0 wt% to 30 wt%, compared to aniline. Those composites were coated on the titanium substrate to form PANi-TiO2-CNTs/Ti by using a binder agent. Material properties were characterized by IR spectra, X-ray diffraction and thermal analysis. The scanning electron microscope (SEM) images explained that the composite existed in uniform fibers with diameter of about 100 nm if content of CNT was 20–30 wt%. Electrical conductivity measurement, electrochemical impedance spectroscopy (EIS) and cyclic voltammograms showed their excellent electrical performance: electrical conductivity reached approximately 10−1 S/cm, and the resistances of those composite layers were reduced from 18 to 27 times compared to that without CNTs. Warburg diffusion was found by EIS analysis with coefficient data ranging from 10−15 to 10−16 cm2/s only for composites containing CNTs on which also no adsorption resistance occurred. The potentiodynamic and galvanodynamic polarizations explained that the composite contained 20% CNTs, exhibiting a better cyclic voltammetry (CV) diagram because of higher responding current density and a better performance because of yielding a higher current density (300 μA/cm2) at 250 mV versus silver/silver chloride reference electrode in comparison with the other obtained composites in this study, respectively.

BACKGROUND: Ferrate(VI), an environmentally friendly oxidant, has recently emerged as one of the most promising chemicals for water and wastewater treatment due to its versatile application as an oxidant, coagulant, and disinfectant. In this paper, the factors affecting the electrosynthesis process of ferrate using ductile iron anode were studied. Besides, the obtained ferrate was used to remove reactive red 195 (RR195). RESULTS: The optimal ferrate electrochemical synthesis conditions were determined, including 14 mol L−1 NaOH electrolyte solution, a current density of 40 mA cm−2, and electrolysis time of approximately 5–6 h. Furthermore, the kinetics of the ferrate's self‐decomposition reaction was also studied at different temperatures. The decomposition process of ferrate in a 14 mol L−1 NaOH environment followed second‐order kinetics, with reaction rate constants of 4.70 × 10−9, 7.31 × 10−8, 1.95 × 10−7, and 4.70 × 10−7 (L mol−1 s−1) at temperatures of 4, 10, 20, and 30 °C, respectively. RR195 treatment efficiency by ferrate reached 96.3% in a short time of 3 min at pH 3 with a ferrate/RR195 molar ratio of 30/1. CONCLUSION: This study identified the optimal conditions for the electrochemical synthesis of ferrate using a ductile iron anode. Ferrate was stable at a low temperature of 4 °C, so unused ferrate needs to be stored at low temperatures to avoid decomposition of ferrate. This paper presents a study on controlling a rotary inverted pendulum (RIP) system using a hierarchical sliding mode control (HSMC) approach. The objective is to swing up and stabilize the pendulum at a desired position. The proposed HSMC controller addresses the underactuation challenge through a hierarchical structure of sliding surfaces. The particle swarm optimization (PSO) algorithm is used to optimize the controller parameters. Simulations were performed to evaluate the performance of the HSMC controller at different initial pendulum angles, demonstrating its effectiveness in achieving swing-up and stabilization. The integration of the PSO algorithm enhances the controller's adaptability and robustness, emphasizing the benefits of combining optimization algorithms with controller parameter tuning for underactuated systems like the RIP.
