Negli ultimi anni, in particolare a seguito del percorso attuativo della certificabilità, gli enti e le aziende del Servizio Sanitario Nazionale hanno prestato crescente attenzione al disegno, all'implementazione e all'affinamento del proprio Sistema di Controllo Interno. La diffusione di una cultura del controllo sia esterno sia interno ha portato in diversi casi all'istituzione di un servizio di Internal Auditing (di seguito IA) come garante di tale sistema di controlli. Il presente contributo approfondisce le modalità attuative della funzione di IA negli enti e nelle aziende del Servizio Sanitario Nazionale attraverso un'analisi di casi studio delle esperienze regionali che per prime si sono mosse in questa direzione e che risultano più significative, Regione Veneto e Regione Emilia-Romagna.Di ciascuna sono esaminate e poi raffrontate le principali variabili della funzione di IA approfondite dalla dottrina e precisamente: i ruoli, le tipologie e i processi di audit, le variabili organizzative e le caratteristiche delle risorse umane impiegate nella funzione. Ne emergono due modalità attuative tra loro distinte che, seppur condizionate dai rispettivi contesti di riferimento, contengono anche interessanti punti di contatto e significativi spunti di riflessione per le altre Regioni che si stanno avviando nel percorso di implementazione della funzione di IA.

Il decentramento in ambito sanitario è da tempo considerato uno dei principali strumenti per far progredire i sistemi sanitari e per rispondere alle esigenze di equità di accesso alle cure.La dottrina ha identificato in questo trasferimento del potere decisionale "da pochi a molti" il presupposto della valorizzazione del ruolo del management, in questo caso, massimamente responsabilizzato nella risposta agli utenti.Per tale ragione nella definizione di una strategia di decentramento appare centrale il coinvolgimento proprio del management allo scopo di definire preventivamente il suo possibile contributo.Obiettivo del presente lavoro è, pertanto, l'analisi del contributo del management nella definizione di una strategia di decentramento del SSN.L'articolo approfondisce il caso della Repubblica di Tunisia come esempio significativo di Paese a medio-basso reddito.

Hospitals in most of the countries in transition face the challenge to raise efficiency in a rapidly evolving health sector. In this framework, the Albanian Ministry of Health has outsourced the sterilization and operating theatre support(SOTS) services of all the public hospitals. The aim of this article is to evaluate the impact of the outsourcing of SOTS services on the productivity of the surgical services. A case study analysis was conducted in the Country’s largest hospital. Surgical activity data of all the surgical services were analyzed. Furthermore, interviews with key stakeholders were conducted in order to enable a more detailed exploration of the results. Compared with the previous twelve months, a relevant increase in the total number of surgical interventions performed by the hospital was registered in the twelve months after the introduction of the new organizational model (þ14%); this significant increase occurred in almost all the surgical services. Results of the interviews highlighted several organizational-, supply capacity-, and confidencerelated determinants behind the productivity gains. These findings have significant implications for poorly performant—but increasingly competitive and evolving—hospital sectors of countries in transition, in which the outsourcing of SOTS services may represent a key tool for enhancing the productivity.

Il presente studio intende offrire un contributo al dibattito sulla Gestione Sanitaria Accentrata (GSA). Nello specifico, propone uno strumento innovativo, vale a dire la redazione di un sistema di procedure amministrativo-contabili della GSA che, travalicando i confini organizzativi della GSA stessa e abbracciando una logica integrata di sistema del SSR, mappa i flussi di risorse afferenti al fabbisogno sanitario. Attraverso la disamina di due casi studio particolarmente emblematici (quello della GSA in Lombardia e quello di Azienda Zero in Veneto), lo studio mostra che le procedure sistemiche e integrate della GSA (i) sono utilmente applicabili in contesti diversi, in quanto in grado di coglierne e valorizzarne le peculiarità organizzative e contabili; (ii) costituiscono un valido strumento di risposta alle principali problematiche della GSA soprattutto in termini di governo e controllo della filiera Regione-GSAaziende sanitarie.

The lack of resources has driven healthcare management studies to recognise Private and Public Partnerships (PPP) as the solution in managing efficiency, effectiveness and profitability of technological innovation. Recent researchers have identified the buying model called Managed Equipment Service (MES), in which purchaser and seller share the risks of technology’s management and use, as an optimal problem-solving method. The aim of this research is to analyse the potential economic-financial and organizational benefits of MES introduction in healthcare, in particular for the acquisition of the diagnostic imaging technologies. The research applies the case study method. We have examined a representative Italian Local Health Authority for its dimensions, managerial characteristics and case-mix offered but, especially, for its high health standards. In the study we have involved the strategic figure of radiographer in diagnostic imaging. The data has been elaborated through statistical analysis to observe the correlations. Evidence demonstrates that MES costs are lower than all the other planning and purchasing methods analysed (between 11% - 239%). When correlations between age of facilities and effectiveness of provided services is strong, MES grants the maintenance of a lower average age of technologies and a more efficient use of resources and significant margins of profitability. The emerging model of PPP is a possible solution to the current issues surrounding economic and financial sustainability of healthcare systems, considered the fewer resources and the technological progress. MES in healthcare appears to be a highly efficient way of managing innovation, in particular for diagnostic imaging technologies. Diagnosis Related Groups (DRGs) are the most used payment system in health care and the best way of introducing efficiency and effectiveness in its management. For this reason, they are particularly needed for transition countries which have to improve the quality and the quantity of the services delivered to customers. Albania especially is currently living in a period of deep innovation. The consolidation of its democracy, the increasing wealth of the population and the growth of the social needs require a new management approach for public administration and the construction of a more compliant model with the EU standards. This transformation also deeply involves the healthcare sector, which is actually redefining the financing, information system and accountability. For these reasons Albania could be a relevant case study in order to observe, starting from the first steps, the path for introducing a DRG payment system in modern health care. Using the case study method, this paper analyses the background, the governance and the financing of Albanian health care, focusing on the actual reform strategies. The results of the research have been the basis for defining the “National Guidelines of Quality Standards for Introduction of DRGs-based Payment”, adopted by the Albanian Ministry of Health (in 2017 became Ministry of Health and Social Protection), which represents an early roadmap for the introduction of DRGs and its future implementation to the whole of Albanian health care.

MPM is one of the most serious professional cancer with a high frequency in subjects previously exposed to asbestos. Several trials have been made or are ongoing to define an effective therapy to allow a longer survival and to reduce its burdensome management by the health systems. The few MPM Care Pathways (CP), which have been suggested by groups of experts of scientific community, are only a list of practicable interventions without the structure of a pathway so the aims of this paper are the declination of possible cases of a MPM CP and the quantification of the costs using a regional consensus CP, verifying its application. The diagnostic and therapeutic procedures of the regional CP were defined starting to a sample of 34 patients treated in public regional hospitals for MPM, diagnosed in 2015, and having.at least a one year follow up. The costs were attributed using the regional price list for outpatient health activities and the DRG for hospitalized activities. The formed cost accounting model was applied in order to calculate the cost for the public regional administration. Last, the obtained results were discussed with surgical thoracic surgeons of the three regional university hospitals. The developed cost accounting model has identified three cases of a MPM CP. They present a common diagnostic phase (7.246,90€) following by three possible treatments, differently combinable between them: surgery (10.808,00€), chemotherapy (164,00€ + drug cost for access) and observation (41,00€). The cost will be due to the specific combination of the three treatments plus the diagnostic phase, predictable according to patient’s age and staging, which can afford each patient affected by MPM. The contribution of the cost accounting model applied to the consensus regional CP for MPM allows to assess the care’s appropriateness of the cases actually treated. It could be used also for CPs of other diseases and as a method for institutional financing of the route. Negli ultimi anni il necessario contenimento della spesa pubblica ha determinato l’avvio di un percorso di riforma nella Sanita che ha riguardato l’evoluzione del sistema contabile e l’introduzione di nuove metodologie di controllo, finalizzate a garantire nuovi standard per l’informativa contabile delle aziende sanitarie. Tale innovazione e definita "certificabilita" ed e stata introdotta dal Ministero delle Salute attraverso la definizione di un percorso comune a tutte le Regioni. Risulta evidente l’intenzione di avvicinare il modello di controllo contabile degli enti del Servizio Sanitario Nazionale a quello delle imprese private attraverso l’introduzione dell’istituto della revisione contabile. Il lavoro analizza i progetti triennali predisposti dalle Regioni finalizzati alla rimozione degli ostacoli organizzativi, contabili e normativi dei sistemi sanitari regionali per il raggiungimento degli standard di certificabilita proposti dal Ministero della Salute. I piani redatti dalle Regioni si configurano quindi come presupposto all’introduzione dell’istituto della certificazione dei bilanci. Dall’analisi di tali documenti emerge il quadro evolutivo del controllo della spesa sanitaria in Italia e la fotografia dello stato dei suoi sistemi amministrativo-contabili regionali.

Museums have identified Web 2.0 as the ideal tool for the achievement of their objectives of promoting culture and involving users; indeed, it offers the conditions to allow the exchange between the interested parties. In the literature, while the use of websites by traditional museums has been extensively studied, a gap emerges with regard to the description and the use of the websites by particular museums which are hosted in working hospitals. The paper aim is to assess the prospective of a website for further the relationship and communication between the Santa Maria Nuova museum and its digital audience. The data highlighted the achievement of excellent results in terms of audience and involvement through the website, although this reality is still very young (only three years of activity) and there are ample possibilities for its digital development.

Il sistema sanitario albanese si trova attualmente in una fase di profonda trasformazione, cosi come l’intero Paese sta vivendo un momento di riflessione sul proprio futuro dopo il tumultuoso sviluppo di questi ultimi anni. L’ammissione dell’Albania tra i futuri potenziali membri dell’Unione Europea ha peraltro rafforzato la necessita di innovare il sistema sanitario nazionale. Questo lavoro analizza l’attuale modello di finanziamento della sanita albanese mostrando le prospettive di sviluppo futuro. In particolare, e delineato il passaggio da un modello basato sulla spesa storica, a uno incentrato sul finanziamento delle prestazioni sanitarie erogate dalle strutture operanti sul territorio. L’introduzione di logiche manageriali consentira quindi di sviluppare un sistema di tariffazione basato sui costi, di implementare un sistema di rilevazione nelle strutture universitario-ospedaliero e di definire un modello di convenzionamento per la sanita privata. L’articolo offre quindi un contributo ai policy makers chiamati a delineare le future "vie" di sviluppo del sistema sanitario albanese nei prossimi anni.
