Abduction of one arm preferentially activates erector spinae muscles on the other side to stabilise the body. We hypothesise that the corticospinal drive to the arm abductors and the erector spinae may originate from the same hemisphere. In 18 subjects, transcranial magnetic stimulation (TMS) was applied using an angle double-cone coil placed symmetrically over the vertex. Motor evoked potentials (MEP) could not be evoked systematically seated at rest but could be evoked bilaterally in erector spinae muscles during unilateral arm abduction. TMS was applied at 110% and 120% motor threshold (MT) for the contralateral erector spinae muscle when an arm was abducted against resistance. The electromyographic (EMG) activity in the erector spinae at L4 vertebral level during contralateral arm abduction was significantly higher (P < 0.05) than in the ipsilateral erector spinae. The mean (±S.E.M.) latencies of MEPs in the contralateral muscle to TMS at 120%MT (left 16.0 ± 0.8 ms; right 17.0 ± 0.8 ms) were significantly (P < 0.05) longer than in the ipsilateral erector spinae (13.9 ± 1.0 ms; 16.6 ± 0.4 ms). In two of six subjects from the same group, it was possible to elicit MEPs by TMS applied selectively to one hemisphere using a figure-of-eight coil. MEPs ipsilateral to the TMS had longer latencies than contralateral MEPs. The study revealed an unexpectedly longer rather than shorter latency of the MEP recorded from the lumbar erector spinae muscles when co-activated during abduction of the opposite arm. A speculative explanation is that TMS might activate back muscles contralateral to arm abduction via an uncrossed, ipsilateral corticospinal tract that is slower conducting than the conventional crossed corticospinal tract. The study has implications for the design of measures to promote recovery and rehabilitation of motor function in disorders such as stroke and spinal cord injury.

Survival after volunteer unrelated donor (VUD) stem cell transplantation (SCT) is influenced by matching for human leucocyte antigens (HLA). We analysed the effects of serological and molecular typing at HLA-A, -B, -C and -DRB1 in 100 patient/VUD pairs from a single transplant centre. Patients received SCT for good risk [chronic myeloid leukaemia in first chronic phase (CML-CP1), n = 55] or poor risk ( n = 45) diseases after myeloablative conditioning and T-cell depletion with alemtuzumab. By serological typing, 70 pairs were fully matched, whereas molecular typing revealed 10 pairs with additional mismatches. The day 100 transplant related mortality was 15%. Acute graft versus host disease (GvHD) grades III–IV occurred in 11%, whilst extensive chronic GvHD in 13% of evaluable patients. There was no statistical difference in GvHD rates between patients who received grafts from fully matched or from mismatched donors. In univariate analysis the disease risk group and CMV seronegativity of recipient and donor were the only significant predictors for survival, with 3-year survival probabilities of 71.2% for CML-CP1 and 28% for poor risk diseases. In the poor risk group, HLA mismatches had a negative impact on survival ( p = 0.003) and progression free survival ( p = 0.009) contrary to CML-CP1 patients, in whom HLA mismatches at molecular or serological level did not have any impact.

Prospective experimental.The aim of this study was to develop a quantitative sensory test (QST) that could be used for assessing the level and the density (degree of impairment) of spinal cord injury (SCI) and for monitoring neurological changes in patients with SCI.National Spinal Injuries Centre, Stoke Mandeville Hospital, Buckinghamshire Hospitals NHS Trust, UK.Perceptual threshold to 3 Hz cutaneous electrical stimulation was measured in 30 control subjects and in 45 patients with SCI at American Spinal Injuries Association (ASIA) sensory key points for selected dermatomes between C3 and S2 bilaterally. Electrical perceptual threshold (EPT) was recorded as the lowest ascending stimulus intensity out of three tests at which the subject reported sensation. The level of SCI according to EPT results was established for right and left sides as the most caudal spinal segment at which patient's EPT was within the control range (mean +/- 2 standard deviation (SD)). The level of SCI, according to EPT, was then compared with clinical sensory level derived according to ASIA classification.In the control group, EPT depended on the dermatome tested and was lowest for T1 (1.01 +/- 0.23 mA, mean +/- SD) and highest for L5 (3.32 +/- 1.14 mA). There was strong correlation between corresponding right and left dermatomes and between repeated assessments. In the SCI group, the level of lesion according to EPT and clinical testing was the same in 43 of the 90 tests (48%). In 37 cases (41%), the EPT level was higher than the clinical level, and in 10 cases (11%), it was lower. Below the level of lesion in incomplete SCI and in the zone of partial preservation in complete SCI, the EPT values in most dermatomes were raised compared with the control group.EPT is a simple, reproducible QST that can assess both the level and the density of SCI. It seems to add sensitivity and resolution to the standard clinical testing and could be a useful adjunct in longitudinal monitoring of patients with SCI for research purposes during natural recovery and therapeutic interventions.International Spinal Research Trust (ISRT), UK, Grant CLI001.

The cortical silent period (CSP) following transcranial magnetic stimulation (TMS) of the motor cortex can be used to measure intra-cortical inhibition and changes in a number of important pathologies affecting the central nervous system. The main drawback of this technique has been the difficulty in accurately identifying the onset and offset of the cortical silent period leading to inter-observer variability. We developed an automated method based on the cumulative sum (Cusum) technique to improve the determination of the duration and area of the cortical silent period. This was compared with experienced raters and two other automated methods. We showed that the automated Cusum method reliably correlated with the experienced raters for both duration and area of CSP. Compared with the automated methods, the Cusum also showed the strongest correlation with the experienced raters. Our results show the Cusum method to be a simple, graphical and powerful method of detecting low-intensity CSP that can be easily automated using standard software.

Glucose- and caffeine-containing energy drinks are said to influence the cognitive and cellular function within the brain. In this study, we have used the size of motor-evoked potentials (MEPs) produced in response to transcranial magnetic stimulation (TMS) of the motor cortex as an index of corticospinal excitability after ingestion of Lucozade and control drinks of glucose-containing or caffeine-containing carbonated water or carbonated water alone. With local ethical approval and informed consent, 10 healthy volunteers took part; surface electromyographic (EMG) recordings were taken from the thenar muscles of the dominant hand. In each assessment, 15 TMS stimuli were delivered over the motor cortex at an intensity of 1.1 T. Six subjects ingested a 380-ml bottle of carbonated Lucozade drink containing 68 g of glucose and 46 mg caffeine. Four subjects took part in three control trials drinking: (A) carbonated water with caffeine, (B) carbonated water with glucose and (C) carbonated water alone. Assessments were made before and at 30-min intervals after each drink. Mean fasting blood glucose concentrations and mean areas of MEPs rose after the Lucozade, remaining elevated for 90 min. Similar rises in MEP areas were seen in trials after drinking carbonated water with caffeine or with glucose, but not after drinking carbonated water alone. No change was seen in the M-wave evoked by electrical stimulation of the ulnar nerve. We conclude that Lucozade can affect the size of MEPs to activation of the motor cortex with fixed-intensity TMS. The underlying mechanism is likely to relate to the combined effects of caffeine and glucose on the brain.

We review the evidence that activation of the cerebral cortex can lead to movements of the vocal folds and possibly to cough. Electrical stimulation of the motor cortex can cause movements of the vocal folds and vocalizatioin, but cough has not been reported. The motor pathways are via the nucleus ambiguous and possibly the nucleus retroambuigualis in the brainstem. In humans, activation of the cerebral cortex by transcranial magnetic stimulation can cause motor potentials in the intrinsic laryngeal muscles and corresponding surface potentials. The relationships between the cortical sensation related to cough, the voluntary control of cough and the involvement of reflex pathways remain to be clarified.

Neurological testing tools for measuring and monitoring somatosensory function lack resolution and are often dependent on the clinician testing. In this study we have measured perceptual threshold (PT) to electrical stimulation of the skin and compared it with two-point discriminative ability (TPDA) in 12 control subjects. Tests were made on both sides of the body at American Spinal Injury Association (ASIA) key points on seven spinal dermatomes (C3 (neck), C4 (shoulder), C5 (upper arm), C6 (thumb), T8 (abdomen), L3 (knee), L5 (foot)) and in the mandibular (chin) and maxillary (cheek) fields of the trigeminal (V) nerve. Electrical stimulation (0.5 ms pulse width; 3 Hz) was applied via a self-adhesive cathode and an anode strapped to the wrist or ankle. The stimulus intensity was adjusted and PT was recorded as the lowest current at which the subject reported sensation. Sites were tested in random order. Indices for both TPDA and PT differed according to the dermatome tested but there was no correlation between TPDA and PT for any dermatome. There was good correlation between results from equivalent dermatomes on left and right sides for both PT and TPDA. Women frequently had lower mean (+/- S.E.) PTs and better TPDA than men; differences were significant (P < 0.05) for PT on the knee (women, 1.31 +/- 0.15 mA; men, 2.05 +/- 0.26 mA) and the foot (women, 2.90 +/- 0.19 mA; men, 4.13 +/- 0.28 mA) and for TPDA on the thumb (women, 3.8 +/- 0.2 mm; men, 7.8 +/- 1.3 mm) and the knee (women, 17.8 +/- 1.6 mm; men, 27.1 +/- 4.0 mm). Four subjects repeated the experiment on another day and the results correlated well with the first test for PT (r2, 0.62) and TPDA (r2, 0.48). PT differs between dermatomes in a predictable way but does not relate to TPDA. PT is easy to measure and may be a useful assessment tool with which to monitor recovery or deterioration in neuropathies, neurotrauma or after surgery.

Study design: Longitudinal. Objectives: (1) To perform standard clinical neurological examinations and establish the pattern of clinical change with time following incomplete spinal cord injury (iSCI). (2) To establish the pattern of change in corticospinal electrophysiological function with time after iSCI. (3) To correlate clinical with electrophysiological findings. Setting: The National Spinal Injuries Centre, Stoke Mandeville Hospital, Aylesbury, UK and Imperial College School of Medicine, Charing Cross Hospital, London, UK. Methods: Neurological assessments and classification were performed according to American Spinal Injuries Association and International Medical Society of Paraplegia (ASIA/IMSOP) standards. Twenty-one patients (ages 18–72 years) with iSCI (level C2–C7, ASIA impairment grades C–D) and 10 healthy control subjects (ages 27–57 years) were studied. Electrophysiological tests of corticospinal function were carried out using transcranial magnetic stimulation (TMS) of the motor cortex and electromyographic (EMG) recordings from thenar muscles. Both tests were performed on a number of occasions, beginning 19–384 days and ending 124–1109 days post-injury, and the group data were pooled into time epochs of 50 or 100 days post-injury for analysis. Seven of the patients were studied on seven or more occasions and were also assessed individually. Results: Individual and pooled data indicated that neurological scores improved progressively and tended to stabilise by around 300 days post-injury. When the patients were first assessed, the mean latency for motor evoked potentials (MEPs) and inhibition of voluntary EMG were significantly different from control values. There was no significant change in latency on subsequent sessions for either the grouped or individual patient data. There was no correlation between clinical assessment and electrophysiological data. Conclusion: We conclude that the weakened inhibition seen following iSCI is established within a few days of the time of spinal cord trauma. We argue that reduced corticospinal inhibition may be a prerequisite for the recovery of useful motor function. Sponsorship: The work was supported by a project grant from The Wellcome Trust. Synchronisation of motor unit discharges is commonly seen in hand muscles of normal man but is absent following neurologically complete spinal cord injury and reduced after stroke. These findings support the notion that some corticospinal inputs to motoneurones are shared and contribute to the observed synchrony of discharge. In this study we have examined motor unit discharge in hand muscles below the level of an incomplete spinal cord injury in an attempt to relate strength of synchrony to the integrity of the corticospinal tract. Eight patients with incomplete spinal cord injury (neurological level C3-C7) and eight control subjects took part in the study. The patients had sustained injury 14-191 weeks prior to the recordings and had since regained good motor function in their hands. Two concentric needle electrodes were inserted into the first dorsal interosseus muscle which subjects were instructed to contract weakly so that potentials from individual motor units could be reliably identified on both recordings. Synchrony was detected by constructing cross-correlograms between the discharges of pairs of individual motor units. The amount of synchronous firing was determined from the magnitude of any peak in the cross-correlogram, as the probability above chance (XP) of one motor unit firing with respect to the other and vice versa. The degree of synchrony was lower (P < 0.05) in the patient group (mean XP 0.06) than in the control group (mean XP 0.09). The incidence of significant synchrony was lower in the patient group (41.8 %) than in the control group (92.9 %). The mean (+/- S.E.M.) frequency of motor unit discharge was slightly lower (P < 0.05) in patients (9.7 +/- 0.4 impulses s-1) than controls (10.8 +/- 0.5 impulses s-1). The mean width of synchrony peaks was narrower (P < 0.05) in patients (11.4 +/- 1.1 ms) than controls (13.2 +/- 0.6 ms). We conclude that the weaker synchrony of motor unit discharge in incomplete spinal cord injury may reflect permanent damage to some corticospinal axons.

Lateral ventricular volumes were monitored and quantified using accurately registered magnetic resonance images (MRIs) in six healthy individuals 30 min before and up to 4 h after ingestion of a glucose drink. The volume of the lateral ventricles increased by an average (+/- S.E.M.) of 2.4 +/- 0.4% as blood glucose levels rose from 4.8 +/- 0.2 mmol l-1 to 8.4 +/- 0.4 mmol l-1. This was followed by a peak decrease of 5.99 +/- 3.3% below initial fasting volumes as blood glucose levels fell to 5.0 +/- 0.3 mmol l-1. We suggest that the secondary volume decrease demonstrates a homeostatic process of brain volume regulation for which the mechanism remains uncertain.

The size of compound motor evoked potentials (cMEPs) to transcranial magnetic stimulation of the motor cortex was measured in the relaxed first dorsal interosseous muscle of the nondominant hand (ndFDI) during different levels of voluntary contraction in the homonymous muscle of the dominant hand (dFDI). cMEP responses in the ndFDI became larger when the dFDI was contracted to forces ranging 10-70% of maximum voluntary contraction. Variability in the amplitude of the cMEP responses in ndFDI decreased when dFDI was contracted. Comparison with cMEPs to spinal cord stimulation suggested a large component of the facilitation was occurring at a cortical level. The amplitude of cMEP responses in ndFDI also increased when the tibialis anterior muscle of the leg on the contralateral side was contracted. The observed facilitation of motoneurons during contraction of contralateral muscles might involve a transcallosal pathway modulating the excitability of one cortex when the other is activated.

Transcranial magnetic stimulation (TMS) of the motor cortex was used to produce compound motor evoked potentials (cMEPs) in the first dorsal interosseus (FDI) muscle. The size of cMEPs was measured as an index of corticospinal excitability before and after initiation of a simple reaction task (SRT). The SRT, consisting of an abduction of the right index finger against a vertical support in response to a 1 kHz cueing tone, was performed in 6 healthy male subjects. cMEPs were facilitated when timed to occur just before the fastest simple reaction time (fSRT). When the cMEP was placed 15.5 ± 1.5 ms before the fSRT, its amplitude increased to 176 ± 36% of the control response seen in the relaxed state (no SRTs). Facilitation of the cMEP increased to 382 ± 43% of the control when it was placed 11.9 ± 1.5 ms after the fSRT. The facilitation of cMEP responses prior to the SRT is discussed with particular reference to the premovement potential that may be recorded over the cortex prior to a voluntary movement.

We have investigated variability in the amplitude of compound motor evoked potentials (cMEPs) in right and left thenar and wrist extensor muscles in response to synchronous bilateral transcranial magnetic stimulation (TMS) of the motor cortices using two figure-of-eight stimulating coils. Trials of 50 stimuli revealed a wide range of variability in cMEP amplitudes in relaxed muscles (coefficient of variation, range 0.22-1.12). The amplitudes of the cMEPs in one muscle correlated positively with those in the others. The r2 values (mean +/- SEM) were 0.27 +/- 0.06 for muscles on the same side of the body and 0.19 +/- 0.04 for muscles on opposite sides. Employing the ECG to trigger TMS, clamping the coil relative to the head or altering the orientation of the coil all failed to affect the variability of cMEPs. We conclude that fluctuations in excitability of the corticospinal pathway give rise to the variability in the response to TMS, that they are wide-ranging with respect to the muscles affected, and include a bilateral component. We argue that the variability reveals fluctuations in excitability mainly at the cortical rather than the spinal level. We suggest that measures of variability might provide an indication of cortical activity in conditions where consciousness and voluntary movement are compromised.

Surface magnetic stimulation has been applied directly over skeletal muscle (triceps surae) in decerebrated cats. Recordings were made of the twitch contraction and electromyographic responses in triceps surae, and of the centripetal nerve volley in the sciatic nerve or spinal roots. Stimulus/response curves were established up to the maximum output of the magnetic stimulator. Neuromuscular blockade abolished the twitch contraction and muscle action potential leaving the nerve volley unaffected. Raising the stimulator output to its maximum increased the size of the nerve volley but failed to produce any muscle response. We conclude that magnetic stimulation applied directly to skeletal muscle excites via stimulation of motor nerve axons. The maximum output of the stimulator was incapable of exciting muscle fibers by direct depolarization. Use of magnetic stimulation in the clinical appraisal of muscle function should be interpreted in the knowledge that the stimulator elicits contraction only indirectly through nerve stimulation.

BackgroundIn this first investigation of motor function in schizophrenia using transcranial magnetic stimulation (TMS), the general hypothesis tested was that this methodology could be used to investigate the disruption of corticospinal inhibitory processes suggested by cognitive and psychophysiological paradigms.MethodNine drug-free DSM–IV schizophrenic patients were compared with nine age- and sex-matched normal subjects. Electromyographic (EMG) recordings were made from the thenar muscles of the dominant hand during sustained, weak voluntary contraction. TMS over a particular threshold applied to the motor cortex would elicit a compound motor evoked potential (cMEP) followed by a period of suppression of EMG.ResultsThe latency of cMEPs following TMS was significantly shorter in the schizophrenic patients. The two groups did not differ significantly with respect to mean latency of suppression of EMG activity, or stimulus thresholds for either cMEPs or EMG suppression.ConclusionThese findings could be the result of a relative lack of corticospinal inhibition of motor responses; a change in the site of TMS activation; or an abnormality of peripheral nervous function in schizophrenia. Drug effects were unlikely since seven of the patients were drug-naïve.

Study design: Preliminary longitudinal clinical trial. Objectives: To test the efficacy of repetitive transcranial magnetic stimulation (rTMS) in modulating corticospinal inhibition and improving recovery in stable incomplete spinal cord injury (iSCI). Setting: National Spinal Injuries Centre, Stoke Mandeville Hospital, Bucks, UK and Division of Neuroscience, Imperial College Faculty of Medicine, Charing Cross Hospital, London, UK. Methods: Four stable iSCI patients were treated with rTMS over the occipital cortex (sham treatment) and then over the motor cortex (real treatment). Patients were assessed using electrophysiological, clinical and functional measures before treatment, during sham treatment, during the therapeutic treatment and during a 3-week follow-up period. Results: Cortical inhibition was reduced during the treatment week. Perceptual threshold to electrical stimulation of the skin, ASIA clinical measures of motor and sensory function and time to complete a peg-board improved and remained improved into the follow-up period. Conclusion: In this preliminary trial, rTMS has been shown to alter cortical inhibition in iSCI and improve the clinical and functional outcome. Objective: To test the hypothesis that chronic fatigue syndrome (CFS) is associated with altered cerebral metabolites in the frontal and occipital cortices. Method: Cerebral proton magnetic resonance spectroscopy (1H MRS) was carried out in eight CFS patients and eight age- and sex-matched healthy control subjects. Spectra were obtained from 20 × 20 × 20 mm3 voxels in the dominant motor and occipital cortices using a point-resolved spectroscopy pulse sequence. Results: The mean ratio of choline (Cho) to creatine (Cr) in the occipital cortex in CFS (0.97) was significantly higher than in the controls (0.76; P=0.008). No other metabolite ratios were significantly different between the two groups in either the frontal or occipital cortex. In addition, there was a loss of the normal spatial variation of Cho in CFS. Conclusion: Our results suggest that there may be an abnormality of phospholipid metabolism in the brain in CFS.

A rhythmic modulation of motor unit discharge at 17-24 Hz has been observed in the tibialis anterior muscle of subjects who display a high degree of short-term synchrony. Motor unit discharges were recorded both from extensor digitorum communis in the arm and tibialis anterior in the leg during voluntary isometric contractions. Time domain analysis of motor unit discharges in both muscles revealed a prominent central peak in the cross-correlograms that was interpreted as short-term synchrony. In tibialis anterior, but not in extensor digitorum communis, the primary, central peak was accompanied by secondary peaks. These were symmetrically or unilaterally present at an average time lag of 52 ms (range 37-68 ms) with respect to the primary peak. In both muscles other peaks occurred at time lags equivalent to the mean interspike interval of motor unit discharge (range 80-150 ms). In all subjects frequency domain analysis of the discharges of pairs of motor units revealed prominent peaks of coherence in the range 17-24 Hz. The actual frequency matched closely the reciprocal of the time lag interval between primary and secondary peaks of the cross-correlogram of the same motor units. Coherence was less evident for motor unit discharges in extensor digitorum communis and no prominent peaks occurred in the range 17-24 Hz. We propose that the secondary peaks and the rhythmicity may involve recurrent inhibition activated by the synchronous discharge of motor units. The presence of this pattern of discharge in tibilias anterior but not extensor digitorum communis would fit with recent animal and human work suggesting that recurrent inhibition is poorly represented in certain distal muscles of the forelimb.

The direction of the current induced by transcranial magnetic stimulation (TMS) over the motor cortex has been observed to influence the threshold and latency of evoked muscle responses. This study investigates the effect of TMS-induced current orientation (ICO) over the prefrontal cortex, on a specific cognitive task (memory-guided saccade). TMS was applied with a figure-of-eight coil, placed at one of eight different orientations over the prefrontal cortex. The most effective ICO was antero-lateral, which is a different optimal ICO from that seen over the hand area of the motor cortex. This demonstrates that ICO can alter the effect of TMS on cognitive functions and that ICO is an independent variable that should not be ignored when designing TMS studies.

The effect of long-term endurance and resistance training on central fatigue has been studied using transcranial magnetic stimulation by exercising the biceps brachii to exhaustion and recording motor-evoked potentials from the non-exercised homologous biceps. Three groups of eight healthy individuals took part: two groups of individuals who had more than 8 years of athletic training in either an endurance or resistance sport, and a group of controls. The size of a motor-evoked potential (area of averaged rectified response) was significantly depressed in all three groups in the non-exercised arm after exhaustive exercise of the opposite arm. Recovery of motor-evoked potentials occurred earlier in endurance athletes (20 min) than in control participants (30 min) and resistance athletes (30 min). Dexterity and maximum voluntary contraction of the biceps for the non-exercised arm were not depressed in any group. In a separate session, the limit of endurance time for the biceps was reduced significantly following exhaustive exercise of the biceps of the other arm for resistance athletes and control participants, whereas there was no change in the endurance athletes. Our findings suggest that athletic training has an effect on the mechanism of central fatigue that may be specific to the nature of training.

INTRODUCTION This report summarizes preliminary findings of three quantitative sensory tests (QSTs) that were evaluated as part of the International Spinal Research Trust (ISRT) Clinical Initiative study. ISRT commissioned this work in preparation for future clinical trials of new spinal cord injury (SCI) interventions. The wider Clinical Initiative study assesses numerous clinical and neurophysiological tests of motor, sensory, and autonomic function that could be used for monitoring the efficacy of new therapeutic interventions. The ISRT Clinical Initiative has a few specific requirements. Firstly, to minimize functional loss to the patient in case of any neurological deterioration, ISRT anticipates that the first clinical trials of spinal cord repair sponsored by them will be conducted in patients with thoracic lesions. Therefore, the newly developed tests must be applicable in the thoracic region, for which SCI diagnosis currently relies mainly on clinical sensory testing. Secondly, the first interventions are expected to produce only minor improvements and possibly over a few spinal segments only; therefore, the newly developed tests must be able to detect these small changes. The tests presented here were developed with these requirements in mind, although they can be used in patients with any level and type of injury. An overall progress review of the ISRT Clinical Initiative study was presented in the ISRT lecture at the International Spinal Cord Society's Annual Scientific Meeting in Beijing, China (October 2003) and was published in 2004 by Ellaway et al. [1]. The present article includes three of the sensory tests discussed in Ellaway et al. [1] and provides additional data acquired since that publication. However, at the time this article was prepared, the ISRT Clinical Initiative study was still ongoing and final results were unavailable. Hence, all the results reported here should be treated as preliminary. Once complete, final results will be submitted for publication as separate articles in peer-reviewed journals. The three tests discussed in this article are QSTs that assess the perceptual thresholds (PTs) (lowest stimulus intensity that a subject can perceive) for three types of stimulation: electrical, vibration, and thermal. All three stimulation types had been used previously for assessing sensory function, mainly in peripheral neuropathies [2-6], radiculopathies [7-8], and brachial plexus lesions [9]. Some QSTs have been used in patients with incomplete SCI [10-12]. For this study, the techniques were adapted to suit patients with SCI of any level, including thoracic, and of any impairment grade. Between them, the three QSTs were expected to include both of the sensory modalities tested in clinical examination (i.e., pinprick and light touch); thermal thresholds would predominantly test spinothalamic function, vibration threshold would test posterior column function, and electrical threshold would test both sensory functions. METHODS AND PRELIMINARY RESULTS The ISRT Clinical Initiative study was approved by the Aylesbury Vale Local Research Ethics Committee. Control subjects and patients with SCI were given a written information sheet and a verbal explanation of all the procedures. Written consent was obtained from all subjects. For each of the three tests (electrical, vibration, and thermal PTs), normative values and reproducibility of the technique were established in control subjects before performing the test in the patients with SCI. Wherever possible, stimuli were applied over American Spinal Injury Association (ASIA) sensory key points or, at least, within ASIA dermatomes (Figure 1). All patients also had a clinical neurological examination and classification performed according to ASIA standards [13-14] so that the QST results could be correlated with clinical sensory examination results. This study was designed to investigate corticospinal excitability of lumbar muscles using transcranial magnetic stimulation (TMS) in patients with chronic low back pain and correlate this with self-rated measures of disability and pain.Twenty-four patients with chronic low back pain and 11 healthy control subjects were used in this study. TMS was delivered through an angled double-cone coil, with its cross-over on the vertex and a posterior-to-anterior current flow in the brain. Electromyographic (EMG) recordings were made from erector spinae (ES) muscles at the fourth lumbar level. Motor cortical excitability was assessed using motor threshold (MTh) for motor evoked potentials (MEPs) and threshold for silent period (SP) during facilitation of the back muscles. Latency, duration, and area of MEPs and SPs were also measured.The latency, duration, and size of MEPs and SPs did not differ between the left and right ES muscles in either the patients or the control subjects and also did not differ between the patients and the control subjects. However, there was a significantly higher MTh and threshold for the SP in the patients as compared with the control subjects; the full significance of this requires further investigation. Interestingly, there was a positive correlation between the self-rated measure of disability (the Oswestry Disability Index score) and both the MTh and the threshold for the SP in the patients. There was also a positive correlation between the self-rated index of back pain and the threshold for the SP in the patients. This finding of an association between clinical and neurophysiologic measures reinforces the need for further research to establish the clinical relevance of these rises in MTh and SP threshold.In summary, this study has revealed that corticospinal excitability, driving ES muscles close to the site of pain, is lowered in patients with chronic low back pain.

Objective: The clinical and functional assessment of back muscles in human spinal cord injury (SCI) has received little attention. The aim of this study was to develop a method to assess the level of a thoracic spinal cord lesion based on the reflex activation of back muscles. Methods: In 11 control subjects and in 12 subjects with clinically complete thoracic SCI (T2–T12), either a spinous process or an erector spinae muscle was prodded to elicit short latency reflexes recorded electromyographically at the spinal level of stimulation. An electromagnetic servo, attached to a blunt probe, applied stimuli at a frequency of 1 Hz and amplitude of 3 mm. Two trials of 50 mechanical prods were conducted at each site. Results: Reflexes were evoked in control subjects in 82% of trials when the spinous process was prodded, and in 80% of trials when the muscle was prodded. In contrast, reflexes in SCI subjects could be elicited in 90–100% of trials two segments either above or below the lesion. Reflex responses in control subjects had a mean (SEM) latency of 5.72 (0.53) ms when the spinous process was prodded, and 5.42 (0.42) ms when the muscle was prodded. In the SCI subjects, responses had slightly (but insignificantly) longer latencies both above and below the lesion to either stimulus. The amplitude of reflex responses, expressed as a percentage of the background EMG, was on average 2–3 times larger at the three vertebral levels spanning the lesion in SCI subjects than at sites above or below the lesion or at any level in control subjects. Conclusion: We propose that the size of these mechanically evoked reflexes may be useful in determining the level of thoracic SCI. Furthermore, the reflexes might provide a valuable tool with which to monitor recovery after an intervention to repair or improve function of a damaged spinal cord.

Patients can often recover good motor function in muscles below an incomplete spinal cord injury. Within a few days of injury natural processes lead to down-regulation of inhibitory pathways within the motor cortex than can be demonstrated electrophysiologically. We believe that this natural change might encourage motor recovery by allowing an increased excitability of surviving descending corticospinal neurones. Repetitive transcranial magnetic stimulation (rTMS) can produce similar changes in corticospinal inhibition in normal uninjured individuals, albeit rather short-lasting. In this preliminary study we have delivered a sham rTMS over one week followed by another week of real treatment to four stable incomplete spinal cord injury patients who already showed reduced inhibition compared with controls. Intracortical inhibition was further reduced during the week of treatment but recovered basal levels within the three-week follow-up period. Longer-term improvements were seen in the clinical scores for both motor and sensory function, perceptual threshold to electrical stimulation of the skin and the time taken to complete a standard peg-board test. Although the measurable electrophysiological effects of rTMS are short-lived it would appear that functional recovery persists for at least three weeks after the treatment. Spinal cord injury patients might be more susceptible to the plastic cortical changes evoked by rTMS than non-injured individuals. This preliminary study provides promising data on which to base a larger investigation with the aim of substantiating the use of rTMS as a tool for routine use in rehabilitation.

Spontaneous electromyographic (EMG) activity in the soleus muscle of the rat varies with the changing gravitational force in parabolic flight, presumably in an appropriate way to resist the load. We investigated how decreased and increased gravitational force affects EMG in human back and arm muscles and to what extent the motor cortex is responsible for any modulation seen.Three healthy subjects stood during 10 parabolas consisting of periods (duration 20-25 s) of 1.8 G, then 0 G, and then 1.8 G. EMG recordings were made from right deltoid and left and right erector spinae (ES) muscles and transcranial magnetic stimulation (TMS) was applied to the motor cortex to produce motor evoked potentials (MEPs) in target muscles.In one exemplary subject, EMG levels and MEP areas increased in the left ES and right ES during periods of 0 G, which was less pronounced with the arm abducted. No significant changes were seen in EMG levels or MEP areas during periods of 1.8 G. Pooled data from the three subjects showed a similar pattern, revealing a facilitation of MEP responses in left and right ES muscles in periods of 0 G.EMG levels and MEP areas in ES muscles increased during periods of 0 G, suggesting that back muscle activity is "turned on" to stabilize the axial skeleton when the vertical compression forces present on Earth are removed. Further analysis suggested that microgravity produced activation of ES muscles through an increase in corticospinal excitability.

Clinical practice and scientific research may soon lead to treatments designed to repair spinal cord injury. Repair is likely to be partial in the first trials, extending only one or two segments below the original injury. Furthermore, treatments that are becoming available are likely to be applied to the thoracic spinal cord to minimise loss of function resulting from damage to surviving connections. These provisos have prompted research into the improvement of clinical and physiological tests designed (1) to determine the level and density of a spinal cord injury, (2) to provide reliable monitoring of recovery over one or two spinal cord segments, and (3) to provide indices of function provided by thoracic spinal root innervation, presently largely ignored in assessment of spinal cord injury. This article reviews progress of the Clinical Initiative, sponsored by the International Spinal Research Trust, to advance the clinical and physiological tests of sensory, motor and autonomic function needed to achieve these aims.

Patients suffering from sciatica may develop altered patterns of corticospinal drive to muscles of the leg. Electromyographic recordings were made bilaterally from tibialis anterior and lateral gastrocnemius muscles during transcranial magnetic stimulation of the motor cortex from nine patients with unilateral sciatica and seven healthy controls. The mean thresholds for eliciting motor evoked potentials (MEPs) and the silent period were higher in the patients than in the control subjects. In addition, there was a positive correlation between the patients' self rated score of disability (the Oswestry Disability Index) and their threshold for MEPs and the silent period. This study suggests that the chronic pain experienced by the patients leads to a reduction in corticospinal drive to the legs. These changes might help to relax muscles close to the perceived site of pain and so alleviate symptoms. Investigation of these patients following remedial treatment will allow us to establish if the changes are transient in nature.

The maximum voluntary muscle force can vary throughout the day; typically being low in the morning and high in the evening. The nature of this possible variation has been investigated with respect to corticospinal excitability. Six healthy subjects were studied. Maximum voluntary contraction (MVC) in the thenar muscles was measured. In addition, we monitored several indices of corticospinal excitability using electromyographic (EMG) recording and transcranial magnetic stimulation (TMS) of the motor cortex. Motor evoked potentials (MEPs) were recorded while relaxed and at 10% MVC when the silent period was assessed as an index of corticospinal inhibition. Readings were taken every 3 h for 24 h. MVC of the thenar muscles did not change significantly over the 24 h. The mean areas, latencies and durations of MEPs did not show significant changes over the 24-h test period with the muscle relaxed or contracted; however, MEP area did vary between sessions at all stimulus intensities suggesting non-time-of-day-dependent changes in corticospinal excitability. Furthermore, the extent and duration of the silent period seen after the MEP in the contracted muscle did not change significantly over the 24 h of the experiment at any stimulus intensity. These results provide evidence that the MVC force of the thenar muscles and their responses to TMS are stable throughout the course of the day and suggest that, in hand muscles, corticospinal excitability may not be subject to circadian variation.

This study examined post-exercise changes in corticospinal excitability in five 'elite' rowers and six nonrowers. Transcranial magnetic stimulation (TMS) was delivered to the motor cortex and bilateral electromyographic (EMG) recordings were made from erector spinae (ES) muscles at L3/L4 spinal level and from the first dorsal interosseous (FDI) muscle of the dominant hand. Each subject completed two exercise protocols on a rowing ergometer: a light exercise protocol at a sub-maximal output for 10 min and an intense exercise protocol at maximum output for 1 min. A trial of ten magnetic stimuli was delivered before each of the protocols and, on finishing exercise, further trials of ten stimuli were delivered every 2 min for a 16 min period. Amplitudes of motor-evoked potentials (MEPs) in each of the three test muscles were measured before exercise and during the recovery period after exercise. The non-rowers showed a brief facilitation of MEPs in ES 2 min after light and intense exercise that was only present in the elite rowers after intense exercise. In the period 4-16 min after light exercise, the mean (+/- S.E.M.) MEP amplitude (relative to pre-exercise levels) was less depressed in the elite rowers (79.4 +/- 2.1%) than in the non-rowers (60.9 +/- 2.5%) in the left ES but not significantly so in the right ES. MEP amplitudes in FDI were significantly larger in the elite rowers, averaging 119.0 +/- 3.1% pre-exercise levels, compared with 101.2 +/- 5.8% in the non-rowers. Pre-exercise MEP latencies were no different in the two groups. After light exercise MEP latencies became longer in the elite rowers (left ES, 16.1 +/- 0.5 ms; right ES, 16.1 +/- 0.4 ms; dominant FDI, 23.4 +/- 0.2 ms) than in the non-rowers (left ES, 15.0 +/- 0.3 ms; right ES, 15.2 +/- 0.3 ms; dominant FDI, 21.5 +/- 0.2 ms). There were no differences in MEP depression or latency between elite rowers and non-rowers after intense exercise. We conclude that the smaller degree of MEP depression in the elite rowers after light exercise reflects less central fatigue within corticospinal control pathways than that seen in the non-rowers. The longer latency of MEPs seen in the elite rowers may reflect recruitment of more slower-conducting fatigue-resistant motor units compared with the non-rowers. These differences may be because the energy requirements for the non-rowers during light exercise are closer to their maximum capacity, leading to more fatigue. This notion is supported by the lack of any difference between groups following intense exercise when both groups were working at their own maximum.

STUDY DESIGN Motor-evoked responses to transcranial magnetic stimulation of the motor cortex were recorded from erector spinae and deltoid muscles while the arm was abducted voluntarily in 10 normal subjects. OBJECTIVE To understand the neuronal substrate for the activation of the contralateral erector spinae muscle when the opposite arm is abducted. BACKGROUND DATA When a standing individual abducts an arm, the center of gravity is altered; to avoid falling, trunk muscles become activated on the contralateral side. METHODS Surface EMG activity was recorded from the right deltoid and left and right erector spinae muscles. Subjects maintained abduction of their right arm to 90 degrees at five different levels of isometric force in standing and lying postures. Transcranial magnetic stimulation was delivered to the motor cortex, producing motor-evoked responses in the three muscles during arm abduction and while relaxed. RESULTS EMG activity in the left erector spinae increased with the force of right arm abduction in both postures. EMG levels in right erector spinae showed no consistent change with right arm abduction force. As arm abduction force was increased, motor-evoked responses were facilitated in deltoid and the left erector spinae but not the right erector spinae in both postures. The pattern of motor-evoked potential facilitation with arm abduction force tended to plateau between 50% and 70% maximum voluntary contraction in the deltoid, whereas it continued to climb more linearly in the left erector spinae. CONCLUSIONS Facilitation of erector spinae with arm abduction remains evident in the lying posture when spinal postural stabilization mechanisms are presumably reduced. Similar facilitation profiles have been seen previously with changing voluntary activation of erector spinae in a trunk extension task, supporting the notion that during arm abduction the drive to the contralateral erector spinae has a corticospinal origin.

Body movements in man are frequently observed in relation to musical rhythms. In this study we have investigated the effect of strongly rhythmic music on corticospinal drive to the ankle extensor and flexor muscles involved in foot tapping. Surface electromyographic (EMG) recordings were made from tibialis anterior (TA) and lateral gastrocnemius (LGN) muscles in 12 normal subjects. Rock music with a strong rhythmic beat or white noise was played. Transcranial magnetic stimulation (TMS) was delivered to the motor cortex in time with the music to produce motor evoked potentials (MEPs) in both muscles while relaxed. During white noise trials nine subjects showed a significant correlation of MEP areas in TA with LGN, compared with only three subjects during music. Overall, there was a significant decrease in the correlation coefficient during music. We conclude that correlated variations in MEP areas between the muscles seen during white noise can be destroyed in the presence of music. This may be due to sub-threshold variations in corticospinal excitability to ankle extensors and flexors, which are time-locked to the musical beat but out of phase with one another.

The pathogenesis of chronic fatigue syndrome (CFS) remains unknown. Thresholds and latencies of motor evoked potentials (MEPs) in response to transcranial magnetic stimulation (TMS) are normal but intracortical inhibition has not been investigated. Eleven patients with CFS were compared with 11 control subjects. Each patient completed a questionnaire using visual analogue indices of pain, fatigue, anxiety and depression. Subjects released a button to initiate simple (SRTs) and choice reaction time (CRTs) tasks; for each task, movement times were measured between release of the initiation button and depression of a second button 15 cm away. Subjects held a 10 % maximum voluntary contraction in the thenar muscles of their dominant hand while TMS was applied to the motor cortex; the duration and extent of inhibition of surface electromyographic (EMG) activity were assessed at stimulus strengths above and below the threshold for MEPs. Patients had significantly (P0.05) higher mean indices of fatigue than of pain, anxiety or depression. Mean (+/- S.E.M.) SRTs (but not CRTs) were longer in patients (309 +/- 45 ms) than in controls (218 +/- 9 ms). Movement times were longer in patients for both SRTs and CRTs. TMS thresholds, expressed as a percentage of the maximum stimulator output, were not significantly (P0.05) different in both groups for both MEPs (patients, 34 +/- 3%; controls, 36 +/- 3%) and inhibition of voluntary contraction (patients, 29 +/- 2%; controls, 34 +/- 4%). The duration and extent of inhibition did not differ significantly between groups at any stimulus strength. The pattern of change in duration and extent of inhibition with increasing stimulus intensity was no different in the two groups. The duration and extent of corticospinal inhibition in patients with CFS did not differ from controls, adding further evidence to the notion that the feeling of fatigue and the slowness of movement seen in CFS is not manifest in corticospinal output pathways.

We used transcranial magnetic stimulation (TMS) to study corticospinal excitability to erector Spinae (ES) muscles during graded voluntary contractions in bilateral trunk extension (BTE) and forced expiratory breath holding (FEBH) in normal individuals. Motor evoked potentials (MEPs) could be produced in all subjects in the absence of voluntary activation. At maximum voluntary contraction, levels of surface electromyographic (EMG) activity were 4 times greater during BTE than FEBH. When EMG was normalized to maximum. MEP amplitudes increased in proportion to contraction in both tasks. MEPs in FEBH were compared with extrapolated values at similar EMG levels in BTE and were found to be larger. EMG and MEPs in left and right ES were symmetrical throughout the range of contractions in both tasks. ES muscles have a facilitation pattern similar to that previously shown in leg muscles, but subtle differences at low levels of EMG suggest that the facilitation is dependent on the task.

The pathogenesis of chronic fatigue syndrome (CFS) remains unknown. In particular, little is known of the involvement of the motor cortex and corticospinal system.Transcranial magnetic stimulation (TMS) was used to assess corticospinal function in terms of latency and threshold of motor-evoked potentials (MEPs) in thenar muscles. Reaction times and speed of movement were assessed using button presses in response to auditory tones.Patients had higher (P.05) self-assessed indices of fatigue (7/10) than for pain (5/10), anxiety (4/10) or depression (3/10). Mean (+/-S.E.M.) simple reaction times (SRTs) were longer (P.05) in the patients (275+/-19 ms) than in the controls (219+/-9 ms); choice reaction times (CRTs) were not significantly longer in the patients. Movement times, once a reaction task had been initiated, were longer (P.05) in the patients in both SRTs (patients, 248+/-13 ms; controls, 174+/-9 ms) and CRTs (patients, 269+/-13 ms; controls, 206+/-12 ms). There was no difference (P.05) in threshold or latency of MEPs in hand muscles between the patients (threshold, 54.5+/-2.2% maximum stimulator output [% MSO]; latency 22+/-0.3 ms) and controls (threshold 54.6+/-3.6% MSO; latency 22.9+/-0.5 ms). Regression analysis showed no correlation (P.05) of SRTs with either threshold for MEPs or fatigue index.Corticospinal conduction times and excitability were within the normal range despite a slower performance time for motor tasks and an increased feeling of fatigue. This suggests that the feeling of fatigue and the slowness of movement seen in CFS are manifest outside the corticospinal system.

OBJECTIVES—Motor evoked potentials (MEPs) and inhibition of voluntary contraction to transcranial magnetic stimulation (TMS) of the motor cortex have longer latencies than normal in patients with incomplete spinal cord injury (iSCI) when assessed using surface EMG. This study now examines the modulation of single motor unit discharges to TMS with the aim of improving resolution of the excitatory and inhibitory responses seen previously in surface EMG recordings. METHODS—A group of five patients with iSCI (motor level C4-C7) was compared with a group of five healthy control subjects. Single motor unit discharges were recorded with concentric needle electrodes from the first dorsal interosseus muscle during weak voluntary contraction (2%-5% maximum). TMS was applied with a 9 cm circular stimulating coil centred over the vertex. Modulation of single motor unit discharges was assessed using peristimulus time histograms (PSTHs). We have examined input-output patterns in the corticospinal system after incomplete spinal cord injury. The amplitude of the motor evoked potential (MEP) to transcranial magnetic stimulation (TMS) was used to study the patterns of recruitment, with increasing stimulus intensity, and facilitation, with increasing voluntary contraction, in thenar muscles of 12 patients with incomplete spinal cord injuries and 13 control subjects. The patients had all suffered spinal cord injury at a segmental level rostral to C8 and T1, the segments supplying innervation of thenar muscles. The patients showed a less pronounced increase in MEP amplitude with increasing strength of TMS compared with the controls. Specifically, at a stimulus strength of 120% threshold and above, the patients showed significantly smaller MEPs relative to the maximum ulnar nerve M-wave response than the controls. The patients also showed a less steep pattern of facilitation with voluntary drive. The MEP continued to increase up to 50% maximum voluntary contraction (MVC) whereas the controls reached a plateau around 10% MVC. The results indicate that the patients show modified corticospinal recruitment and facilitation of the motoneurone pool. We speculate that the function of the adapted corticospinal system after spinal cord injury might be to regulate and modulate drive to motoneurones originating from segmental and other descending inputs. We discuss how such a modified corticospinal system might be of functional benefit to the patients.

OBJECTIVES (1) A biochemical investigation of the motor cortex in patients with incomplete spinal cord injury and normal control subjects using proton magnetic resonance spectroscopy (MRS). (2) To relate any altered biochemistry with the physiological changes in corticospinal function seen after spinal cord injury. METHODS a group of six patients with incomplete spinal cord injury who showed good recovery of motor function were selected. The patients were compared with five healthy control subjects. Electromyographic (EMG) responses of thenar muscles to transcranial magnetic stimulation (TMS) of the motor cortex showed that inhibition of cortical output was weaker in the patients than the controls. Proton MRS data were collected from a plane at the level of the centrum semiovale. Two 4.5 cm3 voxels in the motor cortex and a third voxel in the ipsilateral occipital cortex were examined in the patients and control subjects. RESULTS The mean level of N- acetylaspartate (NAA), expressed relative to the creatine (Cr) peak (NAA/Cr), was significantly increased in the motor cortex of the patients compared with their ipsilateral occipital cortex or either cortical area in the controls. No differences between patients and controls were seen for any of the other metabolite peaks (choline (Cho), glutamate/glutamine (Glx) or the aspartate component of NAA (AspNAA)) relative to Cr. Choline relative to Cr (Cho/Cr) was higher in the motor cortex of the control subjects than in their ipsilateral occipital cortex. This difference was not present in the patients. CONCLUSIONS Raised NAA/Cr in the motor cortex of the patients probably results from increased NAA rather than a decrease in the more stable Cr. The possible relevance of a raised NAA/Cr ratio is discussed, particularly with regard to the changed corticospinal physiology and the functional recovery seen in the patients.

Discharges of gamma motoneurones were recorded from cut filaments of the nerve to the gastrocnemius medialis muscle in the cat decerebrated at an intercollicular level. Gamma motoneurones exhibited a background discharge in the absence of intentional stimulation, or could be made to discharge by continuous, innocuous stimulation of the skin of the heel. The discharges were periodic and regular (low coefficient of variation of interspike intervals), and no correlation was observed between the discharges of pairs of individual gamma efferents. Electrolytic lesion of the ipsilateral pontine and medullary reticular formation in the nucleus subcoeruleus, the nucleus reticularis gigantocellularis or the nucleus reticularis magnocellularis, invariably decreased regularity of discharge and resulted in short term synchrony. Lesions of the peri-aqueductal grey, the nucleus raphe dorsalis or the midline raphe nuclei did not induce synchrony. Surgical lesions in the locus coeruleus caused irregular firing and synchrony only when the lesion extended into the adjacent nucleus subcoeruleus. We conclude that monoaminergic neurones of the nucleus subcoeruleus, or a closely associated tegmental field, with axons descending through the gigantocellularis and magnocellularis fields, are the most likely origin of the bulbospinal control of synchronizing influences on gamma motoneurone discharge.

Frequency of firing and regularity of discharge of human motor units, and short term synchrony between pairs of motor units, have been assessed in extensor digitorum communis (EDC) and tibialis anterior (TA) muscles in control subjects and in clinically complete paraplegic subjects. The discharge pattern of TA motor units in paraplegia ranged from extremely regular to very irregular for different motor units whereas in the control population, and in EDC of both groups, there was a narrow, but intermediate, range of regularity. There was little difference in the incidence and degree of short term synchrony (STS) in EDC between paraplegic and normal subjects. In contrast, virtually no STS of motor units was observed in the TA muscles of the paraplegic group whereas control subjects exhibited approximately the same amount of STS in their TA and EDC muscles. It is concluded that the extra burden placed on arm muscles in paraplegia does not change the amount of synchronisation between motor units. Furthermore, section of the spinal cord does not increase STS as predicted from lesions of the reticulospinal tract in cats. This may reflect the coincidental removal of supraspinal synchronising inputs of motoneurons or the reorganisation of synaptic inputs in chronic paraplegia.

This article reviews methods that have been devel- oped as part of a clinical initiative on improving outcome meas- ures for motor function assessment in subjects with spinal cord injury (SCI). Physiological motor outcome measures originally developed for limbs—transcranial magnetic stimulation (TMS) of the motor cortex to elicit motor-evoked potentials (MEPs) and mechanical stimulation to elicit spinal reflexes—have been extended to muscles of the trunk. The impetus for this develop- ment is the lack of a motor component in the American Spinal Injury Association clinical assessment for the thoracic myo- tomes. The application of TMS to the assessment of limb mus- cles is reviewed, followed by consideration of its application to the assessment of paravertebral and intercostal muscles. Spinal reflex testing of paravertebral muscles is also described. The principal markers for the thoracic SCI motor level that have emerged from this clinical initiative are (1) the threshold of MEPs in paravertebral muscles in response to TMS of the motor cortex, (2) the facilitation pattern and latency of MEPs in intercostal muscles during voluntary expiratory effort, and (3) the absence of long-latency reflex responses and the exaggeration of short-latency reflex responses in paravertebral muscles.
