Contains fulltext : 218150.pdf (Publisher’s version ) (Open Access) BACKGROUND: Studies have shown co-contraction of jaw and neck muscles in healthy subjects during (sub) maximum voluntary jaw clenching, indicating functional inter-relation between these muscles during awake bruxism. So far, coherence of jaw and neck muscles has not been evaluated during either awake or sleep bruxism. OBJECTIVE: The objective of this study was to evaluate the coherence between jaw and neck muscle activity during sleep bruxism. METHODS: In a cross-sectional observational design, the electromyographic activity of jaw (masseter, temporalis) and neck (sternocleidomastoid, trapezius) muscles in individuals with "definite" sleep bruxism was measured using ambulatory polysomnography (PSG). Coherence for masseter-temporalis, masseter-sternocleidomastoid and masseter-trapezius was measured during phasic and mixed rhythmic masticatory muscle activity episodes using coherence-analysing software. Outcome measures were as follows: presence or absence of significant coherence per episode (in percentages), frequency of peak coherence (FPC) per episode and sleep stage. RESULTS: A total of 632 episodes within 16 PSGs of eight individuals were analysed. Significant coherence was found between the jaw and neck muscles in 84.9% of the episodes. FPCs of masseter-temporalis were significantly positively correlated with those of masseter-sternocleidomastoid or masseter-trapezius (P < .001). Sleep stages did not significantly influence coherence of these muscular couples. CONCLUSION: During sleep bruxism, jaw and neck muscle activation is significantly coherent. Coherence occurs independently of sleep stage. These results support the hypothesis of bruxism being a centrally regulated phenomenon.

To improve oral health for frail and care-dependent older people, both intra- and extramurally, in the Euregio Rhine-Waal area in the Netherlands and Germany, we inventoried barriers to oral care for the target group according to the literature, the organisation of oral care in both countries and the implications of this organisation for daily and professional (oral) healthcare and oral care. Results show most identified barriers are common to both countries, but the organisation of oral healthcare differs in both countries. The main differences lie in the financing and organisation of oral care in the intramural situation. In the Netherlands, this is to a large degree regulated and organised on the basis of the Chronic Care Act (Wlz), using the Verenso Oral Care Directive for care-dependent clients as a base for enforcement. In Germany, on the other hand, the provision of oral care in the home situation is more effectively facilitated. In both countries, various initiatives have recently been employed to improve, among other things, information supply, education and financing of oral healthcare.Om de mondgezondheid voor kwetsbare en zorgafhankelijke ouderen, zowel intra- als extramuraal, in het Euregiogebied Rijn-Waal in Nederland en Duitsland te verbeteren, is geïnventariseerd wat de belemmeringen zijn op het vlak van mondzorg voor de doelgroep volgens de literatuur, hoe de mondzorg is georganiseerd in beide landen en wat de implicaties van deze organisatie voor de (mond)zorg en mondverzorging zijn. Resultaten laten zien dat belemmeringen veelal gelijk zijn, maar dat de organisatie in beide landen verschilt. De grootste verschillen liggen in de financiering en inrichting van mondzorg in de intramurale situatie. Deze is in Nederland in sterkere mate gereguleerd en georganiseerd op basis van de Wet langdurige zorg met als uitgangspunt de Verenso-richtlijn ‘Mondzorg voor zorgafh ankelijke cliënten in verpleeghuizen’. In Duitsland is daarentegen het leveren van mondzorg in de thuissituatie beter gefaciliteerd. In beide landen zijn recentelijk verschillende initiatieven ontplooid ter verbetering van onder meer de informatievoorziening, scholing en financiering van mondzorg.

MATERIALS AND METHODS: The Chinese sample comprised 1463 dentulous (>/= 1 tooth in each jaw) and 124 edentulous (in one or both jaws) participants aged >/= 40 yrs. The Vietnamese sample comprised 2820 dentulous and 253 edentulous participants aged >/= 20 yrs. Food avoidance due to chewing difficulties was scored for regionally common 4 soft and 4 hard foods. Dental status was classified according to the multi-level hierarchical dental functional classification system (HDFC) based on the number and location of teeth and posterior occlusal pairs. Associations were analyzed using multivariate logistic regression analyses. RESULTS: For dentulous participants, the chance of avoiding foods was significantly larger with < 10 teeth in each jaw (OR = 2.26 (Chinese sample), respectively 1.74 (Vietnamese sample)), incomplete anterior region (OR = 1.78, respectively 1.84), "impaired" premolar region (OR = 2.22, respectively 1.71), or "impaired" molar region (OR = 2.46, respectively 1.84). Edentulous participants had twice the chance of avoiding foods (OR = 2.01 respectively 2.20). Avoiding foods was significantly associated with higher age. Participants of low SES (Chinese sample, OR = 1.93) and females (Vietnamese sample, OR = 1.27) had a larger chance of avoiding foods. CONCLUSIONS: Avoiding foods was significantly associated with reduced dentitions, edentulousness, and higher age; low SES only in the Chinese and being female only in the Vietnamese sample. CLINICAL RELEVANCE: Incomplete anterior regions, "impaired" premolar or molar regions, and especially edentulousness can be considered significant risk indicators for food avoidance.

Item does not contain fulltext BACKGROUND: There is lack of reliable predictors for success of conventional complete denture (CCD) therapy, which in turn might affect the effectiveness of subsequent implant-retained overdenture (IOD) therapy. PURPOSE: To investigate relationships between digitally obtained geometrical mandibular residual ridge measures and perceived CCD-stability. MATERIALS AND METHODS: 30 CCD wearing patients (67.9 ± 7.0 years) for whom a new set of CCDs was advised, were treated with new CCDs. Digitalized mandibular gypsum models were measured using the Geomagic Studio 2013 software. Data were obtained for (1) height, width, and cross-section surface area of the residual ridge at different locations (midline, premolar, and anterior edge of retromolar pad) and (2) denture base surface area. Scatter plots and multivariate regression analyses were used to investigate associations between the geometric data and denture base surface area, and correlated with denture stability scores (Spearman rank test). RESULTS: Scatter plots showed that best model fit for denture base surface area was mean ridge height (R(2) = 0.906). Multivariate regression showed that height at premolar location (p = 0.001) had largest effect on denture base surface area (R(2) = 0.796). In traditional medicine, a classical triad applies: 1. after investigation symptoms of illness lead to 2. a diagnosis, followed by 3. a proposal for treatment by the health care provider. In wish fulfilling medicine, the emphasis lies on patient's wishes. Wish fulfilling medicine concerns medical treatment, often at the request of the patient, and should be distinguished from shared decision-making,a form of communication seeking to meet the preferences of the patient and to actively involve the patient in his treatment. In the Netherlands, in accordance with the Healthcare Quality, Complaints and Disputes Act, health care providers have to offer good care at a good level, which is safe, effective, efficient and client-orientated, offered in a timely fashion and geared to the real needs of the client. Good care has to meet professional standards. In this way, fulfilling patients' wishes for treatment without a medical need can be restricted by law.

Objectives To identify relationships between masticatory ability and age, and dental and prosthodontic status amongst an institutionalized elderly dentate population in China. Materials and methods A sample of 512 elders living in eight nursing homes in Qingdao was categorized based on a hierarchical dental functional classification system with and without tooth replacements. Masticatory disability scores (MDSs) were analyzed using multiple regression models with only age, and age and dentition variables for participants having ≥ 10 natural and those having Results Overall, associations between MDS and age, number of teeth, and number of teeth replaced by dental prostheses were identified. Most important dentition factor is the presence of three to four premolar pairs. Teeth added by partial removable dental prostheses compensate impaired masticatory ability due to tooth loss for 50% compared to natural teeth.

Background Sleep bruxism is a phenomenon associated with masticatory muscle hyperactivity. While stretching of limb and trunk muscles has been extensively studied for musculoskeletal disorders, little is known about the effectiveness of stretching of masticatory muscles in the management of bruxism. Objective The aim of this study is to determine the effectiveness of stretching of the masticatory muscles for sleep bruxism. Methods Twenty four pain-free individuals with sleep bruxism were randomly assigned to either an intervention group or a control group. Both groups were given sleep hygiene advice and the intervention group additionally received muscle-stretching exercises for 10 days. Primary outcome measures, bruxism bursts and episodes per hour of sleep, were measured by ambulant polysomnography. Secondary outcome measures were among others pain-free active maximum mouth opening (MMO) and masseter pressure pain threshold (PPT). Results The number of bruxism episodes per hour of sleep increased more in the intervention group than in the control group (by 1.1 episodes, P = 0.066), as did the number of bruxism bursts per hour of sleep (by 8.6 bursts, P = 0.049). MMO and PPT increased significantly more in the intervention group (by 3.2 mm, P = 0.020; and by 1.0 kg/cm2 , P = 0.036, respectively). Conclusion Static stretching of the masticatory muscles resulted in a minor increase in sleep bruxism episodes (not significant) and bursts (significant). It also led to a significant increase in MMO and PPT. Therefore, masticatory muscle stretching was not effective in reducing sleep bruxism in the absence of pain and/or dysfunction.

Contains fulltext : 218226.pdf (Publisher’s version ) (Open Access) INTRODUCTION: The purpose of this study was to evaluate the perceptions of dentists and dental hygienists regarding their own and each other's roles in interprofessional collaboration following a clinical interprofessional educational programme (CIEP) as part of their graduate programme at Radboud university medical center (Radboudumc) and HAN University of Applied Sciences (HAN UAS), Nijmegen, the Netherlands, respectively. Perceptions were obtained at the end of their study (2014) and two years after their graduation (2016). METHODS: A qualitative study was conducted using a questionnaire with open-ended questions. Sixty-two dentists and thirty-eight dental hygienists were invited. Theory-based analyses were applied in combination with principles of Grounded Theory to analyse and synthesise the collected data from the open-ended questions. RESULTS: At baseline, 100% responded. At follow-up, thirty-two dentists (51.6%) and twenty-six dental hygienists (68.4%) responded; twenty-seven dentists (43.5%) and eighteen dental hygienists (47.4%) were included. Many similarities were found between baseline and follow-up regarding perceptions of each other's expertise and responsibility, learning from and with each other, and the behaviour of students and the "novice" professionals in interprofessional collaboration. Both dentists and dental hygienists experienced the CIEP as useful for interprofessional collaboration. The "novice" dentists and dental hygienists indicated that in dental practices interprofessional collaboration was less common. CONCLUSION: The CIEP resulted in more understanding amongst dentists and dental hygienists with regard to interprofessional roles, but in practice the "novice" dentists and dental hygienists face difficulties in applying the interprofessional roles.

Item does not contain fulltext AIMS: To assess the effectiveness of a variety of physical treatments in the management of tension-type headache (TTH) in children. METHODS: This review is reported in accordance with the PRISMA guidelines and was registered in the PROSPERO database (CRD42014015290). Randomized and nonrandomized controlled trials that examined the effects of all treatments with a physical component in the management of TTH in children and compared these treatments to a placebo intervention, no intervention, or a controlled comparison intervention were included. The Physiotherapy Evidence Database (PEDro) criteria for bias assessment and the Grades of Recommendation, Assessment, Development, and Evaluation (GRADE) Working Group criteria were used to assess the quality of the body of evidence. The outcome measures were pain, functioning, and quality of life. Only RCTs were included in the meta-analyses. RESULTS: An initial search produced 10,464 published articles. Of these, 17 were relevant trials, including 1,815 participants. The overall GRADE rating of the included studies was moderate, and 11 of the 17 studies could be used in the meta-analyses. The effectiveness of physical treatments in terms of a reduction of pain of 50% or more showed a risk ratio (RR) of 2.37 (95% CI: 1.69 to 3.33). Relaxation training was the most evaluated intervention and proved to be significantly effective (RR: 3.00 [95% CI: 1.94 to 4.63]). In children having TTH combined with temporomandibular disorders, occlusal appliances were effective (RR: 2.58 [95% CI: 1.37 to 4.85]). CONCLUSION: This review supports the use of physical treatments to reduce pain in children with TTH.

Contains fulltext : 225280.pdf (Publisher’s version ) (Closed access) OBJECTIVES: The aim of this systematic review was to assess intervention rates of direct composite restorations in the worn dentition based on data published in clinical trials. METHODS: Searches of electronic data bases, grey Literature and hand searches were completed, and selection criteria were applied at the title, abstract and full paper stages. Unavailable full papers or papers with unsuitable data were excluded. A reference search was conducted, and a final set of papers were selected for data analysis. Data were extracted and noted as any intervention required or performed, or intervention free. Descriptive statistics were used to summarise the findings and the odds ratios were calculated. RESULTS: 1683 titles were found in the initial searches. On application of selection criteria, 140 abstracts were selected (Kappa 0.885), 17 papers were chosen by 3 reviewers (Kappa 0.914 - 0.942) and 12 selected for statistical analysis (Kappa 0.924). A total of 3540 composites in 386 participants were analysed, with 1919 in the anterior region, and 1044 in the posterior region of these 11.7 % required intervention. Annual Intervention Rates (AIR) ranged from 0.8 to 17.9% reflecting the variation in reporting. CONCLUSION: There was wide variation between studies on the type of intervention. Overall the intervention rate of 11.6 % shows a favourable outcome but suggests some form intervention may be needed. CLINICAL SIGNIFICANCE: Direct composites remain a viable option to treat tooth wear but the outcome varies. Provided patients appreciate that some maintenance may be needed they are an effective intervention.

Background Even though bruxism has been shown to be associated with several psychological factors, few studies have been performed on its relationships with anger and frustration. Objective This study aimed to determine the association between self-reported awake and sleep bruxism and anger and frustration. Methods In this longitudinal observational study, 55 healthy adult participants with "possible" bruxism reported their experienced level of bruxism and several psychosocial factors and lifestyle factors for 28 consecutive days using a personal logbook. The logbook consisted of a daily diary and a weekly questionnaire composed of Dutch versions of validated questionnaires. The primary outcome data were analysed using multiple regression models. Results An increase in SB of 1 unit (on a scale ranging from 1 to 10) was associated with an increase in the anger-scale of 0.03 units, and the frustration-scale of 0.04 units. However, the random intercepts were 0.22 for anger and 0.19 for frustration, meaning that there were major differences between individuals. For awake bruxism (AB), the effects were an increase of 0.04 for anger with a random intercept of 0.21 and an increase of 0.03 for frustration with a random intercept of 0.06. The effects of anger and frustration on both SB and AB were not statistically significant. Conclusion The association between anger and frustration and self-reported bruxism is small on group level. In individual cases, anger and frustration and self-reported bruxism may be co-existent.

Wish fulfilling medicine refers to medical procedures applied without a direct medical need. In wish fulfilling medicine, the wish of the patient is dominant, but wish fulfilling medicine is also promoted indirectly by healthcare providers, (pharmaceutical) companies and healthcare insurers. Wish fulfilling medicine often concerns the enhancement of appearance or performance; therefore, wish fulfilling medicine is also referred to as (human) enhancement (therapy). The line between traditional and wish fulfilling medicine is vague: the border between illness and health, normal and abnormal functioning is not sharply defined and is relative to time and place. In the Netherlands, wish fulfilling medicine is not covered in the basic package provided by healthcare insurers and is paid for by the patients themselves. However, 'pay yourself' is not a decisive criterion for wish fulfilling medicine. With new biotechnological developments the domain of wish fulfilling medicine is expanding. Some dental treatments can be considered as wish fulfilling dentistry, for example in the context of cosmetic dentistry, orthodontics, or dental implantology. Although wish fulfilling medical treatments do not cure disease, they can promote health.Met wensgeneeskunde worden medische handelingen aangeduid die zonder direct medische noodzaak worden uitgevoerd. Item does not contain fulltext OBJECTIVE: To investigate the effectiveness of a new zinc-reinforced glass-ionomer in comparison to a common high-viscous glass-ionomer cement (HVGIC) used in multiple-surface ART-prepared cavities. The hypothesis tested was that the cumulative survival percentage of the new HVGIC is higher than that of the common HVGIC over 2 years. METHODS: A randomized triple-blind parallel group clinical trial was used. A total of 218 participants, average age 15.4 years (SD=0.2), with an occluso-proximal carious lesion in a permanent (pre-) molar were included. Restorations using test (ChemFil Rock) and control (Fuji IX GP) restoratives were placed according to the ART method by four calibrated operators. Restorations were evaluated after one week (baseline), and at 6-, 12-, 18- and 24 months from replicas and coloured photographs according to the ART restoration assessment criteria by two independent evaluators. Restoration survival curves were estimated using the Kaplan-Meier method and difference between dependent and independent variables tested with a Wald (chi-square) test. RESULTS: There was a statistically significant difference in cumulative survival percentages between the two types of restorations at 2 years (P=0.02). A higher percentage of multiple-surface restorations of Fuji IX GP (95.4%) than ChemFil Rock (85.3%) survived. Gender (P=0.64), operator (P=0.56) and cavity size (P=0.81) had no effect on the survival of the type of restoration observed. Type of tooth (P=0.004) and type of jaw (P=0.05) showed an effect. Severe wear was the major reason for restoration failure (ChemFil Rock=7; Fuji IX GP=1). SIGNIFICANCE: ChemFil Rock appears not to be a viable alternative to Fuji IX GP in restoring ART multiple-surface cavities in permanent posterior teeth. 01 september 2019

To assess associations amongst masticatory performance, dental functional status, and perceived chewing problems with common soft and hard foods.387 adult participants with reduced natural dentitions were included and underwent an oral examination, completed a written structured questionnaire and performed a two-color chewing gum test for masticatory efficiency. A multivariate regression model was used to analyze associations between masticatory efficiency and a number of dental conditions, with age, gender and perceived chewing difficulties as background variables. Likelihood ratios were calculated to analyze the relationship between masticatory efficiency and dental conditions within a validated hierarchical dental functional classification system expressed by (1) number of teeth in upper and lower jaw, (2) completeness of anterior regions, (3) number of premolar occluding pairs, and (4) number of molar occluding pairs.The regression model showed a significant positive association between unmixed ratio and self-reported problems with chewing (p = .004), and age (p 0.001), and negative associations with having a sufficient premolar region (p 0.001). In the hierarchical dental functional classification system, the likelihoods of having a high unmixed ratio and chewing problems with hard foods were highest for the predictor 'less than 10 teeth in each jaw' (LRs 4.01, respectively 13.06).In partially dentates people, perceived chewing difficulties were associated with lower objective masticatory efficiency. Having fewer than 10 teeth in each jaw and having an impaired premolar region were best predictors for low masticatory efficiency.This study shows that having fewer than 10 teeth in each jaw and having an impaired premolar region are associated with low masticatory efficiency and chewing problems. Preservation of at least 10 teeth in each jaw and 3 to 4 premolars is important for preventing chewing difficulties.

Item does not contain fulltext OBJECTIVE: In a cross-sectional study, we aimed to determine (1) the effect of spinal muscular atrophy (SMA) type 2 and 3 on mandibular function reflected as masticatory performance, mandibular range of motion, and bite force and (2) the predictors of mandibular dysfunction. METHODS: Sixty patients with SMA type 2 and 3 (mean age 32.3 years, SD 17.4 years) and 60 age-matched controls filled out questionnaires about impairments of mandibular function. All participants underwent detailed clinical examination to document the mandibular range of motion including maximal mouth opening, bite force, and masticatory function. RESULTS: All mandibular movements, including mouth opening, lateral range of motion, and protrusion of the mandible, were reduced in patients with SMA type 2 and 3 compared to healthy controls (p < 0.001). Maximal bite force was 19% lower in patients than controls, and more in patients with SMA type 2 than type 3. The strongest predictive factor was SMA type for impairment of mandibular range of motion (R(2) = 0.82) and weakness of neck muscles for bite force (R(2) = 0.47). CONCLUSIONS: Reduced mandibular mobility and bite force are common complications in SMA. SMA type and neck muscle strength are important correlates of these complications. We provide further evidence for clinically relevant bulbar involvement in patients with SMA.

MATERIALS AND METHODS: Dental status of 512 elders (>/=60 years) from eight nursing homes in Qingdao were analyzed in terms of prevalence of decayed (D), missing (M), filled (F), and replaced teeth (R). Multivariate logistic regression was applied to determine relationships with the background variables age, gender, and SES. Prevalence of D, M, and F was analyzed also for separate dental regions. For determining oral functionality, prevalence of dentitions with >/=20 teeth without and with tooth replacements was plotted against age. RESULTS: Mean number of D varied from 3.8 at 60 years to 4.6 at 90 years, M from 3.6 at 60 years to 6.7 at 90 years for the lower jaw, and from 3.0 at 60 years to 8.0 at 90 years for the upper. Mean number of F in each jaw was low: 0.2 at 60 years to 0.4 at 90 years. Gender and SES effects were limited. Molars had significantly higher prevalence of D and M than premolar and anterior teeth. Seventy percent of participants of 60 years had >/=20 natural teeth and 12 % at 90 years. Including tooth replacements, 96 % at 60 years, and 84 % at 90 years had >/=20 teeth. CONCLUSIONS: In this sample of institutionalized elders, dental status of the majority of participants did not represent a functional dentition without tooth replacements. CLINICAL RELEVANCE: Institutionalized Chinese elders showed relatively low numbers of decayed teeth but high numbers of missing teeth.

In 2010, the revised 3-year master's programme was introduced in the Netherlands, which meant that the full programme of dental education was extended to 6 years. In Nijmegen, this was structured to include a set of profile programmes, next to the existing curriculum. Three profiles were chosen, one of which was the Surgical Profile. The aim of this programme was that the more complex procedures and the treatment of medically compromised patients would be carried out by those students in the 6-year educational programme who participated in the Surgical Profile. The experiences of students following this profile were evaluated by means of a questionnaire. In a second questionnaire, distributed 1 year after the students had graduated, respondents were asked whether they were still making use of the skills that they had been taught. The conclusion was that the training of master's students in dental surgery results in a high degree of satisfaction. The study revealed moreover that 93% of the students who completed the Surgical Profile put the skills they had been taught to use during the first year as dentists and even wanted to build on that knowledge by means of relevant post-graduate courses. Wish fulfilling medicine comprises medical procedures applied without a direct medical need. In such procedures, the medical-ethical principles can come under pressure: the autonomy of the patient, when wishes originate from social pressure; beneficence when the (underlying) aim and consequences are unclear; and 'doing no harm', when that appears to be impossible. The principle of justice, too, could come under threat when especially those with a privileged socio-economic background can take advantage of wish fulfilling medicine. Regardless of whether it concerns wish fulfilling medicine or conventional medicine, respect for human dignity and the individual integrity of the patient continue to be paramount. In care ethics and moral ethics, the qualities necessary in a proper caregiver are emphasised, such as caring, compassion, commitment, honesty and personal dedication. Wish fulfilling medicine is the subject of significant ethical debate. Important aspects arising from this debate are that the risks of harm should be limited, human dignity and integrity should be respected, people should genuinely be helped and the principle of justice should be upheld.Met wensgeneeskunde worden medische (be)handelingen zonder direct medische noodzaak aangeduid. The aim was to determine the efficacy of mastication training in Duchenne muscular dystrophy using chewing gum for 4 weeks. In all, 17 patients and 17 healthy age-matched males participated. The masticatory performance was assessed using a mixing ability test and measuring anterior bite force before, shortly after and 1 month after the training. In the patient group the masticatory performance improved and remained after 1-month follow-up, no significant changes in anterior maximum bite force was observed after mastication training. In the healthy subject the bite force increased and remained at the 1-month follow-up; no significant differences in masticatory performance were observed. Mastication training by using sugar-free chewing gum in Duchenne muscular dystrophy patients improved their masticatory performance. Since bite force did not improve, the working mechanism of the improvement in chewing may relate to changes of the neuromuscular function and coordination, resulting in improvement of skills in performing mastication.

The ultimate strength of a dental prosthesis is defined as the strongest loading force applied to the prosthesis until afracture failure occurs. Important key terms are strength, hardness, toughness and fatigue. Relatively prevalent complications of single- and multi-unit fixed dental prostheses are porcelain and ceramic fractures. Afactor which also plays a role is the functional loading force from the entire orofacial system. With respect to the strength of multi-unit fixed dental prostheses, the length of the arch span between the abutment teeth, the pontic with the connectors and the possible cantilevers are the critical components. Components of the configuration ofabutment teeth of single- and multi-unit fixed dental prostheses which are relevant for its strength are the convergence angle and the design of(the area above) the (cervical) outline. Finally, the thickness of the porcelain or the ceramic (veneers) ofmetal-ceramic and all-ceramic single- and multi-unit fixed dental prostheses is of importance.

For the manufacture of single- and multi-unit fixed dental prostheses, effective communication between dentist and dental technician is required. Mutual insight concerning the (im)possibilities of available treatments and technical options is prerequisitefor this communication. The manufacture of single- and multi-unit fixed dental prostheses involves 4 phases: recording the required detailed information on the relevant teeth and the occlusal system, the technical adjustments, the technical design and the technical fabrication. These phases can be accomplished through an analogue or (semi)digital procedure. Pioneering developments are computer aided design and computer aided manufacturing (CAD/CAM), and computerised milling machines. Associated with this are 3 manufacturing methods which can be distinguished: the dental practice method, the dental laboratory method and the milling centre method. Materials applied are metal alloys and ceramics, while resins are used for provisional and transitional constructions. Due to the fact that the choice of material in the analogue procedure is limited, CAD/CAM offers more options, the digital procedure is expected to gain ground gradually. It is expected that this development will provide an impulse to higher quality.

The degree to which single- and multi-unit fixed dental prostheses are able to withstand loading forces is dependent, among other things, on the quality of their retention and resistance. The quality of the retention and resistance of the configuration of an abutment tooth prepared for a metal and metal-ceramic single-unit fixed dental prosthesis is determined by the configuration's convergence angle, the height, the volume, the interocclusal space, the cervical outline design, the additional preparations, the quality of the (build-up) restoration, and the surface roughness. A silicate ceramic single-unit fixed dental prosthesis is attached through adhesion using a composite cement, but the retention and resistance of an oxide ceramic single-unit fixed dental prosthesis is dependent on the abutment tooth configuration. Most types of multi-unit fixed dental prosthesis have the following additional retention and resistance determining factors: the position in the occlusal system, the number of abutment teeth and their mutual configurations, and the length of (cantilever) pontics. A resin-bonded fixed partial denture's retention and resistance are determined by its bonding as well as its enamel surface coverage and its resistance preparations.

Item does not contain fulltext OBJECTIVES: The aims of this study were to compare 2-year cumulative survival rates of amalgam and atraumatic restorative treatment (ART) restorations in primary molars and to investigate the determinants of the survival rate of restorations. MATERIALS AND METHODS: A controlled clinical trial using a parallel group design was carried out on 258 children aged 6-7 years old, allocated to two treatment groups: conventional restorative treatment using amalgam and ART using high-viscosity glass ionomer. A total of 364 amalgam restorations and 386 ART restorations were placed by three pedodontists in 126 and 158 children, respectively, and were evaluated after 0.5, 1, and 2 years. Restorations were placed in vital primary molars with neither pain nor signs of pulp involvement. The survival analysis was conducted using the proportional hazard rate regression model with frailty correction. RESULTS: The 2-year cumulative survival rates for all amalgam (77.3%) and ART (73.5%) restorations were not statistically significantly different, but there was an effect of "type of surface" (single/multiple) and "cavity filling time" on the survival rates. Both amalgam and ART single-surface restorations had higher survival rates than multiple-surface restorations of the same material. Secondary caries was responsible for 36 and 38% of failures in amalgam and ART restorations, respectively. Mean time for restoring all type of cavities with amalgam and ART restorations was 13.6 and 13.7 min, respectively. CONCLUSIONS: Amalgam and ART restorations presented similar survival rates over a 2-year period for all, single-surface, and multiple-surface restorations. CLINICAL RELEVANCE: In the cause of finding alternatives to amalgam, ART restorations using high-viscosity glass ionomer might be a suitable option for managing cavitated dentine carious lesions in vital primary molars.

In a correctly functioning occlusal system, the design of the occlusal parts of single-unit and multi-unit fixed dental prostheses is generally determined by the maximum intercuspation. Determining and recording the maxillomandibular relationships is only required in case the adjacent teeth do not offer adequate support or reference to put the maxillary and mandibular cast in maximum intercuspation or in case the existing occlusion needs a well-structured alteration. The maxillomandibular relationships can be determined and registered analogously and digitally and on that basis the maxillary and mandibular casts can be mounted in a (virtual) occludator or articulator. In the absence of a distinct occlusal plane, one may consider first carrying out a facebow recording and transfer. Usually, the accuracy of determining and recording the maxillomandibular relationships does not increase when using more complicated methods.

A single- or multi-unit fixed dental prosthesis can be attached to the abutment teeth through mechanical retention and gap sealing or by adhesion. For sealing the gap, water-soluble cements are appropriate, such as zinc phosphate, polycarboxylate, and (resin-modified) glasionomer cement. Attachment through adhesion can be performed with composite cement. If the hard tooth tissue is prepared adequately, composite cement provides sufficient adhesion, but self-adhesive composite cement is now also available. For the adhesion of the composite cement to the restorative materials of the single- or multi-unit fixed dental prosthesis, surface sandblasting, silanizing, and tin coating and the application of a metal primer or chemically active composite are available. Cementing a single- or multi-unit dental prosthesis involves 3 phases: 1. Cleansing the single- or multi-unit dental prosthesis and the abutment tooth/teeth; 2. Preparing the hard tooth tissue, mixing the cement and placing the single- or multi-unit dental prosthesis; 3. Removing the excess cement.

As a result of the introduction of oral implants, it is also possible to treat patients who have a reduced occlusal system with implant-supported fixed dental prostheses. Many publications report the successful application of implant-supported single- and multi-unit fixed dental prostheses. However, it is questionable if implants are also successful in periodontally compromised patients. With respect to implant treatment, roughly 3 categories of periodontally compromised patients can be distinguished: patients who have not been treated for periodontitis, patients who have been treated for periodontitis and have stable periodontal health, and those who have been treated but have not achieved stable periodontal health. For the first group, periodontal treatment is required. Inserting implants is only indicated in cases showing steady improvement of post-operative periodontal health. The second category has no contraindication for inserting implants. For the third group, inserting implants is contraindicated absolutely. When treatment with oral implants is indicated, meticulous aftercare and a surgical treatment which enables adequate oral hygiene self-care are strict requirements.

The preprosthetic treatment is a phase of the so-called patient-centred oral healthcare cycle. The goal of the preprosthetic treatment is the elimination of pathological conditions and of uncertainties and risks to the greatest extent possible. Teeth in a reduced occlusal system can be distinguished functionally as strategic, non-strategic, and undesirable. The most important objective of the preprosthetic treatment is to establish a positive prognosis for the strategic teeth. Specific aspects relevant to the preprosthetic treatment are: referral to a specialist, requesting a second opinion, inserting 1 or more oral implants, transitional treatments, and occlusal adjustments. Subsequently, the preprosthetic treatment is evaluated to assess whether healthy circumstances have been established for the intended treatment with single- or multi-unit fixed dental prostheses.

Objectives To assess, in older people with different levels of care-dependency 1) which frailty- and non-frailty related predisposing, enabling and need factors are associated with a) dental service use (DSU) frequency, b) changed DSU after the onset of care-dependency, c) brushing frequency, and d) changed brushing frequency since the onset of care-dependency; and 2) if unfavorable oral health care behavior is related to unfavorable oral health outcomes. Methods Bivariate analyses and multivariable logistic regression analyses were performed to evaluate data from 126 Dutch care-dependent people aged ≥ 65 on oral and general health, psychological and social issues. Results Lower DSU frequency was mainly related to non-frailty-related predisposing factors, especially being edentate (OR = 3.75; CI: 1.20–11.71; p = 0.023) and lower socioeconomic status (OR = 1.74; CI: 0.97–3.14; p = 0.065); lower DSU frequency since the onset of care-dependency to frailty-related enabling and need factors, especially ‘difficulty going to the dentist’ (OR = 4.98; CI:1.85–13.36; p = 0.001) and clinically assessed treatment need (OR = 3.23; CI:1.24–8.42; p = 0.016); lower brushing and changed (reduced) brushing frequency to frailty-related enabling factors, and, in case of reduced frequency, significantly to ‘not being capable of summoning the effort to brush’ (OR = 8.28; CI: 1.44–47.56; p = 0.018) and high care-dependency level (OR = 4.14; CI: 1.05–16.36; p = 0.043). Elders with lower and especially those with reduced DSU and brushing frequencies since the onset of care-dependency, had generally worse oral health outcomes and related quality of life. Conclusions Oral health care behavior, especially lower brushing and DSU frequency since the onset of care-dependency, is related to specific frailty-related factors in a care-dependent older population. Clinical significance Oral care-providers should be alert to the role of specific frailty-related factors, which are likely to reduce DSU and brushing frequency in the course of increasing frailty.

Item does not contain fulltext PURPOSE: The aim of this study was to analyze the clinical fit of metal-frame partial removable dental prostheses (PRDPs) based on custom trays used with alginate or polyvinyl siloxane impression material. MATERIALS AND METHODS: Fifth-year students of the Nijmegen Dental School made 25 correct impressions for 23 PRDPs for 21 patients using alginate, and 31 correct impressions for 30 PRDPs for 28 patients using polyvinyl siloxane. Clinical fit of the framework as a whole and of each retainer separately were evaluated by calibrated supervisors during framework try-in before (first evaluation) and after (second evaluation) possible adjustments (score 0 = poor fit, up to score 3 = good fit). Framework fit and fit of the denture base were evaluated at delivery (third evaluation). Finally, postinsertion sessions were evaluated and total number of sessions needed, sore spots, adjustments to the denture base, and reported food-impaction were recorded. RESULTS: No significant differences in clinical fit (of the framework as a whole, for the retainers, or for the denture base) were found between the groups in the three evaluation sessions. Differences were not found for postinsertion sessions with one exception: in the alginate group, four subjects reported food impaction, versus none in the polyvinyl siloxane group. CONCLUSION: Clinical fit of metal-frame PRDPs based on impressions with custom trays combined with alginate or polyvinyl siloxane was similar.

Muscle-stretching exercises are a recommended part of several international guidelines for musculoskeletal disorders and may be effective in management of the jaw muscle activity that gives rise to bruxism. However, most studies of muscle-stretching exercises have mainly focused on their influence on performance (eg, range of motion, coordination, and muscle strength) of the limb or trunk muscles of healthy individuals or individuals with sports-related injuries. Very few have investigated stretching of the human masticatory muscles and none muscle-stretching exercises in the management of (sleep) bruxism. This article reviews the literature on muscle-stretching exercises and their potential role in the management of sleep bruxism or its consequences in the musculoskeletal system.

There is also a high prevalence of dysphagia in nursing home residents and they suffer more often from comorbidity and multimorbidity. This puts nursing home residents at higher risk of (mortality from) NHAP. Therefore it is important to gain more insight into the incidence of NHAP and the associated medical conditions in nursing home residents with dysphagia. OBJECTIVE: To investigate possible associations between NHAP and dysphagia in nursing home residents and to search for a medical risk profile for NHAP. DESIGN: A retrospective cross-sectional study. SETTING: Three nursing homes in The Netherlands. PARTICIPANTS: 416 electronic medical files of nursing home residents aged 65 or older living in 3 nursing homes. METHODS: Data about age, gender, diagnosis of dysphagia and/or pneumonia, medical diagnosis and possible cause of death of the nursing home residents were extracted from electronic medical files. RESULTS: The data of 373 electronic medical files were analyzed. A significant difference in the prevalence of dysphagia was found between the nursing homes (p < 0.001). The incidence of NHAP was 5-12% in the participating nursing homes. Statistically significant higher incidence of NHAP was found in residents with dysphagia (p = 0.046). Residents with dysphagia had statistically significantly more diseases compared to residents without dysphagia (p = 0.001). Logistic regression analyses revealed no statistically significant associations between NHAP and the number of diseases and the ICD-10 diseases. CONCLUSIONS: Dysphagia was found to be a risk factor for NHAP. Awareness of the signs of dysphagia by nurses and other care providers is important for early recognition and management of dysphagia and prevention of NHAP.

The aim of the study was to examine if the daily application of 0.05% chlorhexidine oral rinse solution is effective in reducing the incidence of aspiration pneumonia in nursing home residents with dysphagia. Associations between background variables (age, gender, dysphagia severity, care dependency, medication use, number of medical diagnoses, teeth and dental implants, and wearing removable dentures) and the incidence of aspiration pneumonia were also examined. METHODS: This study is a multicenter study in which for 1 year participants with dysphagia in the intervention group received the usual oral hygiene care with the addition of a 0.05% chlorhexidine oral rinse solution, whereas participants in the control group received only oral hygiene care. RESULTS: Data of 103 participants in 17 nursing homes were analyzed. Survival analysis showed no significant difference in the incidence of pneumonia between both groups (Cox regression, HR = 0.800; 95% CI [0.368-1.737], p = 0.572). Cox regression analysis for Functional Oral Intake Scale (FOIS)-level showed a significant risk of the incidence of pneumonia (HR = 0.804; 95% CI [0.656-0.986], p = 0.036). After adjustment for Group and FOIS-level, Cox multivariate proportional hazard regression analysis showed that the variables age, gender, Care-dependency Scale-score (CDS) number of diseases, medication use, number of teeth, and the presence of dental implants or removable dentures were not significantly associated with the incidence of pneumonia. CONCLUSIONS: Chlorhexidine oral rinse solution 0.05% as an adjunctive intervention in daily oral hygiene care was not found to reduce incidence of aspiration pneumonia. The requested number of participants to achieve sufficient power was not established and high drop-out rate and non-structural compliance was present. The power was considered to be sufficient to analyze the associations between the background variables and the incidence of pneumonia in the included nursing home residents with dysphagia. Dysphagia was found to be a risk factor for aspiration pneumonia. TRIAL REGISTRATION: Registration in The Netherlands National Trial Register: TC = 3515. Item does not contain fulltext OBJECTIVES: To assess and analyse OHRQoL of people with shortened dental arches (SDA) in a long-term cohort study. METHODS: All participants of a long-term cohort study on SDA who were still attending the university dental clinic and still had an SDA (SDA group) with 3-5 posterior occluding pairs and intact anterior areas, an SDA plus removable dental prosthesis (SDA plus PRDP group) or complete dental arch (CDA group) completed the Dutch version of the Oral Health Impact Profile (OHIP-49NL) and additional questions on satisfaction with their dental status (yes/no). Mann-Whitney tests were performed for OHIP total scores and OHIP domain scores. The chance that the difference in median OHIP scores between the groups was larger than 6 OHIP units, was calculated by a bootstrapping procedure. RESULTS: 10 participants were eligible for SDA group and 11 for CDA group. The SDA plus PRDP group (n=1) was excluded from analyses. Mean follow-up period was 29.3+/-5.5 for SDA group and 36.7+/-5.5years for CDA group. Mean OHIP-49NL score was 13.9+/-10.9 for SDA group and 11.3+/-8.6 for CDA group. Differences in mean total scores and mean scores per domain were not statistically different. The probability that a difference in median OHIP total scores between groups was larger than 6 OHIP units was 0.25. Both groups showed high percentages of satisfaction with dental condition, except for dental appearance. CONCLUSION: OHRQoL of people with a long-term SDA condition was similar to that of people with CDA. CLINICAL SIGNIFICANCE: People having SDA for long periods are expected to report similar OHRQoL and satisfaction levels/oral comfort as people with CDA.

An occlusal system that does not function well, has a negative impact on the functional level of the oral system and the orofacial system. In purely mechanical terms, a limited anatomical reduction of the occlusal system has limited implications for the functional level since the occlusal system has several forms of adaptation, reserve, and compensation. However, following the loss of (parts of) teeth, an occlusal system may be anatomically reduced to such an extent that restoration of the functional level is required, for instance by the use of implant-supported single- and multiple-unit fixed dental prostheses. The mechanical strength of a tooth and a single-tooth fixed dental prosthesis on a similar tooth type is not essentially different. But the same cannot be said of the mechanical strength of a multiple-unit fixed dental prosthesis, because the strength is controlled by the mechanoreceptors in the periodontal ligaments of the abutment teeth. This control system is disturbed by the insufficiency of or, when oral implants are involved, the absence of mechano-receptors. It is unknown whether this is causing noticeable problems.

A resin-bonded fixed partial denture is a prosthetic construction which can replace I or several teeth in an occlusal system and which comprises a pontic element which is adhesively attached to 1 or more abutment teeth. To compensate for the limited shear strength of the adhesive layer, the Jixed partial denture is occlusally supported by the abutment(s). A direct resin-bonded fixed partial denture is made of composite, reinforced or not by a frame of flexible metal or fiber material. For an indirect resin-bonded fixed partial denture, a metal, fibre-reinforced composite or ceramic substructure is fabricated in a dental laboratory. The basic principle of a resin-bonded fixed partial denture is minimal invasiveness. However, a restoration in an abutment tooth requires a certain occlusal space which is realized by tooth preparation. Resistance preparations may be performed to improve the longevity of resin-bonded fixed partial dentures. Both financially and biologically, a resin-bonded bridge is a cost-effective prosthetic construction. The longevity is limited, but when the construction fails the negative consequences for the abutments are generally limited, which leaves open several types of other treatments.

Occlusion concepts based on functional aspects offer more solid ground in the diagnostic process and in the treatment of (reduced) dentitions than morphologically and mechanically oriented occlusion concepts. Nevertheless, for occlusal reconstruction morphologically oriented guidelines are necessary. These guidelines are based on the border movements and positions of the mandible in the orofacial system, and on the location and modelling of the occlusal contacts in the occlusal system. The modelling of single- and multi-unit fixed dental prostheses must harmonize with the occlusal system. Moreover, an important feature is the relation of the anterior teeth which enables mutually protected occlusion. Characteristics of a healthy orofacial and occlusal system are: absence of pathology, perceived sufficient oral functions, variability inform and function, and adaptive capacity. When designing single- or multiunit fixed dental prostheses, a pragmatic starting point is to maintain the existing occlusion and the existing speech pattern unless arguments can be provided for alterations. The occlusal design should aim at optimizing oral functions, such as mandibular and occlusal stability.

After (extensive) loss of tooth tissue, vital teeth can be built up completely with composite restoration material. Sometimes, the application of additional retentive preparations is indicated. In exceptional cases, a root canal treatment is indicated, following which a root canal post is applied if necessary. Endodontically treated multi-rooted teeth can be built up entirely with composite, utilizing the pulp chamber and root canal accesses for retention. However, in single-rooted teeth a root canal post is necessary, in which case a prefabricated root post is used. Traditionally, metal posts were used in combination with a cast, indirect build-up restoration. Subsequently, directly fabricated cores, combining metal posts with amalgam or, at the present time, composite restoration material, became customary. The present trend is to use prefabricated or individually fabricated fibre-reinforced posts. After restoring a tooth with a build-up restoration, a combined build-up restoration can be chosen, or a direct or indirect crown single tooth prosthesis.

Als eerste moet aan de hand van een risicoprofiel de frequentie van periodieke mondonderzoeken worden vastgesteld. Doelstellingen van de periodieke mondonderzoeken zijn de preventie en zo nodig de behandeling van pathologische aandoeningen en complicaties. In preventief opzicht moet aandacht worden geschonken aan voorlichting en instructie, orale biofilm en tandsteen, niet-functionele activiteiten, harde gebitsweefsels, parodontale en peri-implantaire weefsels en speeksel. That requires aftercare, too. First, the frequency of routine oral examinations should be assessed, using an individual risk profile. The objectives of the routine oralexaminations are the prevention and, when necessary, the treatment of pathological conditions and complications. With regard to prevention, attention should be paid to information and instruction, oral biofilm and calculus, non-functional activities, hard tooth tissues, periodontal and peri-implant tissues, and saliva. Subsequently, it can be determined whether the intended durability and profitability have been achieved or can still be achieved, whether or not through indicated adjustments. Special attention should be paid to endodontically treated teeth. Restorative, repair or replacement treatments may be indicated in case of complications, such as loose single- ormulti-unit fixed dental prosthesis, fracture of a fixed dental prosthesis unit, lost tooth pulp vitality, tooth root fracture, and implant or implant abutment problems.

The question whether a patient needs extensive and invasive treatment, such as single-tooth fixed prosthesis, multi-teeth fixed prosthesis or oral implants, can be established only in a dialogue with mutual respect between the healthcare provider and the patient. With respect to this question, the following principles have to be considered: the patient's autonomy, beneficence, non-maleficence, and justice. These principles are reflected in legislation and are implicitly applied in complaint and disciplinary jurisdiction. When the autonomy of a patient is not sufficiently respected by a healthcare provider, a patient complaint might be the result, such as a complaint of impertinent treatment, lack of informed consent, or poor patient documentation. If beneficence or non-maleficence principles are violated, an intrinsic treatment complaint might be the result, such as a complaint of inadequate treatment planning, often based on inadequate examination, and complications or untimely failure of single-tooth fixed prosthesis, multi-teeth fixed prosthesis or oral implants. Respecting the previously mentioned principles promotes appropriate healthcare to the patient also in complex care with other healthcare providers.
