Incidences of brain death due to nicotine overdose by e-cigarettes have been increasing. In such cases, liver donation has been not reported because of the secondary damage to the liver due to metabolism of large amounts of nicotine. However, kidneys have been considered acceptable for transplant. Here, we present a successful case of liver transplant from a brain-dead donor due to intentional nicotine ingestion who did not have extensive steatosis. To the best of our knowledge, this is the first report of such a case.

Introduction Liquid nicotine exposure is becoming more common in the pediatric population. Toxicity may occur with exposure to small quantities given the high concentrations in solutions available commercially. Effects can include altered mental status, seizure, and death. Methods This simulation-based case involves the identification and management of a toddler presenting with acute liquid nicotine exposure, with emphasis on the general approach to the acutely ill pediatric patient, consideration in toxic exposures, and the presentation of nicotine exposure. Providers should assess airway, breathing, and circulation while concurrently providing supportive care for an actively seizing simulated patient, necessitating appropriate selection of medications and acute airway management. Additionally, providers must maintain a broad differential diagnosis and obtain a focused history to narrow that differential and identify toxic exposure as a cause of the patient's presentation. Preparatory and didactic material is provided to help the instructor prepare the simulation environment, guide learners through the case, and debrief with learners afterward. Results We implemented this curriculum with four pediatric emergency medicine fellows and 15 pediatric residents during two sessions. Feedback was overwhelmingly positive; participants who completed evaluations reported high levels of confidence with knowledge and skills directly related to the educational objectives after participation (mean Likert scores of 4.9 out of 5 in response to effectiveness of the case in teaching evaluation and management of nicotine toxicity). Discussion This comprehensive resource will aid in offering continuing education for providers and specifically in educating learners with regard to acute liquid nicotine exposure in a child.

INTRODUCTION This uncontrolled study examined the outcome of a program of progressive cigarette reduction, using nicotine gum, as a prelude to complete cessation among 116 smokers (70 men, 46 women; mean age = 45.7 years, SD = 12.65; mean baseline smoking = 28.3 cigarettes/day) who sought treatment in a smokers' clinic but did not want to quit abruptly. METHODS Subjects participated in a two-stage program consisting of a 4-month reduction phase followed by a 6-month abstinence phase. The aim was to reduce the number of cigarettes smoked daily by at least 50% by Week 8 and to quit at the end of Week 16. During the reduction phase, subjects used nicotine gum (2 or 4 mg) to progressively decrease smoking. During the abstinence phase, subjects used any type of nicotine replacement therapy (NRT) to remain smoke free. Psychological treatment and NRT were provided at no cost to participants. RESULTS At Month 2, 76 subjects (68%) achieved the target of 50% reduction. At the target quit date in Week 16, 66 subjects (57%) achieved carbon monoxide-validated abstinence and 45 subjects (39%) maintained continuous abstinence at the 6-month follow-up. No symptoms of nicotine overdose were detected. DISCUSSION A program of progressive cigarette reduction using nicotine gum is feasible in practice and may be a useful strategy for smokers who are unable or unwilling to quit by abrupt cessation.
