3. A retrospective head-to-head comparison of the Lugano classification and PERCIST for FDG-PET/CT response assessment in diffuse large B-cell lymphoma.

- Abstract
Background: Diffuse large B-cell lymphoma (DLBCL) is the most common form of lymphoma. European guidelines recommend FDG-PET/CT for staging and end of treatment (EOT) response assessment, mid-treatment response assessment is optional. We compared the Lugano classification and PET Response Criteria In Solid Tumours (PERCIST) for FDG-PET/CT response assessment in DLBCL head-to-head., Methods: We retrospectively included patients with DLBCL who underwent first-line R-CHOP(-like) therapy (2013-2020). Interim and EOT FDG-PET/CT response were reevaluated using the Lugano classification and PERCIST. Response was dichotomized into complete metabolic response (CMR) versus non-CMR (interim and EOT) and responders versus nonresponders (interim only). The cutoff for nonresponse at interim was a Deauville score of 5 (DS5) with the Lugano classification and a partial metabolic response with ≤66% reduction in SUL
peak using PERCIST (PERCIST66)., Results: In multivariable Cox regression (N = 170), DS5 at interim, PERCIST66 at interim, non-CMR at EOT with the Lugano classification and non-CMR at EOT with PERCIST were predictive of progression-free survival (PFS). The Lugano classification and PERCIST agreed perfectly at interim and EOT and with 98.4% for the identification of nonresponders at interim. The accuracy for predicting events within 2 years of diagnosis was 84.2% for DS-5 at interim, 87.6% for PERCIST66 at interim, 86% for non-CMR with the Lugano classification at EOT and 83.3% for non-CMR with PERCIST at EOT., Conclusion: The Lugano classification and PERCIST were equally predictive of PFS. Nonresponse at interim and non-CMR at EOT were predictive of poor PFS with comparable accuracy for predicting events within 2 years., (© 2023 The Authors. - Abstract
In recent years, many people have experienced different problems and challenges in using the national Danish health portal sundhed.dk, as they find it difficult to obtain desired information about their own laboratory test results and treatment plans. Therefore, the aim of this study is to find solutions, to make patients laboratory results easily accessible and understandable for the users. To achieve this aim there will be used two participatory design methods, video observation and questionnaires. The results shows that only 43.5% normally understand their test results, whereas the remaining participants need help to understand their results.

- Abstract
Introduction: Transcervical resection of the endometrium (TCRE) is a first-line surgical treatment of abnormal uterine bleeding. However, many women experience unsuccessful results, causing hysterectomy in up 17% of cases. The aim of this study was to describe the odds of hysterectomy in women with abnormal uterine bleeding, treated with TCRE and levonorgestrel intrauterine contraceptive device (TCRE + LNG-IUCD) or TCRE alone. The secondary aim was to analyze the rate of amenorrhea., Material and Methods: Designed as a retrospective cohort study, and conducted at Odense University Hospital, Denmark, the study included women with abnormal uterine bleeding, who underwent TCRE from January 2013 to December 2015. The decision of treatment with respect to LNG-IUCD was at the woman's discretion. Data were collected from medical records and a self-reported retrospective bleeding-pattern questionnaire. A multivariate regression model was used, enabling adjustment for potential and identified confounders., Results: Out of 432 women, 276 (62%) consented to inclusion and of these, 16 (4%) were excluded. In total 88 (34%) received combined treatment and 172 (66%) received TCRE alone. Ten women (11%) treated with TCRE + LNG-IUCD underwent hysterectomy, compared with 27 (16%) treated with TCRE alone (OR = 0.69, 95% CI 0.28-1.56; P = .34). Multivariate analysis disclosed a significant effect of TCRE + LNG-IUCD (OR = 0.35, 95% CI 0.13-0.97; P = .04) on hysterectomy. The presence of fibromas was shown to increase the odds of treatment failure, resulting in hysterectomy (OR 2.69, 95% CI 1.15-6.31; P = .02). Furthermore, the incidence of amenorrhea was 59% in the TCRE + LNG-IUCD group and 36% in the TCRE alone group (OR = 2.56, 95% CI 1.46-4.49; P < .01)., Conclusions: The study showed significantly lower odds of hysterectomy in the TCRE + LNG-IUCD group when adjusted for confounders. - Abstract
Introduction. Cefuroxime is an important antibiotic to treat several serious infections. Rapid elimination through the kidneys and the variation in MICs of various susceptible pathogens such as Escherichia coli, Klebsiella pneumoniae, Staphylococcus aureus and Streptococcus pneumoniae give rise to dosing issues, especially in otherwise healthy patients. Aim. To investigate the probability of target attainment (PTA) for obtaining the optimal dosage regimens for cefuroxime in healthy young people. Methodology. Two weeks apart 750 and 1500 mg cefuroxime were administered as an intravenous bolus to 20 healthy volunteers (mean age: 27 years). Population modelling and simulation studies were done based on the obtained data for cefuroxime plasma concentration. Results. With a target value of time above MIC ( T
>MIC ) greater than 50 % the simulations revealed that a PTA of >99 % is obtained for S. pneumoniae with a dosage regimen of 750 mg q12h. For E. coli and K. pneumoniae the PTA was <90 % even with the highest, simulated dosage of 1500 mg q6h. For S. aureus a dosage of 1500 mg q8h gave a PTA above 97 %. Conclusions. S. pneumoniae is most likely treatable with a two-daily dose of 750 mg cefuroxime. Not treatable are K. pneumoniae and E. coli . For S. aureus 1500 mg q8h constitutes an optimal dosing schedule.- Published

7. Independent effects of endurance training and weight loss on peak fat oxidation in moderately overweight men: a randomized controlled trial.

- Abstract
Endurance training increases peak fat oxidation (PFO) during exercise, but whether this is independent of changes in body weight is not known. The aim of the present study was to investigate the effects of endurance training with or without weight loss or a diet-induced weight loss on PFO and on key skeletal muscle mitochondrial proteins involved in fat oxidation. Sixty moderately overweight, sedentary but otherwise healthy men were randomized to 12 wk of training (T), diet (D), training and increased caloric intake (T-iD), or continuous sedentary control (C). Isoenergetic deficits corresponding to 600 kcal/day were comprised of endurance exercise for T and caloric restriction for D. T-iD completed similar training but was not in 600 kcal deficit because of dietary replacement. PFO and the exercise intensity at which this occurred (FatMax) were measured by a submaximal exercise test and calculated by polynomial regression. As intended by study design, a similar weight loss was observed in T (-5.9 ± 0.7 kg) and D (-5.2 ± 0.8 kg), whereas T-iD (-1.0 ± 0.5 kg) and C (0.1 ± 0.6 kg) remained weight stable. PFO increased to a similar extent with 42% in T [0.16 g/min; 95% confidence intervals (CI): 0.02; 0.30, P = 0.02] and 41% in T-iD (0.16 g/min; 95% CI: 0.01; 0.30, P = 0.04) compared with C, but did not increase in D (P = 0.96). In addition, the analysis of covariance showed that changes in both PFO (0.10 g/min; 95% CI: 0.03; 0.17, P = 0.03) and FatMax (6.3% V̇o2max; 95% CI: 1.4; 11.3, P < 0.01) were independently explained by endurance training. Objective: First-degree relatives (FDRs) of patients with type 2 diabetes may exhibit a disproportionately elevated risk of developing insulin resistance, obesity, and type 2 diabetes when exposed to physical inactivity, which to some unknown extent may involve low-grade inflammation. We investigated whether subjects who are nonobese FDRs show signs of low-grade inflammation before or after exposure to short-term physical inactivity., Research Design and Methods: We studied 13 healthy FDR subjects and 20 control (CON) subjects matched for age, sex, and BMI before and after 10 days of bed rest (BR). Insulin sensitivity was measured by the hyperinsulinemic euglycemic clamp. Key low-grade inflammation mediators were measured in arterial blood and microdialysate from subcutaneous abdominal (SCAAT) and femoral adipose tissue. Adipokine mRNA expression was determined in SCAAT., Results: Before BR, FDR subjects displayed insulin resistance, elevated plasma C-reactive protein, leptin, and monocyte chemoattractant protein (MCP)-1, high interleukin (IL)-6, and MCP-1 expressions, as well as low adiponectin and leptin expressions. FDR subjects responded to BR by decreasing plasma adiponectin and IL-10 expression and increasing plasma expression of IL-10 and tumor necrosis factor-α. In contrast, CON subjects responded to BR by increasing plasma adiponectin and adiponectin expression and by decreasing SCAAT microdialysate leptin., Conclusions: Young and nonobese FDR of patients with type 2 diabetes exhibit low-grade inflammation, which is further and disproportionately aggravated when exposed to physical inactivity. The study provides support for the notion that people at increased risk of type 2 diabetes should avoid even short periods of physical inactivity.

- Abstract
Adipokines play important regulatory roles in the pathophysiology of obesity and insulin resistance. We measured plasma and interstitial concentrations of the adipokines adiponectin, resistin, leptin, monocyte chemoattractant protein-1 (MCP-1), interleukin-6 (IL-6) and interleukin-8 (IL-8) in subcutaneous, abdominal and femoral adipose tissue using calibrated, large-pore microdialysis technique in 8 healthy, lean men on 2 experimental days. The interstitial leptin concentration was 2.5-fold higher in subcutaneous, femoral than abdominal adipose tissue (P<0.05), but no regional differences were found for the remaining adipokines (P>0.05). Adiponectin and leptin concentrations were higher in plasma than subcutaneous adipose tissue (approximately 25-fold and approximately 2-fold, respectively, P<0.05), whereas MCP-1, IL-6 and IL-8 concentrations were higher in subcutaneous adipose tissue than plasma (approximately 100-fold, approximately 200-fold and approximately 1000-fold, respectively, P<0.05). Resistin concentrations did not differ significantly between compartments. Adipose tissue blood flow (ATBF) showed no regional difference (P>0.05). The intra- and inter-subject variations of all investigated adipokines as well as of ATBF were substantial (coefficient of variation: 4-177%). In conclusion, interstitial leptin concentrations are approximately 2.5-fold higher in subcutaneous, femoral than abdominal adipose tissue, which might be a potential mechanism behind the health-benefits of "pear-shape". Furthermore, subcutaneous adipose tissue has a marked production of pro-inflammatory adipokines.

Objective: The predictive value of perceived risk of breast cancer has previously mainly been compared to statistical risk models. We aimed to compare the women's perceived risk with the observed risk of a breast cancer diagnosis., Methods: Women referred to mammography at a Danish public hospital (N=1021) were asked to complete a questionnaire about 2 weeks before the examination. The women were asked to rate their perceived risk of having breast cancer on a six-point scale with the anchor points very low risk (1) and very high risk (6). The perceived risk was compared with the diagnosis from the mammography 1-3 weeks later. Multivariate logistic regression was used to adjust for age, category of referral, family history of breast cancer, and previous mammography., Results: The questionnaire was completed by 665 (65%) of the women. Compared to women without breast cancer, a larger percentage of women with breast cancer had estimated their risk as high. The risk difference was 16.5% (95% CI 2.5-30.6), using a cut-off between 4 and 5 on the six-point scale. The odds ratio was 3.6 (95% CI 1.4-8.9) after adjustment for age, category of referral, family history of breast cancer, and previous mammography. The sensitivity, specificity, and predictive value were 0.17, 0.96, and 23%, respectively., Conclusion: Women who rated their risk as high had a statistically significant increased risk of having a breast cancer diagnosed at the subsequent mammography. The odds ratio based on the women's risk perceptions remained of the same magnitude after adjustment for category of referral, suggesting that information given by the women contains independent predictive value.

- Abstract
Arterial blood gas analyses provide information about the patient's oxygenation, ventilation, and perfusion. Studies have shown that most errors occur during the pre-analytical phase (the processes prior to the actual analysis of the sample). 1st Automatic is an extra module to the existing blood gas analyzer and a partial automation of the pre-analytical phase which is designed to reduce these errors. The objective was to investigate whether 1st Automatic is compatible to the conventional method. Quantitative measurements of pH, pCO2, pO2, Na+, K+, Ca2+, cGlu, cLac and tHb were performed on 21 paired whole blood specimens collected into the standard blood gas syringes and safePICO syringes, both designed for the ABL800 FLEX. During the collection the pre-analytical errors were minimised. No clinically significant differences were observed in the nine blood gas parameters. The differences were statistically insignificant, with the exception of cNa+, cCa2+ and tHb. There was a good consistency between the results using the new automated procedure and the conventional method. It was not possible to show any clinically significant difference between the two procedures.

- Abstract
The term "endothelial dysfunction" refers to the inability or attenuated effect of the endothelial cells in participating in the relaxation of the adjacent smooth muscle, thus causing less vasodilation. Although endothelial dysfunction is often seen in patients with type 2 diabetes, it does not necessarily follow that insulin resistance and (or) hyperglycemia is causing the inability to respond properly to vasodilatory stimuli. Rather, this could be related to the impact of concomitant cardiovascular risk factors that are almost invariably present in patients with type 2 diabetes. The impact of physical training - or the opposite, inactivity - on endothelial function is not fully elucidated. Some studies have shown positive effects of physical training, whereas others have not. In general, physical training can improve endothelial function when this is impaired. However, physical training does not seem to have any effect on endothelial function when this is normal.

Protein kinase C (PKC) may be involved in growth regulation. In the present study the relationship between body weight, and thereby age, and the activity of PKC in muscle as well as in rapidly growing overloaded muscle were investigated. PKC activity in music was linearly inversely correlated to rat weight in both soleus (r = -0.59, P less than 0.05) and in plantaris (r = -0.74, P less than 0.01) muscles. During compensatory hypertrophy. PKC activity per muscle was maximally increased compared with the contralateral control muscles after 4 days in both soleus (126%) and in plantaris (105%) but had returned to basal levels by the 9th day. The data are in agreement with a role for PKC in muscle growth.

- Abstract
The malignant neuroleptic syndrome is a rare and fatal iatrogenic syndrome which requires rapid and effective symptomatic treatment in a somatic hospital, possibly in an intensive care unit. Relatively little medicine (neuroleptics) is employed in child psychiatry and the syndrome is, therefore, rare in child psychiatric departments. A well-defined syndrome is concerned and it is important to be able to recognise and diagnose this.

15. Airborne lidar measurements of smoke plume distribution, vertical transmission, and particle size.

- Abstract
Observations were made of a dense smoke plume downwind from a forest fire using the ALPHA-1 two-wavelength downward-looking airborne lidar system. Facsimile displays derived from lidar signatures depict plume dimensions, boundary layer height, and underlying terrain elevation. Surface returns are interpreted in terms of vertical transmission as function of cross-plume distance. Results show significantly greater plume attenuation at 0.53-microm wavelength than at 1.06-microm, indicating ~0.1-microm mean particle diameters or the presence of gaseous constituents that absorb the visible radiation. These results demonstrate the potential of multiple-wavelength airborne lidar for quantitative analysis of atmospheric particulate and gaseous constituents.
