4. Association of continuity of carer and women's experiences of maternity care during the COVID-19 pandemic: A cross-sectional survey.

Abstract

BACKGROUND: Recent research highlights the impact of the COVID-19 pandemic on maternity services, although none to date have analysed the association between continuity of carer and how women felt about the changes to pregnancy care and birth plans. AIM: To describe pregnant women's self-reported changes to their planned pregnancy care and associations between continuity of carer and how women feel about changes to their planned care. METHODS: A cross-sectional online survey of pregnant women aged over 18 years in their final trimester of pregnancy in Australia. FINDINGS: 1668 women completed the survey. Most women reported at least one change to pregnancy care and birthing plans. Women receiving full continuity of carer were more likely to rate the changes to care as neutral/positive (p<.001) when compared with women who received partial or no continuity. DISCUSSION: Pregnant women experienced many changes to their planned pregnancy and birth care during the COVID-19 pandemic. Women who received full continuity of carer experienced fewer changes to care and were more likely to feel neutral/positive about the changes than women who did not receive full continuity of carer.

7. Trends in percentages of gestational diabetes mellitus attributable to overweight, obesity, and morbid obesity in regional Victoria: an eight-year population-based panel study

Abstract

Background: Gestational diabetes mellitus (GDM) is the fastest growing type of diabetes in Australia with rates trebling over the past decades partially explained by rising obesity rates and maternal age among childbearing women. Percentage of GDM attributable to obesity has been documented, mostly focusing on metropolitan populations. In parts of regional (areas outside capital cities) and rural Australia where overweight, obesity and morbid obesity are more prevalent, intertwined with socioeconomic disadvantage and higher migrant communities, trends over time in adjusted percentages of GDM attributed to obesity are unknown. Methods: In this population-based retrospective panel study, women, without pre-existing diabetes, delivering singletons between 2010 and 2017 in a tertiary regional hospital that serves 26% of Victoria’s 6.5 million Australian population were eligible for inclusion. Secular trends in GDM by body mass index (BMI) and age were evaluated. The percentage of GDM that would have been prevented each year with the elimination of overweight or obesity was estimated using risk-adjusted regression-based population attributable fractions (AFp). Trends in the AFp over time were tested using the augmented Dickey-Fuller test. Results: Overall 7348 women, contributing to 10,028 births were included. The age of expecting mothers, their BMI, proportion of women born overseas, and GDM incidence significantly rose over time with GDM rising from 3.5% in 2010 to 13.7% in 2017, p < 0.001, increasing in all BMI categories. The incidence was consistently highest among women with obesity (13.8%) and morbid obesity (21.6%). However, the highest relative increase was among women with BMI < 25 kg/m2, rising from 1.4% in 2010 to 7.0% in 2017. 9. Women, clinician and IT staff perspectives on telehealth for enhanced gestational diabetes mellitus management in an Australian rural/regional setting

Abstract

INTRODUCTION: Women in rural and regional areas encounter challenges when accessing care for gestational diabetes mellitus (GDM). A telehealth initiative for GDM care in an urban setting demonstrated positive effects on achieving glycaemic targets without compromising quality of care, but consumer and health service staff perspectives have not been explored. This research aimed to identify the profiles of women accessing care for GDM in a large regional hospital with a rural catchment in Victoria, Australia as well as gain insight into the views of the women with GDM, clinicians and IT staff on the acceptability and feasibility of a GDM telehealth in this setting. METHODS: Clinical and demographic characteristics of women accessing the GDM service between October 2016 and October 2017 were audited. Semi-structured interviews were completed with nine patients, three clinical staff and two IT service staff. Quantitative and qualitative data were analysed descriptively and thematically, respectively. RESULTS: Telehealth was viewed favourably by women and staff, with many perceived benefits identified around mitigating challenges of accessing care, and service capacity and provision. Concerns were raised around potential costs incurred by women and health services in accessing telehealth initiatives. Staff highlighted that moderation of workloads and coordination of telehealth services would be essential to the success of a future telehealth initiative. CONCLUSION: This article contributes important knowledge around GDM care in rural and regional settings and the perspectives of women with GDM, clinicians and technical support staff. Women and health services staff consider telehealth a feasible and acceptable alternative to current GDM care and address many of the barriers and impacts of attending care in person. Perceived benefits to patients and health services need to be balanced against the concerns around the work and costs to deliver GDM telehealth services.

12. Validation of 24-hour dietary recall in Ugandan children by simultaneous weighed food assessment

Abstract

Background: Undernutrition remains highly prevalent in African children, highlighting the need for accurately assessing dietary intake. In order to do so, the assessment method must be validated in the target population. A triple pass 24 hour dietary recall with volumetric portion size estimation has been described but not previously validated in African children. This study aimed to establish the relative validity of 24-hour dietary recalls of daily food consumption in healthy African children living in Mbale and Soroti, eastern Uganda compared to simultaneous weighed food records. Methods: Quantitative assessment of daily food consumption by weighed food records followed by two independent assessments using triple pass 24-hour dietary recall on the following day. In conjunction with household measures and standard food sizes, volumes of liquid, dry rice, or play dough were used to aid portion size estimation. Inter-assessor agreement, and agreement with weighed food records was conducted primarily by Bland-Altman analysis and secondly by intraclass correlation coefficients and quartile cross-classification. Results: 19 healthy children aged 6 months to 12 years were included in the study. Bland-Altman analysis showed 24-hour recall only marginally under-estimated energy (mean difference of 149kJ or 2.8%; limits of agreement -1618 to 1321kJ), protein (2.9g or 9.4%; -12.6 to 6.7g), and iron (0.43mg or 8.3%; -3.1 to 2.3mg). Quartile cross-classification was correct in 79% of cases for energy intake, and 89% for both protein and iron. The intraclass correlation coefficient between the separate dietary recalls for energy was 0.801 (95% CI, 0.429-0.933), indicating acceptable inter-observer agreement. Conclusions: Dietary assessment using 24-hour dietary recall with volumetric portion size estimation resulted in similar and acceptable estimates of dietary intake compared with weighed food records and thus is considered a valid method for daily dietary intake assessment of children in communities with similar diets. The method will be utilised in a sub-study of a large randomised controlled trial addressing treatment in severe childhood anaemia.

Abstract

BSTRACT From 1971-1980, studies were conducted at Fresno, California, to identify and quantify, where possible, the soil and water chemistry, subsurface geologic, hydrologic, biologic, and operational factors that determine the long term (10-year) effectiveness of basin type artificial ground water recharge through alluvial soils. This paper updates previous findings and refers to publications that describe the geology beneath the basins and regional geology that determine the transmission and storage properties for local ground water management and chemical quality enhancement. High quality irrigation water from the Kings River was used for recharge. Construction and land costs for the present expanded facility 83 ha (205.2 ac) using three parcels of land were $1,457,100. The nine-year annual mean costs for only canal water, maintenance, and operation were $110.42/ha·m ($13.62/ac·ft) based on an average recharge rate of 1338 ha·m/yr (10,848 ac·ft/yr) at 86 percent facility efficiency. The measured end of season recharge rate averaged 14.97 ± 0.24 cm/day. The 10-year mean actual recharge rate based on actual water delivered, total ponded area, and total days of recharge was 12.1 cm/day. [ABSTRACT FROM AUTHOR]

34. SOIL AND GROUNDWATER SALINIZATION BENEATH DIVERSIFIED IRRIGATED AGRICULTURE

Abstract

Evaluation of salinization of ground water in an area of diverse irrigation agriculture requires a field-scale approach. Data reduction methods are suggested and illustrated that relate four crop classes (no crop, grapes, row and truck crops, and orchards) to magnitudes and variabilities in soil salinity to a depth of 6.1 m, and evaluate relative importance of the crop classes as contributors to soil salinity. Data on soil salinity were used to obtain a projected ground-water salinity probability curve. The ground-water salinity probability curve based on domestic water well sampling over a 3-year period compared favorably with the projected curve.

35. Ground‐Water Quality Related to Irrigation with Imported Surface or Local Ground Water

Abstract

Ground‐water quality in an arid irrigated area that imports high quality surface water was compared with an adjacent up‐gradient area that uses local pumped ground water. A large irrigation canal separates the two areas which are up‐gradient from the pumping depression of the Fresno‐Clovis, California metropolitan area, which is dependent on ground water. Intensive sampling (154 wells) was done in the fall of 1972 after it was shown by previous sampling that the ground‐water electrical conductivity (EC), NO3−, and Cl−content had not significantly changed since 1967. Diagrammatic distribution maps for ground‐water quality showed great variability. The Wilcoxon‐Mann‐Whitney nonparametric statistical tests showed the significance probabilities for EC, NO3−, and Cl−differences above and below the canal were 0.093, 0.114, and < 0.00006, respectively. The significance of these differences are determined by the reader or decision‐maker by his chosen error probability, the error risk he is willing to take in rejecting the null hypothesis of no difference when it is true. Areas of higher ground‐water NO3−and Cl−were generally related to soil drainage‐recharge and agricultural use. The use of local ground water without supplemental surface supplies above the canal has led to ground‐water EC, NO3−, and Cl−concentrations to be 9.5, 18.6, and 91.8% higher, respectively, than below the canal which uses mostly high quality surface water.

36. Hammer Seismic Timing as a Tool for Artificial Recharge Site Selection

Abstract

Hand‐carried hammer impact seismic timing equipment utilizes the physics of seismic wave refraction to explore stratigraphic changes deeper then those found on soils maps. In alluvial areas, artificial recharge and liquid waste disposal sites are selected from sandy, single‐grained soil series noted for their high water‐transmitting properties through the surface 2 m. The surface horizon seismic velocity exhibited by these soils in the San Joaquin Valley has a very narrow range (V1= 315–440 m/sec) which gives good definition to the refraction off the second horizon (V2= 610–2,100 m/sec). Accurate information on depth and horizontal continuity of this second horizon can be obtained in areal surveys. A third horizon may often be described, but its areal continuity is usually masked by intermediate discontinuous layers above it of equal or lesser refractivity. All these horizons are of hydraulic significance for ground‐water recharge by water spreading.
