We report on a rare case of major intra-abdominal bleeding that occurred in a patient undergoing late breast reconstruction with a transversus rectus abdominis musculocutaneous (TRAM) flap. During the flap-raising procedure, a small rupture was found at the base of the superior mesenteric artery, probably caused by electric current. The ruptured vessel required immediate laparotomy and suturing, and the final outcome of the patient was good. The massive bleeding, almost 8-fold as compared with three normal cases, caused clear activation of coagulation seen with elevated TAT values at the end of the operation, and remarkably enhanced fibrin turnover with elevated D-dimer, up to 25-fold from baseline, on the first postoperative day. As a result of major bleeding, the fibrinolytic system was also activated: a biphasic elevation of tPA was seen, first immediately after the bleeding event at 2 h, and later on the first postoperative day, followed by fibrinolytic shutdown associated with increased PAI-1 values.

Summary Coagulation and fibrinolysis activities in relation to trauma, surgery and thrombosed microanastomoses were studied during free-flap surgery in eight patients with lower-extremity defects due to recent trauma or chronic ulcers. One patient had an intraoperative thrombosis, and three more patients required reoperations on the same day due to postoperative thromboses; one of these also required a second reoperation due to flap failure. The baseline level of fibrinogen was slightly elevated in all patients except one, and was significantly higher in the patients who underwent reoperation. At the end of the primary surgery, distinct thrombin generation (TAT and F1+2) was seen in three patients with excessive bleeding, and all three later underwent reoperations. One of these patients generated excessive thrombin on the eighth postoperative day, upon removal of a necrotic flap. Thrombin generation (F1+2) was also seen at baseline in the patient with the intraoperative thrombosis, and persisted on the first postoperative day. D-dimer at baseline was higher in patients with recent trauma, and in two of these, both of whom underwent reoperations on the same day, D-dimer remained high perioperatively. Resistance to fibrinolysis with increased PAI-1 levels was seen in these two patients at the time of reoperation. In all, TAT and F1+2 were associated with the threat of flap failure. A preoperative hypercoagulable state and excessive bleeding during the operation were predictors of reoperation. The markers for coagulation and fibrinolysis could be used preoperatively to target antithrombotic control, and postoperatively to detect the threat of flap failure. Meticulous haemostasis during surgery might help to diminish the need for reoperations.

Cancer patients are subjected to increased systemic risk of thrombotic events and may therefore be at higher risk of even local thrombosis compromising the outcome of reconstructive microsurgery. Coagulation and fibrinolysis activities were studied serially during and after reconstructive microsurgery in seven patients with oropharyngeal cancer or sarcoma in the extremities. A preoperative hypercoagulable state was found in four patients (elevated fibrinogen, TAT, F1+2, or D-dimer); two of these patients also had a local thrombotic event. In all patients, the plasma markers generally varied perioperatively: fibrinogen decreased, whereas TAT and subsequently D-dimer increased at the end of the operation. However, tPA and PAI-1 remained unaltered, except in the patients with thrombosis in whom PAI-1 activity increased progressively during the operation. F1+2 was also clearly elevated in these two patients at the time of thrombosis. Preoperative assessment of hypercoagulability for this group of patients could be helpful in targeting meticulous antithrombotic protection.

The association between obesity and the outcome of pedicled transverse rectus abdominis musculocutaneous (TRAM) flaps was studied in 12 patients. Obesity was assessed preoperatively by body mass index (BMI) and waist:hip circumference ratio (WHCR). The thickness of the abdominal fat and muscles was measured preoperatively with ultrasonography on the abdomen and during the nine postoperative months on the flap. Marginal or fat necrosis was more common among patients with lower body type fat distribution (WHCR less than 0.80) than in patients with medium or upper body type fat distribution. BMI and abdominal muscle and fat thicknesses were not associated with marginal or fat necrosis of the flaps. We conclude that lower body (female type) fat distribution may be associated with marginal cutaneous or fat necrosis in pedicled TRAM flaps.

According to clinical studies there are fewer circulatory complications with the free TRAM flap than with the pedicled TRAM flap. In this haemodynamic study, we measured perioperative cutaneous blood flow in 11 free TRAM flaps with laser Doppler flowmetry (LDF) and transcutaneous oxygen tension (ptcO 2 ). We also studied 4 pedicled TRAM flaps with an additional microvascular anastomosis. Our results suggest that skin blood flow in the free TRAM flap is superior compared to our previously presented results of the pedicled TRAM flaps, and also superior to pedicled TRAM flaps with additional microvascular anastomoses of the inferior epigastric vessels to "supercharge" them.

A study was conducted to assess the effect of intraoperatively administered inotropic agents on blood flow in the recipient and donor vessels, during breast reconstruction with a muscle sparing free TRAM flap. Twenty-one consecutive patients were randomized into 3 groups receiving either dopamine, dobutamine, or placebo. When the flap and all vessels had been fully dissected but not yet divided, the study drug was administered intravenously for 15 minutes. Hemodynamic parameters and transit-time flow of the thoracodorsal and inferior epigastric arteries were monitored. Both dobutamine and dopamine infusions resulted in significant raises in cardiac output and mean arterial pressure. However, while dobutamine resulted in a higher cardiac output (P = 0.001) and a decrease in systemic vascular resistance (P = 0.028), the increase in mean arterial pressure was greater with dopamine (P = 0.002). Only the dobutamine group showed increased blood flow, in both the thoracodorsal (P = 0.043) and the inferior epigastric (P = 0.043) arteries. If vasoactive agents are needed during microvascular anesthesia, dobutamine seems to be more advantageous than dopamine.

Endothelin-1 (ET-1) may contribute to vasoconstriction and flap blood flow during microvascular surgery. Calcium antagonists have suppressed ET-1 release in some studies. The effect of 5 mg of felodipine, a vasodilating calcium antagonist, administered the evening and morning before a microvascular TRAM (transverse rectus abdominis musculocutaneous) flap breast reconstruction, on perioperative plasma ET-1 concentrations, peripheral temperature gradient (Tgrad), rectal temperature (Trect), flap skin blood flow with transcutaneous oxygen tension (ptcO2), heart rate (HR) and mean arterial pressure (MAP) was evaluated in this clinical, randomised, double blind, prospective study. Felodipine did not cause any statistically significant changes in ET-1 levels, Tgrad, Trect, Ptc O2, flap survival or pre- or intraoperative MAP. HR was significantly higher in the felodipine group before induction and at 10 min postoperatively. We conclude that in patients undergoing a microvascular TRAM flap breast reconstruction, preoperatively administered felodipine has no effect on perioperative ET-1 levels, temperature changes, or flap skin blood flow.

The role of endothelin-1, a potent vasoconstrictor released by vascular endothelium, in the vasoconstriction that develops after prolonged operations is not clear. This study was performed in order to determine if there was any relationship between endothelin-1 and the degree of vasoconstriction during prolonged plastic surgery. Plasma concentrations of endothelin-1, skin-forearm temperature gradient (Tgrad), rectal temperature, mean arterial pressure (MAP) and heart rate (HR) were measured at nine predetermined times before, during and after operation in nine women undergoing breast reconstruction with a pedicled transverse rectus abdominis musculocutaneous flap. Development of cutaneous or fat necrosis of the flap was assessed clinically and with ultrasound. Concentrations of endothelin-1 before induction were increased (median 8.9 (25-75% quartiles 5.5-12.5) pg ml-1). During operation they were approximately 3 pg ml-1 and after operation approximately 5 pg ml-1. Tgrad was approximately 4 degrees C before induction and after operation, indicating marked vasoconstriction; during operation it was about zero, indicating vasodilatation. There was a statistically significant correlation between endothelin-1 concentrations and Tgrad (Spearman non-linear correlation) (r = 0.32, P = 0.004) and between endothelin-1 and MAP (r = 0.25, P = 0.02), but not between endothelin-1 and HR or development of minor cutaneous or fat necrosis of the flap (five patients). We conclude that increased plasma concentration of endothelin-1 is associated with the extent of peripheral vasoconstriction.

Changes in the cutaneous blood flow of the pedicled TRAM flap for breast reconstruction were studied in 15 patients with laser Doppler flowmetry (LDF) and transcutaneous oxygen tension (ptcO2). One patient was excluded from the study. The LDF value increased to 127 +/- 15% of the base line on the random and to 151 +/- 13% (p less than 0.01) on the axial side after dissection of the random side of the flap. Ligation of the deep inferior epigastric artery caused a decrease to 57 +/- 8% (p less than 0.001) on the random and to 78 +/- 11% on the axial side. The random side ptcO2 decreased from 48 +/- 2 mmHg [6.4 +/- 0.3 kPa] to 17 +/- 5 mmHg [2.3 +/- 0.7 kPa] (p less than 0.001) after dissection of the flap and remained near zero for one week. Eight patients developed minor cutaneous necrosis on the random side, probably because the superior epigastric system could not adequately nourish the TRAM flap. Low LDF and ptcO2 values after pedicle ligation and a negative response to oxygen stimulation predicted skin necrosis.
