4. Relationship between Body Roundness Index and cognitive impairment in middle-aged and older adults: a population-based cross-sectional study.

Abstract

Background: Cognitive impairment is a growing public health concern, particularly in aging populations. Obesity, as measured by various indices, has been linked to cognitive decline, but the relationship between Body Roundness Index (BRI) and cognitive impairment remains unclear. This study aims to evaluate the association between BRI and cognitive impairment in a rural, low-income, low-education population in China and to determine if BRI can be used as an independent predictor of cognitive decline. Methods: This cross-sectional study included the participants aged 35–95 years from rural Tianjin, China. The mean age of the study population was 64.35 ± 7.58 years. Data were collected through face-to-face interviews, physical examinations, and laboratory tests. Cognitive function was assessed using the Mini-Mental State Examination (MMSE), and BRI was calculated and grouped into quartiles. Univariate and multivariate logistic regression analyses were performed to examine the relationship between BRI and cognitive impairment. Subgroup analyses were conducted to explore interactions between BRI, age, gender, and hypertension. The dose–response relationship was analyzed using restricted cubic spline models. Results: Of the participants, 36.5% had cognitive impairment. Multivariate analysis showed that women, individuals aged 65 and over, and those with hypertension had a higher risk of cognitive impairment. Participants in the second quartile of BRI had a 31% lower risk of cognitive impairment compared to the first quartile (OR: 0.69, 95% CI: 0.51–0.94, p = 0.017). Subgroup analysis revealed that BRI was significantly associated with cognitive impairment in individuals under 65, but not in older participants. The dose–response relationship between BRI and MMSE score showed an inverted U-shaped curve, with the weakest association observed around a BRI of 4.49. Conclusion: Body Roundness Index, in conjunction with age, gender, and hypertension, can serve as a useful predictor of cognitive impairment, particularly in younger populations. Early identification of individuals at risk through BRI may facilitate timely interventions, reducing the burden of cognitive decline on patients and healthcare systems. [ABSTRACT FROM AUTHOR]

5. Pulse Pressure and Other Cardiovascular Risk Factors Associated with Multiple Carotid Plaques in a Rural Chinese Population: A Population-Based Cross-Sectional Study.

Abstract

This study aimed to investigate the association between these Blood pressure (BP) components examined in this study, including systolic blood pressure (SBP), diastolic blood pressure (DBP), pulse pressure (PP), and mean arterial pressure (MAP), with the presence of multiple carotid plaques in a low-income rural Chinese population. Methods: This population-based cross-sectional study was conducted from April 2014 to January 2015, involving participants from the Tianjin Brain Study. Participants aged 45 years and older with diagnosed carotid plaques were included. Data on demographics, medical history, and lifestyle factors were collected through interviews and existing records. Blood pressure measurements were taken in a quiet room, following the standard procedures recommended by the American Hypertension Association (AHA), and ultrasonographic examinations were performed to identify and quantify carotid plaques. Multivariate logistic regression model was used to explore the association between blood pressure components and multiple plaques. The blood pressure component with a positive univariate analysis was included in different models, each adjusted for age, sex, body mass index (BMI), and the ratio of high-density lipoprotein cholesterol (HDL-C) to low-density lipoprotein cholesterol (LDL-C). Results: The study found that 41.8% of participants had multiple carotid plaques, with a higher prevalence in men compared to women. Multivariate logistic regression analysis revealed that each 1-mmHg increase in systolic BP (SBP) was associated with a 0.9% increase in the prevalence of multiple carotid plaques (OR = 1.009; 95% CI 1.004– 1.014; P < 0.001). Each 1-mmHg increase in pulse pressure (PP) was associated with a 1.2% increase (OR = 1.012; 95% CI 1.006– 1.018; P < 0.001), and each 1-mmHg increase in mean arterial pressure (MAP) was associated with a 1.1% increase (OR = 1.011; 95% CI 1.003– 1.019; P = 0.005). Participants with a history of hypertension had a significantly higher prevalence of multiple carotid plaques compared to normotensive individuals. Notably, grade 2 hypertension showed a significant association with multiple carotid plaques (OR = 1.554; 95% CI 1.135– 2.127; P = 0.006). In addition, male sex, older age, and low BMI were all associated with a higher risk of multiple carotid plaques (P all< 0.05). Conclusion: This study provides critical evidence on the relationship between BP components and multiple carotid plaques, with significant implications for patients, physicians, and society. By prioritizing BP management, particularly focusing on PP, which demonstrates the strongest association with carotid plaques, as well as targeting higher-risk populations such as males, older individuals, and those with low BMI, preventive measures against carotid atherosclerosis can be enhanced. Abstract

Genetic studies of blood pressure (BP) traits to date have been performed on conventional measures by brachial cuff sphygmomanometer for systolic BP (SBP) and diastolic BP, integrating several physiologic occurrences. Genetic associations with central SBP (cSBP) have not been well-studied. Genetic discovery studies of BP have been most often performed in European-ancestry samples. Here, we investigated genetic associations with cSBP in a Chinese population and functionally validated the impact of a novel associated coiled-coil domain containing 93 (CCDC93) gene on BP regulation. An exome-wide association study (EWAS) was performed using a mixed linear model of non-invasive cSBP and peripheral BP traits in a Han Chinese population (N = 5,954) from Beijing, China genotyped with a customized Illumina ExomeChip array. We identified four SNP-trait associations with three SNPs, including two novel associations (rs2165468-SBP and rs33975708-cSBP). rs33975708 is a coding variant in the CCDC93 gene, c.535C>T, p.Arg179Cys (MAF = 0.15%), and was associated with increased cSBP (β = 29.3 mmHg, P = 1.23x10-7). CRISPR/Cas9 genome editing was used to model the effect of Ccdc93 loss in mice. Homozygous Ccdc93 deletion was lethal prior to day 10.5 of embryonic development. Ccdc93+/- heterozygous mice were viable and morphologically normal, with 1.3-fold lower aortic Ccdc93 protein expression (P = 0.0041) and elevated SBP as compared to littermate Ccdc93+/+ controls (110±8 mmHg vs 125±10 mmHg, P = 0.016). Wire myography of Ccdc93+/- aortae showed impaired acetylcholine-induced relaxation and enhanced phenylephrine-induced contraction. RNA-Seq transcriptome analysis of Ccdc93+/- mouse thoracic aortae identified significantly enriched pathways altered in fatty acid metabolism and mitochondrial metabolism. Plasma free fatty acid levels were elevated in Ccdc93+/- mice (96±7mM vs 124±13mM, P = 0.0031) and aortic mitochondrial dysfunction was observed through aberrant Parkin and Nix protein expression. Together, our genetic and functional studies support a novel role of CCDC93 in the regulation of BP through its effects on vascular mitochondrial function and endothelial function. Author summary: More than 1000 loci have been reported in previous gene discovery efforts for blood pressure and hypertension. cSBP is a unique blood pressure trait that has not been as extensively studied as peripheral BP, and correlates of cSBP may provide a better indication of future cardiovascular events. In this study we replicated several BP associations in a Han Chinese cohort and found a novel association with a coding variant in CCDC93 (rs33975708, c.535C>T, p.Arg179Cys, MAF 0.15%) which was strongly associated with higher cSBP. To model loss of Ccdc93 and functionally validate the human genetic association, a new transgenic mouse was created. Ccdc93 homozygous deletion was embryonic lethal, and mice with heterozygous loss of Ccdc93 exhibited impaired arterial relaxation and enhanced contractile responses. Aortic RNA-seq analysis of Ccdc93+/- heterozygous mice identified metabolic dysregulation and mitochondrial dysfunction, which was validated by features of elevated plasma free fatty acids and increased aortic Parkin and Nix protein expression. Improved knowledge of the genes involved in BP regulation is needed to develop effective therapies to manage elevated BP, and this study identified the gene CCDC93 as a novel regulator of blood pressure and vascular function. Abstract

Background: The coronavirus disease (COVID-19) pandemic has accentuated the need for effective clinical skills training in infectious diseases. This study aimed to explore the influencing factors of infectious disease clinical skills training based on scenario simulation teaching for medical staff in China. Methods: This hospital-based, cross-sectional study was conducted at the Third People's Hospital of Shenzhen between March and December 2022. Scenario simulation teaching was applied, and factors such as gender, educational level, professional background, and previous experience were examined to determine their impact on qualification outcomes. Results: The study included participants primarily between the ages of 20–40 years, with a higher proportion of women holding university degrees. Nurses and physicians were more likely to qualify, indicating the significance of professional backgrounds. Women showed a higher likelihood of qualifying than men and higher educational attainment correlated with better qualification rates. Prior experience with protective clothing in isolation wards was a significant determinant of successful qualification. Multivariate analysis underscored the influence of sex, education, and previous experience on training effectiveness. Conclusion: Scenario simulation is an effective strategy for training clinical skills in treating infectious diseases. This study highlights the importance of considering sex, education, professional background, and prior experience when designing training programs to enhance the efficacy and relevance of infectious disease training. 40. Health care reform and stroke prognosis in low-income Chinese populations from 1992 to 2018

Abstract

Background Improving maternal and newborn health remains one of the most critical public health challenges, particularly in low- and lower-middle-income countries. To overcome this challenge, interventions to improve the continuum of care based on real-world settings need to be provided. The Ghana Ensure Mothers and Babies Regular Access to Care (Embrace) Implementation Research Team conducted a unique intervention program involving over 21 000 women to improve the continuum of care, thereby demonstrating an intervention program’s effectiveness in a real-world setting. This study evaluates the implementation process of the Embrace intervention program based on the RE-AIM framework. Methods A cluster-randomized controlled trial was conducted in 32 sub-district-based clusters in Ghana. Interventions comprised of four components, and to evaluate the implementation process, we conducted baseline and endline questionnaire surveys for women who gave birth and lived in the study site. The key informant interviews of health workers and intervention monitoring were conducted at the health facilities in the intervention area. The data were analyzed using 34 components of the RE-AIM framework and classified under five general criteria (Reach, Effectiveness, Adoption, Implementation, and Maintenance). Results In total, 1480 and 1490 women participated in the baseline and endline questionnaire survey, respectively. In the intervention area, 83.8% of women participated (reach). The completion rate of the continuum of care increased from 7.5% to 47.1%. Newborns who had danger signs immediately after birth decreased after the intervention (relative risk = 0.82, 95% confidence interval = 0.68-0.99) (effectiveness). In the intervention area, 94% of all health facilities participated. Mothers willing to use their continuum of care cards in future pregnancies reached 87% (adoption). Supervision and manual use resolved the logistical and human resource challenges identified initially (implementation). The government included the continuum of care measures in their routine program and developed a new Maternal and Child Health Record Book, which was successfully disseminated nationwide (maintenance). Conclusions Following the RE-AIM framework evaluation, the Embrace intervention program was considered effective and as having great potential for scaling across in real-world settings, especially where the continuum of care needs to be improved.

Thus, we aimed to explore the secular trends in the prognosis of stroke in a low-income population in China.Methods: This was a population-based stroke surveillance study that included all of the participants in the Tianjin Brain Study. Stroke management and outcomes were evaluated for all stroke patients. Trends in the burden of stroke were assessed as annual percentage changes.Results: Overall, 1462 stroke cases occurred from 1992 to 2018, and 58.7% of patients were male. The rates of diagnosis by neuroimaging and of hospitalization increased noticeably across sexes and ages for all stroke types, and the greatest increases were observed among elderly women; diagnosis by neuroimaging increased by 4.7% and the rate of hospitalization increased by 12.4% (all P< 0.001).
