Abstract

Abstract Aggressive B-cell non-Hodgkin lymphomas (NHL) in children, adolescents, and young adults (CAYA) include Burkitt lymphoma (BL), diffuse large B-cell lymphoma (DLBCL), and a subset of high-grade tumors with features intermediate between these entities whose genetic and molecular profiles have not been completely elucidated. In this study, we have characterized 37 aggressive B-NHL in CAYA, 33 with high-grade morphology, and 4 DLBCL with MYC rearrangement (MYC-R), using targeted next-generation sequencing and the aggressive lymphoma gene expression germinal center B-cell-like (GCB), activated B-cell-like (ABC), and dark zone signatures (DZsig). Twenty-two tumors had MYC-R without BCL2 breaks, and two MYC-non-R cases had BCL6 translocations. MYC-R cases, including DLBCL, carried BL-related mutations and copy number alterations. Conversely, MYC-non-R lymphomas had alterations in the B-cell receptor signaling/NF-κB pathway (71%). DZsig was expressed in 12/13 of MYC-R tumors but only in 2/10 of MYC-non-R GCB tumors (P

Abstract

Quantum Computing (QC) is a rapidly evolving research field that has garnered significant attention due to its potential to revolutionize various domains such as cryptography, optimization, and machine learning. In this article, we conduct an extensive analysis of the evolution of QC research within the realm of High Performance Computing (HPC) over a span of ten years, up to 2023. Through bibliometric analysis and advanced science mapping techniques, we uncover key thematic areas that have emerged in the field, including quantum algorithms, simulation, parallel-computing, deep learning, machine learning, and encryption. This analysis highlights the interdisciplinary nature of QC, which intersects with disciplines such as physics, mathematics, computer science, and materials science. Furthermore, our study elucidates the close relationship between HPC and QC, showcasing how advancements in one field can significantly impact the other. The findings of this study not only provide valuable insights into the past trends and research landscape but also serve as a guide for future research directions, enabling the advancement of knowledge and fostering innovation in computer science. 3. Rapid loss of organic carbon and soil structure in mountainous grassland topsoils induced by simulated climate change

Abstract

Mountainous grassland soils are considered one of the most unique biological hotspots, rich in organic carbon (OC). At the same time, they are exposed to great threats, as climate warming is more pronounced in mountainous regions than in lowland areas. In this study, we assessed the effect of simulated warming (+1K, +2K, and + 3 K) on OC stocks and soil structure in grassland soils of the Northern Limestone Alps in Germany by translocating plant-soil mesocosms from high- (1260 m a.s.l., Rendzic Phaeozem) and mid- (860 m a. s. l., Haplic Cambisol) to low-elevation (600 m a.s.l). Plant-soil mesocosms were exposed to both extensive and intensive grassland management practices. Four years after translocation, we observed a rapid decrease of topsoil SOC stocks under intensive (−1.0 t C ha yr−1) and extensive management (-2.2 t C ha yr−1), under the highest temperature increase. Intensive management with about 1 t C ha−1 yr−1 higher manure C return than extensive management (1.6 vs. 0.8 t C ha−1 yr−1 intensive and extensive, respectively) may explain the difference in SOC losses between different management treatments. Under both management practices, the loss of SOC was mainly associated with a decrease of large macroaggregates, at both management practices. In addition, different aggregate specific OC loss rates resulted in an altered distribution of OC among the aggregate size classes. Our study provides evidence that simulated climate change induced a rapid and substantial decline of SOC in mountainous, OC-rich grassland soils, which may be attributed to decreased physical OC protection within large macroaggregates. Optimized grassland management in form of increased application of organic fertilizers could only partially offset the SOC loss by improved formation of small macroaggregates.

Abstract

Background: The emergence of new SARS-CoV-2 variants with significant immune-evasiveness, the relaxation of measures for reducing the number of infections, the waning of immune protection (particularly in high-risk population groups), and the low uptake of new vaccine boosters, forecast new waves of hospitalizations and admission to intensive care units. There is an urgent need for easily implementable and clinically effective Early Warning Scores (EWSs) that can predict the risk of complications within the next 24–48 hr. Although EWSs have been used in the evaluation of COVID-19 patients, there are several clinical limitations to their use. Moreover, no models have been tested on geographically distinct populations or population groups with varying levels of immune protection. Methods: We developed and validated COVID-19 Early Warning Score (COEWS), an EWS that is automatically calculated solely from laboratory parameters that are widely available and affordable. We benchmarked COEWS against the widely used NEWS2. We also evaluated the predictive performance of vaccinated and unvaccinated patients. Results: The variables of the COEWS predictive model were selected based on their predictive coefficients and on the wide availability of these laboratory variables. The final model included complete blood count, blood glucose, and oxygen saturation features. To make COEWS more actionable in real clinical situations, we transformed the predictive coefficients of the COEWS model into individual scores for each selected feature. The global score serves as an easy-to-calculate measure indicating the risk of a patient developing the combined outcome of mechanical ventilation or death within the next 48 hr. Conclusions: The COEWS score predicts death or MV within the next 48 hr based on routine and widely available laboratory measurements. The extensive external validation, its high performance, its ease of use, and its positive benchmark in comparison with the widely used NEWS2 position COEWS as a new reference tool for assisting clinical decisions and improving patient care in the upcoming pandemic waves. Abstract

PurposeThis study aims to analyze the lean supply chain management (LSCM) strategy's role as a mechanism to address technology uncertainty and provide organizations with competitive advantage.Design/methodology/approachAn empirical study was conducted of 276 Spanish focal firms in industrial sectors that occupy an intermediate position in the supply chain to investigate the influence of technology uncertainty on LSCM implementation and the latter's effect on operational performance and competitiveness. A covariance-based structural equation model (CB-SEM) was used to test three hypotheses.FindingsTechnological uncertainty encourages progress in the implementation of lean throughout the supply chain and so is a mechanism that not only brings stability to the focal company but also improves its performance and puts it in a better competitive position.Practical implicationsManagers are suggested to consider the strategic integration with supply chain partners and the establishment of long-term relationships based on trust and commitment advocated by LSCM to enhance organizations' capabilities and effectively and flexibly respond to technological changes.Originality/valueThis study focuses on the effects of environmental uncertainty on the supply chain. The past literature has focused on the behavior of individual firms to deal with uncertainty, but this work shifts the level of analysis to the supply chain. Therefore, the strategic change to deal with what is happening in the environment is now switched to the supply chain level.

Abstract

Post-transplant lymphoproliferative disorders (PTLD) represent a broad spectrum of lymphoid proliferations, frequently associated with Epstein-Barr Virus (EBV) infection. The molecular profile of pediatric monomorphic PTLDs (mPTLD) has not been elucidated and it is unknown whether they display similar genetic features as their counterpart in adult and immunocompetent (IMC) pediatric patients. In this study, we investigated 31 pediatric mPTLD after solid organ transplantation, including 24 diffuse large B-cell lymphomas (DLBCL), mostly classified as activated B-cell, and seven Burkitt lymphoma (BL), 93% of which were EBV positive. We performed an integrated molecular approach, including fluorescence in situ hybridization, targeted gene sequencing and copy-number (CN) arrays. Overall, PTLD-BL carried mutations in MYC,ID3, DDX3X, ARID1A or CCND3 resembling IMC-BL, higher mutational burden than PTLD-DLBCL and less CN alterations than IMC-BL. PTLD-DLBCL showed a very heterogeneous genomic profile with fewer mutations and CN alterations than IMC-DLBCL. Epigenetic modifiers and genes of Notch pathway were the most recurrently mutated in PTLD-DLBCL (both 28%). Mutations in cell cycle and Notch pathways correlated with worse outcome. All seven PTLD-BL were alive after treatment with pediatric B-cell Non-Hodgkin Lymphoma protocols, whereas 54% of DLBCL patients were cured with immunosuppression reduction, rituximab and/or low-dose chemotherapy. These findings highlight the low complexity of pediatric PTLD-DLBCL, their good response to low intensity treatment and the shared pathogenesis between PTLD-BL and EBV+ IMC-BL. We also suggest new potential parameters that could help in the diagnosis and the design of better therapeutic strategies for these patients.

Abstract

Abstract Background Memory responses to the antigens that an individual encounters throughout life may vary with the intensity and duration of antigen contacts or even with changes in immune status over time. This work aims to characterise specific responses to latent CMV, seasonal influenza and novel SARS-CoV-2 infections in immunocompetent individuals over 60 years of age. Specific cellular and humoral responses were identified by IFN-γ and granzyme B release by ELISpot and antibody level measurement. T lymphocyte subpopulation phenotypes were characterised by flow cytometry. Results Cellular and humoral responses to these viruses were detected in almost all patients. Influenza and SARS-CoV-2 cellular responses were positively correlated. There was no significant correlation between CMV and influenza or SARS-CoV-2 responses although both were consistently lower in CMV-seropositive patients. CMV responses were negatively correlated with the levels of the least differentiated subsets of T lymphocytes, and positively correlated with the most differentiated ones, contrary to what happened with the influenza responses. Nevertheless, SARS-CoV-2 cellular responses were negatively correlated with the most differentiated CD8+ T lymphocytes, while humoral responses were negatively correlated with the least differentiated T lymphocytes. Responses to the three viruses were correlated with a Th1/Th2/Th17 balance in favour of Th1. Conclusions The results indicate that memory responses differ depending on the durability of the antigen stimulus. Cellular responses to novel pathogens resemble those generated by seasonal but not CMV infection. Subpopulation distribution and the level of specific T lymphocytes against previous pathogens could be used as immunocompetent status biomarkers in older adults reflecting their ability to generate memory responses to new pathogens.

Abstract

Abstract Background Multiple trauma injuries are the leading cause of death and disability in people under the age of 45 and require prompt and specialised care. However, medical and nursing education programmes do not always include specific training in emergency pre-hospital care, resulting in a lack of basic practical skills in trauma management. Objective To develop and validate two instruments for assessing nursing students’ competence in cervical collar and spinal board application in simulated pre-hospital emergency scenarios. Method This is an instrumental study that involves the development of two assessment instruments and the evaluation of their psychometric properties in a sample of 392 nursing students. Content validity was assessed using expert judgement, by calculating the content validity ratio (CVR) for each item and the scale level content validity index average (S-CVI/Ave) for the instruments. Exploratory factor analysis using the MINRES extraction method and Promax rotation was performed to analyse the performance of the items and structure of the rubrics. Internal consistency was analysed using the Omega coefficient and inter-rater agreement was assessed using Cohen’s Kappa coefficient. Results Initially, two rubrics were obtained: one with six items for cervical collar placement (S-CVI/Ave = 0.86) and one with nine items for spinal board placement (S-CVI/Ave = 0.81). Both had a single-factor structure, with all items having factor loadings greater than 0.34 for the cervical collar rubric and 0.56 for the spinal board rubric, except for item 2 of the cervical collar rubric (λ = 0.24), which was subsequently removed. The final cervical collar rubric (five items) had an overall internal consistency of 0.84 and the spinal board rubric had an overall internal consistency of 0.90, calculated using the Omega statistic. The weighted Kappa coefficient for each item ranged from acceptable (0.32) to substantial (0.79). These results show that we have successfully developed two sufficiently valid instruments to assess the immobilisation competencies proposed in the objective of the study. Conclusion Whilst further research is needed to fully establish their psychometric properties, these instruments offer a valuable starting point for evaluating nursing students' competence in cervical collar and spinal board application in simulated pre-hospital scenarios.

23. Long-term resilience of mineral associated organic matter in soils of a Mediterranean forest burned in 1994 at different severities

Abstract

Wildfires are a common disturbance in Mediterranean ecosystems; nevertheless, in temperate and higher latitude ecosystems the frequency and severity of wildfires increased because of prolonged heat waves, drought periods and changes in fire regime. High wildfire frequency and increased burned surface area may affect soil organic carbon (SOC) dynamics. In our study, we assessed the effects on different SOC pools (labile, intermediate and passive) in two areas of the Cadiretes Massif in Spain 28 years after a wildfire event. The area was burned at different severities (LS: low severity; HS: high severity) and were compared with a control (unburned) adjacent area. A physical fractionation method was used to isolate the OC and associated to dissolved organic carbon (DOC), particulate organic matter (POM) the mineral associated organic matter (MAOM). In addition, we used 13C NMR spectroscopy to get information about the quality of the different OC fractions. Results displayed that an increase in wildfire severity caused a decrease of SOC and N in the bulk soil. This was mainly observed in the labile fractions (DOC; POM). No differences were observed in the OC associated to the MAOM with the increase of the burning severity indicating that the OC protected in the mineral fraction was resilient towards wildfires.

Abstract

Purpose The purpose of this paper is to systematically identify the key intellectual developments of the lean supply chain management (LSCM) field from its very first beginning by analyzing the publications that have been highly cited. Design/methodology/approach The citation classics method has been used to examine the origins and evolution of the research field. In total, 74 articles have been identified as having the highest impact on LSCM research published from its beginnings to the present day. Key aspects and links among the scientific contributions are studied by descriptive, bibliometric, network and content analysis. Findings Publication and citation trends, methodologies used, top journals, most influential authors, institutions and countries, collaboration patterns and evolution of the most used keywords are some of the aspects studied. Three main research areas are identified: LSCM foundations and implementation, lean-agile supply chain and performance assessment and the role of sustainability, which offers a holistic view of the field and enables the identification of potential avenues for future research susceptible to contribute to advancing the knowledge on LSCM. Originality/value This study provides a useful insight into the development of the LSCM field and can help researchers as it explores the state-of-the-art of LSCM by identifying the most relevant contributions in the area, the main research lines addressed by the scientific community in this topic and opportunities for further research on LSCM for years to come.

This paper presents a systematic literature review (SLR) of the current knowledge about Lean Supply Chain Management (LSCM) and performance relationships. The purpose is to identify the studied aspects set and to propose a novel classification of the literature on LSCM and performance relationships and to discuss the conceptual and empirical evidence that identifies existing interrelationships. The analysis has enabled two research lines to be addressed: (a) LSCM performance-based models, and (b) LSCM’s impact on performance. Since the beginning of the COVID-19 pandemic, Hospital Universitario 12 de Octubre (H12O) evolved its EHRs: it identified, modeled and standardized the concepts related to this new disease in an agile, flexible and staged way. Thus, data from more than 200,000 COVID-19 cases were extracted, transformed, and loaded into an i2b2 repository. This effort allowed H12O to share data with worldwide networks such as the TriNetX platform and the 4CE Consortium.

One approach to verifying the quality of research data obtained from EHRs is auditing how complete and correct the data are in comparison with those collected by manual and controlled methods. This study analyzed data quality of an EHR-derived dataset for COVID-19 research, obtained during the pandemic at Hospital Universitario 12 de Octubre. Data were extracted from EHRs and a manually collected research database, and then transformed into the ISARIC-WHO COVID-19 CRF model. Subsequently, a data analysis was performed, comparing both sources through this convergence model. More concepts and records were obtained from EHRs, and PPV (95% CI) was above 85% in most sections. In future studies, a more detailed analysis of data quality will be carried out.

Abstract

Aims Consequences of climate change and land use intensification on the nitrogen (N) cycle of organic-matter rich grassland soils in the alpine region remain poorly understood. We aimed to identify fates of fertilizer N and to determine the overall N balance of an organic-matter rich grassland in the European alpine region as influenced by intensified management and warming. Methods We combined 15N cattle slurry labelling with a space for time climate change experiment, which was based on translocation of intact plant-soil mesocosms down an elevational gradient to induce warming of +1 °C and + 3 °C. Mesocosms were subject to either extensive or intensive management. The fate of slurry-N was traced in the plant-soil system. Results Grassland productivity was very high (8.2 t - 19.4 t dm ha−1 yr−1), recovery of slurry 15N in mowed plant biomass was, however, low (9.6–14.7%), illustrating low fertilizer N use efficiency and high supply of plant available N via mineralization of soil organic matter (SOM). Higher 15N recovery rates (20.2–31.8%) were found in the soil N pool, dominated by recovery in unextractable N. Total 15N recovery was approximately half of the applied tracer, indicating substantial loss to the environment. Overall, high N export by harvest (107–360 kg N ha−1 yr−1) markedly exceeded N inputs, leading to a negative grassland N balance. Conclusions Here provided results suggests a risk of soil N mining in montane grasslands, which increases both under climate change and land use intensification.

36. How soil sodification and pH restrict microbially mediated organic carbon turnover and aggregate formation: An artificial soil microcosm study

Abstract

Exchangeable sodium can have pronounced influences on physicochemical soil properties whereas the combined impact on microbial turnover of organic carbon (OC) remains elusive. In this work, we aimed to differentiate the effects of exchangeable sodium and soil pH on microbially mediated aggregate formation and turnover of cattle slurry. We incubated the soils under controlled laboratory conditions using artificial soil model minerals containing quartz grains, montmorillonite and goethite. The montmorillonite was pre-treated with NaCl solutions of sodium adsorption ratios (SAR) 0, 1 and 5 which resulted in exchangeable sodium percentages (ESP) of 1, 7 and 19 on average. The soil pH was adjusted within two treatments to 7.5 and 8.5 for each ESP at the start of the incubation. We incubated these six treatments with and without cattle slurry ground to < 200 µm after addition of a combined microbial inoculum, extracted from a Cambisol (pHH2O 7.5, Germany) and a Calcaric Solonchak (pHH2O 9.3, Spain) added to all samples. The microcosms were incubated with three replicates over a period of 30 days at constant pF of 2.2. The CO2 emission measurements of the microcosms with exchangeable sodium indicated a delayed respiration. The respiration under ESP 19 increased rapidly within the first days of incubation, whereas it was more delayed under ESP 7 until 15 days of incubation. The delayed CO2 respiration might be related to inhibited structural formation in treatments with higher exchangeable sodium. To test this, we are investigating the data on water-stable aggregation by wet sieving. The delayed CO2 respiration was reflected in lower microbial biomass, extracted after the incubation. The microbial biomass under ESP 19 and pH 8.5 was highest whereas the amount of leached C after two rainfall events (at day 7 and 15) was lowest, which could be related to a higher microbially mediated OC sequestration. The composition of exchangeable cations was monitored before and after the whole incubation which might help explaining the processes of microbially mediated aggregate formation and microbial carbon turnover under different levels of exchangeable sodium.

Abstract

Soil salinization is an emerging problem worldwide as a result of unsustainable land management practices and climate change. However, salt-affected soils under agricultural use could act as a C sink if these negative effects can be offset by combination of sustainable land management practices (SLM). In this study, we assessed the effect of (i) intensive tillage along with flood irrigation (IT); (ii) combination of no-tillage with pruning residues (branches and leaves) as mulch, and drip-irrigation (NT + PM); and (iii) combination of reduced tillage with the incorporation of pruning residues and drip-irrigation (RT + PI), on physico-chemical soil parameters, aggregate stability, amount and quality of organic matter fractions and soil organic carbon (SOC) sequestration in a lemon tree orchards (Citrus limon var. Verna) under semi-arid climate conditions. The RT + PI management system showed a decrease in salinity and bulk density, and increased soil porosity, soil OC and N stocks, and percentage of OC-rich macroaggregates as compared to the IT system. The aggregate-occluded particulate organic matter fraction (oPOM) played a key role in macroaggregate stability. The NT + PM treatment also showed positive effects on the investigated soil properties, but this was limited to the upmost topsoil (0−5 cm). The IT management system revealed highest values of salinity and bulk density, and considerably lower SOC stocks. Moreover, a degradation of soil structure with a low percentage of macroaggregates depleted in SOC was observed. We conclude that the incorporation of pruning residues in combination with reduced tillage and drip-irrigation is an effective management system to improve soil structure and facilitate SOC sequestration. Therefore, conventional management systems based on intensive tillage and flood irrigation should be abandoned in salt-affected soils under semi-arid climate conditions in favour of systems with higher organic matter inputs incorporated into the soil combined with measures to reduce the salt content.

Abstract

Grassland management and climate change may have profound effects on soil organic carbon (SOC) storage in temperate grasslands, but to date only limited information on the long-term SOC development derived from grassland monitoring sites is available. In 1986, the Bavarian State Research Center for Agriculture established 20 long-term grassland soil monitoring sites in the state of Bavaria, Germany. We analyzed SOC changes and their main controlling factors in these monitoring sites between 1989 and 2016 in four intervals using the machine learning algorithm conditional random forest. The results showed that changes of SOC stocks in grassland monitoring sites were affected by the multifactorial and interactive nature of changing climate conditions, pedogenic-topographic factors and management practices. Seasonal climate variables explained the highest variability of SOC stock changes in long-term grassland sites, followed by organic fertilizer application. Increasing autumn precipitation led to decreased SOC stocks, whereas increasing spring and summer precipitation led to increased SOC stocks on sites at high elevations with low slopes. Furthermore, we observed that the organic fertilizer application resulted in an increase and/or preservation of SOC stocks, whereas conventional farming with a low degree of organic fertilization resulted in SOC stock decreases. Regarding the acceleration of climate change and the complex interactions between site conditions, changing climate and management, there is an urgent need to collect improved management data in future long-term studies.

Abstract

p'The role of fluvial sedimentary areas as organic carbon sinks remains largely unquantified. Little is known about mechanisms of organic carbon (OC) stabilization in alluvial sediments in semiarid and subhumid catchments where those mechanisms are quite complex because sediments are often redistributed and exposed to a range of environmental conditions in intermittent and perennial fluvial courses within the same catchment. The main goal of this study was to evaluate the contribution of transport and depositional areas as sources or sinks of 'span classCombining double low line"inline-formula"'CO2'/span' at the catchment scale. We used physical and chemical organic matter fractionation techniques and basal respiration rates in samples representative of the three phases of the erosion process within the catchment: (i) detachment, representing the main sediment sources from forests and agricultural upland soils, as well as fluvial lateral banks; (ii) transport, representing suspended load and bedload in the main channel; and (iii) depositional areas along the channel, downstream in alluvial wedges, and in the reservoir at the outlet of the catchment, representative of medium-and long-Term residence deposits, respectively. Our results show that most of the sediments transported and deposited downstream come from agricultural upland soils and fluvial lateral bank sources, where the physicochemical protection of OC is much lower than that of the forest soils, which are less sensitive to erosion. The protection of OC in forest soils and alluvial wedges (medium-Term depositional areas) was mainly driven by physical protection (OC within aggregates), while chemical protection of OC (OC adhesion to soil mineral particles) was observed in the fluvial lateral banks. However, in the remaining sediment sources, in sediments during transport, and after deposition in the reservoir (long-Term deposit), both mechanisms are equally relevant. Mineralization of the most labile OC (the intra-Aggregate particulate organic matter ('span classCombining double low line"inline-formula"''i'M'/i'pom'/span') was predominant during transport. Aggregate formation and OC accumulation, mainly associated with macroaggregates and occluded microaggregates within macroaggregates, were predominant in the upper layer of depositional areas. However, OC was highly protected and stabilized at the deeper layers, mainly in the long-Term deposits (reservoir), being even more protected than the OC from the most eroding sources (agricultural soils and fluvial lateral banks). Altogether our results show that both medium-and long-Term depositional areas can play an important role in erosive areas within catchments, compensating for OC losses from the eroded sources and functioning as C sinks. © Author(s) 2019. Abstract

The capacity of soils to store organic carbon represents a key function of soils that is not only decisive for climate regulation but also affects other soil functions. Recent efforts to assess the impact of land management on soil functionality proposed that an indicator- or proxy-based approach is a promising alternative to quantify soil functions compared to time- and cost-intensive measurements, particularly when larger regions are targeted. The objective of this review is to identify measurable biotic or abiotic properties that control soil organic carbon (SOC) storage at different spatial scales and could serve as indicators for an efficient quantification of SOC. These indicators should enable both an estimation of actual SOC storage as well as a prediction of the SOC storage potential, which is an important aspect in land use and management planning. There are many environmental conditions that affect SOC storage at different spatial scales. We provide a thorough overview of factors from micro-scales (particles to pedons) to the global scale and discuss their suitability as indicators for SOC storage: clay mineralogy, specific surface area, metal oxides, Ca and Mg cations, microorganisms, soil fauna, aggregation, texture, soil type, natural vegetation, land use and management, topography, parent material and climate. As a result, we propose a set of indicators that allow for time- and cost-efficient estimates of actual and potential SOC storage from the local to the regional and subcontinental scale. As a key element, the fine mineral fraction was identified to determine SOC stabilization in most soils. The quantification of SOC can be further refined by including climatic proxies, particularly elevation, as well as information on land use, soil management and vegetation characteristics. To enhance its indicative power towards land management effects, further “functional soil characteristics”, particularly soil structural properties and changes in the soil microbial biomass pool should be included in this indicator system. The proposed system offers the potential to efficiently estimate the SOC storage capacity by means of simplified measures, such as soil fractionation procedures or infrared spectroscopic approaches.

Abstract

This work aims to describe how EHRs have been used to meet the needs of healthcare providers and researchers in a 1,300-beds tertiary Hospital during COVID-19 pandemic. For this purpose, essential clinical concepts were identified and standardized with LOINC and SNOMED CT. After that, these concepts were implemented in EHR systems and based on them, data tools, such as clinical alerts, dynamic patient lists and a clinical follow-up dashboard, were developed for healthcare support. In addition, these data were incorporated into standardized repositories and COVID-19 databases to improve clinical research on this new disease. In conclusion, standardized EHRs allowed implementation of useful multi- purpose data resources in a major Hospital in the course of the pandemic.

Abstract

BackgroundThe propagation of COVID-19 in Spain prompted the declaration of the state of alarm on March 14, 2020. On 2 December 2020, the infection had been confirmed in 1,665,775 patients and caused 45,784 deaths. This unprecedented health crisis challenged the ingenuity of all professionals involved. Decision support systems in clinical care and health services management were identified as crucial in the fight against the pandemic.MethodsThis study applies Deep Learning techniques for mortality prediction of COVID-19 patients. Two datasets with clinical information (medication, laboratory tests, vital signs etc.) were used. They are comprised of 2,307 and 3,870 COVID-19 infected patients admitted to two Spanish hospital chains. Firstly, we built a sequence of temporal events gathering all the clinical information for each patient. Next, we used the temporal sequences to train a Recurrent Neural Network (RNN) model with an attention mechanism exploring interpretability. We conducted extensive experiments and trained the RNNs in different settings, performing hyperparameter search and cross-validation. We ensembled resulting RNNs to reduce variability and enhance sensitivity.ResultsWe assessed the performance of our models using global metrics, by averaging the performance across all the days in the sequences. We also measured day-by-day metrics starting from the day of hospital admission and the outcome day and evaluated the daily predictions. Regarding sensitivity, when compared to more traditional models, our best two RNN ensemble models outperform a Support Vector Classifier in 6 and 16 percentage points, and Random Forest in 23 and 18 points. For the day-by-day predictions from the outcome date, the models also achieved better results than baselines showing its ability towards early predictions.ConclusionsWe have shown the feasibility of our approach to predict the clinical outcome of patients infected with SARS-CoV-2. The result is a time series model that can support decision-making in healthcare systems and aims at interpretability. The system is robust enough to deal with real world data and it is able to overcome the problems derived from the sparsity and heterogeneity of the data. In addition, the approach was validated using two datasets showing substantial differences. This not only validates the robustness of the proposal but also meets the requirements of a real scenario where the interoperability between hospitals’ datasets is difficult to achieve.

This exciting and wide-ranging volume examines the construction and dissemination of the image of female power during the Renaissance.Chapters examine the creation, promotion, and display of the image of women in power, and how the artistic and cultural patronage they developed helped them craft a self-image that greatly contributed to strengthening their power, consolidating their political legitimacy, and promoting their authority. Contributors cover diverse models of sixteenth-century female power: from ruling queens, regents, and governors, to consorts of sovereigns and noblewomen outside the court. The women selected were key political figures and patrons of art in England, France, Castile, the Low Countries, the Holy Roman Empire, and Italian city states.
