ObjectivesTo determine preoperative factors associated to myocardial injury after non-cardiac surgery (MINS) and to develop a prediction model of MINS.DesignRetrospective analysis.SettingTertiary hospital in Spain.ParticipantsPatients aged ≥45 years undergoing major non-cardiac surgery and with at least two measures of troponin levels within the first 3 days of the postoperative period. All patients were screened for the MANAGE trial.Primary and secondary outcome measuresWe used multivariable logistic regression analysis to study risk factors associated with MINS and created a score predicting the preoperative risk for MINS and a nomogram to facilitate bed-side use. We used Least Absolute Shrinkage and Selection Operator method to choose the factors included in the predictive model with MINS as dependent variable. The predictive ability of the model was evaluated. Discrimination was assessed with the area under the receiver operating characteristic curve (AUC) and calibration was visually assessed using calibration plots representing deciles of predicted probability of MINS against the observed rate in each risk group and the calibration-in-the-large (CITL) and the calibration slope. We created a nomogram to facilitate obtaining risk estimates for patients at pre-anaesthesia evaluation.ResultsOur cohort included 3633 patients recruited from 9 September 2014 to 17 July 2017. The incidence of MINS was 9%. Preoperative risk factors that increased the risk of MINS were age, American Status Anaesthesiology classification and vascular surgery. The predictive model showed good performance in terms of discrimination (AUC=0.720; 95% CI: 0.69 to 0.75) and calibration slope=1.043 (95% CI: 0.90 to 1.18) and CITL=0.00 (95% CI: −0.12 to 0.12).ConclusionsOur predictive model based on routinely preoperative information is highly affordable and might be a useful tool to identify moderate-high risk patients before surgery. However, external validation is needed before implementation.

Objetivo. Analizar la desigualdad en la atención prenatal de los nacidos vivos de Colombia en 2018 según el tipo de aseguramiento en salud. Material y métodos. Análisis de diferencias en la media de controles prenatales por régimen de aseguramiento y los Odds ratio ajustado por variables sociodemográficas. Resultados. Existen diferencias en la media de control prenatal por regímenes de aseguramiento en 2018. Las madres afiliadas al subsidiado tienen 2.2 veces más probabilidad de tener menos de cinco controles comparado con contributivo, con mayor riesgo en las madres menores de 19 años, con más de cuatro embarazos, residentes en la Orinoquía y en área rural, así como las que no tienen esposo o compañero. Conclusiones. Aunque Colombia ha alcanzado la cobertura universal, existen desigualdades entre regímenes de aseguramiento. Este hallazgo puede orientar al gobierno en la definición de políticas en salud materna y apoyar la gestión de la atención prenatal de las aseguradoras. Magíster en Epidemiología Maestría Objective. To analyze insurance health disparities of the prenatal care in Colombian´s live newborns in 2018. Material and methods. Analysis of differences in the prenatal control average by health insurance type and the odds ratio adjusted for sociodemographic variables. Results. There are differences in the prenatal control average by type insurance health in 2018. Affiliated mothers who were in subsidized regimen have 2.2 times more probability to get five less prenatal controls compared with people who are in contributory regime (IC95% 2.22-2.32) , with higher risk in mothers under 19 years old, with more than four pregnancies, residents in Orinoquía and rural areas, as well as those without a husband or partner. Conclusions. Although Colombia has achieved universal coverage, there are disparities between insurance regimes. This finding can guide the government in defining maternal health policies and support the management of prenatal care for insurance companies.

La presente investigación tuvo como objetivo analizar, describir y mejorar los conocimientos básicos que los padres tienen con respecto a la pérdida auditiva, amplificación auditiva y la metodología auditivo oral, aspectos importantes tomando en cuenta que sus hijos presentan una discapacidad auditiva y que están inmersos en una educación auditivo oral, en la cual se requiere el apoyo de los padres para la rehabilitación de sus hijos. En cuanto a la metodología, esta investigación es cuantitativa, ya que usa recolección de datos para probar hipótesis con base en la medición numérica y el análisis estadístico para establecer patrones de comportamientos. El tipo de estudio es descriptivo, ya que, busca especificar las características de un grupo de padres de familia de niños con pérdida auditiva, los cuales fueron sometidos a un análisis para hallar el nivel de conocimiento sobre pérdida auditiva, la amplificación auditiva y la metodología auditivo oral. La muestra estuvo conformada por todos los padres de familia de los niños con pérdida auditiva del colegio “Fernando Wiese Eslava”. Se trabajó con todas las familias que tienen como mínimo un año de permanencia en el colegio. Los resultados permiten exponer que los padres de familia de niños con pérdida auditiva del Colegio Fernando Wiese Eslava obtuvieron un nivel de conocimientos dentro del promedio, donde no se encontraron diferencias significativas en cuanto al nivel de conocimientos de los padres de familia en función al género, ni al tiempo de permanencia, sin embargo, si existen diferencias en cuanto al grado de instrucción de los mismo. The present research aimed to analyze, describe and improve the relation between dad – kid according to the basic knowledge that fathers have about the hearing loss, hearing amplification and oral hearing methodology, important aspects talking about sons that presents hearing impaired and who are immersed into a hearing oral education. In regard to the methodology, this research is quantitative, analyzed with hypothesis based on numerical measurement and statistical analysis establishing behavior patterns. The type of investigation is descriptive, because try to specify the characteristics a group of parents have with hearing loss kids, who was evaluated to analyze how much they know about hearing loss, hearing amplification and oral hearing methodology. The sample consisted in the parents of kids with hearing loss from the “Fernando Wiese Eslava” school. Analyzing all the families with a minimum of a year in the school. The results let us explain the parents with hearing loss kids from the “Fernando Wiese Eslava” school have a knowledge within the average, without differences on the gender, or the time in the school, however there is a difference in regard to the grade of education between them all. - Abstract
Generalized eruptive histiocytoma is considered an extremely rare subtype of non-Langerhans cells histiocytosis. In the literature, there are few reports of this disease that mainly affects adults. In this report, we present a case of generalized eruptive histiocytoma in an elderly patient who had presented symptoms for over two months. Multiple erythematous papules, asymptomatic and symmetrically distributed were observed on the trunk and limbs. Histological examination showed a dense mononuclear cell dermal infiltrate. In the immunohistochemical analysis, the cells were CD68 positive, but CD1a, S100 and CD34 negative. A diagnosis of generalized eruptive histiocytoma was established. The aim of our paper is to report a case of a very rare disease, whose subtype and affected age group are even more unusual.
