Overall, the indications for operating on spinal disorders are increasing as reflected by the total number of spine surgeries. Spine surgery usually involves decompression and/or fusion of one or more spine levels (1, 2, 3). However, regardless of the pathology and surgical technique used, there is a variable percentage of patients who remain symptomatic and with functional disability (2, 4, 5).

El total desconocimiento acerca de dos cruces procesionales que forman parte de instituciones museísticas portuguesas, hasta su adquisición, en la segunda mitad del siglo pasado, hace inviable cualquier investigación archivística, condicionando el conocimiento sobre estas cruces. Nuestros esfuerzos se centran, pues, en un estudio comparativo, considerando para eso ejemplos de producción portuguesa y española correspondientes al periodo comprendido entre finales del siglo XV y la centuria siguiente. Ciertas características evidenciadas parecen corresponder a un modelo «europeo»; al mismo tiempo, muestran particularidades de carácter más regional, que nos acerca, con las reservas necesarias, hacia la producción de los centros españoles.

Patient positioning on the surgical table is a critical step in every spine surgery. The most common surgical positions in spine surgery are supine, prone and lateral decubitus. There are countless lesions that can occur during spine surgery due to patient mispositioning. Ulnar nerve and brachial plexus injuries are the most common nerve lesions seen in malpositioned patients. Devastating complications due to increased intraocular pressure or excessive abdominal pressure can also occur in prone decubitus and are real concerns that the surgical team must be aware of. All members of the surgical team (including surgeons, anesthesiologists and nurses) should know how to correctly position the patient, identify possible positioning errors and know how to avoid them in order to prevent postoperative morbidity. This work pretends to do a review of the most common positions during spine surgery, alert to errors that can happen during the procedure and how to avoid them.

In tokamak and other fusion devices, magnetic control is the main tool that allows to regulate the plasma current, position and shape; it is in charge of actuating the desired plasma current waveform, steering the plasma position to a given set point and maintain the plasma shape close to a prescribed plasma equilibrium. This work describes the application of several physics concepts and computational tools in order to obtain a novel optimal controller for the plasma centroid position, which has been implemented and tested in the real-time plasma control system at the ISTTOK tokamak. A key point for the development of the new control system was the installation of a recently upgraded hardware, that numerically integrates in real-time the magnetic probes signals.

RESUMO Objetivo: Avaliar a qualidade de vida relacionada com a saúde e a incapacidade no primeiro mês após a alta para domicílio de todos os sobreviventes de COVID-19 grave internados por mais de 24 horas no Serviço de Medicina Intensiva. Metódos: Estudo realizado no Serviço de Medicina Intensiva do Centro Hospitalar Universitário São João, entre 8 de outubro de 2020 e 16 de fevereiro de 2021. Aproximadamente 1 mês após a alta para domicílio, uma enfermeira com experiência em medicina intensiva realizou uma consulta telefônica a 99 sobreviventes, aplicando os questionários EuroQol Five-Dimensional Five-Level e World Health Disability Assessment Schedule 2.0 - 12 itens. Resultados: A média de idade da população estudada foi de 63 ± 12 anos, e 32,5% foram submetidos à ventilação mecânica invasiva. O Simplified Acute Physiology Score médio foi de 35 ± 14, e o Índice de Comorbilidades de Charlson foi de 3 ± 2. O tempo de internamento em medicina intensiva e no hospital foi de 13 ± 22 e 22 ± 25 dias, respectivamente. A média da Escala Visual Analógica da EuroQol foi de 65% (± 21), sendo que apenas 35,3% dos sobreviventes não apresentaram ou tiveram problemas ligeiros para realizar suas atividades habituais, a maioria com algum grau de dor/desconforto e ansiedade/depressão. O World Health Disability Assessment Schedule 2.0 - 12 itens, mostrou incapacidade marcada em retomar o trabalho habitual ou atividades comunitárias e na mobilidade. O uso de ambas as ferramentas sugeriu que o estado de saúde dos sobreviventes seria pior do que a sua percepção. Conclusão: A identificação precoce de sequelas pode ajudar a definir fluxos e prioridades para a reabilitação e reinserção após a COVID-19 grave.

Glioblastoma multiforme (GBM) is a very aggressive and heterogeneous glioma. Currently, GBM is treated with a combination of surgery, radiotherapy, chemotherapy (e.g. temozol­amide) and Tumour Treating Fields. Unfortunately, the mean survival is still around 15 months. This poor prognosis is associated with therapy resistance, tumor recurrence, and limited delivery of drugs due to the blood-brain barrier nature. Nanomedicine, the application of nanotechnology to medicine, has revolutionized many health fields, specifically cancer diagnosis and treatment. This review explores the particularities of different nanosystems (i.e., superparamagnetic, polymeric and gold nanoparticles, and liposomes) as well as how they can be applied to the treatment and diagnosis of GBM. As described, the most of the cited examples are on the preclinical phase; however, positive results were obtained and thus, the distance to achieve an effective treatment is shorter every day.

When the COVID-19 pandemic hits Portugal in early March 2020, medical doctors, engineers and researchers, with the encouragement of the Northern Region Health Administration, teamed up to develop and build, locally and in a short time, a ventilator that might eventually be used in extreme emergency situations in the hospitals of northern Portugal. This letter tells you the story of Pneuma, a low-cost emergency ventilator designed and built under harsh isolation constraints, that gave birth to derivative designs in Brazil and Morocco, has been industrialized with 200 units being produced and is now looking forward to the certification as a medical device that will possibly support a go-to-market launch. Open intellectual property (IP), multidisciplinarity teamwork, fast prototyping and product engineering have shortened to a few months an otherwise quite longer idea-to-product route, clearly demonstrating that when scientific and engineering knowledge hold hands great challenges can be successfully faced.

Glioblastoma multiforme (GBM) remains a challenging disease, as it is the most common and deadly brain tumour in adults and has no curative solution and an overall short survival time. This incurability and short survival time means that, despite its rarity (average incidence of 3.2 per 100,000 persons), there has been an increased effort to try to treat this disease. Standard of care in newly diagnosed glioblastoma is maximal tumour resection followed by initial concomitant radiotherapy and temozolomide (TMZ) and then further chemotherapy with TMZ. Imaging techniques are key not only to diagnose the extent of the affected tissue but also for surgery planning and even for intraoperative use. Eligible patients may combine TMZ with tumour treating fields (TTF) therapy, which delivers low-intensity and intermediate-frequency electric fields to arrest tumour growth. Nonetheless, the blood–brain barrier (BBB) and systemic side effects are obstacles to successful chemotherapy in GBM; thus, more targeted, custom therapies such as immunotherapy and nanotechnological drug delivery systems have been undergoing research with varying degrees of success. This review proposes an overview of the pathophysiology, possible treatments, and the most (not all) representative examples of the latest advancements.

Smart cities are, nowadays, an unavoidable and growing reality, supported on software platforms that support city management, through the processing and presentation of a large number of data, obtained from sensors used throughout the cities. Low-power wide area networks (LPWAN) leverage the sensorization process; however, urban landscape, in turn, induces a high probability of change in the propagation conditions of the LPWAN network, thus requiring active monitoring solutions for assessing the city LPWAN network condition. Currently existing solutions usually consider the existence of only one type of LPWAN network to be monitored. In this paper, an architecture for aggregation of metrics from heterogeneous LPWAN networks is presented. The architecture, named IoTMapper, combines purpose build components with existing components from the FIWARE and Apache Kafka ecosystems. Implementation details for the LPWAN networks are abstracted by adapters so that new networks may be easily added. The validation was carried out using real data collected for long-range wide-area network (LoRaWAN) in Lisbon, and a simulated data set extrapolated from the collected data. The results indicate that the presented architecture is a viable solution for metrics aggregation that may be expanded to support multiple networks. However, some of the considered FIWARE components present performance bottlenecks that may hinder the scaling of the architecture while processing new message arrivals.

In source localization problems, the relative geometry between sensors and source will influence the localization performance. The optimum configuration of sensors depends on the measurements used for the source location estimation, how these measurements are affected by noise, the positions of the source, and the criteria used to evaluate the localization performance. This paper addresses the problem of optimum sensor placement in a plane for the localization of an underwater vehicle moving in 3D. We consider sets of sensors that measure the distance to the vehicle and model the measurement noises with distance dependent covariances. We develop a genetic algorithm and analyze both single and multi-objective problems. In the former, we consider as the evaluation metric the arithmetic average along the vehicle trajectory of the maximum eigenvalue of the inverse of the Fisher information matrix. In the latter, we estimate the Pareto front of pairs of common criteria based on the Fisher information matrix and analyze the evolution of the sensor positioning for the different criteria. To validate the algorithm, we initially compare results with a case with a known optimal solution and constant measurement covariances, obtaining deviations from the optimal less than 0.1%. Posterior, we present results for an underwater vehicle performing a lawn-mower maneuver and a spiral descent maneuver. We also present results restricting the allowed positions for the sensors.

Intermittent vesical self-catheterisation is a legitimate and safe technique that has been reported since the 1970s as a solution for the treatment and prevention of vesical urinary complications resulting from spinal cord injury. This practice, using clean technology, has been asserting itself as one of the best alternatives for people with neurogenic bladder. However, adherence is not complete due to some barriers imposed to this procedure by the injured, with emphasis on positioning, agility, and visual impairment. The solutions presented today to support self-catheterisation are expensive equipment that does not allow patients with advanced levels of spasticity to have their autonomy. A biomechanical support device was developed to aid self-catheterisation, mainly aimed at women with spasticity, filling the gap in the existing products. Despite the main objective of self-catheterisation, the system’s design made it possible to quantify the strength of the adductors for the sitting position during the execution of the adduction movement, particularly relevant for spastic patients. The device’s production was entirely carried out using the FDM methodology, with 3D printers, and its design and operation were thought to overcome the physical and psychological barriers imposed by the users. The system was first tested with a group of healthy volunteers to obtain a pattern of the adductors force in a sitting position and after with a group of spastic volunteers. The obtained data allows to compare the adductor force data and optimize the system, with particular functionalities for spastic patients, with the implementation of a motorised version and a visualization camera. The system, its developments, and results obtained are present and discussed.

The increased development of camera resolution, processing power, and aerial platforms helped to create more cost-efficient approaches to capture and generate point clouds to assist in scientific fields. The continuous development of methods to produce three-dimensional models based on two-dimensional images such as Structure from Motion (SfM) and Multi-View Stereopsis (MVS) allowed to improve the resolution of the produced models by a significant amount. By taking inspiration from the free and accessible workflow made available by OpenDroneMap, a detailed analysis of the processes is displayed in this paper. As of the writing of this paper, no literature was found that described in detail the necessary steps and processes that would allow the creation of digital models in two or three dimensions based on aerial images. With this, and based on the workflow of OpenDroneMap, a detailed study was performed. The digital model reconstruction process takes the initial aerial images obtained from the field survey and passes them through a series of stages. From each stage, a product is acquired and used for the following stage, for example, at the end of the initial stage a sparse reconstruction is produced, obtained by extracting features of the images and matching them, which is used in the following step, to increase its resolution. Additionally, from the analysis of the workflow, adaptations were made to the standard workflow in order to increase the compatibility of the developed system to different types of image sets. Particularly, adaptations focused on thermal imagery were made. Due to the low presence of strong features and therefore difficulty to match features across thermal images, a modification was implemented, so thermal models could be produced alongside the already implemented processes for multispectral and RGB image sets.

This paper presents an economic and environmental cost analysis of a representative Portuguese housing dwelling unit using an innovative lightweight adjustable membrane partitioning system, developed within a research project. The environmental and economic costs of this system, both on the construction and occupancy phases, were compared with the Portuguese conventional dividing wall solution, in hollow brick, simulated on the same reference dwelling, using the same layout and remaining constructive elements. The occupancy phase costs were evaluated by computer simulation, which allowed determining how the occupancy schedule of daytime and night time occupied areas may influence the cooling and heating needs for different solar orientations.

The city of Lisbon, as any other capital of a European country, has a large number of issues regarding managing waste and recycling containers spread throughout the city. This document presents the results of a study promoted by the Lisbon City Council for trialing LPWAN (Low-Power Wide-Area Network) technology for the waste management vertical under the Lisbon Smart City initiative. Current waste management is done using GSM (Global System for Mobile communications) sensors, and the municipality aims to use LPWAN in order to improve range and reduce costs and provisioning times when changing the communications provider. After an initial study, LoRa (Long Range) and LoRAWAN (LoRa Wide Area Network) as its network counterpart, were selected as the LPWAN technology for trials considering several use cases, exploring multiple distances, types of recycling waste containers, placements (underground or surface) and kinds of commercially available waste level measurement LoRa sensors. The results showed that the underground waste containers proved to be, as expected, the most difficult to operate correctly, where the container itself imposed attenuation levels of 26 dB on the LoRa link budget. The successful results were used to promote the deployment of a city-wide LoRa network, available to all the departments inside the Lisbon City Council. Considering the network capacity, the municipality also decided to make the network freely available to citizens.

Cancer is a major health concern and the prognosis is often poor. Significant advances in nanotechnology are now driving a revolution in cancer detection and treatment. The goal of this study was to develop a novel hybrid nanosystem for melanoma treatment, integrating therapeutic and magnetic targeting modalities. Hence, we designed long circulating and pH-sensitive liposomes loading both dichloro(1,10-phenanthroline) copper (II) (Cuphen), a cytotoxic metallodrug, and iron oxide nanoparticles (IONPs). The synthetized IONPs were characterized by transmission electron microscopy and dynamic light scattering. Lipid-based nanoformulations were prepared by the dehydration rehydration method, followed by an extrusion step for reducing and homogenizing the mean size. Liposomes were characterized in terms of incorporation parameters and mean size. High Cuphen loadings were obtained and the presence of IONPs slightly reduced Cuphen incorporation parameters. Cuphen antiproliferative properties were preserved after association to liposomes and IONPs (at 2 mg/mL) did not interfere on cellular proliferation of murine and human melanoma cell lines. Moreover, the developed nanoformulations displayed magnetic properties. The absence of hemolytic activity for formulations under study demonstrated their safety for parenteral administration. In conclusion, a lipid-based nanosystem loading the cytotoxic metallodrug, Cuphen, and displaying magnetic properties was successfully designed.

Teaching software engineering in its many different forms using traditional teaching methods is difficult. Serious games can help overcome these challenges because they allow real situations to be simulated. However, the development of serious games is not easy and, although there are good practices for relating game design patterns to teaching techniques, there is no methodology to support its use in a specific context such as software engineering. This article presents a case study to validate a methodology that links the Learning and Teaching Functions (LTF) to the Game Design Patterns (PIB) in the context of Software Engineering Education. A serious game was developed from scratch using this methodology to teach software estimation (a specific topic of software engineering). An experiment was carried out to validate the effectiveness of the game by comparing the results of two different groups of students. The results indicate that the methodology can help to develop effective educational games on specific learning topics.

A criação do novo bispado de Elvas no século XVI, exigia a construção de um Paço Episcopal que servisse de residência a todos os bispos que futuramente governassem a diocese. A sua construção, ampliação, e os muitos enriquecimentos empreendidos por alguns desses eclesiásticos, idealizados e concretizados de acordo com o gosto vigente, permitiu atribuir às suas casas uma atmosfera compatível com o estatuto inerente ao desempenho do seu cargo. A extinção da diocese em 1881 e a sua execução no ano seguinte, ditariam novas funcionalidades a este edifício, encontrando-se na Lei da Separação o momento histórico em que uma significativa parte da sua componente artística - pintura, mobiliário, ourivesaria, faiança, paramentaria, azulejaria, livraria, etc - se dispersa por inúmeras instituições culturais.

The capability to anticipate a contact with another device can greatly improve the performance and user satisfaction not only of mobile social network applications but of any other relying on some form of data harvesting or hoarding. One of the most promising approaches for contact prediction is to extrapolate from past experiences. This paper investigates the recurring contact patterns observed between groups of devices using an 8-year data-set of wireless access logs produced by more than 70000 devices. This effort permitted to model the probabilities of occurrence of a contact at a predefined date between groups of devices using a power law distribution that varies according to neighbourhood size and recurrence period. In the general case, the model can be used by applications that need to disseminate large data-sets by groups of devices. As an example, the paper presents and evaluates an algorithm that provides daily contact predictions, based on the history of past pairwise contacts and their duration.

O presente estudo revela-nos a existência de António Coelho [Pinhão], um ourives lisboeta na 2.ª metade do século XVII, autor de peças – religiosas e civis – encomendadas por D. Diogo de Sousa, arcebispo de Évora. A documentação conhecida possibilita traçar os percursos individuais e conjuntos de ambos os medianeiros, possibilitando-nos uma maior proximidade às questões artísticas. Entre estas salientamos os processos de encomenda, as tipologias e até mesmo perceções de gosto do referido arcebispo, que a correspondência trocada entre ambos os interlocutores permite identificar.

Após alguns anos em que o transporte ferroviário teve um declínio acentuado, ultimamente tem vindo a assistir-se, em vários países, à modernização de linhas férreas antigas, nomeadamente procedendo à reabilitação da via-férrea. Vários desses casos referem-se a vias que se pretende que venham a servir essen - cial mente o transporte de mercadorias. Durante a reabilitação das vias férreas existem vantagens técnicas, económicas e ambientais em manter, se possível, os materiais existentes, em particular a camada de balastro ferroviário contaminado com solos finos, sob as novas camadas de reforço. Este trabalho apresenta resultados de estudos realizados com o objetivo de reabilitar uma via-férrea para o transporte de mercadorias, localizada em África. Para além da caracterização realizada in situ, construíram-se trechos experimentais para testar diversas soluções estruturais para reabilitação da plataforma ferroviária e realizaram-se modelações numéricas da via, contemplando essas soluções. Os resultados dos estudos permitiram concluir que é possível levar a cabo o reforço estrutural da plataforma, de forma eficiente, em termos técnicos e económicos.

Apresentam-se as conclusões resultantes de três estudos experimentais realizados com base no ensaio com Dilatómetro de Marchetti (DMT) em depósitos aluvionares dos rios Vouga, Mondego e Tejo. Os resultados obtidos são comparados com ensaios de laboratório e in situ convencionais. Descreve-se ainda uma primeira tentativa de utilização deste ensaio em solos residuais, através de um estudo experimental realizado em solos residuais graníticos do Porto.
