Purpose: Separation anxiety is a state of extreme anxiety, felt by an individual in case of separation from attachment figure. It was aimed to investigate attachment styles, temperament traits, childhood trauma and dissociative experiences of university students diagnosed with adult separation anxiety disorder (ASAD). Methods: Sixty five university students, diagnosed to have ASAD by a structured clinical interview were compared with healthy students via˝Adult Attachment Style Questionnaire (AAQ)," "Childhood Trauma Questionnaire (CTQ)," "Temperament Evaluation of Memphis, Pisa, Paris and San Diego Auto-questionnaire (TEMPS-A)," and "Dissociative Experiences Scale (DES)." Results: Twenty two (33.8%) students with ASAD had problems with orientation and getting used to university. According to AAQ, students with ASAD had higher prevalence of insecure attachment styles, and especially high ambivalent attachment styles and to CTQ, students with ASAD had significantly higher scores in all subscales except physical abuse. Overall DES score was higher in students with ASAD compared to the control group. DES score and history of physical disease in the family were independent associates of ASAD. Conclusion: DES score and history of physical disease in the family were independent associates of ASAD among university students. Hence, early awareness of dissociative symptoms and family history might help early identify students with ASAD.

Background: Schizophrenia is a chronic mental disorder, characterized byacute exacerbation and remission phases. Immune system has a role in the pathophysiology of schizophrenia. High mobility group box-1 (HMGB-1) is a macrophage secreted protein activating immune cells to produce cytokines. The aim of this study was to evaluate HMGB-1 levels among patients with schizophrenia both in acute exacerbation and remission phases. Methods: Consecutive schizophrenia patients in acute exacerbation and remission phases were enrolled and compared with each other and with age-sex matched healthy subjects. Patients were assessed with the Scale for the Assessment of Positive Symptoms (SAPS), Scale for the Assessment of Negative Symptoms (SANS), Brief Psychiatric Rating Scale (BPRS), Clinical Global Impression Scale (CGI). Results: Mean HMGB-1 levels were not significantly different in acute exacerbation phase versus remission phase schizophrenia patients (2.139±0.564 g/L vs. 2.326± 0.471 g/L, p=0.335) and both were individually higher than the control group (1.791±0.444 g/L, p=0.05 for acute exacerbation vs control, p=0.002 for remission vs control). In remission phase schizophrenic patients, HMGB-1 levels were positively correlated with Scale For The Assessment of Positive Symptoms (r=0.447, p=0.015) and BPRS (r=0.397, p=0.033) scores and HMGB-1 levels were independently associated with BPRS. Conclusions: Serum HMGB-1 levels were shown to be increased in patients with schizophrenia patients irrespective of phase, there were no differences between patients in acute exacerbation and remission phase in terms of biomarker and HMGB-1 levels were related to symptom severity according to psychiatric scales among patients in remission phase of schizophrenia.

Objectives: SMYRNA-HF study is a prospective multicenter registry study to determine the profiles of patients with heart failure (HF) in Turkey. This study aimed to present the baseline characteristics of preliminary cohort by comparing them according to different HF phenotypes. Methods: The first SMYRNA-HF cohort included outpatients with HF from 9 centers. Patients were classified into three HF phenotypes as HF with reduced ejection fraction (HFrEF), mildly reduced EF (HFmrEF), and preserved EF (HFpEF) as recommended by guidelines. Results: Overall, 298 patients were included in this preliminary analysis that 57% of the patients were classified as having HFrEF, 33.3% as having HFpEF, and 9.7% as having HFmrEF. Female gender was more common in HFpEF (p = 0.003). Age, frequency of diabetes mellitus, use of angiotensin-converting enzyme inhibitors/angiotensin receptor blocker, use of beta-blocker, use of daily loop diuretic, heart rate, blood urea nitrogen levels, lipid profiles, hemoglobin, white blood cell, platelet levels were similar among three HF phenotypes. Body mass index (BMI) (p < 0.001), frequency of hypertension (HT) (p < 0.001), and atrial fibrillation (AF) (p = 0.015) were higher in HFpEF. Ischemic etiology (p < 0.001) was less frequent in HFpEF. Use of mineralocorticoid receptor antagonist was higher in HFrEF (p < 0.001). Conclusions: Our study presented the baseline characteristics of outpatients with HF in Turkey. There were significant differences among HF phenotypes in terms of gender, BMI, frequency of HT, AF, and ischemic etiology. Treatment implementations seem to follow the guidelines. Although the rates are low, new treatment approaches recommended in the most recent guidelines seem to enter clinical practice.

Objective: Long-lasting care process can result in the withdrawal of the mother with children diagnosed with cerebral palsy (CP) from the social environment and cultural activities while leading also to physical exhaustion. The present study evaluates the levels of depression and anxiety, suicidal ideation, and behaviors in mothers with children diagnosed with CP.Methods: The study included 57 healthy mothers with children aged 2–16 years who have been diagnosed with CP and 41 healthy mothers with healthy children as a control group. The Hospital Anxiety and Depression Scale (HADS), the Scale for Suicidal Ideation (SSI), and the Suicidal Behavior Scale (SBS) were administered to all participants.Results: The sociodemographic status of the mothers of children with CP was similar to that of the mothers with healthy children (p>0.05). Background Depression is a major public health problem in all ages. Besides other chronic physical diseases, it is one of the most common psychiatric disorders observed in the elderly. In this study, it was aimed to determine the prevalence of depressive symptoms and related factors and to draw attention to occult suicide probability in elderly people without significant disabling disease. Methods Individuals aged 65 and over, who were randomly selected from a family healthcare centre, were enrolled into the study. Geriatric Depression Scale (GDS) was used to assess prevalence of depression and Suicide Probability Scale (SPS) for risk of suicide. Results There were 323 elderly (F/M: 150/173). Mean age was 73.3 ± 6.1 years. The majority had chronic physical illnesses (77.7%). Mean GDS and SPS scores were 11.07 ± 4.6 and 62.91 ± 8.24, respectively. Prevalence of depression was 27.6%. In multivariable logistic regression analysis, living without a spouse (P = 0.047, Exp(B) = 1.744, 95% CI = 1.008-3.01), dissatisfaction with life (P ˂ 0.001, Exp(B) = 5.327, 95% CI = 3.02-9.36) and having no leisure time activity (P = 0.047, Exp(B) = 1.846, 95% CI = 1.009-3.37) were found to be the independent predictors of presence of depression, as indicated by GDS. In logistic regression analysis, dissatisfaction with life (P ˂ 0.001, Exp(B) = 2.631, 95% CI = 1.53-4.50) was found to be an independent predictor of high SPS scores. Conclusion In elderly individuals without overt disabling disease, dissatisfaction with life, absence of leisure time activities, and a life without a partner seem to significantly increase the likelihood of overt depression. On the other hand, there is occult increase in suicide risk in the presence of dissatisfaction with their lives in the elderly.

FUNDAMENTO: Tem-se observado que a depressão é preditora de reinternação e mortalidade na insuficiência cardíaca. O hormônio da paratireoide é um biomarcador novo e promissor que pode predizer a internação, a capacidade funcional e a mortalidade na insuficiência cardíaca. OBJETIVO: Nosso objetivo foi investigar a associação da depressão aos níveis séricos de hormônio da paratireoide em pacientes com insuficiência cardíaca sistólica. MÉTODOS: Cem pacientes ambulatoriais consecutivos com IC sistólica com fração de ejeção do ventrículo esquerdo < 40% foram examinados prospectivamente. Todos os pacientes foram submetidos a exames laboratoriais, incluindo análises de peptídeo natriurético cerebral e de hormônio da tireoide. Os pacientes foram convidados a completar o Inventário de Depressão de Beck-II. RESULTADOS: Cinquenta e um pacientes (51%) apresentavam escore de BDI ruim (escore de BDI > 18). Esses pacientes apresentavam níveis de hormônio da paratireoide significativamente mais elevados em comparação com aqueles com bons escores de BDI (133 ± 46 pg/ml versus 71 ± 26 pg/ml, p < 0,001). No modelo de regressão logística multivariada, constatou-se que o nível do hormônio da tireoide (razão de chances (OR) = 1.035, p = 0,003), fração de ejeção do ventrículo esquerdo (OR = 0,854, p = 0,004), classe funcional III / IV (OR = 28,022, p = 0,005), C-reactive protein (CRP) (OR = 1,088, p = 0,020) e presença de edema pré-tibial (OR = 12,341, p = 0,033) constituíam preditores independentes de depressão moderada a importante após o ajuste de outros possíveis fatores de confusão. CONCLUSÃO: Pacientes com insuficiência cardíaca sistólica com depressão moderada a importante apresentavam níveis séricos elevados de hormônio da tireoide e CRP, capacidade funcional ruim e fração de ejeção do ventrículo esquerdo mais baixa. A associação da depressão com esses parâmetros pode explicar a contribuição da depressão para a internação e a mortalidade na insuficiência cardíaca. In multivariable logistic regression model, PTH level (Odds ratio (OR) = 1.035, p = 0.003), LVEF (OR = 0.854, p = 0.004), NYHA functional class III/IV (OR = 28.022, p = 0.005), C-reactive protein (CRP) (OR = 1.088, p = 0.020), and presence of pretibial edema (OR = 12.341, p = 0.033) were found to be independent predictors of moderate to severe depression after adjustment of other potential confounders. CONCLUSION: Systolic HF patients with moderate to severe depression had higher serum levels of PTH and CRP, poor functional status and lower LVEF. The association of depression with such parameters might explain the contribution of depression to hospitalization and mortality in HF.

WOS: 000296230200011, PubMed ID: 21968258, Objectives: We aimed to evaluate echocardiography-guided hemodynamic determinants of quality of life (QoL) via the Medical Outcomes Study Short Form (SF-36) questionnaire in patients with mild to moderate mitral stenosis (MS). Methods: Eighty consecutive patients with rheumatic MS, who were admitted to the outpatient department, were enrolled into the study upon obtaining informed consent. Forty age-sex-matched healthy individuals were enrolled as a control group. Results: All subscale scores and total SF-36 scores were significantly lower in the patient group representing a worse QoL. In multivariable logistic regression analysis, only mean pulmonary artery pressure (OR 1.138, 95% CI 1.049-1.234, p = 0.002) was found to be an independent predictor of poor QoL in patients with mild to moderate MS. Conclusion: During follow-up of MS patients before intervention, physicians should consider that mean pulmonary artery pressure is the main factor which influences the patients' QoL.
