Aim: to demonstrate the need for a detailed differential diagnosis and selection of therapy in a patient with decompensated liver cirrhosis of combined etiology (HCV infection and primary sclerosing cholangitis). Key points. The patient came to the clinic with complaints of shortness of breath with minimal physical activity, abdominal enlargement, swelling of the legs, yellowness of the skin, and severe weakness. The complaints arose two months after suffering from left-sided focal pneumonia. Laboratory tests revealed signs of systemic inflammation, liver failure, and acute kidney injury. According to the results of instrumental studies, massive hydrothorax was noted in the right pleural cavity. The patient underwent a series of thoracentesis, and a total of about four liters of non-inflammatory pleural fluid was evacuated. Differential diagnosis was based on the presence of dyspnea and respiratory failure. The patient received effective antiviral therapy with drugs using an interferon-free regimen. Subsequently, conservative therapy was carried out, against the background of which the symptoms regressed and the patient’s condition improved. Conclusions. Hepatopleural syndrome is a serious complication in patients with decompensated liver cirrhosis, although it does not always appear secondary to massive ascites. To resolve hepatic hydrothorax, it is necessary to carry out diuretic therapy, replacement transfusion therapy with albumin preparations, and if there is a large amount of fluid in the pleural cavities, therapeutic and diagnostic thoracentesis is recommended.

Aim: to review the modern approaches to the diagnosis and treatment of C. difficile-associated disease in adults and present the resolution of the Expert Council held on March 25, 2023 in Moscow.General provisions. C. difficile is the most important nosocomial pathogen which spores are also commonly found in the environment. Microbiota impairment, primarily due to the use of antibacterial drugs, is a key stage in the development of C. difficile-associated disease. A search for an infection should be carried out only in patients with diarrhea, and it is advisable to use at least 2 laboratory methods. The drug of choice for first-line treatment is vancomycin. If drug treatment is ineffective or the patient has recurrent clostridial infection, fecal microbiota transplantation should be considered. The probiotic strain Saccharomyces boulardii CNCM I-745 has a direct inhibitory effect on C. difficile toxin A, promotes normalization of the intestinal microbiota composition, and decreases the inflammatory reaction in colonic mucosa colonized with a toxigenic strain of C. difficile.Conclusions. Addition of the probiotic strain Saccharomyces boulardii CNCM I-745 to antibacterial therapy promotes both primary and secondary prevention of C. difficile-associated disease.

The Vladimirskoe deposit is one of the largest gold ore deposits in the southeastern part of the Eastern Sayan. This work is devoted to the localization of mineralized zones and the determination of the formation time of gold mineralization.The geological and structural analysis has shown that second-order discontinuous disturbances accompanied by zones of beresitization, calcification, sulfidization and dyke bodies are the main ore-controlling structures of gold mineralization. For the first time, the age of rocks of dyke complexes and ore mineralized zones of the deposit has been established by 40Ar/39Ar dating methods.

The subject. The interaction of modern national legal systems and the international legal system is still the most controversial legal phenomenon. An exclusively national approach to solving issues of the general theory of international law could not and cannot justify expectations and close the need for legal comprehension. The situation is aggravated by the fact that, at the level of universal international law, the solution of issues on the agenda with the help of positive methods of legal regulation, and, consequently, the implementation of the norms of existing international treaties today are criticized, tested and often completely violated. The implementation of the norms of existing international treaties is increasingly faced with attempts to refute their postulates using the "customs" under which those "rules" are veiled. In such conditions, national legal systems may experience an increased "feeling of jealousy" to their sovereignty and try to "close" as much as possible using the principle of non-interference in the internal affairs of the state.Purpose of the study. In such conditions, it is most important to choose the right vector in improving international law, and not order, law based on normative principles. It is important to correctly "choose the key" to harmonizing the will of sovereign states of our time. The article is devoted to the search for a more correct way of developing international law through its interaction, and not counteraction with the national one.Methodology. The research was carried out using a formal legal interpretation of international legal acts, as well as a comparative analysis of Russian and foreign legal literature. Structural and systemic methods are also the backbone of the study. The conclusions of the work are based on dialectical unity and the struggle of opposites, as well as on interacting deduction and induction in relation to legal systems.The main results. The growing trend towards fragmentation of international law leads to a reduction in the base of sources in the universal sphere of international law. The intensification of regionalization and the creation of regional unions of sovereign states is becoming a source of polar processes: on the one hand, supranational control over the observance of international law by states within their legal system increases, on the other hand, natural situations of non-execution of decisions of international judicial bodies arise. Such situations, without a proper assessment of the reasons for the issuance of the international acts of law enforcement themselves, can lead to unfounded criticism. There is an impressive amount of work in the field of correlation between international and national law, as well as in the field of enforcement of decisions of international judicial bodies. Despite this, in the field of practical implementation of the norms of international law, there remains a lack of doctrinal developments. Such a situation will inevitably lead to attempts to create a semblance of a norm, an escalation of confrontation, and an inability to reach agreement on issues on the agenda. The situation at the level of the universal international legal system is aggravating, therefore, the verification of the limits of competence of regional education authorities, in particular of supranational judicial control bodies, becomes even more important.Сonclusions. The author comes to the conclusion that, on the one hand, in the sphere of universal international law, there is a reduction in the base of sources of international law. On the other hand, in the field of regional international integration, on the contrary, the number of sources of international legal regulation is increasing, and the process of their implementation is intensifying. Universal international law based on the principles of the UN Charter is the most qualitative regulator of the field of international public relations. However, at the present stage of development of the international community, regional integration may well come to its aid: through regional interaction, the consolidation of the wills of sovereign states can and should be achieved a legal constructive dialogue on key issues of already universal international law. Using the approaches of national legal regulation exclusively and unilaterally, it is impossible to productively approach issues of international law: neither in the field of creating international organizations of various types, nor in matters of fulfilling obligations under international law. According to the results of the study, it was concluded that only non-confrontational interaction of specialists in the field of international and national (primarily constitutional) law can provide the construction of the most effective model of interaction of these legal systems. This requires a transformation of the basic approaches to the issues of interaction between international and national law. The basic unity of the general theory of law and the specific features of legal systems should be the starting points for doctrinal research of existing legal structures and the practice of their implementation.

Aim. To study the structure of bacteria exometabolites of the Bacillus velezensis BZR336g strain using the GC×GC‐MS method when cultivated on nutrient media differing in the content of microelements.Material and Methods. The object of the study is the B. velezensis BZR336g bacterium strain. It shows promise for the development of a biopreparation on its basis for protecting plants against phytopathogenic fungi. Exometabolites were isolated from the liquid culture and their metabolomic profile was analysed with the GC×GC‐MS method using a LECO Pegasus BT‐4D device.Results. Among the identified compounds one can see the precursors of biologically active metabolites of bacteria of the strain B. velezensis BZR336g. Analysis of the total accumulation of fatty acids and their analogs, as well as of amino acids and peptides, by B. velezensis BZR336g bacteria on nutrient media differing in the content of metal ions, shows that Co2+ ions in combination with Mn2+ and Zn2+ ions play an important role in inducing their synthesis. The presence of the Co2+ ion in the nutrient medium has the greatest influence on the synthesis of peptide components. If it is removed from the nutrient medium, the amount of amino acids and peptides decreases by almost two times. Benzene compounds can act as precursors of aromatic hydrocarbons, which are also of interest in the analysis of bacterial metabolism, since they have antimicrobial activity.Conclusions. The number of compounds that are precursors of lipopeptide metabolites depends on the content of metal ions in the nutrient medium. This fact may vary depending on the combination in which the metals are present.

Aim. An efficacy assessment of steroid therapy in SARS-CoV-2 patients aged over 50 years with lung damage over 50 % (grade 3–4).Materials and methods. A retrospective study of 158 SARS-CoV-2 patients hospitalised in April—June 2020 was conducted under the inclusion criteria: age over 50 years, chest computed tomography (CT) for COVID-19-asso-ciated pneumonia, C-reactive protein (CRP) >50 mg/L, anticoagulant therapy, no contraindications to steroids, no biologic therapy. Cohort 1 patients (n = 96) received dexamethasone 4–12 mg/day, cohort 2 (n = 62) — a standard non-dexamethasone therapy.Results. Dexamethasone treatment associated with a significant alleviation of COVID-19-associated pneumonia in CT score (p = 0.001), reduced fibrinogen (p = 0.001), a trend to CRP reduction by day 8–10 and lower demand for oxygen therapy, including ventilatory support (p = 0.001). Mortality rate was 19.8 and 75.8 % in cohorts 1 and 2, respectively (p = 0.001).Conclusion. Dexamethasone therapy 4–12 mg/day in SARS-CoV-2 patients aged 50+ years with grade 3–4 CT changes receiving LMWH from start of hospitalisation significantly improved CT scores and reduced mortality.

The mineralogical and geochemical features, as well as the sequence of formation of aillikite and calcite carbonatite (CC) with pyrochlore are described for the massif of alkaline ultramafic carbonatite complexes Belaya Zima located in East Siberia. Until now, mutually exclusive information presents the temporal relationships of carbonatites and lamprophyres of the Belaya Zima massif.The sample marking the contact of aillikite and CC was comprehensive studied using analytical methods, e.g. XRF (ARL-9900XP spectrometer, ThermoFisher Scientific), ICP-MS (Element Finnigan MAT), SEM (MIRA 3 LMU (Tescan Ltd)), transmission and ore microscopy (AxioScope. A1, Zeiss), 40Ar/39Ar age determination of micas (Argus mass spectrometer, Micromass). The data obtained indicate a later formation of CC relative to aillikites and probable separation of the carbonatite melt from a single picrite-carbonatite source.

The present research featured the development of urban planning policy in small towns of the Krasnoyarsk Territory in 1950s–1960s, when it was designed by the local authorities and included construction, repairs, national, historical, cultural, environmental, and natural features of the terrain, etc. The authors developed some criteria for the classification of small towns and traced the evolution of urban planning policy from individual to mass construction. The new principles that appeared in 1950s–1960s included the natural and climatic conditions and aimed at strengthening the country's defense. The new course of economic, architectural and urban development modernized and changed the structure of the economy in small towns. The research was based on the historical-chronological and geopolitical method that covered the strategic, economic, and socio-cultural development of Siberia in general. The research contributes to the development of urban planning policy and helps to improve city management practices.

Aim. A clinical description of end-stage hereditary haemochromatosis manifested with chronic alcohol abuse.Key points. A 50-yo patient referred with marked general weakness as a major complaint. The patient had a history of long-term alcohol consumption at toxic doses, putative cirrhosis, paroxysmal atrial fibrillation, type 2 diabetes mellitus. The patient's severity on admission was conditioned by marked hypotension. Further examination aimed at excluding occult gastrointestinal bleeding, adrenal insufficiency, decompensated heart failure. Bronze skin and icteric sclerae were positive. Blood tests revealed severe macrocytic hyperchromic anaemia, thrombocytopae-nia, hyperbilirubinaemia, hypoalbuminaemia, hypocoagulation, elevated transaminases, hyponatraemia, elevated creatinine (CKD DPI 63 mL/min), severe hyperferritinaemia. Faecal occult blood test and EGDS for bleeding were negative. Abdominal ultrasound exposed signs of liver cirrhosis and portal hypertension (ascites, splenomegaly). Echocardiographic evidence of dilated cardiomyopathy of all chambers, a reduced 24% ejection fraction at absent acute myocardial infarction. Primary haemochromatosis was suspected upon high ferritin, transferrin iron saturation and multiple organ dysfunction. Genotyping revealed the HFE 845G > A variant diagnostic of haemochromatosis type 1. Clinical diagnosis: Primary disease: haemochromatosis (homozygous variant HFE 845G > A (A/A)): liver cirrhosis, Child-Pugh class C. Portal hypertension: splenomegaly, ascites. Dilated cardiomyopathy. Diabetes mellitus. Complications: multiple organ dysfunction (SOFA 16). Liver failure: jaundice, hypoalbuminaemia, hypocoagulation. Cardiac rhythm and conduction disorder: paroxysmal atrial fibrillation. Acute cardiac failure with underlying CHF IIb, NYHA class 3. Acute renal failure (anuria) with underlying CKD stage 3 (CKD DPI 63 mL/min). Moderate macrocytic hyperchromic anaemia. Acute and chronic adrenal failure. Despite a cardiovascular and renal failure compensation therapy and albumin transfusion, the patient died. Autopsy revealed a marked organ infiltration with haemosiderin (heart, stomach, liver, pancreas, lungs, kidneys, adrenal glands).Conclusion. The case describes a classical clinical manifestation of haemochromatosis: bronze skin hyperpigmentation, liver cirrhosis, diabetes mellitus, cardiomyopathy, adrenal insufficiency. Terminal haemochromatosis, severe cardiac and renal failure decompensation precluded phlebotomy and chelation therapy. A lethal outcome was conditioned by multiple organ dysfunction with underlying massive haemosiderin deposition in most organs.

Purpose: To evaluate the possibility of registration of the photopic negative response (PhNR) without pupil dilatation in healthy individuals to determine the dysfunction of neurons of the inner retina.Methods: 12 healthy persons (23 eyes) aged 24 to 40 years were examined. Refraction anomalies did not exceed 0.5 diopters. A PhNR was evaluated in photopic ERGs recorded on a blue background with red flashes of four intensities (0.375, 0.75, 1.5, 3.0 cd·s/m2) using the RETIport/scan21 (Roland Consult). First, ERG was recorded with the natural pupil (3.5 ± 0.2 mm). Then, the recording was repeated after drug mydriasis (average pupil size 8.7 ± 0.1 mm).Results. In eyes with natural pupil width, in comparison with drug mydriasis, PhNR amplitudes in responses to weakest stimuli (0.375 cd·s/m2) were significantly reduced. The effect of pupil size on the PhNR amplitude from a baseline was virtually absent in responses to higher strength flashes. This phenomenon can be associated with an irregular distribution of light over the retina with small pupil size, the effect of which is maximal in responses to weak flashes. For the maximal brightness stimuli (3.0 cd·s/m2), differences were found between groups of non-dilated and dilated pupils in the amplitudes of the b-wave and PhNR from the b-peak, which may reflect a decrease in the number of stimulated photoreceptors during ganzfeld stimulation when the pupil is not dilated.Conclusion. The effect of pupil size on the parameters of the PhNR was most significant in the ERG with minimal flash strength and it decreases in responses to flashes of higher brightness. If it is necessary to record the ERG without drug mydriasis, especially in screening studies, it is recommended to limit the protocol to estimate the PhNR from a baseline in responses to maximal flashes, and use the relative parameter — the amplitude ratio of PhNR/b. The first part of the review was focused on modern methods of instrumental diagnostics of primary open-angle glaucoma [1]. Diagnostic possibilities and informativeness of objective measurable parameters were discussed with regard to special criteria, called clinical endpoints. The second part of the review is dedicated to finding methods of early diagnosis and criteria of the progression of glaucomatous optic neuropathy.

Currently, the mitochondrial component is becoming increasingly significant in the development and progression of glaucoma optic neuropathy. Since mitochondria have their own genetic apparatus, the study of qualitative and quantitative changes in mitochondrial DNA (mtDNA) becomes part of the disease diagnosis, in addition to studying the functional characteristics of the organelles alone. Moreover, the inability to explain the nature of the disease by known mutations of nuclear DNA contributes to the the study of mtDNA. In this review, we briefly discuss the mitochondrial genetics and the role of mitochondrial haplogroups in the diseases manifestation. We summarize the accumulated data on the qualitative and quantitative changes in mtDNA in patients with glaucoma. We discuss the approach to prevent the inheritance of mutant mtDNA as a part of assisted reproductive technologies, as well as the first steps towards the mtDNA heteroplasmy level manipulation and direct mtDNA editing.

The review addresses the management of primary glaucoma as a socially significant multifactorial disease. The main reasons that impede the timely diagnosis and treatment of patients with glaucoma are indicated: blurring of boundaries, conventionality of standards, and lack of individualized approach to treatment. The main risk factors for the development of glaucoma are highlighted, with special attention to hereditary predisposition and the role of “medicine of the future” in managing glaucoma. Four fundamental principles are described: personalization, prediction, prevention and participative attitude (P4 medicine). Advanced scientific understanding of the key risk factors for the development and progression of glaucoma, together with a modern personalized and personified approach will further develop precise individual strategies for the prevention and treatment of the disease.

The review is focused on modern methods of instrumental diagnostics of primary open-angle glaucoma. Diagnostic possibilities and informativeness of objective measurable parameters are discussed with regard to special criteria, called clinical endpoints.

Purpose. To find the connection between the parameters of the corneoscleral shell and lamina cribrosa (LC) and hemodynamic parameters of the eyes with primary open angle glaucoma (POAG). Materials and methods. The study involved 111 eyes of 63 patients, including 41 eyes without ophthalmic pathology (control group, mean age M ± SD 66.9 ± 18.2 yrs), 22 eyes with the initial glaucoma stage (group 1, mean age 65.9 ± 11.3 yrs), 31 eyes with the developed glaucoma stage (group 2, mean age 69.2 ± 17.3 yrs), and 17 eyes with the advanced glaucoma stage (group 3, mean age 69,7 ± 7.3 yrs). LC parameters and the density of surface (SVL) and deep (DVL) vascular layers were measured using Spectralis OCT2 with an angiography module (Heidelberg Engineering, Germany) in the enhanced deep imaging (EDI) mode using AngioTool software. Corneal hysteresis (CH) and corneal resistance factor (CRF) were determined using ORA (Ocular Response Analyzer, Reichert, USA). The rigidity coefficient (E) of the corneoscleral shell was measured by a GlauTest-60 tonograph (Russia) operation in the differential tonometry mode. The elasticity coefficient (s) was determined by a modified differential tonometry technique. The rheographic index (RI) and pulse blood volume (PBV) were measured using transpalpebral rheoophthalmography. Results. A statistically significant correlation was revealed between CH and SVL (р = 0.005, r = 0.288), LC thickness and SVL and DVL (р = 0.001, r = 0.374 and р = 0.003, r = 0.397, respectively). A negative statistically significant correlation was found between E and RI (р = 0.000, r = -0.538) as well as between E and PBV (р = 0.001, r = -0.376). A similar correlation was revealed between s and theses parameters (RI and PBV; р = 0.027, r = -0.404 and р = 0.024, r = -0.410, respectively). E increase is accompanied by a decrease in the SVL density (р = 0.000, r = -0.376); besides, PBV is negatively correlated with the LC depth (р = 0.022, r = -0.257). Conclusion. In POAG, deteriorated blood supply of inner ocular shells (decreased RI, PBV, SVL and DVL density) correlates with (a) increased corneoscleral rigidity (manifested in the increase of rigidity and elasticity coefficients, and the decrease of CH), and (b) decreased LC thickness and its increased depth. Increased rigidity of the corneoscleral shell contributes to an LC posterior displacement. It must be concluded that LC thickness is a more sensitive biomechanical parameter that changes even in the initial stages of POAG and has a high diagnostic value.

The paper reports on promising methods of drug delivery in glaucoma treatment: contact lenses, punctum plugs, intraocular implants, subconjunctival and supraciliary injections, ocular rings, hydrogels, nanocarriers.

, were measured for 57 patients, including 49 patients aged 51-75 (ave. 66.0 ± 1.7 years) with various stages of POAG and the control group of 8 patients aged 61-68 (ave. 63.7 ± 0.9 years) with no eye pathology (except for age-related cataract). All patients with POAG received hypotensive therapy. Of these, 24 patients received one of prostaglandin analogues (Glauprost) as monotherapy or in combination with beta-blockers and/or carbonic anhydrase inhibitors during no less than 3 months, and 25 patients only received hypotensive therapy beta-blockers and/or carbonic anhydrase inhibitors. The patients’ examination consisted of elastotonometry by Maklakov with three loads having different weights, modified Schiotz differential tonometry using a GlauTest 60 tonograph, measurement of corneal compensated intraocular pressure (IOPcc) and corneal hysteresis (CH) by Ocular Response Analyzer (ORA, Reichert, USA). Results. Coefficient К (2.45 ± 0.15) of patients with stage II POAG (mean age 65.0 ± 1.5 years) who did not use prostaglandin analogues was significantly higher (p < 0.05) than that of patients of the same age group (mean age 66.5 ± 1.8 years) with the same glaucoma stage who received Glauprost (К = 2.09 ± 0.17), which implies a higher risk of glaucoma progression. When prostaglandin analogues were used, CH level proved to be reliably (p < 0.05) higher (9.10 ± 0.4 mm Hg) as compared to patients who received no such therapy (8.14 ± 0.7 mm Hg): this is viewed as a favorable prognostic criterion for the POAG course. A less pronounced decrease of K and increase of CH after prostaglandin analogue instillations than in stage II POAG was also noted in patients with stage III POAG. Conclusion. Prostaglandin analogue Glauprost has a favorable impact on biomechanical parameters of the corneoscleral eye shell, especially in patients with stage II POAG. It lessens glaucoma progression risk induced by disturbed elastic properties of the cornea and the sclera and their ratio // Russian Ophthalmological Journal, 2017; 1: 15-9.

The literary review presents an analysis of modern views of the biomechanical properties of lamina cribrosa (LC) in glaucoma. It is shown that LC deformations in glaucomatous eyes are connected with their morphological and structural disorders at macro- and micro levels even in the early stages of glaucoma development. New visualization technologies of ocular structures, in particular, optical coherent tomography with enhanced depth imaging (EDI) enable in vivo evaluation of biomechanical characteristics of LC and its stress-strain from the data on the changing shape of its anterior surface (curvature index), thickness and deepness, pore dimensions, presence and severity of LC tissue defects. LC changes inherent to glaucoma are significant criteria for the estimation of the risk of glaucoma emergence and its early diagnostics, determination of the stage of the pathological process, and prediction of its progression rate. Primary or secondary disturbance of blood supply of eye tissues plays a leading role in the pathogenesis of deterioration of visual functions in various ophthalmopathological processes, including primary open angle glaucoma (POAG). For this reason, the study of intraocular hemodynamics can be viewed as a promising direction in clinical ophthalmology. Of particular importance is the evaluation of blood flow in the uveal tract, since it accumulates more than 80 % of blood entering the eye, thus being the primary collector of blood flow and implements the nutritional function in the inner shells of the eye. Rheoophthalmography is one of the main methods of determining the blood supply of the uveal tract. Reaching a prolonged hypotensive effect in the postoperative period is one of the main tasks of modern glaucoma surgery. The purpose of this work is to develop a new fully biodegradable drainage structure, made of domestically produced materials, and to use modified sinustrabeculectomy (STE) in the surgical treatment of primary open angle glaucoma (POAG) and refractory glaucoma. Materials and methods. The operation was performed in 27 patients (27 eyes) aged 50-70 (averagely 59.5 ± 1.9 years), who had been operated for POAG II (9 patients, 33.3 %) or POAG III (18 patients, 66.7 %) of staged b - c. For drainage, we used polyglycolic thread, which is an absorbable synthetic suture material. Complete absorption of drainage takes 60 to 90 days. The examination included standard methods (visometry, tonometry, computer perimetry, tonography, optic nerve HRT. In the early postoperative period optical coherence tomography of the anterior segment (ASOCT) was used, which was followed by ultrasound biomicroscopy (UBM). The follow-up period was up to 1 year. Results.1-3 months after the surgery, antihypertensive efficacy was noted in 100 % of cases (27 eyes), and 9-12 months after the surgery the figure was 98 % (26 of 27 eyes). Conclusions. We consider the differential splitting scheme on physical processes on the system one-dimensional equations viscous compressible fluid mixtures movement. Purpose: to investigate hypotensive efficiency of new surgical intervention at terminal primary and neovascular glaucoma.Methods. Operation has been executed on 35 eyes of 35 patients with terminal primary (23 eyes) and secondary (12 eyes) neo- vascular glaucoma. A fornix based conjunctival flap was prepared in inferior part of a limbus. 1.5 mm transparent scleral incision 3 mm from limbus was performed. Goniocyclodialysis with spatula was carrying out. then 0.3 ml of viscoelastic solution was entered in cyclodialysis cleft. On sclera put cryoapplications, located in two ranks (on 6 in everyone). the period of postoperative follow-up has averaged 15.6±1.3 months.Results: In 1 month IOP did not exceed 25 mm hg and the painful syndrome has been stopped in all cases. In 1 year after operation normalization IOP took place on 27 eyes (77.2%), from them on 8 was local hypotensive therapy added. With 8 patients (22.8%) IOP instillation of 2 preparations remained at level of 28-30 mm Hg, however the painful syndrome was absent.Conclusion: Long-term hypotensive effect of new operation is reached at the expense of the double mechanism: reduction of production of aqueous humor for the account cryopexy of ciliary body and improvements aqueous outflow for formations cyclodialysis cleft.
