University counseling centers (UCCs) are tasked with delivering effective mental health interventions amidst increasing enrollment and declining resources. A growing number of UCCs have implemented session limits to meet demand with limited resources. To further understand the impact of this trend, we examined data obtained from a nationwide sample of 15,802 clients seeking treatment from 580 therapists at 32 UCCs utilizing the Behavioral Health Measure-20 (BHM-20). We compared psychotherapy outcomes to the number of clinical staff at UCCs utilizing either explicitly defined session limits or ambiguously defined session limits. There was a significant interaction between the way the sample of UCCs defined annual session limits, number of staff, and therapy outcomes. Specifically, higher therapy outcomes were observed at UCCs with explicitly defined session limits and fewer available staff. The ratio of enrolled students to clinical staff was not a predictor of outcome. Implications for clinical practice and agency session limits policy are discussed.

Psychotherapy has historically focused on symptom reduction. However, there are calls for increasing our understanding of how psychotherapy impacts clients' flourishing. In this study, licensed mental health professionals and trainees across different clinical settings with diverse therapeutic orientations were interviewed about their perspectives on the cultivation of flourishing in psychotherapy, as well as their own personal experiences with flourishing. We were interested in understanding how therapists define flourishing, how they integrate flourishing into their work with clients, and any barriers or catalysts for engaging in this process. To do so, we conducted eight focus groups, and interviews were analyzed using grounded theory to form a hierarchy of categories. The results were used to create a three-part grounded theory. Results suggested that flourishing is a distinct concept from well-being that involves active engagement, emotional connection and meaning making. Our three-part grounded theory of flourishing elucidates how flourishing can be pursued by guiding clients towards their experiences of suffering in order to engage in meaning making and to identify their values, as well how systemic barriers can be navigated. In this longitudinal study (N = 98), we examined whether drinking for suppression reasons moderated the relation between depressive symptoms and hooking up for self-affirmation reasons and negative hookup consequences. No moderation was found for hooking up for self-affirmation reasons, but the effect for negative hookup consequences approached significance. When drinking for suppression reasons is low, participants with fewer depressive symptoms reported more negative consequences, and participants with more depressive symptoms reported fewer negative consequences 2 months later. Results are discussed within the context of sex education and mental health programming for young adults.

We responded to the need for replication in psychotherapy research by extending a prior naturalistic study documenting the effectiveness of long‐term psychodynamic treatment. We employed three novel analytic strategies and evaluated their influence on substantive interpretations offered in the prior study. The sample consisted of clients (N = 387; Mage = 31.61; SD = 11.13; 58.9% female; 79.1% Caucasian/White) receiving outpatient treatment at a psychodynamic training clinic. Results replicated the original two‐class solutions for social functioning, depression and life satisfaction. Novel to the replication analyses, each trajectory for social functioning and depression exhibited nonlinear change and results advanced research on a theorised mechanism of effective psychodynamic treatment. Results showed that change in experiential avoidance was a possible mechanism between early‐treatment change in social functioning and later‐treatment changes in depression symptoms and life satisfaction. Clinical implications focus on the influence that initial levels of symptoms and well‐being may have on early and later phases of treatment, monitoring progress throughout treatment, and how to promote symptom and well‐being improvement. Objective: This study aimed to develop an updated brief self-report post-session measure, suitable for collecting systematic feedback on clients' session reactions in the context of measurement-based care (MBC). Method: The Session Reactions Scale-3 (SRS-3; 33 items) was developed by extending and adjusting the Revised Session Reactions Scale. In Study 1, the psychometric properties of the SRS-3 were tested on N = 242 clients. In Study 2, a brief version of the SRS-3 (SRS-3-B; 15 items) was developed using a combination of conceptual, empirical, and pragmatic criteria. In Study 3, the psychometric properties of the SRS-3-B were tested on a new sample of N = 265 clients. Results: Exploratory factor analysis supported the use of the SRS-3-B as a two-factor (helpful reactions, hindering reactions) or unidimensional (overall session evaluation) instrument. The SRS-3-B was meaningfully related to another process measure (Individual Therapy Process Questionnaire) both on the item and factor levels. Conclusions: The SRS-3-B is a reliable process measure to elicit rich and clinically meaningful feedback from clients within the MBC context and as a research instrument to assess the helpful and hindering aspects of therapy sessions. This pilot study involved the development and initial testing of an applied Multicultural Orientation Performance Task (MCO‐PT) and coding system. The MCO‐PT was comprised of seven brief video simulations of cultural exchanges. Seventy‐four graduate students recorded responses and then completed measures of desirable responding and multicultural competence. Research assistants coded the videos on three dimensions: cultural humility, cultural opportunities and cultural comfort (ICCs 0.61–0.97). Exploratory factor analysis yielded three factors that corresponded with the MCO constructs (alphas 0.77–0.97). Cultural opportunities were positively correlated with multicultural competence, and none of the subscales were associated with social desirability. [ABSTRACT FROM AUTHOR]

The current retrospective study examined whether clients' (N = 176) perceptions of their psychotherapists' multicultural orientation (MCO) were associated with their psychological functioning, working alliance, and real relationship scores. Moreover, we tested whether clients' perceptions of the working alliance and the real relationship mediated the relationship between clients' perceptions of their psychotherapists' MCO and psychological functioning. The results showed that clients' perceptions of their psychotherapists' MCO were positively related to working alliance, real relationship, and psychological functioning. Only clients' ratings of the working alliance mediated the relationship between clients' perceptions of their psychotherapists' MCO and psychological functioning. Thus, because clients perceive their psychotherapists as being more oriented toward cultural issues, they may view the therapist as being more credible and may gain a sense of comfort in the therapeutic process. In turn, clients' strong alliance facilitates improvement in psychological well-being.
