Abstract

Objective: Numerous factors are considered to impact on the rate of complications during salvage total laryngectomy procedures. Neck dissection could be one of these factors. This study analysed the pattern of lymph node metastasis and rate of occult neck disease during salvage total laryngectomy as well as the impact of neck dissection on survival and complication rates. Method: This was a retrospective analysis of a prospectively maintained laryngectomy database in two large tertiary teaching hospitals. Results: The rate of occult neck disease was 11.1 per cent. Most cases with occult neck disease had rT4 disease. Patients with complications, advanced tumour stage and positive margins had a significant decrease in overall survival. Patients receiving elective neck dissection did not have any survival benefit. Positron emission tomography-computed tomography showed a very high specificity and negative predictive value. Conclusion: According to the low risk of occult neck disease when using contemporary imaging techniques as well as the lack of impact on survival, conservative management of the neck should be considered for crT1-T3 recurrence. 19. Respiratory strategy at birth initiates distinct lung injury phenotypes in the preterm lamb lung

Abstract

BACKGROUND: A lack of clear trial evidence often hampers clinical decision-making during support of the preterm lung at birth. Protein biomarkers have been used to define acute lung injury phenotypes and improve patient selection for specific interventions in adult respiratory distress syndrome. The objective of the study was to use proteomics to provide a deeper biological understanding of acute lung injury phenotypes resulting from different aeration strategies at birth in the preterm lung. METHODS: Changes in protein abundance against an unventilated group (n = 7) were identified via mass spectrometry in a biobank of gravity dependent and non-dependent lung tissue from preterm lambs managed with either a Sustained Inflation (SI, n = 20), Dynamic PEEP (DynPEEP, n = 19) or static PEEP (StatPEEP, n = 11). Ventilation strategy-specific pathways and functions were identified (PANTHER and WebGestalt Tool) and phenotypes defined using integrated analysis of proteome, physiological and clinical datasets (MixOmics package). RESULTS: 2372 proteins were identified. More altered proteins were identified in the non-dependent lung, and in SI group than StatPEEP and DynPEEP. Different inflammation, immune system, apoptosis and cytokine pathway enrichment were identified for each strategy and lung region. Specific integration maps of clinical and physiological outcomes to specific proteins could be generated for each strategy. CONCLUSIONS: Proteomics mapped the molecular events initiating acute lung injury and identified detailed strategy-specific phenotypes. This study demonstrates the potential to characterise preterm lung injury by the direct aetiology and response to lung injury; the first step towards true precision medicine in neonatology.

25. Cypriot migration and settlement in Britain

Abstract

This is a social scientific study of the extent, character and causes of Cypriot migration and settlement in Britain between the years 1945 and 1966. It is based largely on published sources, mainly statistical. Some 75,000 Cypriots, mostly young adults, emigrated from Cyprus to Britain during the period - numbers increasing up to around 1960, and then declining sharply. A variety of circumstances affecting the movement are considered, the most important of which are found to be economic conditions in Britain and Cyprus. Cypriot settlement in Britain amounted in all to more than 100,000 persons in 1966, and was highly concentrated in London. The demographic and economic characteristics of London Cypriots are analysed in detail, and the settlers are found to have developed a thriving and remarkably self-sufficient economic base for community life as an ethnic minority in modern Britain. While their migration has been roughly coincident with that of recent 'coloured' immigrants to Britain from other Commonwealth countries, the character of Cypriot settlement has more in common with that of Jewish immigrants to Britain over half a century previously.

Abstract

Research has been conducted on the 5-cyanofuran-2-[1,2,3,5-diselenadiazolyl]. The preparation and the crystal structure of this compound are reported and the results indicate that this crystal structure consists of diselenadiazolyl pi-dimer stacks which run parallel to the x direction and that the asymmetric unit consists of four pi-dimer units.
