Abstract

Oxidative stress is a pathological hallmark of neurodegenerative tauopathic disorders such as Alzheimer's disease and Parkinson's disease-related dementia, which are characterized by altered forms of the microtubule-associated protein (MAP) tau. MAP tau is a key protein in stabilizing the microtubule architecture that regulates neuron morphology and synaptic strength. The precise role of reactive oxygen species (ROS) in the tauopathic disease process, however, is poorly understood. It is known that the production of ROS by mitochondria can result in ultraweak photon emission (UPE) within cells. One likely absorber of these photons is the microtubule cytoskeleton, as it forms a vast network spanning neurons, is highly co-localized with mitochondria, and shows a high density of aromatic amino acids. Functional microtubule networks may traffic this ROS-generated endogenous photon energy for cellular signaling, or they may serve as dissipaters/conduits of such energy. Experimentally, after in vitro exposure to exogenous photons, microtubules have been shown to reorient and reorganize in a dose-dependent manner with the greatest effect being observed around 280 nm, in the tryptophan and tyrosine absorption range. In this paper, recent modeling efforts based on ambient temperature experiment are presented, showing that tubulin polymers can feasibly absorb and channel these photoexcitations via resonance energy transfer, on the order of dendritic length scales. Since microtubule networks are compromised in tauopathic diseases, patients with these illnesses would be unable to support effective channeling of these photons for signaling or dissipation. Consequent emission surplus due to increased UPE production or decreased ability to absorb and transfer may lead to increased cellular oxidative damage, thus hastening the neurodegenerative process.

Abstract

Background: Increasing research suggests that gait abnormalities can be a risk factor for Alzheimer’s Disease (AD). Notably, there is growing evidence highlighting this risk factor in individuals with amnestic Mild Cognitive Impairment (aMCI), however further studies are needed. The aim of this study is to analyze cognitive tests results and brain-related measures over time in aMCI and examine how the presence of gait abnormalities (neurological or orthopedic) or normal gait affects these trends. Additionally, we sought to assess the significance of gait and gait-related measures as prognostic indicators for the progression from aMCI to AD dementia, comparing those who converted to AD with those who remained with a stable aMCI diagnosis during the follow-up. Methods: Four hundred two individuals with aMCI from the Alzheimer’s Disease Neuroimaging Initiative (ADNI) database were included. Robust linear mixed-effects models were used to study the impact of gait abnormalities on a comprehensive neuropsychological battery over 36 months while controlling for relevant medical variables at baseline. The impact of gait on brain measures was also investigated. Lastly, the Cox proportional-hazards model was used to explore the prognostic relevance of abnormal gait and neuropsychological associated tests. Results: While controlling for relevant covariates, we found that gait abnormalities led to a greater decline over time in attention (DSST) and global cognition (MMSE). Intriguingly, psychomotor speed (TMT-A) and divided attention (TMT-B) declined uniquely in the abnormal gait group. Conversely, specific AD global cognition tests (ADAS-13) and auditory-verbal memory (RAVLT immediate recall) declined over time independently of gait profile. All the other cognitive tests were not significantly affected by time or by gait profile. Abstract

Loss-of-function mutations in the genes associated with primary microcephaly (MCPH) reduce human brain size by about two-thirds, without producing gross abnormalities in brain organization or physiology and leaving other organs largely unaffected [Woods CG, et al. (2005) Am J Hum Genet 76:717–728]. There is also evidence suggesting that MCPH genes have evolved rapidly in primates and humans and have been subjected to selection in recent human evolution [Vallender EJ, et al. (2008) Trends Neurosci 31:637–644]. Here, we show that common variants of MCPH genes account for some of the common variation in brain structure in humans, independently of disease status. We investigated the correlations of SNPs from four MCPH genes with brain morphometry phenotypes obtained with MRI. We found significant, sex-specific associations between common, nonexonic, SNPs of the genes CDK5RAP2 , MCPH1 , and ASPM , with brain volume or cortical surface area in an ethnically homogenous Norwegian discovery sample ( n = 287), including patients with mental illness. The most strongly associated SNP findings were replicated in an independent North American sample ( n = 656), which included patients with dementia. These results are consistent with the view that common variation in brain structure is associated with genetic variants located in nonexonic, presumably regulatory, regions.

6. Descriptive Characteristics of Patients Hospitalized with Congestive Heart Failure: A Brief Summary.

Abstract

Background: There are approximately 5.1 million patients with chronic, or congestive, heart failure (CHF) in the United States, accounting for one million hospital admissions, 6.5 million hospital days, and $37.2 billion in healthcare expenditure. The cost derives mainly from inpatient services including length of stay. The impetus has been to decrease LOS while improving patient outcomes. HF is associated with higher prevalence of C-Diff among hospitalized patients with common bacterial infections, even while adjusting for other C-Diff risk factors. Additionally, patients with HF and C-Diff had a significantly higher in-hospital mortality. There is a gap in the evidence related to the effects of patient and hospital characteristics on the outcomes of inpatients with chronic heart failure. Method: This project was the first part of a larger study designed to investigate the prevalence and factors affecting inpatients hospitalized with CHF, using a longitudinal dataset from national inpatient samples. ICD 9 codes (CHF-428) were used for the extraction of the variables. This report focuses only on the descriptive characteristics of the sample patient population. Results: The results showed that there were 697, 334 men and women hospitalized with chronic heart failure (46.88% male and 53.12% female, with an average age of 76.08 years). The ethnic distribution of patients were 74.64% White; 14.07% Black; 6.41% Hispanic; 1.72% Asian; 0.22% Native American; and 2.60% (other). The average LOS was 6.03 days with an average cost of $76,764 per patient, with a 94.88% survival rate after hospitalization. Conclusion: These preliminary results indicate that patients hospitalized with chronic heart failure are primarily White, older, have lower household incomes, and exhibit diabetes as the major comorbidity, with approximately half of the patient admissions at urban teaching hospitals. These results may be helpful in identifying patterns in patient/hospital characteristics which could lead to improved patient outcomes and lower re-hospitalization rates. [ABSTRACT FROM AUTHOR]

Abstract

Oxidative stress is a pathological hallmark of neurodegenerative tauopathic disorders such as Alzheimer's disease and Parkinson's disease-related dementia, which are characterized by altered forms of the microtubule-associated protein (MAP) tau. MAP tau is a key protein in stabilizing the microtubule architecture that regulates neuron morphology and synaptic strength. When MAP tau is degraded in tauopathic disorders, neuron dysfunction results. The precise role of reactive oxygen species (ROS) in the tauopathic disease process, however, is poorly understood. Classically, mitochondrial dysfunction has been viewed as the major source of oxidative stress and has been shown to precede tau and amyloid pathology in various dementias, but the exact mechanisms are not clear. It is known that the production of ROS by mitochondria can result in ultraweak photon emission (UPE) within cells. While of low intensity, surrounding proteins within the cytosol can still absorb these energetic photons via aromatic amino acids (e.g., tryptophan and tyrosine). One likely absorber of these photons is the microtubule cytoskeleton, as it forms a vast network spanning neurons, is highly co-localized with mitochondria, and shows a high density of aromatic amino acids. Functional microtubule networks may traffic this ROS-generated endogenous photon energy for cellular signaling, or they may serve as dissipaters/conduits of such energy to protect the cell from potentially harmful effects. Experimentally, after in vitro exposure to exogenous photons, microtubules have been shown to reorient and reorganize in a dose-dependent manner with the greatest effect being observed around 280nm, in the tryptophan and tyrosine absorption range. In this paper, recent modeling efforts based on ambient temperature experiment are presented, showing that tubulin polymers can feasibly absorb and channel these photoexcitations via resonance energy transfer, on the order of dendritic length scales and neuronal fine structure. Since microtubule networks are compromised in tauopathic diseases such as Alzheimer's and Parkinson's dementias, patients with these illnesses would be unable to support effective channeling of these photons for signaling or dissipation. Consequent emission surplus due to increased UPE production or decreased ability to absorb and transfer may lead to increased cellular oxidative damage, thus hastening the neurodegenerative process., (Copyright © 2017 Elsevier B.V. Abstract

The aim of our study was to examine whether there is an association between blood pressure reactivity to the cold pressor test in African Americans who engaged in different levels of physical activity. We examined the systolic pressure, diastolic pressure, mean arterial blood pressure, heart rate, cardiac index, total peripheral resistance, and forearm blood flow during a two-minute cold pressor test in 15 aerobic, physically active and 15 physically inactive, normotensive young adult African-American males. Peak oxygen consumption varied as a function of physical activity, and was significantly higher in the physically active than in the physically inactive subjects (54.5 +/- 1.5 vs 36.8 +/- 0.7 ml kg(-1) min(-1)) (P<.05). During the cold pressor test, consisting of immersing the foot in ice water, the change in cardiovascular responses were similar between the physically active and the physically inactive groups. These results suggest that regular physical activity may not contribute to an attenuated blood pressure response to behavioral stress of the cold pressor test in normotensive young adult African-American males.

Objective: To determine the association between alcohol intake and the risk of developing age-related macular degeneration (AMD)., Design: Case control study., Participants: The sample consisted of 3072 adults 45 to 74 years of age with macular changes indicative of AMD who participated in a nationally representative sample of the first National Health Nutrition and Examination Survey (NHANES-1) between 1971 and 1975: (a) the ophthalmology data set and (b) the medical history questionnaire., Main Outcome Measures: Alcohol intake and the risk of developing AMD were measured. AMD was determined by staff at the National Eye Institute by fundoscopy examination using standardized protocol., Results: Overall, 184 individuals (6%) had AMD. We observed a statistically significant but negative association between AMD and the type of alcohol consumed in a bivariate model (OR 0.86; 95% CI 0.73, 0.99). In the same model, age maintained a consistently strong association with AMD (OR 1.08; 95% CI 1.06-1.11; P < .001). Among the different types of alcohol consumed in NHANES-1 (beer, wine, and liquor), the effect of wine, either alone (OR 0.66; 95% CI 0.55-0.79) or in combination with beer (OR 0.66; 95% CI 0.55-0.79) or liquor (OR 0.74; 95% CI 0.63-0.86), dominated the negative association observed between AMD and alcohol type. Additionally, a statistically significant and negative association between wine and AMD was noted after adjusting for the effect of age, gender, income, history of congestive heart failure, and hypertension (OR 0.81; 95% CI 0.67-0.99)., Conclusion: Moderate wine consumption is associated with decreased odds of developing AMD. Health promotion and disease prevention activities directed at cardiovascular disease may help reduce the rate of AMD-associated blindness among older people. The nature and pathophysiology of this association warrant further investigation.

Abstract

Objectives: Patients with congestive heart failure are often undernourished. The measurement of resting energy expenditure has served as the basis upon which estimates of daily caloric needs have been developed. Resting energy needs, however, in heart failure patients are unknown. We have developed a new equation to predict resting energy needs in heart failure patients that takes into account easily measured clinical variables and symptom severity., Design: Observational., Setting: Baltimore VA Medical Center., Subjects: Forty male patients with heart failure aged, 57-85 y; (27 Class II; 13 Class IV)., Measurements: Resting metabolic rate was measured by indirect calorimetry, fat-free mass and fat mass by dual energy X-ray absorptiometry, peak VO2 by a treadmill test. Symptom severity was measured by the New York Heart Association classification. Ejection fraction, plasma albumin and plasma glucose were also assessed., Results: Stepwise regression analysis showed that body weight, fasting glucose, plasma albumin and New York Heart Association classification accounted for 83% of the variation in resting energy needs. The regression equation had a root mean square error of 130 kcal (544 kJ) per day. The equation is: RMR (kcal/d) = 12.2 (wt, kg) + 1.6 (glucose, gm/dl) + 103 (NYHA; III, IV)-144 (albumin, mg/dl) + 755. Moreover, prediction equations based on observations in healthy individuals significantly underestimated resting energy needs in heart failure patients., Conclusion: We offer a new equation to predict resting energy needs in heart failure patients based upon readily available clinical measurements. Further studies are needed to cross-validate our equation.
