- Abstract
The endoperiodontal lesion is an inflammation that contains both endodontic and periodontal coverage. The intimate relationship between the dental pulp and the periodontal tissue is given through the apical foramen and branching of the main canal. This close relationship between tissues results in the possibility of exchange of pathogenic microorganisms and contamination of one or both tissues. As consequence, there may be clinical and imaging manifestations in which their signs and symptoms can be confused, resulting in misdiagnosis. Due to the complexity of diagnosing and establishing a treatment plan for endoperiodontal lesions due to their similarities, this work aims to present a clinical case of an alteration with clinical characteristics similar to an endoperiodontal lesion, with emphasis on the importance of implementing correct strategies for diagnosis and establishment of an effective treatment for therapeutic success. Endodontic treatment is common in dentistry and for a correct diagnosis it is necessary to observe clinical and radiographic signs, taking into account pulp and periapical changes in the element to be treated. Studying this frequency can contribute to the literature, elucidating whether the greatest number of cases is occurring in young or elderly patients, women or men, anterior or posterior teeth. The objective of this study was to analyze the prevalence of endodontically treated teeth, through 900 panoramic radiographs. A cross-sectional study was carried out evaluating the prevalence of teeth treated endodontically from a sample of 900 panoramic radiographs, corresponding to exams performed in 2020, in a radiological clinic located in central west Brazil. The sample consisted of 540 (60%) female subjects and 360 (40%) male subjects, aged between 18 and 88 years old, being more prevalent for Individuals <31 years old with (34.9%) Sample. The prevalence of endodontically treated teeth was analyzed, the results showed 1,236 teeth in this condition, individually analyzing the upper right central incisor was the most frequent 9.22%, while for group analysis the upper teeth were 7.06%. Therefore, there was a higher prevalence of teeth treated endodontically in the maxilla, the tooth with the highest treatment index, the right upper central incisor. Women were more affected and it was observed that with increasing age, the number of teeth treated endodontically also increases. [ABSTRACT FROM AUTHOR]

Introduction: Root canal fillings are a decisive factor in the success of endodontic treatment. This systematic review evaluated animal studies using histopathological parameters to determine the effect of root canal filling on the healing of apical periodontitis. Methods: A search was conducted in PubMed, Lilacs, Scielo, Science Direct and BBO using indexed and non-indexed keywords. Studies published from 2003 to 2019 were selected according to inclusion and exclusion criteria. Results: Five animal studies met eligibility criteria and were included in the systematic review for a qualitative analysis. Conclusion: The extension of the root canal filling may affect the healing of apical periodontitis when bacteria remain in the canal. Root canal sealers with antimicrobial properties were not effective in eliminating bacteria remaining in the root canal, nor in promoting the complete healing of apical periodontitis. MINERAL aggregates , *EPOXY resins , *CALCIUM silicates , *CEMENT , *DENTAL cements , *CALCIUM hydroxide - Abstract
The search for a high-performance filling cement has encouraged several kinds of research with different materials of calcium hydroxide, epoxy resin, and, more recently, MTA (Mineral Trioxide Aggregate). Calcium silicate bioceramic cement is the finalproduct of physical improvement and chemical qualities of MTA, considered as a third-generation smart paste, with bioactive properties promisors in endodontics. The objective of this paper was to review the properties and the physical-chemical characteristics of two third-generation bioceramic cement and compared they with an epoxy resin-based cement considered the gold standard in root canal filling (AH Plus). Thus, we conduct a review with 78 studies, published in different scientific journals, in the last 20 years. After analyzing the articles, we found that bioceramic cements showed superior mechanical stability and mechanical resistance based on ISO standards, in different stages, when compared with other materials. So, this study showed that bioceramic cements have been considered promissory due to their physical-chemical properties approach ISO standards and surpass AH Plus cement in some aspects that justify their application in Dentistry Clinical. - Abstract
The complete filling of the root canal system depends on the use of a material that drains and seals irregularities in the anatomy of the canal. This work aims to analyze the efficiency of Bio-C Sealer, Sealer Plus and AH Plus endodontic sealers in occupying the pulp cavity irregularities. 33 human lower incisors were selected. after biomechanical preparation with reciprocating X1 Blue files, were filled using single-cone technique. After 48 hours, the teeth were sectioned transversely at a distance of 5mm from the root apex and the most cervical fragment was photographed. The images had the total canal area, gutta-percha and sealer-filled and empty areas measured in pixels using Axion Vision software. Of the 11 teeth filled with AH Plus sealer 1.56% of the root canal area remained empty, for Sealer Plus the percentage was 1.90% and for Bio-C Sealer 1.32%. All sealers tested were effective in filling the root anatomy irregularities reaching a percentage above 98%, with BioC Sealer having the lowest void indexes, followed by AH Plus and Sealer Plus. Confocal laser scanning microscopy (CLSM) was used to investigate the penetration of endodontic sealers into the dentinal tubules after retreatment using two different obturation techniques.Thirty mandibular premolars were prepared up to instrument F3 (ProTaper Universal, Dentsply) and filled with Endofill using the single cone technique. The canals were retreated using Mtwo instruments. Reobturation was performed with the Bio-C sealer mixed with a fluorophore dye (Fluo-3) using either the lateral condensation technique (group LC) or the single cone technique (group SC) (n = 15). Teeth were sectioned 2, 4, and 6 mm from the apex and analyzed with CLSM to assess the penetration of the sealer into the canal perimeter and the maximum depth of penetration of the sealer into the dentinal tubules. Data were analyzed using ANOVA and the Student-t and Holm-Sidak tests. In the apical segment, the penetrated perimeter was significantly higher in the LC group than in the SC group (p 0.05); no significant difference was found in the middle and cervical segments (p 0.05). In terms of penetration depth, no significant differences were found for any of the segments studied (p 0.05).The LC technique promoted a higher percentage of canal circumference penetrated by the sealer than the SC technique in the apical segment after endodontic retreatment.CLSM demonstrated that the LC technique promoted a higher percentage of canal perimeter penetrated by the Bio-C sealer than the SC technique in the apical segment of mandibular premolars after retreatment.

Introduction: There are several techniques to perform am obturation, being the lateral condensation technique the most commonly used because it is simple, easy to perform and does not require any special device and can be used in most clinical situations, also allowing the control of the extension. during compaction. Objective: The objective of this study was comparing the lateral condensation’s obturation techniques with single cone and hybrid Tagger regarding the filling’s quality of the root canal. Methods: 30 human unirradicular teeth was used, which the coronary opening was performed with spherical and frustoconical diamond drills. After the canal exploration with Lima K 10 and 15 was visually determined the working length (CT) of the tooth that will correspond to the actual tooth length minus one millimeter (mm). The canal instrumentation was realized with files with the system ProDesign M until the instrument 40.05. During the instrumentation the canis was irrigated with 2.5% sodium hypochlorite and for final irrigation, 17% EDTA was used. After the canis’s drying, the teeth were randomly distributed in three groups (n10) according the technique used to obturate the root canal: G1 lateral condensation; G2 single cone technique; G3 hybrid Tagger technique. The filling quality was assessed by conventional and digital radiographs by 2 calibrated examiners, which were assigned scores to quantify the presence of spaces in the obturator material in the 3-thirds root. Result: The results were analyzed statistically by Kruskal Wallis test with the Dunn post-hoc test’s means (α <0.05). There were no differences in the middle and apical thirds between the techniques used for the obturation of the canals, however, both in the cervical third the worst results were observed for G2 (single cone). Conclusion: It was concluded that the Hybrid Tagger technique has the best canal sealing. [ABSTRACT FROM AUTHOR]

- Abstract
The Enterococcus faecalis are microorganisms frequently found in persistent endodontic infections and are probably the best species to adapt to and tolerate the poor ecological conditions in root canal obturation. In order to show which characteristics make E. faecalis so resistant and to verify its susceptibility to different antimicrobial agents, a broad literature review of the literature was performed aiming to analyze the performance of the intracanal medication on this etiologic agent. The search was based on access to the Pubmed and Bireme databases. Fifty manuscripts met the inclusion criteria and were used in this review. The results showed that E. feacalis a rapid recovery from nutritional deprivation and environmental stress, has strong factors of viirulence and is located in places difficult to reach the endodontist. When submitted to different intracanal medicinal products, E. faecalis had a higher susceptibility to those containing chlorhexidine. However, this bacterium still presents a high level of complexity in its elimination and a potential risk to endodontic failure. [ABSTRACT FROM AUTHOR]

This study aimed to evaluate the intratubular penetration of Bio-C Sealer using confocal laser scanning microscopy (CLSM). Sixty canines were divided into four groups (n = 15): Bio-C Sealer, Endo Sequence BC Sealer, Sealer Plus BC, and AH Plus. Three 1-mm thick discs were obtained from each sample and analyzed to determine the area and maximum penetration depth of the sealers. Bio-C Sealer showed maximum penetration in the cervical and middle third, lower than the AH Plus (p < 0.05) but similar to the other calcium silicate sealers evaluated (p > 0.05). In the apical thirds, there was no difference between the sealers (p > 0.05). The area of penetration of Bio-C Sealer was similar to AH Plus and calcium silicate sealers (p > 0.05). Hence, it can be concluded that Bio-C Sealer had similar intratubular penetration as the other tested sealers, and it can be used as an endodontic sealer.

Para as variáveis binárias, não houve diferença observada entre os cimentos avaliados: 24h (OR 1,12 [0,69; 1,80] p = 0,6476), 48h (OR 1,56 [0,76; 3,20] p = 0,2267), 72h (OR 1,38 [0,55; 3,45] p = 0,4893) e 7 dias (OR 2,10 [0,55; 8,01], p = 0,2790). The aim of this study of systematic reviews and meta-analyses was to answer whether bioceramic sealers have better effects on postoperative pain, ability to penetrate dentinal tubules and antimicrobial activity compared to AH Plus® sealer. Two systematic reviews, guided by PRISMA guidelines, were conducted and registered in PROSPERO (CRD4202125928) (Chapter 1) and Open Science Framework (OSF) Registries (https://doi.org/10.17605/OSF.IO/BX7VQ) (Chapter 2). A question was asked based on population, intervention, comparison and outcome (PICO), Chapter 1: “Does the use of bioceramic sealers result in less postoperative pain compared to the use of AH Plus® sealer in endodontically treated patients?”; Chapter 2: “Do bioceramic filling sealers have superior penetration into dentinal tubules and antimicrobial activity compared to AH Plus® filling sealer?”. Search strategies were defined and searches performed in the following databases: PubMed, Scopus, Web of Science, Embase, Cochrane Library, and OpenGrey. Chapter 1 used the Cochrane scale to assess the risk of bias and the GRADE tool to assess the quality of evidence. Meanwhile, Chapter 2 used The Joanna Briggs scale to assess the risk of bias from in vitro studies. Meta-analyses were conducted using the “Meta” package, version 3.6.3, the mean difference (MD) effect measure was calculated for quantitative variables and odds ratio (OR) (Chapter 1) and standardized mean differences (SMD) (Chapter 2), with a 95% confidence interval (CI). The results of Chapter 1 were included 13 articles in the systematic review, 11 were included in the meta-analysis. For quantitative variables, the bioceramic sealer had a lower occurrence of postoperative pain than the AH Plus® sealer in 24 hours (MD - 0.4101 [-0.80; - 0.02], p = 0.0386) and 48h (MD -0.31 [-0.59; -0.03], p = 0.0295). For the binary variables, there was no difference observed between the sealers evaluated: 24h (OR 1.12 [0.69; 1.80] p = 0.6476), 48h (OR 1.56 [0.76; 3.20] p = 0.2267), 72h (OR 1.38 [0.55; 3.45] p = 0.4893) and 7 days (OR 2.10 [0.55; 8.01], p = 0 .2790). Regarding the risk of bias analysis, a low risk was observed for most domains, except for allocation that was considered unclear, while the certainty of evidence ranged from moderate to low. The results of Chapter 2 were a total of 54 studies included, and 16 studies were included in the meta-analysis. Overall, the studies had a low risk of bias. There was no statistical difference between the sealers evaluated for meta-analyses. [dissertation] penetration into dentinal tubules, regardless of the thirds: coronal SMD 0.58 [0.14; 1.31], p = 0.12; average SMD 0.07 [0.54; 0.39], p = 0.75; and apical: SMD 0.08 [0.73; 0.56], p = 0.80. Bioceramic sealers and AH Plus® demonstrated similar antimicrobial action to SMD [3.42; 5.32], p = 0.67 and SMD 0.67 [1.89; 0.55], p = 0.2825. Thus, it is concluded that bioceramic sealers have less postoperative pain in the first 24 and 48 hours, and have similar responses for penetration into dentinal tubules and antimicrobial activity when compared to AH Plus® sealer. The clinical-radiographic control, after endodontic treatment, allows to evaluate and prove the results of endodontic therapy, classifying it as success or failure. Endodontic success can be defined as the final result of endodontic therapy, when the tooth is clinically asymptomatic, functional and without radiographic pathology. This study aimed to evaluate the quality of endodontic treatment performed by undergraduate students at the Faculty of Dentistry, University of Pernambuco, Brazil. The dental records of the sample included all patients seen in 2000 and 2022. Chi-square and Fisher's exact tests were used to calculate the significance level set at 5%. The sample size was 219 patients and it was possible to verify the success in most treatments, observing a percentage of 85.4% (188 patients) and failure in only 31 patients (14.2). Regarding the apical limit of the root canal system obturation, the treatment success was much higher when the obturation was in the apical foramen and in the CDC, 92.0 and 91.4%, respectively and the worst result in cases of sub-obturation , 70.0%, with a statistically significant difference. It is concluded that there was a high percentage of success in endodontic treatment performed by undergraduate students at the Faculty of Dentistry of Pernambuco, indicating that academics are receiving quality education.

Os menores níveis de infiltração ocorreram no grupo Fill Canal, seguido pelos grupos N-Rickeft e Sealer 26., It is an "in vitro" study to evaluate the sealing ability of 3 root canal sealers using a dye leakage/clearing method. 40 lower incisors were instrumented by the step-back technique and divided into 4 groups. The teeth were filled with lateral condensation of guta-percha varying the root canal sealer. Fill Canal, Sealer 26, N-Rickert were tested and compared to the control group in which guttapercha cones with no seater were used. Linear dye penetration was measured after demineralizatÎon and clearing. Statistical analysÏs showed significant differences among the groups. The lowest leakage values were found in the Fill Canal group followed by the N-Rickert and the Sealer 26 groups.
