Abstract

Background In Liberia, an estimated 32% of children under 5 are stunted. Malnutrition and hunger worsened during the country’s civil war and were further exacerbated by the 2014–2016 outbreak of Ebola virus disease. Studies examining adherence to recommended infant and young child feeding practices frequently do so with an emphasis on the knowledge, attitudes and beliefs of mothers and caregivers. Often overlooked are the structural factors that enable or constrain their agency to practise evidence-based recommendations.Methods Between July and December 2017, we surveyed 100 Liberian mothers to assess the sociodemographic factors associated with the risk of severe acute malnutrition in children in Maryland County, Liberia. We also conducted 50 in-depth interviews at two government health facilities to qualitatively explore mothers’ experiences, as well as health workers’ understandings of the determinants of malnutrition in the region. We applied logistic regression to analyse quantitative data and inductive content analysis to thematically interpret qualitative data.Results Mothers were less likely to have a child with severe acute malnutrition if they had an income greater than US$50 per month (adjusted OR (aOR)=0.14, p

Abstract

The transduction of the auditory signal is dependent on the flow of ions within the inner ear. We have generated mice deficient in NKCC1, an ion cotransporter that is thought to be involved in the secretion of K + by the strial marginal cells. Inner ear histology revealed partial to almost total absence of the scala media and collapse of Reissner’s membrane. Ultrastructural analysis showed that Reissner’s membrane consists of 3–4 cell layers instead of the usual two, and a substance of unknown composition is present between Reissner’s membrane and underlying structures. Within the tunnel of Corti, hair cells and supporting cells were difficult to identify. The location of the tectorial membrane was altered, and a precipitate was observed surrounding it. Severe structural defects were noted in the interdental cells and Boettcher cells, and mild defects were observed in the stria vascularis and in type II and type IV fibrocytes. The finding that major defects occur predominantly in cells that are not known to express NKCC1 suggests that loss of NKCC1 results in functional defects in cells expressing NKCC1 and a morphological effect on cell populations downstream in the proposed K + recycling pathway.

Abstract

Anatomic definition of the membranous labyrinth in the clinical setting remains limited despite significant technological advances in magnetic resonance imaging (MRI). Recent developments in ultra-high resolution imaging for use in the research laboratory on small animals and pathologic specimens have given rise to the field of imaging microscopy. We have delineated for the first time the labyrinthine structures in a human temporal bone cadaver specimen using these novel techniques. This approach to the study of the middle and inner ear avoids tissue destruction inherent in histological preparations using standard light microscopy techniques. Part I of this series focused on bony middle and inner ear anatomy with MicrCT. In Part II, we present high-resolution MicroMR images to highlight the utility of this technique in teaching radiologists and otolaryngologists clinically relevant anatomy focusing on the membranous labyrinth. This anatomy can be further enhanced using 3D volume-rendered images. It is hoped that familiarity with these ex vivo anatomic techniques will encourage further developments in the field of high-resolution clinical imaging for patients with temporal bone pathologies.
